About the Foundation

We are a non-profit, education-focused organization dedicated to promoting improved quality of life for the terminally ill, their families, caregivers, and friends. We offer help to communities and healthcare providers.

Crossroads Hospice Charitable Foundation is dedicated to educating communities on Hospice resources, identifying excellent Hospice care, and by offering grief recovery resources. Our objective is to offer education to community organizations who provide end-of-life care and assist those organizations providing grief recovery support. We offer funding to enhance community awareness supporting end-of-life care and services. Funds given by Crossroads Hospice Charitable Foundation are designated funds to support end-of-life training and grief recovery support classes.

Our mission is dedication to ultimate life. This means that we work toward a better quality of life for the terminally ill and their family, friends, and caregivers.

What We Do

Education and Support. We offer education, free resources and support regarding excellent end-of-life care, caregiver assistance and compassionate resolution services.

Financial Assistance. We provide funds to other non-profit community agencies, healthcare providers, and caregivers who share our goal. Your donation is sent to Crossroads Hospice Charitable Foundation in Tulsa, OK and then distributed back to your loved one's community.

Who We Are

In 1995, before there was a foundation, there was Crossroads Hospice. Grateful family members and friends of patients expressed their appreciation for Crossroads' care in the form of financial donations. These funds were given to promote hospice care for others and to share knowledge of hospice philosophy of care. Crossroads could not accept funds directly, so these donations were passed along to a foundation outside our local communities.

Funds grew, along with awareness that our donors would prefer their contributions be used in the communities where their loved ones had lived. In 2003, Crossroads Hospice Charitable Foundation was formed to assure this promise could be kept.

Our mission is to help healthcare professionals better understand the role of hospice care in maximizing the quality of life for terminally ill patients, their caregivers, families, and friends. We accomplish our mission by focusing on education regarding excellent end-of-life care and by providing information and resources for use in care of the terminally ill, support for caregivers, and grief resolution for the bereaved.
