Coordinating Care Across Wisconsin: Innovating Healthcare Through Partnership Grants

This five-year grant funding opportunity is intended to catalyze innovative healthcare service models through partnerships across communities, systems, and sectors in rural Wisconsin. Through these grants, the Wisconsin Department of Health Services (DHS) aims to address those points in a care continuum where community members are most likely to experience barriers to health-related services or fall out of care, and to directly address fragmented systems of care. Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.Application Phases

Applications for funding will be released in two distinct phases:

• Phase 1: Planning grant.
  • Applicants will submit a letter of application for an initial six-month planning or pilot grant.
  • The planning period will provide an opportunity for awardees to receive technical assistance for application development, build a strong foundation for program implementation, and develop strong partnership models.
  • We recognize that programs may be at different stages of development, from initial idea generation to ready-to-go projects and partnerships.
  • We welcome a range of planning period activities.
• Phase 2: Full award.
  • Planning grant awardees will be eligible to submit an application for a full award covering the remaining four-year grant period.
  • The full application will be released in February 2027.
  •  Only recipients of an approved letter of application will be eligible to apply for the full award.
  • Full award applications should reflect and build on the work done during the planning period and provide more in-depth program designs.

Workforce Innovation Grant: Healthcare Employment, Access, and Rural Transformation (WIG:HEART)The Workforce Innovation Grant: Healthcare Employment, Access, and Rural Transformation (WIG:HEART) grant program funds competitive grants for innovative projects that develop transformational solutions to workforce challenges that impact the quality and availability of healthcare in Wisconsin's rural regions and communities across Wisconsin. Grants support healthcare industry employers in finding workers and help workers prepare for and connect to better, higher-quality, and more family-sustaining jobs.

The four-year program is scheduled to run from Oct. 30, 2026 to Sept. 30, 2030. The current application is for Year 1 of the grant, which will run from Oct. 30, 2026 to Sept. 30, 2027.

The Joseph Thomas Wyss Hockey Scholarship, offered by The Community Foundation of Southern Wisconsin, supports graduating seniors from select high schools who are active participants in hockey. Eligible students must plan to attend an accredited college or university and have been involved in the Monroe Avalanche Hockey or Monroe Youth Hockey programs. The Stebnitz Builders, Inc. Scholarship, managed by the Community Foundation of Southern Wisconsin, supports graduating seniors from Walworth County high schools pursuing trade-related fields. Casey’s Donation Requests support vital community needs in Education, Hunger, and Community Servants. Eligible 501(c)3 organizations within Casey’s communities can apply through the online Donation Request Form. Requests are reviewed continuously, with feedback provided via email within one month. Priority is given to organizations that haven't received donations in the current giving calendar year, which runs from May 1 to April 30.

The Clean Sweep Grants program, managed by the Wisconsin Department of Agriculture, Trade, and Consumer Protection, provides funding to municipalities and Tribes for the safe disposal of household hazardous waste, agricultural pesticides, and prescription drugs. Key deadlines include May 1 for eligibility inquiries, September 15 for pre-applications, and November 15 for full proposals. Midwest Nursing Research Society (MNRS)

The MNRS originally began to provide a place for nurses to collaborate, and collaborate they did! Over time, the founding members began to realize the benefits of having an organization dedicated specifically to nursing research. With some careful attention and a lot of hard work, we have grown into the organization it is today. We're a society where scientists, innovators, and drivers of clinical practice across the career trajectory, come together to share ideas and grow our field through innovative research.

The mission of the MNRS is to generate knowledge, translate evidence into practice, and enhance careers through collaborative engagement, implementation, and dissemination among diverse scholars. Our Vision to be the premier society that advances science, fosters leadership, and drives dissemination to improve health equity for all people.

It is an honor to recognize the important contributions of our nursing colleagues. MNRS accepts nominations for our five very prestigious awards to selected members of the Society who have made significant and notable contributions to the field of nursing through research. 

Distinguished Contribution Award

The purpose of the Distinguished Contribution Award is to recognize the scientific contribution of a member who has conducted or promoted research, which has enhanced the science and practice of nursing in the MNRS region.

HOLES FORE HOPE/Minnesota PGA Associate Scholarship Program

The HOLES FORE HOPE/Minnesota PGA Associate Scholarship program is designed to award scholarships to Associates of the Minnesota PGA Section. Holes Fore Hope has committed funds to help Section Associates offset the cost of the PGA of America Associate Program. The amount of the scholarship(s) is/are determined by the availability of funds. Payments will be made to the Associate upon completion of the Level they are completing. This year, Holes Fore Hope and the Assistants' Chapter Board will oversee both the application review and award selection process.

The Minnesota Section PGA, which covers the areas of Minnesota, North Dakota, South Dakota and a small portion of Wisconsin, consists of over 100 Associate members annually. The Minnesota Section PGA is mandated not to discriminate according to federal and state statutes. The final selection process will be made by the Minnesota Section PGA Board of Directors. No announcements will be made before the applicants receive notification from the Minnesota Section PGA. If additional funds become available, the Board may grant additional scholarships to worthy applicants.

Expanded Wisconsin Fast Forward Grants

The objective of the Wisconsin Fast Forward (WFF) expanded grant program is to award funds that reimburse the costs of workforce development activities, as specified in the Grant Program Announcement (GPA).

Teacher Training and Recruitment Grants

This expanded WFF grant program is to recruit and prepare individuals to teach in low-income or urban school districts in Wisconsin as a profession.

Nature's Path's Gardens for Good program empowers communities by funding organic gardens that address food insecurity. Since 2010, it has donated $930,000 CAD to support 120 gardens. In 2023, grants of $6,500 each will be awarded to 20 gardens across the U.S. and Canada. Eligible applicants must be registered non-profits with urban organic agriculture projects that serve low-income communities, promoting sustainable practices and inclusivity.

The NRA Foundation: Wisconsin Grants provide funding for charitable purposes aligned with the NRA Foundation's mission, including promoting firearms safety, educating youth about firearms, and supporting competitive shooting sports. Eligible applicants must be IRS 501(c)(3) recognized entities or government bodies, and must submit necessary documentation. Ineligible organizations include labor groups and private businesses. This initiative aims to foster educational and safety programs while ensuring compliance with IRS regulations.
