Call for Applications:

Community-Led Seed Grants

AHW’s Call for Applications: Community-Led Seed Grants supports community-driven efforts to design new strategies, test innovative ideas, and foster greater collaboration to build a foundation for future endeavors to address Wisconsin’s leading health challenges and produce deeper impact on health and health equity in Wisconsin’s communities. Eligible community-based organizations may request up to $50,000 for projects completed within a 12-month period, beginning Jan. 1, 2027.

To foster a collaborative approach to award selection, applicants will participate in a peer review process to assess proposals submitted to this funding opportunity by their fellow applicants. For the submitted application to be considered for funding, completion of all assigned reviews is required.

Following funding determinations, successful community-led applicant teams will be matched with an eligible MCW faculty member from a pre-formed pool developed through the Call for MCW Academic Partner for Community-Led Grants to serve as an additional resource to the funded community-led projects. 

The Community-Led Momentum Grants, funded by the Advancing a Healthier Wisconsin Endowment, offer up to $250,000 for community-driven health initiatives in Wisconsin. These grants aim to support innovative strategies and programs that enhance the health workforce and promote health equity. Successful projects will engage collaborative teams to address community health needs over a 24-month period, starting July 1, 2026. This funding opportunity encourages partnerships for impactful health solutions.

Investing in Dental Care Technology: Grants to Improve Rural Dental Efficiency and Access

Funding opportunity summary

The Wisconsin Department of Health Services (DHS) is seeking applications for competitive grants to support dental clinics in adopting technologies that improve efficiency and increase access to care in rural and semi-rural communities. Dental practices, particularly in more rural areas of the state, face challenges that result in reduced access to care. These challenges include high rates of uninsured patients, low and unpredictable patient volumes, dental workforce shortages, and limited capital resources to adopt innovative technologies. This funding opportunity seeks to address these challenges through increased access to dental technological innovations, which improve dental services in rural Wisconsin by increasing the speed of appointments while maintaining quality of care. This funding will allow dental practices to purchase technologies to deliver high-quality and efficient services. Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.

Community Health Workers Grants: Strengthening the Rural Health Workforce in Wisconsin

Funding opportunity summary

The Wisconsin Department of Health Services (DHS) seeks to strengthen the Wisconsin rural health workforce by developing and expanding high-quality and impactful community health worker (CHW) programs in clinical and community settings. There are three main components to this funding opportunity:

• Hiring and supporting CHWs to carry out services in rural communities
• Training CHWs on core competencies and standards
• Building organizational capacity to sustain quality CHW service provision

Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.

The credit union’s funding and community priorities include offering competitive financial products, financial education and wellness resources, and support for local communities through scholarships, volunteerism, and sponsorships that strengthen financial stability and community vitality across its service areas.

PCM Credit Union Scholarship Program

The PCM Credit Union (PCMCU) offers an annual scholarship program to support members pursuing post-secondary education. PCMCU will award three (3) $1,000 scholarships to students who are beginning or continuing their education as full-time students. Applicants must be primary members of PCMCU, enrolled full-time in a post-secondary institution, and must not have previously received a PCMCU scholarship (effective 2026). A completed application and essay response must be submitted. The essay prompt reflects the credit union’s “people helping people” philosophy and asks students to explore how schools and credit unions can collaborate to promote financial confidence and responsible decision-making among young people.

The Trillium Foundation Grant aims to improve the lives of Wisconsin citizens, focusing on Older Adults and Individuals with Disabilities, including veterans and underserved communities. Named after a native wildflower, the foundation emphasizes resilience and community support. Eligible applicants must possess nonprofit tax-exempt status and cannot be individuals. The foundation does not fund activities that are already publicly funded or supported by other grantmaking entities.

The F26AS00085 Aquatic Invasive Species Interjurisdictional Grants, funded by the U.S. Fish and Wildlife Service, aim to support interjurisdictional projects addressing aquatic invasive species in the Great Lakes. Eligible applicants include Great Lakes states and tribes, focusing on action-oriented projects that enhance regional priorities. The initiative will fund both sustaining projects, which build on previous efforts, and new projects that present innovative solutions for effective management and prevention of aquatic invasive species.

The Diminished Tree Species Grant Program focuses on restoring native tree species crucial for biodiversity and forest health. By funding scientific research, seed orchard development, and land preservation, the program aims to combat climate change through effective tree restoration projects. The Oak Hill Fund, a private entity, encourages project-specific applications, prioritizing organizations that meet eligibility criteria under Section 501(c)(3) of the IRS. This initiative is vital for sustaining healthy ecosystems and mitigating environmental impacts.

The KCF Scholarships - Rizzo Family Memorial Scholarships Fund, managed by the Kenosha Community Foundation, offers a one-time award of $1,000 to local students pursuing higher education in Wisconsin. Eligible applicants include high school seniors or current students with a minimum GPA of 3.0, focusing on health-related fields. This scholarship aims to support academic advancement and foster community growth by investing in the education of future healthcare professionals.

The Collaborative Health Sciences Program RFP by the University of Wisconsin aims to fund innovative research addressing complex health issues in Wisconsin. With a maximum award of $600,000 over three years, this initiative encourages interdisciplinary teams to explore new directions in health research. Eligible proposals should demonstrate potential for additional funding and focus on improving health equity, addressing diseases, clinical knowledge, and innovative training for health professionals, while ensuring collaboration among diverse disciplines.

The Postdoctoral Grant Program at the University of Wisconsin School of Medicine and Public Health aims to support the growth and professional development of postdoctoral trainees, fellows, and associates. With funding up to $20,000 for one year, recipients can engage in independent research projects, attend workshops, or cover open-access publication fees. This initiative enhances research capacity and promotes health equity in Wisconsin, addressing critical health challenges while fostering career advancement in the medical field.

TC Energy’s Build Strong program supports vital organizations in communities near its operations. By providing grants, scholarships, and in-kind support, the program aims to enhance safety, education, environmental protection, and community well-being. Eligible applicants include non-profits and community groups addressing local needs. Ineligible activities include operational expenses and for-profit endeavors. The program prioritizes projects demonstrating measurable community impact, fostering a collaborative relationship between TC Energy and local stakeholders.

The Thomas J. Cashman Post Graduate Scholarship, awarded by the Environmental Education Foundation, aims to support students from the Geneva Lake area pursuing post-graduate studies in environmentally-related disciplines. This scholarship encourages previous undergraduate winners to apply for continued financial assistance, promoting academic excellence and environmental stewardship. Eligible applicants must demonstrate enrollment in a graduate program and a commitment to an environmentally-focused career, enhancing awareness and education about ecological issues in their community.

Nonprofit organizations with a verifiable U.S. tax ID working in Wisconsin can apply for Encova Foundation of West Virginia grants to support community health, wellness, education, safety, and children’s programs. 501(c)(3) organizations that impact health care or public health, especially those with active Encova associate involvement, may be especially competitive.

The WEDC Capacity Building Grants aim to enhance local development efforts in Wisconsin by supporting nonprofit organizations and educational institutions. These grants facilitate the creation and expansion of cooperative initiatives, ensuring that communities have the resources needed to thrive. Kansas Association of Independent Crop Consultants Scholarship Fund

The Kansas Association of Independent Crop Consultants created this scholarship fund to award to a qualified individual majoring in an agricultural field.

The Community Capacity Grant Program, established by the University of Wisconsin School of Medicine and Public Health, aims to support Wisconsin-based organizations dedicated to advancing health equity and addressing social determinants of health. With grants of up to $20,000, this initiative encourages innovative projects that enhance organizational capacity without covering core operating expenses. By fostering partnerships and mentorship, the program seeks to empower smaller nonprofits to effectively achieve their missions and improve community health outcomes.

The Wisconsin Partnership Program Faculty Grants: Opportunity Grant Program supports innovative research and education projects addressing critical health challenges in Wisconsin. This program offers one-time funding of up to $200,000 over two years for projects with potential to transform fields or apply novel ideas. Applicants must demonstrate a clear path for additional funding and a commitment to health equity, while engaging diverse perspectives. A three-stage review process ensures thorough evaluation of proposals.

The Community Impact Grant Level One: Engage aims to enhance health equity in Wisconsin by supporting community partnerships that address social determinants of health. Open to Wisconsin-based organizations with less than $1 million in annual expenses, grants of up to $250,000 are available for three years. The program fosters collaboration among community leaders and aims to evaluate the impact of initiatives addressing health inequities, ensuring sustainable improvements in health outcomes for diverse populations.

The Gridiron Veterans Scholarship, provided by the Bob Woodruff Foundation, offers essential financial support for military veterans and first responders. This initiative focuses on wellness, rehabilitation, and recovery programs, ensuring participants can access mental health services, physical therapy, and holistic resources. The foundation prioritizes those facing emotional and physical challenges, empowering them to overcome barriers and improve their quality of life through customized, evidence-based support.

The Community Impact Grant Level One: Engage supports community-academic partnerships aimed at addressing social determinants of health and advancing health equity in Wisconsin. Open to small to medium organizations, grants offer up to $250,000 over three years, promoting initiatives that tackle health inequities through authentic community leadership. Collaboration with academic partners, while encouraged, is not mandatory. The program emphasizes sustainability, evaluation, and a commitment to creating systemic change for affected populations.

The GIG Fund: Arts and Wellness offers grants ranging from $2,000 to $15,000 to support projects that blend arts and wellness. Open to non-profit organizations, hospitals, and clinics across the Midwest, this fund aims to enhance community well-being through artistic initiatives. Eligible applicants must engage professional artists and provide activities that promote health and creativity. This funding round supports activities occurring between December 1, 2025, and June 30, 2026, emphasizing the importance of arts in fostering wellness.

The federal Historic Preservation Fund (HPF) is managed by the National Park Service (NPS) and the U.S. Department of the Interior. The HPF is derived from federal offshore oil leases, not from tax revenues. The Catalog of Federal Assistance number is 15.904. In Wisconsin, subgrants from this fund are administered by the Wisconsin State Historic Preservation Office (SHPO).

The SHPO annually passes through 10% of our federal Historic Preservation Fund grant to Certified Local Governments (CLGs) through a competitive subgrant program. All proposed projects must assist Wisconsin historic resources in one of the following ways:

• Surveys of Communities or Neighborhoods
• Nominations of Historic Places to National Register
• Educational Projects about Historic Places
• Planning Documents
• Host CAMP Training

The Dr. Ralph and Marian Falk Medical Research Trust – Falk Catalyst Awards support high-risk, high-reward biomedical research with up to $350,000 in funding over one to two years. These awards aim to address critical scientific challenges and enable the translation of innovative research into clinical practices. Projects focus on identifying biological markers, therapeutic targets, and developing interventions to disrupt disease processes, ultimately improving treatment outcomes for patients.

The Gil Gabanski Student Scholarship, provided by the MGWA Foundation, offers multiple $2,000 awards for undergraduate and graduate students studying at accredited institutions in Minnesota and nearby areas. This scholarship aims to ease educational expenses and foster interest in groundwater-related careers, including hydrogeology and groundwater geology. It also promotes mentorship opportunities for early-career students, honoring Gil Gabanski's legacy in the geosciences community.

The UNICO Foundation offers the Dr. Benjamin Cottone Memorial Scholarship, providing $5,000 to support a U.S. citizen of Italian descent pursuing a medical degree. Eligible candidates must be seniors at accredited colleges or medical schools, with proof of enrollment required. This scholarship not only aids education but also honors the foundation's commitment to community service and heritage, reflecting the core values of the Italian American community.

The Midwest Culture Bearers Award honors and supports nine folk arts practitioners annually, providing each with a $5,000 unrestricted grant. This initiative aims to amplify the essential work of culture bearers across the Midwest, fostering opportunities for recognition and community connection. By prioritizing traditional practices and knowledge sharing, the award not only celebrates cultural heritage but also strengthens the creative roots of the region, ensuring vibrant artistic expression for future generations.
