RFP To Fund Transformative Projects Rooted in the Perception Box™ Framework

This RFP is designed for organizations creating meaningful shifts in how people interpret their experiences, beliefs, and perspectives.

All requests must be project-specific and aligned with the Perception Box framework.

At Unlikely Collaborators, we support people and organizations who are helping others see something about themselves they couldn’t see before. Our work is grounded in the Perception Box, a framework developed by our founder, Elizabeth R. Koch.

The Perception Box is the internal lens through which each of us experiences the world. It is shaped by our beliefs, our past experiences, our identity, and the stories we carry, often without realizing it.

Most of the time, we don’t see this lens. We experience our perspective as reality. But when people begin to notice it, something shifts. There is often more curiosity, less certainty, and more openness to others and new possibilities.

We are not a fit for projects focused primarily on advocacy, external blame, or telling people what to think. We support work that helps people examine how they think.

What this looks like in practice

We are most interested in projects where participants are actively engaged in noticing and questioning their own assumptions. This might include:

• Structured experiences where people reflect on their beliefs and how those beliefs were formed
• Encounters with perspectives that challenge their existing worldview
• Moments of realization where someone recognizes their interpretation may not be the only one
• Practices that build curiosity, self-awareness, and the ability to stay open in the face of difference

Before you get started on your RFP grant application, here’s what to expect:

• At Unlikely Collaborators, we support people and organizations that create meaningful shifts in how individuals see themselves.
• We are not looking for surface-level awareness. We are looking for work that leads to a real moment of recognition, when someone begins to question their own assumptions, reconsider their perspective, or see others differently.
• We prioritize leaders working from lived experience, not distance—people who understand the challenge from the inside and are translating that insight into something others can directly experience.
• The strongest projects are designed to create these moments of reflection in a tangible way. They bring people together in environments that are thoughtful, human, and often unexpected, where insight is felt, not just discussed.

Back in 1984, The Saginaw Community Foundation was founded to enhance the quality of life in our county by providing leadership, education, and support for philanthropy. Today, we still continue this mission of supporting the needs of our community through targeted grants, investments, and scholarships.

In addition to our grantmaking and scholarship programs, the Foundation also offers donor advised funds, which allow donors to recommend charitable grants to organizations of their choice. We also manage several endowed funds, which provide ongoing support for specific causes or organizations.

Saginaw Community Foundation is governed by a Board of Directors composed of community leaders and volunteers. A staff of professionals oversee our day-to-day operations and work with donors and community organizations to identify and address community needs.

CTE Scholarships

Dreaming of a career in welding? Maybe managing your own auto mechanics garage fuels your thoughts or you would love to become an electrician. Perhaps studying to become a chef in a culinary arts program suits your tastes.

No matter what your career path may be, there is little doubt that in recent years interest in Career and Technical Education (CTE) has grown. The increased need for workers skilled in the trades has resulted in larger numbers of students seeking out CTE programs.

Schools offering CTE encompass a wide variety of hands-on education, from HVAC to carpentry to cosmetology. In the past, financial assistance has been hard to come by for these areas of study, but that is no longer the case. Saginaw Community Foundation has several scholarship opportunities for those who have the desire to pursue CTE.

According to leading economists, not everyone needs a four-year degree. In fact, millions of good-paying jobs are opening up in the trades. And some pay better than what the average college graduate makes.

For aspiring welders, electricians, carpenters, and other valued skilled trade workers, there is great opportunity to take advantage of tailored CTE programs. The demand is massive as an estimated 3 million trade jobs in the United States remain vacant.

Our CTE scholarship awards range between $500 – $4,000 typically.

The Patricia and William Hendrick Memorial Scholarship

Long-time Saginaw residents Pat and Bill Hendrick were graduates of St. Mary’s High School in Saginaw. They supported Catholic education their entire lives at the primary, secondary, and higher education level. Pat and Bill believed in and helped further the cause of Catholic education with enthusiasm and financial support all their lives.

To honor Pat and Bill, their family and many friends established this scholarship to help Saginaw County residents who are going on to attend Catholic higher education. Special preference will be given to graduates of Nouvel Catholic Central High School and/or those attending the University of Notre Dame – two organizations which held a special place in their hearts.

Rural Health Transformation Program Funding Opportunity For Public Health Dental Hygiene Teams in Local Health Departments 

About the Program

This project is 100% funded by the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS. The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics.

The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP) — a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth.

Rooted in Health: Rural Dental Access

A key component of Kentucky’s Rooted in Health: Rural Dental Access initiative involves expansion of the PHDH teams that have been serving multiple counties in Kentucky since 2013. Through this grant opportunity, the Kentucky Department for Public Health (DPH) is soliciting applications from qualified organizations to establish new or expand current PHDH teams to expand access to rural dental care. The newly created programs must demonstrate commitment to providing services including:

• Preventive dental hygiene services for children and adults
• Dental hygiene-focused oral health services for residents of long-term care settings as defined in Kentucky’s Dental Practice Act KRS 216B
• Teledentistry-enabled preventive care, education, triage, and care coordination

Program Objectives

Funded PHDH programs must demonstrate their willingness and ability to:

• Increase access to school-based preventive dental hygiene services for all students, regardless of ability to pay.
• Deliver dental hygiene services in long term care facilities through teledentistry consultation with university dentists.
• Utilize teledentistry to expand access to care, improve quality and continuity of care, and enable more rural residents to stay in region for dental care and specialty consultations.
• Integrate Community Health Workers (CHWs) and/or social determinants of health (SDOH) screenings to support patient navigation and continuity of care.
• Achieve measurable improvements in oral health outcomes and service utilization.
• Operate in a cost-effective, sustainable, and compliant manner. Compliance is defined as adherence to the provision of hygiene services to the aboveidentified populations within KRS 313.

Department for Public Health (DPH) - Rural Health Transformation Program Funding Opportunity to Establish or Expand Community Paramedicine Programming

About the Program

The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics.

Funding is designed to support early program development and scaling. Community paramedicine programs are not expected to be fully sustainable during initial funding periods. Awards may vary depending on the proposed budget provided by the applicant and additional funding will be provided in subsequent years.

Program Objectives

Funded programs must demonstrate their willingness and ability to:

• Design and deliver community paramedicine services that address low-acuity, preventative, and high-utilization patient needs in Kentucky’s rural communities.
• Reduce unnecessary emergency department utilization and repeat visits after discharge by supporting community members where they are.
• Strengthen partnerships with rural service sites such as Local Health Departments, Federally Qualified Health Centers, rural clinics, and behavioral health partners, including Community Mental Health Centers (CMHCs).
• Connect community members to the right care before and after emergencies by repositioning EMS as a front-line connector to care.
• Operate in a cost-effective, sustainable, and compliant manner. Compliance is defined as adherence to KBEMS standards for EMS agencies.

Can-Amera Games Scholarship

The Can-Amera Games started in 1972 as an international goodwill sporting competition between Saginaw Township and Galt, Ontario. The games were centered on friendly competition in a variety of sports for youth ages 9-18. As the years progressed, the games expanded to include other townships in Saginaw County, as well as those communities, including Galt, that joined together to become the city of Cambridge, Ontario. The Can-Amera Games were the efforts of an all-volunteer organization that worked year-round to ensure that the games would be successful in meeting the purpose of this friendly international competition. Can-Amera Games ended in 2006 at the completion of the 35th annual event.

All past board members, volunteers, and athletes would like to honor a deserving high school senior who best represents the spirit of competition, sportsmanship, friendship and community involvement with the Can-Amera Scholarship as a way to symbolize the spirit and memory of the Can-Amera Games.

The Foundation for a Healthy Kentucky (FHKY) is a nonprofit organization funded by an endowment that aims to transform our state so that every Kentuckian has what they need to live a healthy life. Since it opened its doors in 2001, the Foundation has invested millions in health policy advocacy, research, and demonstration projects across the Commonwealth.

Partnerships for Success Grants

Partnerships for Success (PFS) provides funding to enhance and support Kentucky community-based organizations that serve communities with limited opportunities. Applicants should focus on reducing alcohol, nicotine (including vaping), and cannabis use/misuse and related consequences by implementing evidence-based prevention programs, practices, and policies that are trauma and resilience informed, and responsive to local and community context. The Foundation for a Healthy Kentucky (FHKY) is facilitating this funding opportunity as one of its pass-through grant programs. The project is supported by the Kentucky PFS 2023 Grant via the Substance Abuse and Mental Health Services Administration (SAMHSA) Grant H79SP083680.

Grants will provide up to $50,000 in funding for 12 months. 

Goal

To provide funding that enhances and supports evidence-based strategies to prevent alcohol, nicotine (including vaping), and cannabis use/misuse in communities that are at higher risk in Kentucky.

Overview

This funding opportunity allows potential grantees to creatively address localized behavioral health-related issues. Funding is intended to support solutions that address the issues, identified by those who live and work locally, as most relevant to communities that are at higher risk in Kentucky.

Proposed projects should:

• Focus on communities with limited opportunities as the primary focus of a proposed or existing project or program, including leadership and decision-making power.
• Make communities that are at higher risk the primary beneficiaries of the project or program's outcomes.
• Address a substance use challenge identified by the community.
• Plan, develop, implement, and/or sustain a prevention initiative.

Established in 1959 by the Oregon Legislature, OSAC provides innovative funding and programs to Oregon students and families.

OSAC Scholarship Program

The OSAC Scholarship Application provides current and future Oregon students one easy application to apply for multiple scholarships at one time. OSAC awards more than $10 million in scholarships annually through more than 600 scholarships.

2026-2027 Associates Scholars Empowerment Program

Daughters of Charity Inc., in partnership with UNCF, created the Associates Scholars Empowerment Program to assist students receiving certificates and two-year degrees.

The Associates Scholars Empowerment Program is a renewable need-based scholarship open to students receiving certificates and two-year degrees.

Students who do not meet the renewal criteria will be placed on probation and given a one semester grace period, while funded, to come into compliance.
