The Utah Department of Health and Human Services will support and serve all individuals and communities throughout Utah. It will do so through effective policy and the operations of an effective and efficient seamless system of services and programs that is centered around the individuals and communities the department serves.

RHTP Evaluation Contract

The purpose of this RFSQ is to obtain statements of qualifications from companies to provide monitoring and evaluation services for the Utah Rural Health Transformation Program (RHTP). 

The RHTP is a five-year federally funded initiative from the Centers for Medicare & Medicaid Services (CMS) to strengthen rural health systems. Utah’s RHTP framework is built upon four strategic pillars: Making Rural Utahns Healthy, Workforce Development, Innovation and Access, and Technology Innovation. These pillars are operationalized through 24 key activities divided into seven integrated initiatives. These initiatives and activities aim to transform rural healthcare delivery, strengthen patient-centered care, and drive measurable improvements in health, coordination, and financial sustainability across rural Utah. A network of partners including other State Agencies, community groups, private sector entities, and academia will implement the program in 25 of Utah’s 29 counties (excluding Salt Lake, Utah, Davis and Weber). 

The RHTP Monitoring and Evaluation Contract will provide critical analytical support, evaluating whether the program achieved the intended outcomes, assessing the impact of the seven initiatives, evaluating innovative pilot activities, and monitoring performance to drive course corrections to maximize impact, and prevent ongoing financial obligations for the State of Utah.

The intent of this RFSQ is to award a contract under Utah Procurement Code 63G-6a Part 15 for the purchase of Design Professional Service Providers and Consultants. The vendor with the highest- ranking will be selected to negotiate a contract at compensation determined to be fair and reasonable. If fair and reasonable compensation, contract requirements, and contract documents cannot be agreed upon with the highest ranked vendor, the state may proceed with negotiations with the next highest ranked vendor. Do NOT include pricing when you submit your qualifications. 

Nonprofit and public entities supporting rural economic development projects in Sanpete County, Utah can apply for the Rural Communities Opportunity Grant, which offers up to US $600,000 for business recruitment, expansion, workforce training, and related infrastructure. Utah Governor's Office of Economic Opportunity administers this program for eligible rural communities across the state.

Nonprofit and 501(c)(3) charitable organizations improving basic needs like hunger, health, or homelessness in Sanpete County, Utah can apply for Utah Community Credit Union’s donation and sponsorship support. Funding may also be available to other eligible counties in Utah, with for-profit sponsorships considered based on marketing and advertising benefits to the credit union.

Sanpete County, Utah county governments can access funding through the Utah Governor's Office of Economic Opportunity to support rural economic development projects. Grants up to Up to US $200,000 annually may fund business recruitment and expansion, workforce training, and infrastructure or capital improvements that strengthen local economies in eligible rural Utah counties.

Nonprofit 501(c)(3) organizations, educational institutions, youth groups, and similar charities in Sanpete County, Utah can seek funding for education and STEM-focused programs, including K–12, higher education, literacy, workforce development, and youth development. PacifiCorp/Pacific Power/Rocky Mountain Power Foundation offers up to US $10,000 for projects serving communities in its service area.

Nonprofits, schools, local governments, and tribal entities in Sanpete County, Utah can seek US $1,000 - US $10,000 for community, cultural, educational, environmental, health, and youth programs. BNSF Foundation provides support to organizations near its rail lines, including projects in STEM education, homelessness and hunger, public safety, and conservation.

Nonprofit 501(c)(3) arts and cultural organizations in Sanpete County, Utah can seek funding for festivals, museums, music, theater, visual arts, and cultural heritage projects. With a mission to support community vitality, Pacific Power Foundation provides grants up to US $10,000 for programs benefiting communities in its Rocky Mountain Power service area.

Health, safety, and wellness 501(c)(3) nonprofits and qualifying public agencies in Sanpete County, Utah can seek PacifiCorp Foundation grants to support health and human services programs. Funding up to Up to US $10,000 can assist initiatives such as addiction services, domestic violence programs, first responders, mental health, food insecurity, and other community wellness efforts within the Pacific Power and Rocky Mountain Power service area.

Sanpete County nonprofit organizations and public entities working on community enhancement or environmental stewardship projects can access funding through PacifiCorp Foundation, with awards up to US $10,000. Eligible projects include affordable housing, community centers, parks and trails, conservation, waste reduction, and water resource management in areas served by Pacific Power and Rocky Mountain Power.

Candidates for the Gordon Lymbery, PhD Memorial Scholarship must have graduated from a Northern Nevada high school, or received a GED, and must be seeking an Associate’s, Bachelor’s or Master’s degree.

Candidates for the Gordon Lymbery, PhD Memorial Scholarship must have graduated from a Northern Nevada high school, or received a GED, and must be seeking an Associate’s degree, Bachelor’s degree, or Master’s degree. Awards for up to $1,000 are available for graduates of any Northern Nevada high school. Please see the Lymbery Certification Scholarship if you are seeking a certificate. This is a one-time award but has the potential to be renewed with reapplication.

Edward Arciniega Scholarship

For graduating seniors from Churchill County High School, Charter School, or from a Churchill County Home-school program, intending to study at a 2-4 year college, having studied a foreign language for at least 4 years. This is a single year award for students who have at least four years of study in a language other than English in grades 6-12, and who have demonstrated an aptitude for language and academic accomplishment. This is a one-time award of up to $1,000.

Cynthia Chapman Memorial Scholarship

Cynthia Chapman was a renowned pianist who was dedicated to the pursuit of science and education. She devoted much of her life developing educational videos for Encyclopedia Britannica. This scholarship is in honor of her exceptional accomplishments and career.

The Cynthia Chapman Memorial Scholarship is designed for students who have a background in music (such as playing an instrument, choir, band, etc.) and who are pursuing a degree in a science related field. This scholarship is for graduating high school seniors from Northern Nevada, with a preference for those from Eureka County. This is a one-time award of up to $2,000.

Austen Marie Path to Music Memorial Scholarship

The Austen Marie Path to Music Memorial Scholarship was established in memory of Austen Marie Lynch to help support fellow musicians pursue a career in music, extending to others the opportunity to fulfill her own dream of becoming a career artist. Austen was a hardworking, tenacious, and talented musician. Her dream was to attend college and become a career musician. She was a singer, a songwriter, a guitarist, a poet, an artist, and a lover of ALL music. At 17 years old, Austen passed from a rare and terminal cancer in the comfort of her own bed surrounded by her loved ones, her guitars, and her record collection. 

This scholarship will be awarded to fellow musicians to attend school and fulfill their dream of becoming a career artist. There will be no GPA or academic requirement, but the applicant must show their dedication to music and ability to overcome life's challenges. Eligible applicants will be graduating seniors from a Nevada high school who plan to attend an institute of higher education to pursue a major or minor in music. This is a one-time award of up to $5,000 and has the potential to be renewed for additional years with reapplication.

Caesars Nevada Team Member Scholarship

This year, Caesars Entertainment is partnering with Phil and Jennifer Satre to match the scholarship funding.

Paid work, internships, and volunteerism will be a weighted selection tool used by the committee. This is a one-time award; awards may be single- or multi-year.

APA Foundation

The mission of the APA Foundation is to advance the art and science of planning through philanthropic activities that provide access to educational opportunities, enrich the public dialogue about planning, and advance social equity in the profession and our communities.

APA Scholarships

The APA Foundation scholarships aim to make the planning profession more accessible to individuals with limited financial means and to attract the best, brightest, and most talented individuals to the field of planning.

These scholarships are intended for students who demonstrate academic merit, a commitment to the field of planning, and financial need. The expanded eligibility includes students enrolled in undergraduate or graduate planning‑related programs at accredited U.S. institutions, reflecting the Foundation’s commitment to broadening access to the profession and strengthening its impact on communities nationwide.

APA Foundation Environmental Scholarship

This scholarship supports students pursuing planning degrees with a focus on environmental or conservation planning. It reflects a commitment to advancing wildlife and environmental conservation through the planning profession and to supporting future planners dedicated to protecting natural resources and sustainable environments. Awards are given to students currently enrolled in degree programs in planning.

The D.K. Shroyer & Alma Davenport Shroyer Memorial Scholarship, managed by the Beckley Area Foundation, aims to support students from Raleigh County, WV, who have completed two semesters of college with a strong academic standing. This need-based scholarship provides opportunities for students pursuing higher education in accredited institutions. The foundation is dedicated to fostering charitable giving to enhance educational success and community growth across Southern West Virginia.

The Delaney L. Wykle Nursing Scholarship, managed by the Beckley Area Foundation, aims to support nursing students from Raleigh and Summers counties in West Virginia. Applicants must demonstrate financial need, possess a minimum 3.0 GPA, and be enrolled in an accredited four-year nursing program. This scholarship helps empower students to achieve their educational goals while enhancing community health services in Southern West Virginia.

GIAR Grandmother’s First-Generation Scholarship

This scholarship is established in the memory and honor of Gramie Georgina, Grandma Isabella, Grandmother Ann, and Grannie Ruth, all first-generation women who were never able to attend college. This scholarship is only accepting renewal applications for 2026.

Helps First-Generation Young Women living in Nevada or Hawaii.

The scholarship funds the undergraduate or graduate college education of first-generation young women of Portuguese, Mexican, Central American or South American ancestry, under the age of 25. The term first-generation is defined here as a young woman who has immigrated to the United States of America and become a naturalized citizen or the daughter of an immigrant of Portuguese, Mexican, Central American and South American Ancestry. The student must be a resident of Nevada or Hawaii.

This scholarship is a one-time award of up to $5,000 based on need (which will be verified through your attached FAFSA and Financial Aid documents). One scholarship may be awarded to cover the full cost of attendance at UNR (up to $25,000), also based on need. All awards have the potential to be renewed with reapplication.

Beverly Joyce White Memorial Scholarship for Hospice Nursing Staff

The scholarship is awarded to hospice nursing staff who are pursuing a certification in hospice palliative care or other related education offerings.

The Byrd Prillerman High School Memorial Scholarship, managed by the Beckley Area Foundation, provides need-based support to Raleigh County students residing in the former Main Gulf area. Preference is given to descendants of past students or teachers of Byrd Prillerman School. This scholarship aims to empower local students in their pursuit of higher education, fostering community growth and educational achievement.

Partners for Environmental Progress (PEP) Scholarship

To be eligible, applicants must meet the following criteria: be an owner, employee or dependent of an owner or employee of the local branch/site of a company whose membership in PEP is in good standing at the time of the application, selection & awarding of the scholarship to the applicant. Applicants must currently be enrolled at the time of the application as a full or part-time student in a two year or four year accredited technical college, or university for secondary or post-secondary work and have a minimum 3.0 GPA with a declared course of study and major in one of the following: environmental science, environmental planning, environmental engineering, or other related science fields of study, including, but not limited to, all engineering fields, marine science, geoscience, fisheries science, forestry, and estuarine science and management.

Poarch Creek Indian Community Scholarship Fund

Annually, the Poarch Creek Indian Community Scholarship Fund will award a minimum of one scholarship to a male student and one to a female student. Applicants must be a resident of the State of Alabama or one of the following counties in Florida: Escambia, Santa Rosa, Okaloosa, Walton, Holmes, Washington or Gadsden. Academic merit, community involvement, and other factors such as financial need and/or workforce demand, will be taken into consideration when reviewing applications

The National Association of Free and Charitable Clinics (NAFC) is the only nonprofit 501c(3) organization whose mission is solely focused on the issues and needs of the medically underserved throughout the nation and the more than 1,400 Free and Charitable Clinics that serve them. The NAFC has earned the Platinum Seal of Transparency from GuideStar and a 4-Star rating from Charity Navigator. Founded in 2001 and headquartered near Washington, D.C., the NAFC is working to ensure that the medically underserved have access to affordable quality healthcare and strives to be a national voice promoting quality healthcare for all. Breast Cancer Screening & Prevention Initiative

The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. By enhancing early detection and preventive care, the initiative seeks to reduce barriers to care and improve health outcomes for women who might otherwise go without critical cancer screening services.

Scholarship Fund Overview

Beyond a shared commitment to educational opportunity, families establish scholarship funds with a variety of goals and purposes in mind. They may offer traditional scholarships or customize fund awards for alternative scholarships.

Currently, CFNJ administers more than 135 scholarship funds. Our scholarship opportunities run the gamut, from those exclusive to a local high school or geographic region to those supporting specific majors or career pursuits. Last year, we awarded over $2 million in scholarship assistance to 271 scholars.

Applicants may be evaluated on academic performance; extracurricular activities; community involvement; paid or unpaid work; and a statement of their personal aspirations, educational goals, and demonstrated unmet financial need in our scholarship review process.

Bego Scholarship

Preference for this scholarship will be given to students who are from recently immigrated families and are U.S. residents, naturalized citizens, or first-generation U.S. citizens. The candidate must be a resident from New Jersey, New York, or Pennsylvania. They also must have been accepted to, have applied for, or plan to attend an accredited four-year college. Applicants will be evaluated based on academic performance, extracurricular activities, community service, work (paid or unpaid), and unmet financial need.

Unless noted otherwise in the scholarship description and criteria statement, applicants can attend accredited schools of their choice within the United States.

CFNJ scholarships awards vary. On average, awards range from $500 to $2,500. However, the award could be larger based on the number of qualified applicants and the parameters set by donors who have established the funds. 

ETV: Linda D. Murphy Village Strivers Scholarship

This scholarship is awarded to a Black/African-American graduating high school senior from Morristown or Morris Township who has demonstrated consistent academic improvement throughout high school. Applicants will be evaluated on their engagement through activities or programs that support the local community (community service, church engagement, or paid or unpaid work experience) and unmet financial need.

Unless noted otherwise in the scholarship description and criteria statement, applicants can attend accredited schools of their choice within the United States.

CFNJ scholarships awards vary. On average, awards range from $500 to $2,500. However, the award could be larger based on the number of qualified applicants and the parameters set by donors who have established the funds. 

J. Oscar and Susan Florence Hunt Scholarship

This scholarship will be awarded to a college-bound, graduating senior from Gloucester or Salem counties. Preference is given to the residents of Swedesboro, Auburn, Pedricktown, and Penns Grove, in the order of priority stated. Scholarships will be awarded based on need and merit.
