Request for Proposals: Vision Screening and Treatment
Vitalyst Health Foundation is seeking proposals for the July 2018-June 2021 Vision Medical Assistance Program (MAP). The goal of the Medical Assistance Program is to improve access to healthcare through direct service for low income individuals, leveraging existing resources and improving the quality of care to underserved populations in Maricopa County.
Vitalyst Health Foundation was created when the St. Luke’s Hospital System was sold in 1995. In keeping with the hospital’s long tradition of charity care, the Medical Assistance Program was established in 1996. Initially the funding included several restricted funds in addition to the Vitalyst general fund. The focus on the funds is primarily Maricopa County. The program continues to fund the Joseph C. Theisen Endowment for substance abuse treatment, the Charles J. Felton Endowment for vision, and the Charles J. Felton Endowment for hearing. Vitalyst will seek proposals for services that leverage existing resources and maximize the benefits provided under the Affordable Care Act. Other objectives are to pilot new technologies and approaches to direct care.

Grant Amount and terms
Grant amounts could generally range up to $80,000 annually, for a three-year award period.
