MH253024 - Aging in Place Pilot Western New York Region

The Office of Mental Health announces enhanced annual operating funding and up to $2 million in capital funding to develop one (1) Pilot Program for aging adults residing in the CR-SRO level of care.

The funding is for programs in OMH's Western New York Region.

The New York State Office of Mental Health (OMH) announces the availability of enhanced annual operating funding and up to $2 million in capital funding to develop one (1) Pilot Program for adults who are aging and are currently, or could be with enhanced supports, residing in the CR-SRO level of care. This pilot program will be developed within an existing CR-SRO program already operated by the applicant. The expectation is for an agency to set aside 20-25 existing units within the CR-SRO program to support long-term care services for older adults and medically complex individuals. Agencies with multiple CR-SROs within 30 minutes travel time of each other may propose to utilize these enhanced services across multiple programs.

Capital funds up to $2 million for minor renovations and/or environmental modifications will also be available. It is OMH’s expectation that this pilot program will prioritize 20 to 25 older adults that require enhanced supports to avoid the need for transfer to a higher level of care such as hospitalization, assisted living residences, and/or nursing homes.

OMH intends to fund one (1) existing CR-SRO program, in OMH’s Western New York region, that can demonstrate an ability to provide appropriate need-based housing services for those aging in the community. Applicants also need to demonstrate their ability to undertake minor renovations and/or environmental modifications to eliminate physical barriers impeding aging and medically complex residents and allow them to remain in their current setting. The capital award will not exceed $2 million dollars. Project scope and budget will be developed by the selected applicant in conjunction with the OMH Bureau of Housing Development and Support after further analysis of the project. Capital and enhanced operating funding will be subject to approval by the New York State Division of the Budget (DOB).

This renewable scholarship is available to students who will or have graduate from a high school located in Chapin, South Carolina. 

Elias Eade Jr. Scholarship

This scholarship was created in honor and memory of Elias Eade Jr. by his children. Elias Eade Jr. was a lifetime resident of Olean who faithfully served God, his country, his family and the Olean community. Mr. Eade was a graduate of Olean High School and St. Bonaventure University and a member of the Olean business community for many years. Mr. Eade was an entrepreneur, leader and a man of great strength and integrity with an unprecedented work ethic. Candidates must be a graduating senior of a high school in Cattaraugus County with good academic standing and leadership skills and who plan to continue their education at a trade school or 2- or 4-year college or university. Candidates must answer an essay prompt of their choice from the three prompts provided by the fund advisors.

Jean M. Brown Nursing Scholarship Fund 

Jean M. Brown graduated from the St. Francis School of Nursing in 1938. She went on to become Caterpillar’s first Industrial Nurse, a role she deeply valued. Inspired by her career, she wanted to help others pursue nursing as well.

Through her will, Ms. Brown established an endowed scholarship to support nursing students with financial need, with preference given to those pursuing a career in Occupational Nursing. The scholarship fund was created following her passing in October 2000.

The Joanie Bostic Memorial Senior Pirate Spirit Award, established by the Decatur County Community Foundation, honors Joanie Bostic's passion for youth and Greensburg athletics. This scholarship recognizes a senior who exemplifies outstanding school spirit through participation in or support of athletics or music. Eligible candidates must be graduating seniors from Greensburg Community High School and submit a statement detailing their school experience and encouragement of Pirate spirit.

The Health and Health Care Disparities grant aims to support innovative research that addresses the health challenges faced by individuals with disabilities, particularly in communities experiencing health disparities. Key application deadlines span from February 2024 to October 2026. This funding opportunity invites research proposals that explore the underlying factors affecting the health and well-being of these populations, promoting interventions that can lead to significant improvements in health equity.

The Tommy Aaron/Charlie Aaron Foundation Scholarship, managed by the North Georgia Community Foundation, supports graduating seniors from Hall County or Gainesville City public schools and Lakeview Academy. This $10,000 scholarship, distributed over four years, promotes academic achievement and integrity through golf. Eligible applicants must have a minimum GPA of 2.5, demonstrate financial need, and exhibit strong character traits. This initiative aims to empower local students to pursue higher education while honoring the values of sportsmanship and respect.

The award assists graduating high school seniors and current undergraduate students from Dutchess and Ulster Counties who have overcome significant personal obstacles and can demonstrate financial need. Preference is given to applicants who have pursued their education without a strong support system, alongside consideration of academic achievement. One scholarship of at least $1,000 is awarded annually, and while it is not renewable, recipients may reapply in subsequent years.

Eligible applicants must be employees of Adams Fairacre Farms, Inc. or members of an employee’s immediate family and maintain a B average or better. The scholarship awards one non-renewable, one-time payment of $2,000, payable directly to the recipient’s school, to support postsecondary education at an accredited institution.

Established in 2024 by the Carroll family, this scholarship honors Rich Carroll’s lifelong commitment to public service and his belief in giving back through the fire service profession. The award supports graduating high school seniors from Dutchess County who plan to pursue postsecondary studies in fire science, emergency response, EMT, or paramedicine. The scholarship encourages students entering these critical service fields to follow a path of helping others and strengthening their communities. One non-renewable scholarship of $2,500 is awarded annually.

Leslie C. and Irene G. Roe Athletic Scholarship

The Leslie C. and Irene G. Roe Athletic Scholarship provides multi-year financial support to graduating high school seniors from Dutchess County who have demonstrated outstanding athletic skills and plan to attend a New York State community college or public college/university, including State University of New York or City University of New York institutions. The scholarship offers a $1,500 award, with the number of recipients varying each year, and is renewable for up to three additional years. Continued funding requires maintaining a minimum C+ GPA and submitting a college transcript and fall tuition bill annually. Awards are payable directly to the student’s school.

The Marie Darlin Prize, administered by the Juneau-Douglas City Museum and overseen by the Juneau Community Foundation, honors exceptional artistic and scholarly works that reflect Southeast Alaska's cultural heritage. Awarded annually, the prize of $5,000 recognizes individuals or collaborations committed to the region's social and cultural history, community advocacy, and Indigenous identity. The scholarship recognizes students who have demonstrated involvement and leadership in community service activities and show the ability to succeed in their chosen course of study. One non-renewable, one-time award of $1,000 is provided and is payable directly to the student’s school.

Established by the family of Maynard and Ferne Brownell, this scholarship supports one graduating high school senior from Beacon High School, John Jay High School, or Roy C. Ketcham High School who plans to attend an accredited college or university. The scholarship provides a $750 award applied to tuition and is renewable for up to four years, contingent upon maintaining a full academic schedule and a B- (or equivalent) GPA, with annual submission of transcripts and tuition bills. Selection is based on academic strength, participation in sports and/or community service, and demonstrated financial need.

Multiple one-time scholarships are available each year, with award amounts ranging from $2,000 to $5,000, payable directly to the student’s school.

Award amounts can reach up to $2,500, aimed at removing barriers to educational pursuits and enhancing overall community wellness.

The Patrick Shinners Memorial Team Player Scholarship, managed by the Cattaraugus Region Community Foundation, provides financial support to student-athletes in Cattaraugus and Chautauqua counties who exemplify teamwork and dedication in football. Established in memory of Patrick Shinners, this scholarship honors those who contribute positively to their teams, regardless of individual accolades. Eligible applicants must be graduating seniors with a commitment to community involvement and team spirit, encouraging all athletes, including those entering the Armed Forces or technical trades.

Rainin Visionary Scientist Award 

The Kenneth Rainin Foundation believes that early support for innovative ideas can lead to improvements in preventing, predicting, diagnosing and treating Inflammatory Bowel Disease (IBD). Our Health grantmaking enables researchers worldwide to test ideas, gather and share data, and collaborate to advance the understanding of IBD.

The Rainin Visionary Scientist Award will provide grants of $300,000 each over two years to support three outstanding postdoctoral investigators whose work has the potential to advance the understanding of IBD and related biological systems. This award is designed to help postdocs based in the US who are actively preparing to transition to an independent faculty position develop innovative research programs and strengthen their trajectory toward independent scientific careers. By supporting promising scientists early in their careers, the Foundation seeks to accelerate discoveries that may ultimately improve the lives of people living with Crohn’s disease and ulcerative colitis.

Competitive proposals will demonstrate:

• A novel or creative scientific approach
• Strong potential to advance understanding of IBD biology or treatment
• Clear intellectual leadership from the applicant
• Feasible research aims that can be accomplished within two years
• A strong mentorship environment that supports career development

The Foundation particularly values projects that introduce new ideas, technologies or conceptual approaches to the study of IBD and relevant scientific disciplines.

Funds may be used for postdoctoral salary support, supplies, other direct research expenses necessary to complete the proposed work and travel to relevant scientific meetings. Awardees are expected to attend the Rainin Foundation Innovations Symposium during the award period. Economy/coach air travel costs for this meeting in San Francisco should be included in the budget.

The award provides $300,000 over two years. The grant period is October 1, 2026-September 30, 2028.

The We Raise Foundation's Emerging Leader Grant supports individuals aged 20-35 at Christian organizations developing innovative programs addressing poverty, violence, and inequality. With a total of $15,000 over two years, the grant allocates $10,000 for program support and $5,000 for leadership development, including coaching and conferences. This initiative aims to foster impactful leaders who inspire, communicate effectively, and engage deeply with their communities, ultimately creating lasting change.

Julie Herrick Easterly Memorial Scholarship

This scholarship has been made possible by the generosity of the family and friends of Julie Herrick-Easterly, who established the Julie HerrickEasterly Memorial Scholarship of the Community Foundation for Muskegon County. Julie Herrick-Easterly, an only child and part of an established family tradition of journalism, worked as a copy editor for various newspapers across the country. Her father, Robert Herrick, was editor of the Muskegon Chronicle for 17 years. At age 34, Julie died from a terminal illness. Following Julie's death, her parents established this scholarship in her memory. Awards from the scholarship are given to a Muskegon area graduating senior who plans to pursue a career in journalism. The award represents an acknowledgment of the student's past achievements in this area of study. To the credit of the Herricks, Julie's love for journalism will always be remembered, and the family tradition will be perpetuated through the lasting gift of education.
