AHW’s Call for Applications: Community-Led Seed Grants supports community-driven efforts to design new strategies, test innovative ideas, and foster greater collaboration to build a foundation for future endeavors to address Wisconsin’s leading health challenges and produce deeper impact on health and health equity in Wisconsin’s communities. Eligible community-based organizations may request up to $50,000 for projects completed within a 12-month period, beginning Jan. 1, 2027.

To foster a collaborative approach to award selection, applicants will participate in a peer review process to assess proposals submitted to this funding opportunity by their fellow applicants. For the submitted application to be considered for funding, completion of all assigned reviews is required.

Following funding determinations, successful community-led applicant teams will be matched with an eligible MCW faculty member from a pre-formed pool developed through the Call for MCW Academic Partner for Community-Led Grants to serve as an additional resource to the funded community-led projects.

MCW Academic Partners for Community-Led Seed Grants

AHW is seeking eligible MCW faculty ready to serve as MCW academic partners with pre-defined key roles on funded Community-Led Seed Grants based on the community-identified need for the academic partner and the MCW faculty member’s interest and expertise. AHW’s Community-Led Seed Grants support community-led project teams to address community-identified and community-prioritized health needs through conducting initial exploration and testing innovative ideas to serve as a foundation for future sustainable efforts to improve health and health equity in Wisconsin’s communities. Community-Led Seed Grant applications are submitted independently by eligible community partners and projects approved for funding will be completed during a 12-month period, beginning Jan. 1, 2027, with an optional six-month no cost extension.

The MCW faculty should be able to commit 10% or 20% FTE at the time of funding to engage with one or two Community-Led Seed Grant projects.

Coordinating Care Across Wisconsin: Innovating Healthcare Through Partnership Grants

This five-year grant funding opportunity is intended to catalyze innovative healthcare service models through partnerships across communities, systems, and sectors in rural Wisconsin. Through these grants, the Wisconsin Department of Health Services (DHS) aims to address those points in a care continuum where community members are most likely to experience barriers to health-related services or fall out of care, and to directly address fragmented systems of care. Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.Application Phases

Applications for funding will be released in two distinct phases:

• Phase 1: Planning grant.
  • Applicants will submit a letter of application for an initial six-month planning or pilot grant.
  • The planning period will provide an opportunity for awardees to receive technical assistance for application development, build a strong foundation for program implementation, and develop strong partnership models.
  • We recognize that programs may be at different stages of development, from initial idea generation to ready-to-go projects and partnerships.

The Great Lakes Restoration Initiative offers up to $1,000,000 for the acquisition of private forest land in the Lake Superior Basin. Eligible applicants include federally recognized tribes, local governments, and nonprofits. The Minnesota Section PGA, which covers the areas of Minnesota, North Dakota, South Dakota and a small portion of Wisconsin, consists of over 100 Associate members annually. The Minnesota Section PGA is mandated not to discriminate according to federal and state statutes. The final selection process will be made by the Minnesota Section PGA Board of Directors. No announcements will be made before the applicants receive notification from the Minnesota Section PGA. If additional funds become available, the Board may grant additional scholarships to worthy applicants.

The Buy Local, Buy Wisconsin Grants program supports the state's agricultural and food industries by addressing marketing and distribution challenges. Administered by the Wisconsin Department of Agriculture, Trade and Consumer Protection, the program provides up to $50,000 per project with a 50% matching requirement. PLTW Engineering - New Program In Partnership With Ardagh Group

Ardagh Group has partnered with PLTW to support education in its local communities where Ardagh employees live and work. This partnership recognizes the critical role education, and access to education, plays in ensuring equal opportunities and in building successful, vibrant communities. It is expected that over the next 10 years, Ardagh sponsored education programs will benefit more than 500,000 PreK-12 students as well as delivering best-in-class teacher training to more than 5,000 teachers across 2,000 schools in Ardagh's U.S. communities.

Program Requirements

The purpose of this grant is to assist the grantee's implementation of the PLTW Engineering program. Grantee must offer the PLTW Engineering program for three academic years. Grantee must offer at least one PLTW Engineering course in the 2027-2028 academic year and must offer one additional course in the 2028-2029 academic year (two different courses in two years). Grantee may implement any PLTW Engineering courses of their choosing. Grantee must complete three years of grant reporting.

The Angler Recruitment, Retention, and Reactivation Grant Program, managed by the Wisconsin Department of Natural Resources, aims to increase angling participation among diverse adult audiences in Wisconsin. This program provides financial support to eligible organizations for activities that foster a safe and ethical fishing experience. By promoting community engagement and education, the grant seeks to enhance the number of active anglers, ensuring sustainable fishing practices and preserving Wisconsin's natural resources.

Museums, other private nonprofits, and local government entities in rural Wisconsin can seek funding to build or upgrade water, sewer, solid waste, and stormwater systems serving their facilities and communities. Grants from USDA: Rural Development support acquisition, construction, and related costs for clean drinking water and waste disposal in eligible rural areas.

The Albrecht Memorial Scholarship, provided by The Community Foundation of Southern Wisconsin, supports students with disabilities from five local high schools in Rock County to pursue post-secondary education at UW-Whitewater. Applicants must demonstrate a minimum GPA of 2.5 and provide proof of disability. This initiative reflects the foundation's commitment to enhancing educational opportunities and matching personal philanthropy with community needs, ensuring a lasting impact in Southern Wisconsin.

The Curt Marzolf Memorial Scholarship, managed by The Community Foundation of Southern Wisconsin, offers financial assistance to graduating seniors from Monroe, Argyle, Juda, or South Wayne high schools pursuing engineering studies at accredited colleges. This scholarship aims to support students who maintain a minimum GPA of 3.0 and encourages applicants with interests in hunting, fishing, or related fields, although this is not a requirement. It reflects a commitment to enhancing educational opportunities in Southern Wisconsin.

The Doug & Janice Steinback Family Scholarship is offered by the Community Foundation of Southern Wisconsin to assist students from Southwest Wisconsin pursuing degrees in agriculture, life sciences, biology, or chemistry.
