Grant Overview

The Land, Health, Community-Chicago (LHC-C) program has recently expanded its focus areas to reflect our commitment, as a Foundation, to supporting holistically healthy communities in the Chicagoland area. We believe that a community’s health is rooted in its ability to access and obtain life sustaining resources for all its residents without undue burden, systemic barriers, and inequitable opportunity. The Land, Health, Community-Chicago Program has one grant cycle in the fall.

Program Focus

We look to fund projects and organizations that support the following:

• Our communities are physically active, values healthy eating, and prioritizes overall wellbeing.
• All communities can thrive. Communities that have faced and continue to face inequitable and unjust policies, laws and practices will be equipped with the resources needed to realize their full potential.

Funding Priorities

• Opportunities for children in/out of school to engage physically with and to learn about the natural world, to eat well, and to care for the planet.
•  A community and/or multigenerational approach to addressing the mental health and wellness of youth.
• Efforts to increase access to healthy and sustainably produced food.
• Grassroots and community led initiatives centered on the pursuit of food justice.

Grants are awarded annually through a competitive process, focusing on areas like arts, education, health, and human services. The foundation aims to make a lasting impact by strategically investing in initiatives that benefit local residents.

Enterprise Community Partners

Enterprise Community Partners is a national nonprofit that exists to make a good home possible for the millions of families without one. Home is where life happens, where plans are made, and futures begin. It is the foundation for dignity, health, education, wealth, and community. Yet rents keep going up, paychecks don’t keep pace, and good homes in strong neighborhoods are increasingly out of reach.

The system doesn’t work. It must be changed, and it must be changed by us.

Enterprise has the breadth, scale, and expertise to do it. We support community development organizations on the ground. We aggregate and invest billions to improve housing and strengthen communities across the U.S. We advance housing policy at every level of government. We build and manage communities ourselves. National Housing Innovation Grant Competition

Home is foundational. It’s where we plant roots, raise and care for our families, and build community bonds. Yet in every corner of the country, millions of people of all ages and backgrounds need a home they can afford.

Wells Fargo is meeting this moment with a powerful grant opportunity. Together with Enterprise, Wells Fargo has launched the third iteration of the Housing Affordability Breakthrough Challenge. The 2026 cycle of the housing innovation competition will identify and propel proven, ready-to-scale solutions that transform current practices and increase housing choice and access.

Eligible applicants will compete for five individual grants of $2 million to advance their innovation and drive meaningful, systems-level change in the housing and adjacent industries. Winners will gain access to mentorship and coaching from industry leaders and experts and join a powerful network of Breakthrough Challenge innovators.

Focus Areas

This third cycle of the Housing Affordability Breakthrough Challenge aims to meet the nation’s affordable housing challenges across all types of communities: Native, rural, suburban, tribal, and urban.

Proposals must encompass one or more of three focus areas:

Innovations must serve residents at these income levels:

• Rental: 80% AMI or below
• Homeownership: 120% AMI or below

The Gladys Brooks Foundation supports innovative projects in libraries, education, hospitals, and clinics. Grants focus on resource endowments, capital construction, and equipment aimed at improving public access to information and educational resources. Established in 1986, it prioritizes organizations with local offices and typically funds grants for one year. Applicants are limited to one submission per calendar year to this and similar foundations, ensuring fair distribution of resources.

The Special Programs for the Aging grant provides funding to enhance the quality of life for older adults in Illinois. Administered by the Illinois Department on Aging, it supports services that promote independence, such as transportation, in-home care, and community services. The program also funds multi-purpose senior centers that offer meals, health screenings, and educational activities, ensuring seniors have access to essential resources and support to remain in their homes and communities.

The Universal Leaf Foundation Grants support 501(c)(3) nonprofit organizations primarily in areas where Universal Leaf Tobacco Company operates. Focus areas include education for at-risk individuals, higher education, civic/community arts, and environmental initiatives. The Foundation emphasizes good corporate citizenship and community engagement through financial support and employee volunteerism. Organizations must operate within designated geographic areas and cannot be involved in political advocacy or restrict services based on demographics. Grants aim to enhance community well-being and educational opportunities.

The Birth Equity Innovation Seed Grants in Illinois aim to support community organizations working to reduce disparities in maternal and infant health. Funding will be available for programs tied to communities most affected by these inequities, facilitating access to essential maternal care services. The Illinois Department of Public Health encourages innovative approaches that establish or maintain programs focused on improving birth equity, particularly for marginalized populations. Grants range from $0 to $1,000,000.

The Illinois Healthy Resilient Communities initiative, supported by the Illinois Department of Public Health, aims to enhance health equity through funding partnerships that target social determinants of health. This Notice of Funding Opportunity (NOFO) invites local health departments and lead organizations to apply for either Capacity Building or Implementation grants. Successful applicants will develop and execute programs that address health disparities and improve community resilience, fostering a healthier Illinois.

The Delta Dental of Illinois Foundation's Community Grants Program aims to enhance oral health among Illinois residents, particularly children. With over $5 million awarded over the past decade, the foundation prioritizes programs focusing on oral health education, access to care, and workforce development. Grants range from $1,000 to $20,000, and organizations can reapply yearly. Eligible applicants must be Illinois-based 501(c)(3) charities or governmental entities, addressing specific community needs in oral health.
