NAFC Patient Care Fund

The NAFC Patient Care Fund supports direct patient services that expand immediate access to care. This funding is designed to help NAFC members meet urgent patient needs by supporting clinical supplies, expanded services, contracted providers, transportation assistance, medication access, and other direct care resources.

The Conservation Alliance Grants program is currently under evaluation, leading to a pause in the member-directed grant cycle as the organization aligns its funding with its strategic plan launching in 2025. This initiative aims to harness the collective efforts of businesses and outdoor communities to protect North America's wild places through impactful and efficient grant-making. Ben & Rose Schwabenbauer Educational Scholarship

Scholarships will be awarded annually from this fund for college-bound students from any Cattaraugus County High School committed to advancing their skills in any aspect of the arts. Preference will be for graduating seniors of Ellicottville, Cattaraugus-Little Valley, West Valley, Salamanca or Franklinville/Ten-Broeck Academy high schools. First preference will be for students pursuing the fine arts, with second preference for students pursuing performing arts fields. This award is not limited to students majoring in art.

The Black Men Building Resources Scholarship, offered by the Grand Rapids Community Foundation, supports African-American undergraduates from Kent County. With awards ranging from $2,000 to $5,000, the program prioritizes applicants with high financial need, ensuring that opportunities are accessible to those who strive for educational advancement in West Michigan.

The 1889 Foundation offers selective sponsorship for educational events in Cambria and Somerset Counties, aiming to enhance community health. Eligible projects must promote the Foundation's focus areas, involve multiple community partners, and demonstrate substantial community support. Organizations must be U.S. tax-exempt non-profits with a proven financial stewardship record. The maximum sponsorship is $500 per year, and requests should be submitted in writing with supporting documentation.

The Horner Foundation is a family-based charity dedicated to partnering with community-based organizations that provide youth, aged 10-19, hands-on experiential learning opportunities through Out of School Time (OST) to foster positive youth development.

We work in 4 geographic regions, determined by where family members live, their specific interests and needs identified by the community.

Organizational Funding Statement

The Horner Foundation strives to be responsive and meet organizations where they are. This means that we may provide multi-year funding and ongoing support to nonprofits who may be launching a pilot project or going through a transition. However, we also believe in the value of diverse funding sources. The Horner Foundation typically funds organizations for 1-2 funding cycles (2-4 years). In rare cases, we may fund an organizations for 3 funding cycles (6 years).

Philadelphia, Pennsylvania: Out of School Time (OST) Organizations - STEM Programs    

Strategy

Since 2013, The Horner Foundation has supported Out of School Time programs working directly with underserved youth aged 10-19 in the City of Philadelphia. For our 2024 funding cycle, we will focus on out-of-school time STEM programming. 

Funding Focus

STEM (Science, Technology, Engineering, and Math): The intention of this funding focus is to inspire members of the next generation to pursue careers in the science, technology, engineering, and math sectors through programming delivered in an inclusive learning environment.

• The program may include coding, robotics, entrepreneurship, engineering, and other STEM-related OST programming aimed at equipping youth with the tools necessary to thrive in a tech driven society.
• This funding focus also includes programs that encourage leadership training and career preparation for young tech leaders in underserved communities.

Scholarship Fund Details

Support for West Michigan Students

Attending college or trade school increases opportunity and opens doors to careers. Donor partners who establish a scholarship fund give students a boost on their path to higher education. Students who complete the general scholarship application are considered for these scholarships.

Open to students who are current or former wards of the court in Kent County, Michigan pursuing an undergraduate college or vocational education, have financial need and a 2.5 minimum GPA.

Must provide a letter of recommendation from a caseworker.

The ECMC Foundation focuses on eliminating equity gaps in postsecondary education by 2040 through strategic grantmaking and investments. They support organizations aiming to remove barriers to completion, build institutional capacity, and transform the postsecondary ecosystem. Eligible entities include nonprofit organizations, government entities, and postsecondary institutions. However, funding is not available for personal scholarships, political activities, or projects outside the U.S. that do not align with the Foundation's mission of diversity and inclusion.
