Rural Health Transformation Program: Building Rural Hospital Capacity

The RHT Program is designed to provide funding for investments that will transform and sustain the way care is delivered in rural communities, specifically to improve access and outcomes, especially for individuals who are uninsured and underinsured. 

Scope of Work

The NJDOH’s Rural Health Transformation Program 2026: Building Rural Hospital Capacity focuses on increasing hospitals’ capacity to provide healthcare services to New Jersey’s rural population. The goal is to build long-term capabilities that will serve the target population for years, even after the program’s funding has ended. To achieve this goal, the program supports targeted initiatives that could include improvements in rural healthcare, workforce development and increased accessibility of healthcare services.

By increasing accessibility, supporting improvements and workforce development, the funding will help strengthen the overall health of rural New Jersey communities, in line with the DOH’s vision of making New Jersey the healthiest state in the union.

By fostering early engagement, the initiative seeks to cultivate a skilled workforce dedicated to advancing cancer research and addressing health disparities in New Jersey.

Sickle Cell Disease Pilot Program 2027

The SCD Pilot Program was established under New Jersey legislation (A4052) with funding appropriated for a three-year pilot. Beginning its third year (September-June) as a noncompeting continuation, the program supports Federally Qualified Health Centers (FQHCs) in select municipalities in delivering coordinated, comprehensive care for individuals living with SCD. The program’s purpose is to improve health outcomes and quality of life by providing an integrated model that combines medical, behavioral, social, and educational services. It emphasizes strong communication pathways with external providers and ongoing education and outreach to patients, caregivers, staff, and the broader community. The New Jersey Department of Health (NJDOH), in collaboration with the New Jersey Department of Human Services (NJDHS), oversees this initiative to address the complex healthcare needs of New Jerseyans affected by SCD.

SCD is a lifelong, inherited condition with several common types, including:

HbSS (Sickle Cell Anemia): The most severe and common form, caused by inheriting a sickle cell gene (“S”) from each parent. It predominantly affects individuals of African ancestry but can also impact people of Southern European, Middle Eastern, or Asian Indian descent.

HbSC (Sickle Cell Hemoglobin C Disease): A usually milder form, caused by inheriting a sickle cell gene from one parent and a gene for Hemoglobin C (“C”) from the other.

HbS Beta-Thalassemia: Results from inheriting a sickle cell gene (“S”) from one parent and a beta-thalassemia gene from the other. Severity varies depending on the beta-thalassemia type (“zero” is severe; “plus” is milder).

Symptoms vary widely and can change over time, ranging from mild to severe. Early signs may include jaundice, fatigue, pain, and swelling in the hands and feet. Serious complications include severe anemia, infections, bone and chest pain, stroke, and organ dysfunction. While SCD is present at birth, many infants do not show symptoms until four to six months of age or later.

The AW Environmental Grant Program, backed by American Water Works Company, Inc., aims to support community-driven projects focused on water conservation and the protection of vital water resources. Since 2012, the foundation has invested over $15 million in innovative initiatives that enhance access to clean water and promote responsible water usage in underserved communities. Eligible applicants include 501(c)(3) charities and educational institutions committed to environmental stewardship.

The FJ Brotherton Charitable Trust Grant aims to support charitable organizations, focusing on arts, education, and social services primarily in the northeastern United States. Established in 2024, the Trust continues the legacy of the Fred J. Brotherton Charitable Foundation. It emphasizes funding for programs that assist the needy or disabled, while adhering to strict eligibility criteria, including a requirement for tax-exempt status under I.R.C. Section 501(c)(3).

Museums and cultural organizations in New Jersey preserving folk and traditional arts can receive US $20,000 through the New Jersey Heritage Fellowship. These awards honor exemplary artists for lifetime achievement and artistic excellence, supporting institutions that showcase and sustain the state’s living traditional arts heritage.

New Jersey 501(c)(3) presenters and eligible government units presenting touring artists can access funding through Mid Atlantic Arts Foundation’s Jazz Touring Network; awards up to US $3,000 to support jazz presentations. Museums that regularly present professional touring artists and meet the three-year history requirement may be eligible for membership and related support.

Organizations eligible for funding must qualify as 501(c)(3) exempt entities under the Internal Revenue Code. The foundation's mission emphasizes providing vital assistance to students and advancing cancer research initiatives.

The Giants Foundation Grant supports nonprofit organizations focused on community issues, health improvement, and youth empowerment. Established in 1993 by the New York Giants, the foundation aims to provide financial assistance to eligible organizations in the NY, NJ, and CT areas. Eligible applicants must be 501(c)(3) nonprofits and either youth organizations or those using funds for youth-related programming in health, education, fitness, or football. The Ena Zucchi Charitable Trust Grant aims to support public health, educational, and social services organizations in New Jersey. Job Opportunities for Building Success

The purpose of Job Opportunities for Building Success is to provide services for Justice-impacted individuals whose workforce conditions have impeded their successful reentry into the community. Participants are eligible to receive career services, training, paid internships, apprenticeships, and supportive services (for example, transportation, ID obtainment, and clothing) to increase employment opportunities.

Individuals who have been impacted by the justice system often face distinct challenges when reintegrating into the workforce. They may encounter various obstacles that necessitate tailored support and personalized strategies to facilitate successful employment placement and retention. By providing these specialized services, we can help bridge the gap and strengthen the opportunities available to them.

The grant seeks to award a total of six recipients statewide. Grant funds will be awarded to selected applicants responding to this competitive Notice of Grant Opportunity (NGO). Selected applicants to this NGO will be responsible for managing the recruitment, enrollment, coaching, training, employment, and retention of program participants.
