Improving Health Outcomes in Patients with ATTR-CM Through the Development of a Shared Decision-Making Tool (Decision Aid)

Overview

Transthyretin Amyloid Cardiomyopathy (ATTR-CM) is a progressive, lifethreatening condition associated with significant morbidity, mortality, and burden on patients, caregivers, and healthcare systems. The multisystemic disease course is often characterized by diagnostic delays, complex treatment pathways, and evolving therapeutic options, which can create uncertainty and decisional conflict for patients and clinicians alike. Shared decision-making (SDM) has emerged as a critical component of patientcentered care1, particularly in complex and preference-sensitive conditions such as ATTR-CM. Well-designed decision aids can support meaningful dialogue between patients and healthcare professionals by improving understanding of disease trajectory, treatment options, risks and benefits, and alignment of care with individual patient values and goals.

Sidney and Carol Aroesty Family Scholarship

Purpose of Scholarship

To provide financial assistance to East High School graduating seniors planning to pursue a degree in any science discipline at a two- or four-year college, university, or technical school.

Number and Size of Awards

One award of $12,000; divided into equal annual installments paid over four consecutive years of undergraduate study. Award is paid in the Fall and divided equally between the Fall and Spring semesters.

Hero Fund America Fund Seeks Applications for its First Responders Grant Round

Hero Fund America (HFA) Fund, a field-of-interest fund of the Community Foundation of Herkimer and Oneida Counties, is seeking applications for its First Responders Grant Round for programs, projects, and other needs that will support education, training, lifesaving equipment, and mental health of fire, emergency medical services (EMS), and law enforcement agencies.

Hero Fund America Fund was founded in partnership with Hero Fund America by Garth Roberts of Sharon Springs, NY. Inspired by the selfless and heroic efforts of first responders during the 2013 Boston Marathon bombings, Roberts initiated the Provincetown First Responders Appreciation Month, honoring 45 Boston EMS paramedics, emergency medical technicians, dispatchers, and their families. The success of that project inspired the 2016 creation of Hero Fund America.

Since 2016, the fund has awarded more than 100 grants totaling over $123,000 to meet the needs of first responders across the nation.

“Grants awarded through the Hero Fund America Fund have helped departments improve their abilities to better serve and protect the civilian population,” said Roberts. “Hero Fund America remains firmly committed to raising both funding and awareness in support of the First Responder community.”

The Council fosters innovative research, nurtures new generations of social scientists, deepens how inquiry is practiced within and across disciplines, and mobilizes necessary knowledge on important public issues.

Driving University Impact: Expanding the Public Exchange Network

With generous support from the William and Flora Hewlett Foundation, the Social Science Research Council is pleased to invite U.S. R1 research universities in SSRC’s College and University Fund for the Social Sciences to apply for funding to join the Public Exchange Network. Public Exchange is a new network that turns university research expertise into real-world action and impact. Foundedat the University of Southern California in 2020 and expanded to Washington University in St. Louis in 2025, Public Exchange connects researchers with government, nonprofit, and industry partners to tackle pressing challenges—from disaster recovery and climate solutions to food insecurity and homelessness prevention. Since its founding, Public Exchange has developed high-impact collaborations between 48 partners and 142 researchers including:

• CLEAN LA and CLEAN STL: One of the largest free soil testing programs for lead in Los Angeles County following the January 2025 wildfires, and a related program in St. Louis to test soil and air for contaminants following the May 2025 EF-3 tornado;
• The California Solar Canal Initiative: An ambitious public-private research collaboration to advance solar panel infrastructure along California’s 4,000 miles of open aqueduct, conserving water and generating renewable energy;
• Food Base LA: LA County’s first food systems data dashboard, helping LA County and non-profit partners target distribution of millions of pounds of food and related resources to families experiencing food insecurity;
• ShadeLA: A people-powered campaign to cool Los Angeles ahead of the 2028 Olympics and Paralympics by advancing natural and built shade cover in the region.

Partners have included the City and County of Los Angeles, the State of California, the City of St. Louis, Yelp, Solar AquaGrid, the United Nations Foundation, Keck Medicine of USC, the Southern California Association of Governments, Santa Clarita Valley Water Agency, the Sustainable Entertainment Alliance, the Los Angeles Regional Food Bank, Department of Angels and many more. In addition to real-world impact positively affecting millions of lives, Public Exchange collaborations have resulted in over $18m in research funding, some helping to incubate new centers or areas of research, more than 40 academic journal publications, hundreds of media hits, and dozens of student career opportunities. 

The Public Exchange Model

Public Exchange is a partner-driven program, working collaboratively with external partners to: (1) define a research scope, timeline, and deliverables; (2) identify the right team of multidisciplinary academic experts to analyze the problem; (3) provide project management from concept and strategy development to project outputs and tools delivery; and (4) manage the public and stakeholder engagement related to the project, as well as communications and design needs. Public Exchange staff are not researchers, but come from government, industry, and non-profit backgrounds, uniquely equipping project teams to understand partners’ needs and lead projects to action. Staff oversee activities including partnership and proposal development, funder cultivation, research translation, dissemination, and communications, impact and scaling strategy, administrative support, cross-sector convening, and more, to maximize the real-world impact of university expertise. Public Exchange builds results-driven teams that deliver measurable impact and lasting solutions for partners.

How the Public Exchange Network Works

No single university – even a large R1 – has enough faculty expertise available to meet the needs of every partner. The Public Exchange Network was designed to help universities work together seamlessly to build robust partnerships and generate more collective impact. Universities joining the network are independent but affiliated members, with full control over their own program and project portfolios. As a national network with local, on-the-ground capacity, Public Exchange Network members can also share approaches that work, scale solutions to common problems, and build cross-regional partnerships together. 

As an institutional partner in the Public Exchange Network, universities will build capacity to develop, test and implement solutions to the most urgent problems in their regions and beyond. Universities joining the Public Exchange Network get:

• The tools, templates and training they need to start right away;
• A shared brand they can plug into on day one;
• A network of faculty, past and potential partners and philanthropic relationships, benefitting everyone in the network. 

SSRC Public Exchange Network Expansion Grants

Public Exchange Network Expansion grants will cover 75-80% of the minimum estimated annual costs for two years to launch a new Public Exchange. To launch a Public Exchange, universities must have the following in place: (1) a staff executive director with experience launching and leading social impact partnerships, (2) a project seed fund, and (3) a modest operations budget. We also recommend some in-kind project management support from an existing university staff member. The grant includes a two-year Public Exchange Executive Director (ED) compensation subsidy of $300,000 to launch the Public Exchange on campus, and a 2-year license to use the Public Exchange brand. New Public Exchange Network members will also receive ongoing support and guidance from current Public Exchange leadership and staff at USC and WashU, including recruitment support and intensive training for hiring and onboarding an executive director, a sub-site on the Public Exchange website, incubation and project management support from current Public Exchange staff, and a suite of tools for developing and implementing Public Exchange projects.

In addition, every university in the Public Exchange Network benefits from central communications support (currently housed at USC), and access to a collaborative network of project partners, researchers, and funder relationships. Separately, SSRC will select a local research fellow to study and document how the Public Exchange model develops within and between host institutions during the grant term, and how the Public Exchange Network universities work together to effect larger-scale change. 

Expectations

Institutions selected to join the Public Exchange Network are expected to:

• Hire an Executive Director who can launch and lead efforts to incubate social impact projects with public and private sector partners;
• Have in place before public launch: (1) an executive director, (2) a seed fund, (3) a modest operations budget, and (4 – recommended) some in-kind project management support. 
• Develop and sustain at least three high-impact research partnerships during the two-year grant tenure;
• Invest modest resources during the grant term that demonstrate commitment to continuing the Public Exchange model after the end of philanthropic support. This is estimated at approximately $85-$90k annually (with some variation considering local labor markets) to support an operations and project seed fund and any additional capital needed for the Executive Director hire beyond the subsidy. 
• Coordinate with the SSRC research fellow who will examine the impact of Public Exchange at their university and across the network. 
• Work with faculty across their university. Public Exchange can live anywhere within a university at the school or central level. The majority of Public Exchange projects to date at USC and WashU have included researchers from two or more schools, and grantees must be able to work with faculty university-wide. 

Middlesex County Medical Society Foundation

The Middlesex County Medical Society Foundation, founded in 1964 and was initially funded by contributions from the first mass polio vaccination in our county.

MCMS Foundation Scholarship Fund

Middlesex County Medical Society Foundation is an independent nonprofit organization that awards scholarships to deserving students from Middlesex County, New Jersey for the study of medicine. Scholarships are awarded to deserving students who are enrolled in an accredited medical school.

The CDC's National Centers of Excellence in Youth Violence Prevention (YVPCs) aim to expand the evidence base for preventing youth violence among individuals aged 10 to 24. This initiative supports rigorous evaluations of prevention approaches, community engagement, and training for early career researchers. By establishing partnerships and focusing on communities with above-average youth violence rates, the YVPCs seek to enhance collaborative efforts and effectively disseminate research findings to inform future prevention strategies.

John I. Smith Charities Scholarship

Established in 1989 by John I. Smith Charities to provide educational incentives and financial assistance to individuals residing in government-subsidized housing with the goal of helping students achieve self-sufficiency and become contributing citizens of the community through educational attainment.

Funding Amount

• $800.00 per semester for up to 4 years.
• At least one scholarship will be awarded annually.
• Award amount and number of awards are determined annually based on the Community Foundation of Greenville’s Spending Policy for Endowment Funds.
• Scholarship funds will be paid per semester to the approved educational institution in the name of the student and applied only to the costs of tuition, room, and board, or required fees.

Greenville County Fire Chiefs’ Association Scholarship

In 1995, the Greenville County Fire Chiefs’ Association established a scholarship fund to promote higher education for firefighters and their spouses or children.

Funding

• A minimum of $5,000 will be awarded annually and the number and amount of each award will be determined annually.
• Scholarship funds are paid to an approved educational institution in the name of the student.
• Scholarship funds are awarded for one academic year and are not subject to automatic renewal. 

4-H Beef Club Fund

The 4-H Beef Club Fund, established by the Whitley County 4H Beef Club, provides educational scholarship support for any member of Whitley County 4-H intent upon studying Agriculture at an accredited college or university in pursuit of a 2 or 4 year degree in the field.

Tampa Jazz Club

The Tampa Jazz Club was incorporated as a non- profit organization in 1995, with the objective of “generating interest in preserving, understanding and performing jazz.”

Tampa Jazz Club Scholarship

The Tampa Jazz Club is committed to encouraging talented young jazz students and to presenting great live jazz in Tampa.

Since 2003, we have awarded Tampa Jazz Club scholarships to graduating area high school seniors who went on to study jazz performance or education. To date, we have awarded $24,750 to 22 graduates.

TSF is committed to improving patient care through support for research, education, innovation, and humanitarian outreach, aiming to advance heart and lung health for all.

As the charitable heart of The Society of Thoracic Surgeons, TSF collaborates globally to drive progress in cardiothoracic surgery. Through the efforts of TSF and its worldwide partners, patients with heart and lung disease experience improved longevity and a high quality of life.

STS Perioperative & Critical Care Conference Resident Scholarship

Purpose

This Resident Scholarship allows CT surgery residents to attend the STS Perioperative & Critical Care Conference. The scholarship covers course registration fees, two nights' lodging at the host hotel, and and expense reimbursement of eligible travel expenses.

Course Description

Attendees of the 2026 STS Perioperative & Critical Care Conference will explore the latest techniques and management strategies in cardiovascular and thoracic (CVT) critical care and enhanced recovery after surgery (ERAS). This two-day event features dynamic and interactive presentations, hands-on demonstrations, scientific abstracts, and expert panel discussions designed for all members of the CVT perioperative and critical care team. St. Ignace Area Community Foundation

The St. Ignace Area Community Foundation (SIACF) was established in 1995 by a group of concerned citizens who recognized a need and found access to some matching grant funds to enhance the initial contributions to the SIACF. Organized as a member of the Community Foundations of the Upper Peninsula (CFUP), Jack Swope, Fred Stearns, and Dr. A.D. (Don) McKinnon secured an anonymous contribution establishing the Education Enhancement Fund and a contribution from the local American Legion Post establishing the American Legion Fund as the initial Funds within SIACF.

Community Service Scholarship

The St. Ignace Community Fund annually awards scholarships assisting students with higher education based largely on acknowledgement of student responsibility, community service, and civic involvement. These Scholarships are made possible through the generosity of donors to the various accounts within the St. Ignace Area Community Fund. Some accounts specifically are awarded to recognize higher education in the areas of special education, teaching, law enforcement, and health care.

Ron Jones Memorial Scholarship 

Established to honor the memory of beloved coach and teacher Ron Jones. This is not an athletic award, rather, it was established to recognize students who have demonstrated a devotion to the community of Hoosick Falls through compassion, action, respect and excellence.

The National Institutes of Health (NIH) is set to announce a Notice of Funding Opportunity (NOFO) for multi-site investigator-initiated clinical trials. This initiative aims to support a range of clinical trials addressing efficacy and implementation research. The funding mechanism includes a two-phase approach (UG3/UH3), with a total award duration of up to seven years. Investigators are encouraged to prepare proposals that demonstrate scientific rationale and operational plans for these important studies.

The KUH P30 Resource Core Center funding opportunity invites applications for centers aimed at enhancing research in Kidney, Urology, and Hematology diseases. These centers will provide essential resources, including data and tools, to the research community, fostering collaboration through a national network with the Center Coordinating Hub. This initiative by the National Institutes of Health aims to drive innovative research and shared expertise for improved health outcomes.

The KUH U24 Center Coordinating Hub aims to foster collaboration among funded Centers within the NIDDK/KUHN mission. This initiative encourages the development and sharing of vital research resources, including data, reagents, and expertise, with the broader scientific community. By creating a national network, the program enhances research capabilities and promotes innovative solutions in kidney, urology, and hematology diseases.

The NIDDK Multi-Purpose Resource-Related Research Centers Cooperative Agreement (U2C) invites applications to enhance research capabilities in areas such as obesity and diabetes. These centers provide specialized services, including technical support and data coordination, primarily to clients at other institutions lacking necessary expertise. This initiative aims to bolster high-impact research through collaboration and experiential training, addressing critical health challenges aligned with NIDDK's mission.

Robert & Grace Karst R.N. Scholarship

This scholarship was created by Grace Karst RN, who dedicated 44 years to the nursing profession and as a nursing home administrator. Her husband Robert served in the Army, was a White House guard, and eventually retired after 36 years working for the railroad.

This award is presented to a Whitley County student intent upon becoming a Registered Nurse.

Scientific & Regulatory Consultants, Inc. Scholarship

Science plays a vital role in our daily lives and includes a wide range of fields such as physician or veterinary services, nursing, microbiology, biology, chemistry, engineering, mathematics, psychology, music, etc. This scholarship views “scientific” very broadly and invites you to be creative.

Your application will be reviewed based upon the quality, not quantity, of the response to each question.

Whitley County Builders’ Association Scholarship

This scholarship is available to a graduating senior who attends Columbia City High School, Churubusco High School, or Whitko High School and is planning to pursue a technical, associate or baccalaureate degree in the building trades field (such as Construction Engineer, Architecture, Building trades, etc.)

Philip & Leta Shriver Scholarship

Philip Shriver grew up in rural South Whitley and graduated from South Whitley High School. Leta Bernice Keel graduated from South Whitley High School in 1940. The two married on April 4, 1943. They made their home on a farm in rural South Whitley.

Leta was employed by Virgil Heinselman’s Locker and G & G Grocery. She was a member of the South Whitley United Methodist Church until she died on June 26, 2005.

A career-long farmer, Phil had a large chicken and egg operation for years, maintaining over 5000 chickens. An advocate for the industry, he was instrumental in stopping an egg carton tax proposed by the State of Ohio. He also worked construction for Morris Darley of South Whitley for many years. Phil was an accomplished welder and metal fabricator, making several crosses that adorn the steeples of area churches.

This scholarship was created as a gift from the estate of Philip Shriver after his death in 2021. It is open to any Whitley County graduating senior who plans to further their education in the field of agriculture or church-related work.

Whitley County 4-H Helping Hands Scholarship

This scholarship will go to a Whitley County senior involved with 4-H. Please contact the 4-H Extension Office for more information. The recipient will be selected during the Whitley County 4-H fair.

Tenney Family Scholarship

Anyone who has pondered how to leave a legacy needs to talk with Georgia Tenney.

It’s no secret that a good chunk of Georgia’s heart is entwined in Whitko Schools. A lifelong Pierceton resident, she spent over 30 years as the Middle School Secretary and is currently serving her third term as an elected school board member. Her Wildcat spirt was shared with her late husband Bob who retired as a custodian for the schools system. Together they cheered on Whitko sports until he could no longer attend games.

When Bob passed in January of this year, a couple of teachers approached Georgia with the idea of establishing a scholarship fund to which the Whitko community can contribute. Georgia put those thoughts into action and contacted the Community Foundation of Whitley County to help her fulfill a dream. It has taken shape in the form of an endowed scholarship that will benefit Whitko grads for generations to come. 

“I have always felt very fortunate in what I have been given and have long wanted to find a way to give back. After Bob passed, I told myself it was time to put a scholarship in place.” In addition to wanting to help local students achieve their educational and career goals, Georgia wanted to share a legacy of charitable giving with her children, grandchildren and great-grandchildren. “I want this to be something of which they can all be proud,” she reflected.

Meritus Healthcare Foundation Scholarship Fund

This fund provides scholarships to deserving students from any Washington County public or private high school accepted into a designated health care program.

Healthcare Assistance Grant

The MS Focus Healthcare Assistance Grant assists uninsured individuals with the cost of visiting their MS specialist as well as dental assistance. The grant, which can pay for one visit and one follow-up, is designed to assist those already diagnosed with MS who may need a visit to their treating physician to renew a prescription or handle other important needs. 

Applicants give MS Focus staff authorization to negotiate costs directly with the provider, and the fee is paid directly to the doctor. 

The Scripps Howard Fund, a public charity established by The E.W. Scripps Company is dedicated to creating informed and engaged communities through journalism education, childhood literacy and local causes. At the crossroads of the classroom and the newsroom, the Fund is a leader in supporting journalism through scholarships, internships, minority recruitment and development and First Amendment causes. The Scripps Howard Awards stand as one of the industry’s top honors for outstanding journalism.

Students with internship experiences are more likely to find employment upon graduation than those without, according to the National Association of Colleges and Employers. During the past decade, more than 500 students have honed their skills in newsrooms across the country, thanks to Scripps Howard internships. The Scripps Howard Fund created the internship program in 2023, in partnership with the Institute for Nonprofit News (INN), aimed at giving students an equal opportunity for high-quality, hands-on experience. INN leads a network of more than 500 independent newsrooms across North America. 

Clinical Research Training Scholarship in Autoimmune Neurology

This award recognizes the vital importance of rigorous clinical and translational research and aims to inspire and support early-career investigators pursuing clinical studies in autoimmune neurology — specifically immune-mediated diseases of the nervous system other than multiple sclerosis.

The scope of this award encompasses research on autoimmune neurologic disorders, including neural antibody-associated diseases of the nervous system, autoimmune encephalitis, immune-mediated epilepsy, autoimmune movement disorders, inflammatory myelopathies, NMOSD, MOGAD, paraneoplastic neurological syndromes, neurological complications of cancer immunotherapies, and inborn errors of immunity/autoinflammatory syndromes affecting the nervous system.

This award will consist of a commitment of $65,000 per year for two years, plus a $10,000 per year stipend to support education and research-related costs for a total of $150,000. Supplementation of the award with other grants is permissible, but to be eligible to apply for this award, the other grant source(s) cannot exceed $75,000 annually.
