Grants Or Sponsorships

To apply for a grant or sponsorship from Carnival Foundation, prospective applicants must complete an online application and include the following information and documents:

• A letter of request with a brief description of the organization, including its legal name, history, activities, purpose and governing board
• Purpose for which the grant is requested
• Grant amount requested
• Listing of other sources of financial support
• Operating budget of the project for which the funds are requested
• Organization’s annual budget
• Copy of the organization’s most recent audited financial statement
• Copy of organization’s most recent 990 filed with the Internal Revenue Service
• Copy of Internal Revenue Service determination letter indicating 501© (3) tax-exempt status as an organization that is not a private foundation
• Organization’s completed W-9 form
• Geographical area(s) served
• Organizations applying for a grant renewal must submit a final report detailing use of the prior grant award. Interim and final reports will be submitted through our online portal.

The Excellence in Medicine Awards, presented by the AMA Foundation, honor exemplary physicians and medical advocates committed to volunteerism, community engagement, and leadership. Since 2005, this prestigious program has celebrated over 100 honorees, recognizing their vital contributions to underserved populations. Awardees receive a $2,500 grant honorarium to support a nonprofit health organization of their choice, collectively awarding over $250,000 to enhance community health and promote equitable care across the nation.

Executive Summary

The New Jersey Department of Health (NJDOH), Division of Health Promotion – Community Health and Wellness Unit (CHWU) addresses leading causes of death and disability from chronic disease and illness. The core mission of the CHWU is to prevent premature death and disability from chronic disease, to understand the risk factors of chronic disease, and to promote healthy behaviors through leadership efforts that address evidence-based policies, systems, and environmental change. To achieve this mission, the CHWU has cultivated a robust group of partners – clinicians, academicians, community advocates, patients, consumer representatives, business leaders and government agencies – across the state of New Jersey to reach high burden populations and communities and complete funded work. Improved quality of life and extended years of healthy living for New Jersey residents are the overall outcomes for all activities undertaken through CHWU programs and services.

The Community Health and Wellness Unit’s – Colorectal Cancer Screening and Prevention Program announces the availability of State Fiscal Year (SFY) 2026 funding and technical assistance for primary care clinics operating in Cumberland, Cape May, Gloucester, Monmouth, Hunterdon, Somerset and Essex counties whose colorectal cancer (CRC) screening prevalence is between 25% and 55%, to implement evidence-based interventions (EBIs) and quality improvement (QI) processes to increase CRC screening in eligible and priority populations. This RFA seeks to increase CRC screening among populations aged 45 to 75 years that have lower colorectal cancer screening prevalence. The NJDOH-CCSPP is seeking up to four (4) primary care clinics to adopt strategies that utilize multi-component EBIs to increase CRC screening and to partner with NJDOH-CCSPP by accepting technical assistance, and support for data collection and evaluation activities.

Funded entities must demonstrate capacity and readiness to implement the program by completing the following:

• Engage in a formal readiness assessment of funded entities’ capacity to implement EBIs. This assessment will be used to select EBIs that will support improved CRC screening.
• Implement multicomponent interventions that combine a minimum of three (3) evidence-based interventions (EBI), as recommended in The Community Guide. These EBIs must focus on different areas of the clinical system (which will increase demand, access, and delivery of screening recommendations). The EBIs of priority for this work includes patient reminders, provider reminders and feedback, reducing structural barriers, and patient navigation.
• Provide high-quality, clinic-level data, including baseline and annual CRC screening prevalence, aggregate data on stool-based tests provided to and returned by patients, and aggregate data on follow-up colonoscopies. Emphasis will be on the capacity to collect data and track the entire CRC screening process patients undergo.
• Support completion of stool tests and colonoscopies, including tracking and follow-up of colonoscopies for positive stool tests

In addition to funding and technical assistance, each awardee will have the opportunity to utilize a free patient navigation service supported by NJDOH. The navigators will be deployed to the awardees primary care clinics site and will provide direct patient referrals and outreach via electronic health records (EHR). Leveraging innovative tools, including a patient chat text concierge, the patient navigators will enhance patient engagement, reduce barriers, and empower informed decision-making. Navigators will also coordinate care, track outcomes, schedule screenings, and ensure timely follow-up to promote comprehensive, equitable colorectal cancer prevention and care.

Breast Cancer Screening & Prevention Initiative

The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. By enhancing early detection and preventive care, the initiative seeks to reduce barriers to care and improve health outcomes for women who might otherwise go without critical cancer screening services.

Food as Medicine Initiative

The Food as Medicine Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to address food insecurity as a critical driver of health outcomes for uninsured and medically underserved patients. Research continues to demonstrate the strong connection between food insecurity and poor health, including increased risk of chronic conditions such as diabetes, hypertension, obesity, and certain cancers. Through this initiative, NAFC will provide $20,000 grants to member free and charitable clinics to implement or expand food insecurity screening and patient-centered interventions that improve access to healthy food and nutrition education as measured by the US Household Food Security Module or Hunger Vital Signs and patient education focused on food as medicine for eligible patients in order to increase the percentage of patients with improved food security scores within one year of project implementation.

Medication Therapy Management (MTM) Initiative

The Medication Therapy Management (MTM) Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to improve medication access, adherence, and patient understanding among uninsured and medically underserved populations. Individuals without health insurance often face significant barriers to obtaining prescription medications and receiving the counseling necessary to manage chronic conditions effectively. Through this initiative, NAFC will provide $30,000 grants to member free and charitable clinics and pharmacies to establish or expand Medication Therapy Management programs aligned with recommendations from the American Pharmacists Association. Funding will support activities such as staff certification in MTM, development of standardized protocols, and patient education to help individuals better understand their medications, manage side effects, and support overall treatment plans. By strengthening pharmacist-led care and improving communication between providers and patients, this initiative seeks to enhance medication adherence, improve chronic disease management, and advance better health outcomes for vulnerable patient populations.

The Menasha Corporation Foundation, established in 1953, supports nonprofit organizations across North America, focusing on community betterment, education, environmental sustainability, and health initiatives. Grants are awarded to projects that enhance the lives of individuals and strengthen communities, particularly in areas where Menasha Corporation operates.
