RFA: RHTP 4.4b Remote Patient Monitoring Implementation Initiative

The Nebraska Department of Health and Human Services invites proposals from Critical Access Hospitals (CAHs), Rural Emergency Hospitals (REHs), Regional Rural Hospitals and Federally Qualified Health Centers to implement initiative 4.4b hospital-to-home Remote Patient Monitoring (RPM) programs across rural Nebraska.

This initiative is funded through the Rural Health Transformation Program (RHTP) and supports Nebraska's strategy to expand innovative care delivery models that improve chronic disease management, strengthen rural health infrastructure, and reduce avoidable hospital utilization.

Remote Patient Monitoring technologies allow providers to track patient health data outside traditional clinical settings using connected devices that transmit clinical data to care teams. These technologies enable earlier detection of clinical deterioration, improve patient engagement, and support continuous care management for rural patients who face barriers to frequent in-person care.

This RFA focuses on expanding hospital-based RPM programs that monitor patients at home, particularly individuals with chronic disease or those recently discharged from hospitals who require close follow-up monitoring.

Program Scope

Funded projects will support the implementation or expansion of hospital-based Remote Patient Monitoring programs designed to improve clinical outcomes and reduce avoidable hospital utilization among rural populations. Projects should focus on hospital-to-home monitoring models that enable hospitals or health centers to remotely track patients' health status using FDA-cleared or FDA-approved monitoring technologies.

RPM programs should prioritize patients with chronic conditions such as:

• Hypertension (excluding hypertensive disorders of pregnancy)
• Diabetes
• COPD
• Cardiovascular disease (excluding heart failure)
• Chronic Kidney Disease
• Behavioral Health
• Other high-risk chronic conditions

RPM programs may also include monitoring for:

• Post-discharge follow-up care
• Sub-acute monitoring of complex patients
• Care management for high-risk populations
• Monitoring of waiver populations or patients receiving community-based services
• Psycho-social emotional well-being engagement

Funding Amounts

• Awards will be made based on project scope, hospital or FQHC size, and the population served.
• Single applications should request no less than $200,000 and no more than $1,000,000.

The Carolyn L. Kuckein Student Research Fellowship, awarded by Alpha Omega Alpha Honor Medical Society, supports first and second-year medical students conducting research over 2 to 12 months. With a total funding of $7,000, students can receive reimbursement for travel expenses up to $1,000 for presenting their findings at national conferences. Eligibility requires enrollment in schools with active AΩA chapters, while those taking a year off for research or holding a PhD are ineligible.

RFA: RHTP 3.3 Rural Health Care Workforce Incentive and Sustainability Model

The Nebraska Department of Health and Human Services, Office of Economic Assistance (DHHS) is issuing this Request for Applications (RFA) for the purposes of entering into a Grant agreement and awarding Federal funds through the Rural Health Transformation Project 3.3 to multiple eligible and qualified agencies. The purpose is to strengthen Nebraska's rural health care workforce by investing in the development and expansion of SNAP Employment & Training (SNAP E&T) Third-Party Partners that create sustainable health care career pathways in rural, frontier, tribal, and other high-need communities across the state.

Total Funding available

• $2,000,000
• The amount of funding each Applicant is eligible for is determined based on need and available funding. Each applicant will be eligible for up to $200,000. If applications exceed available funding, DHHS reserves the right to decrease requested funding allocations to maximize the available funding to best serve Nebraska and the requirements of the RHTP Initiative 3.3 purposes. 

This initiative is intended to:

• Build organizational infrastructure necessary to become an approved SNAP E&T Third-Party Partner.
• Develop sustainable health care workforce pipelines.
• Transition providers to the SNAP E&T 50/50 reimbursement model after the grant period.

Jackson and Rose Marie Jackson Charitable Trust Grant provides crucial financial assistance to organizations supporting physically handicapped individuals within a 100-mile radius of Omaha, Nebraska. With two annual grant cycles, the application process begins with a Letter of Intent, followed by a full application for approved projects. Eligible applicants must have tax-exempt status and complete a final grant report. Grants are not provided for for-profit entities, individuals, or general operating expenses.

Nebraska Public Transit

The Nebraska Department of Transportation's (NDOT) Nebraska Public Transit is more than transportation—it’s a lifeline. It connects residents to healthcare, education, jobs and more while fostering independence, mobility and stronger community connections.

Section 5311, Rural Public Transportation

Federal and State funding is available to support public transportation projects in rural Nebraska. This support includes reimbursement for operating expenses subsidies to purchase transit vehicles and awards to construct or renovate transit-related facilities. Eligible subrecipients of funds to administer a public transportation project include private nonprofit organizations, state or local governmental authorities, operators of public transportation services or intercity bus operators.

The Section 53|| program awards grants for operating assistance on a two-year grant cycle. Applications are due in March prior to the grant cycle ending on September 30 of that fiscal year. However, NDOT may allow new applicants to apply for funding at any time on a case-by-case basis. Existing 53|| program participants can apply for vehicles throughout the fiscal year. Startup rural transit agencies should apply for operating assistance and vehicles) at the same time.

What does this program do?

It provides loans and grants to Microenterprise Development Organizations (MDOs) to:

• To help microenterprises startup and growth through a Rural Microloan Revolving Fund.
• Provide training and technical assistance to microloan borrowers and micro entrepreneurs.

Microenterprise Development Organizations must demonstrate experience in managing a Revolving Loan Fund, or:

• Certify that it or its employees have received education and training from a qualified microenterprise development training entity so that the applicant has the capacity to manage such a revolving loan fund.
• Demonstrate that it is actively and successfully participating as an intermediary lender in good standing under the U.S. Small Business Administration (SBA) Microloan Program or other similar loan programs as determined by the Administrator.

 What kind of funding is available?

• Grants are available to provide technical assistance to rural micro-entrepreneurs or microenterprises, up to $205,000 annually.
  • Funding at the requested level is not guaranteed, and at least 15 percent matching funds are required.
• Loans of $50,000 to $500,000 may be used for establishing a Rural Microloan Revolving Fund managed by the Microenterprise Development Organization.
  • Total aggregate debt is capped at $2.5 million.

What are the loan terms?

• Maximum term is 20 years.
• Two-year payment deferral.
• Must establish a loan loss reserve fund.

What terms are required on loans to ultimate recipients?

• Up to $50,000.
• Fixed interest rate.
• Limited to 75 percent of project cost.

How may the funds be used?

Microlenders may make microloans for qualified business activities and expenses including, but not limited to:

• Working capital.
• Debt refinancing.
• Purchasing equipment and supplies.
• Improving real estate.

The Nebraska Crime Commission is offering up to $5,498,000 for the Community-based Juvenile Services Aid Program. This funding will support initiatives for youth aged 12 to 18, allocated based on various factors including county demographics and poverty rates. HOME Investment Partnerships Program (HOME)

The United States Department of Housing and Urban Development (HUD) allocates HOME funds to Participating Jurisdictions (PJs). The State of Nebraska receives funds as a PJ and the Department of Economic Development (DED) administers the program for the State.

HOME CHDO Operating Program

HDO Operating expenses means reasonable and necessary costs for:

• The operation of community housing development organization. Such costs include salaries, wages, and other employee compensation and benefits; employee education, training, and travel; rent; utilities; communication costs; taxes; insurance; equipment; materials; and supplies. The requirements and limitations on the receipt of these funds by CHDOs are set forth in § 92.300(e) and (f). §92.208(a)

Within the CHDO Operating funding category, the Department may award funding for eligible operating expenses (not project related):

• Assist the organization in maintaining the current level of production and/or development of affordable housing.
• Assist the organization in increasing their capacity; including hiring new staff; and
• Assist the organization in expanding their services or coverage area.

Nebraska Library Commission

The Nebraska Library Commission was created in 1901 and is part of the executive branch of state government. Per state statute, the Commission is responsible for the statewide promotion, development and coordination of library services. In fulfilling these functions the Commission works with all types of libraries – public, school, college and university, special and institutional libraries. Some of the Commission’s services are provided directly to the public and to state government personnel.

Library Improvement Grants

Program Goals

The purpose of the Library Improvement Grants is to facilitate growth and development of library programs and services in Nebraska public and institutional libraries, by supplementing local funding with state funds designated for these purposes.

Applicant projects are encouaged to meet one or more of the following purposes:

• Facilitate access to resources . . . for the purpose of cultivating an educated and informed citizenry;
• Encourage resource sharing among . . . libraries for the purpose of achieving economical and efficient delivery of library services to the public;
• Promote literacy, education, and lifelong learning and to enhance and expand the services and resources provided by libraries, including those services and resources relating to workforce development, 21st century skills, and digital literacy skills;
• Ensure the preservation of knowledge and library collections in all formats and to enable libraries to serve their communities during disasters;
• Promote library services that provide users with access to information through national, state, local, regional, and international collaborations and networks.

Grants require at least a local match of twenty-five percent (25%) of the total project amount. At least 10% must be a cash match. The remaining 15% of the local match can be in-kind costs and/or additional cash.

The Environmental Quality Incentives Program (EQIP) offers financial and technical assistance to agricultural producers for implementing conservation practices that enhance environmental quality and address natural resource concerns. With a total funding of $41 million across nine regions, CTPs play a crucial role in promoting responsible planning and decision-making within local governments and the private sector.

The Nonpoint Source Management Grants (Section 319) provide vital funding for small projects aimed at combating runoff pollution, a significant contributor to water quality degradation. Administered by the Nebraska Department of Environment and Energy, these grants offer up to $15,000, with a 40% non-federal match required. Eligible applicants include government units, educational institutions, and non-profits, making it an essential opportunity for organizations seeking to improve water quality through innovative local projects.

The Small Town Grants program, part of the Clean Water State Revolving Fund, provides matching grants to municipalities with populations of 10,000 or less. With over $10.1 million allocated since July 2020, the program aims to subsidize costly wastewater treatment and sewer projects. Eligibility requires completion of a Needs Survey and a median household income below the state average, ensuring support for communities facing financial hardship.

The Nebraska DEE Emergency Assistance Grants provide crucial financial support for wastewater treatment facilities that have been impacted by natural disasters or unforeseen events. The Child Care Start-Up and Expansion Grant offers financial assistance of up to $10,000 for child care centers and $5,000 for child care homes to enhance service capacity. Eligible applicants include individuals, groups, and existing programs aiming to meet licensing requirements or expand services. This initiative supports new quality child care programs and modifications to existing ones, ensuring providers can maintain operations and child care subsidy enrollment for three years.

The NE Emergency Mini Grant, offered by the Nebraska Department of Health and Human Services, provides funding up to $2,000 for licensed child care centers and homes. This grant aims to support necessary items for licensure based on inspections by relevant authorities. Eligible applicants must maintain good standing and ensure compliance with child care subsidy requirements. The initiative focuses on improving child care quality and accessibility for low-income families, enhancing early childhood development programs.

With an annual investment of approximately $12 million, the foundation addresses food security, promotes water conservation, and enhances community sustainability. Eligible applicants must be recognized charitable organizations, primarily in North America, committed to improving quality of life through innovative solutions and partnerships.
