Overview:

Cardinal Health E3 Grant Program

The Cardinal Health Foundation supports local, national and international programs that improve healthcare efficiency, effectiveness and excellence and the overall wellness of the communities where the Cardinal Health (NYSE:CAH) nearly 37,000 employees live and work. The Cardinal Health Foundation also offers grants to encourage community service among its employees and works through international agencies to donate much-needed medical supplies and funding to those who need them in times of disaster.

Cardinal Health develops and supports programs that help healthcare providers implement best practices that can truly transform patient care. Over the past nine years, the Foundation has invested $8.6 million with 297 grants to hospitals, health systems or other health-related organizations. These projects have produced some very significant outcomes and insights.

We remain committed to helping improve the efficiency and effectiveness of healthcare and in 2017 the E3 program take into account our rapidly changing demographics, the importance of the patient and the family as key partners in care and a renewed appreciation for the value medications, when used appropriately, can play in positive patient outcomes.

We are seeking proposals that include the following criteria in 2017:

  • Supporting the best use of medications, especially at transitions across the continuum of care from the hospital to home/to ambulatory settings.
  • Managing diabetes and/or multiple chronic diseases.
  • Being informed by and implementing emerging innovations that have been tested and are supported by data showing success in improving outcomes, reducing cost or accelerating the rate of change in healthcare.
  • Engaging patients and their caregivers/families as well as healthcare leadership.
  • Publishing or otherwise sharing outcomes.
  • Finding innovative and sustainable ways to effect long-term change.

Organizations may submit more than one proposal for different programs that relate to the specific criteria outlined in the program guidelines and instructions. Organizations should not submit the same proposal for multiple hospitals, however.

    • Non-profit healthcare institutions including those in acute and ambulatory settings working in partnership with each other or independently.
    • Collaboratives are also eligible.
