Minority health and physical education professionals advancing community health education can be honored through the E.B. Henderson Award, open to long-term SHAPE America members across the U.S., including American Samoa. Health and physical education professionals advancing measurement and evaluation in school and community health programs in American Samoa may be eligible for this honor. Grants from Society of Health and Physical Educators help recognize SHAPE America members who have significantly contributed to the Measurement and Evaluation SIG and its mission. This is an individual professional recognition award, not program funding.

Grants from Society of Health and Physical Educators help American Samoa community health education professionals and K–12 health and PE educators gain national recognition for lifetime scholarly work in measurement and evaluation. Current SHAPE America members with substantial, long-term contributions to theory and practice in health and physical education may be nominated, including posthumous candidates within five years of death.

The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. Food as Medicine Initiative

The Food as Medicine Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to address food insecurity as a critical driver of health outcomes for uninsured and medically underserved patients. Research continues to demonstrate the strong connection between food insecurity and poor health, including increased risk of chronic conditions such as diabetes, hypertension, obesity, and certain cancers. Through this initiative, NAFC will provide $20,000 grants to member free and charitable clinics to implement or expand food insecurity screening and patient-centered interventions that improve access to healthy food and nutrition education as measured by the US Household Food Security Module or Hunger Vital Signs and patient education focused on food as medicine for eligible patients in order to increase the percentage of patients with improved food security scores within one year of project implementation.

Medication Therapy Management (MTM) Initiative

The Medication Therapy Management (MTM) Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to improve medication access, adherence, and patient understanding among uninsured and medically underserved populations. Individuals without health insurance often face significant barriers to obtaining prescription medications and receiving the counseling necessary to manage chronic conditions effectively. Through this initiative, NAFC will provide $30,000 grants to member free and charitable clinics and pharmacies to establish or expand Medication Therapy Management programs aligned with recommendations from the American Pharmacists Association. Funding will support activities such as staff certification in MTM, development of standardized protocols, and patient education to help individuals better understand their medications, manage side effects, and support overall treatment plans. By strengthening pharmacist-led care and improving communication between providers and patients, this initiative seeks to enhance medication adherence, improve chronic disease management, and advance better health outcomes for vulnerable patient populations.

The CDC Foundation is authorized by Congress to mobilize philanthropic partners, corporations, organizations and individuals to support the critical health protection mission of CDC and the public health system.

The CDC Foundation manages hundreds of programs each year aimed at protecting communities across the country from a variety of health threats—from chronic disease conditions including cardiovascular disease and cancer, to infectious diseases like malaria and HIV to emergency responses, such as Ebola and Zika.

Contractor for Implementation and Evaluation of Healthy Native Babies Project Materials

The CDC Foundation invites proposals from Tribes, Tribal Serving Organizations (TSOs), and Urban Indian Organizations (UIOs) [T/TSOs/UIOs] to implement and evaluate the implementation of the updated Healthy Native Babies Project (HNBP) materials within their service areas. This initiative aims to strengthen culturally relevant infant safe sleep education, increase community awareness and support Native families in reducing sleep related infant deaths. Selected T/TSOs/UIOs will adapt, disseminate and assess the use of HNBP materials in ways that reflect the cultural, linguistic and community-specific needs of the Native populations they serve.

The Global Health focus area aims to close gaps in quality healthcare by supporting charitable solutions that improve access, bring care closer to where people live, and address the high burden of non-communicable diseases (NCDs) in resource-limited settings. To the extent permissible, the Foundation prioritizes charitable efforts related to cardiometabolic health in resource-limited settings.

K-12 STEM Education

The K-12 STEM Education focus area aims to close the gaps for students from low-income communities by supporting programs that foster new pathways and pipelines to STEM education through focused K-12 efforts, concentrating on the critical middle school years. The Foundation’s K-12 STEM Education grants are focused on initiatives that benefit Marion County, Indiana.

Economic Mobility

The Economic Mobility focus area supports charitable and educational programs that strengthen workforce readiness, postsecondary and skills-based educational pathways, and housing stability to promote long-term prosperity. This support is focused on Marion County, Indiana, with the goal of improving economic opportunity for low-income communities and advancing community well-being.
