Purpose

The purpose of this Request for Proposal (RFP) # COMPADM26002 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to provide technical assistance to Rural Iowa healthcare organizations and providers. This technical assistance will support providers with planning, partnership building, assessment and evaluation, and overall implementation of Hub and Spoke models of care in the areas of maternal and child health, mental/behavioral health, cardiovascular health, and/or chronic disease prevention and management.  

This technical assistance should allow Rural healthcare organizations to achieve success in future Health Hubs Projects of Iowa's Rural Health Transformation Grant. Activities in the Health Hubs Project will include the development of hub and spoke models for health care, including the development of rural provider strategic partnerships and agreements. It will also include developing and implementing arrangements for exchanging best practices, coordinating care across regions via telehealth and referral networks, expanding access to specialty services in a financially sustainable manner, streamlining or centralizing functions to create cost savings, improving the financial viability of rural providers, preserving independence of rural providers, keeping care local in rural communities when appropriate and possible, and developing models of sustainability for rural health care. The Contractor will support the Health Hubs on the items listed above, while also assisting with long-term sustainability planning, workforce development, and telehealth integration. This work implements a portion of the Hometown Connections initiative of the Iowa Healthy Hometowns Project, funded through the Centers for Medicare & Medicaid Services’ Rural Health Transformation Program, opportunity #: CMS-RHT-26-001. 

The High Mountain Hay Fever Children’s Health Fund Grant, established by the Wet Mountain Valley Community Foundation, aims to enhance children's health in Custer County. This grant focuses on addressing physical, emotional, social, and educational needs for children up to 18 years old. Nonprofit organizations and governmental programs in Custer County are eligible to apply, with a preference for collaboration and evidence-based programs. It does not provide funding to individuals.

Ripley County Community Foundation

The Ripley County Community Foundation was established in 1997 with $19,000 by a group of local citizens who had the vision to leave a lasting legacy for future generations.

Sally Morris Community Impact Grants

The purpose of a Sally Morris Community Impact Grant is to assist an organization with a sizable project needed to support and/or enhance its charitable reach. These Grants are available two times per year. Organizations may apply for a Sally Morris Community Impact Grant and one Small Projects Grant each year. 

To ensure your project aligns with our mission, all applications must meet the Community Foundation’s Grant Guidelines and Policies.

The AK Environmental Quality Incentives Program (EQIP) offers financial assistance and conservation planning to agricultural producers in Alaska. This program aims to implement conservation practices that enhance water quality, soil health, and wildlife habitats, while also benefiting agricultural operations. Eligible participants include socially disadvantaged and veteran farmers, who may receive higher payment rates. Applications are accepted until January 15, 2026, encouraging participation from historically underserved producers in implementing vital conservation efforts.

The Sustainable Materials Recovery Program (SMRP) Municipal Grants support municipalities in enhancing recycling, composting, and waste reduction initiatives. Through the Recycling Dividends Program (RDP), eligible cities and towns can receive funding by implementing effective waste management policies. This program encourages local efforts to divert waste from landfills, thus promoting sustainable practices and environmental responsibility. MassDEP evaluates applications based on established criteria, ensuring that communities meet the necessary standards for funding.

The Presser Foundation, established in 1939, supports music education and philanthropy through capital support grants ranging from $5,000 to $100,000. Eligible organizations include non-profit, 501(c)(3) music education and performance entities located within a 75-mile radius of Philadelphia. Projects funded may include construction, renovation, or equipment purchases. The Foundation prioritizes initiatives that enhance music education while excluding non-music related venues and general operating expenses.

The Conservation Small Grants program, administered by the Primate Society of Great Britain, provides awards up to £1,250 to support projects benefiting primate conservation and education. The grants aim to address threats to primate populations, enhance conservation incentives, and direct efforts to vital locations. Additionally, funds from the Born Free Foundation and Trentham Monkey Forest are allocated for specific conservation initiatives, fostering local engagement and expertise in habitat countries.

The Proposition 13 – Water Recycling – Planning Grant offers ongoing funding to assist local agencies in planning water recycling projects. This initiative aims to enhance the use of treated municipal wastewater, thereby augmenting fresh water supplies across California. All community types, including disadvantaged and severely disadvantaged communities, are eligible for financial support. Eligible projects encompass various aspects of water recycling, such as treatment, storage, and groundwater recharge.

The Legacy Youth Alliance Grant supports youth-led projects and organizations in Wapello County, aiming to enhance education and health opportunities for local youth. The fund prioritizes innovative initiatives that demonstrate community impact and collaboration. Eligible applicants include 501(c)(3) non-profits or those with a fiscal sponsor. Notably, it excludes funding for individuals, for-profits, and projects lacking youth focus. The foundation seeks to empower youth through impactful programs.

RFP PHTHOIT27001 for Pharmacy Services for TB, STI, and Immunization

Purpose

The purpose of this Request for Proposal (RFP) PHTHOIT27 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicant to provide Direct Pharmacy Services for Tuberculosis (TB), Sexually Transmitted Infection (STI), and Immunization Programs.

The Waste Reduction and Recycling Incentive Grants Fund supports innovative waste management projects in Nebraska. By funding recycling systems, market development for recyclable materials, and composting initiatives, it aims to enhance sustainability and reduce waste. This program also oversees the Scrap Tire Management Grant, promoting environmentally sound practices. With a focus on integrated solutions, these grants empower communities to implement effective waste reduction strategies and foster a circular economy.

The Family Forest Fish Passage Grant Program in Washington aids private forestland owners in removing barriers that obstruct fish migration, particularly for salmon and trout. Since its inception in 2003, the program has successfully helped landowners remove 433 barriers, restoring over 1,149 miles of fish habitat. This initiative is vital for the recovery of Washington’s fish populations, ensuring that small forest owners can access financial assistance to maintain and improve stream health.

The Community Foundation of the Central Blue Ridge's Youth Philanthropy Council (YPC) is offering $30,000 in grants for nonprofit organizations focused on preventing homelessness and meeting essential needs in Staunton, Waynesboro, and Augusta County. This initiative emphasizes accessible mental health resources and dignity for all residents. Established in 2008, the YPC empowers local youth to engage in philanthropic leadership, ensuring their voices contribute to meaningful community support.

The Mid-South RC&D Council Grant aims to improve the quality of life in Autauga, Bullock, Butler, Elmore, Lee, Lowndes, Macon, and Montgomery counties. Established in 1993, this nonprofit organization focuses on community needs through education and resource enhancement. Key priorities include community improvement, workforce development, emergency preparedness, and youth and adult programs. The council supports initiatives that promote economic development, training, and technology resources to benefit local citizens.

The Equipment Grants program, managed by the Oklahoma Department of Environmental Quality, supports the purchase of solid waste management equipment valued between $5,000 and $50,000. Eligible applicants include government units and recognized organizations in Oklahoma. This funding is intended for one-time purchases, with a requirement for storage and maintenance plans in applications. Ineligible items include buildings and vehicles, ensuring a focused investment in necessary equipment.

The ReFED Catalytic Grant Fund, managed by Refed Inc., supports innovative solutions to minimize food waste and reduce methane emissions, a cost-effective strategy for climate action. This fund prioritizes initiatives that enhance food recycling and improve supply chain efficiency in the beef and dairy sectors. By focusing on proven methods, the grant aims to accelerate impactful changes in the food system, ultimately fostering sustainable practices and collaborative efforts to combat food waste.

2026-2027 AmeriCorps State Formula Grant Competition

Overview

The Mayor’s Office on Volunteerism and Partnerships – ServeDC (ServeDC), is the DC Commission on National and Community Service as such ServeDC provides oversight and support to applicants selected as sub-grantees. Oversight includes monitoring and programmatic site visits, fiscal monitoring, and reporting requirements. Support includes AmeriCorps trainings, program director meetings, and one-on-one technical assistance.

ServeDC was established by an Executive Order in 2000 and is housed in the Office of Community Affairs in the Executive Office of the Mayor. ServeDC is the District of Columbia Government agency dedicated to promoting service as an innovative, sustainable solution to the challenges we face as a community and a nation. ServeDC engages District communities by building partnerships and organizational capacity, serving as the local lead for national volunteer and service initiatives, and providing and promoting meaningful service opportunities throughout the year. This application applies to Single State applicants operating only in the District of Columbia.

The Green School Grant Program, established by the NYS Association for Reduction, Reuse & Recycling (NYSAR3), offers funding to K-12 schools in New York for waste reduction, reuse, recycling, and composting initiatives. With over 100 schools funded since 2005, this program supports new and existing projects, providing up to $1,000 per award. Schools are encouraged to raise awareness about waste minimization while sharing their outcomes with NYSAR3.

The Ogden Codman Trust Grant provides crucial financial support to organizations serving residents of Lincoln, Massachusetts. It focuses on assisting individuals in need with food, shelter, medical assistance, and educational grants. The Trust encourages community engagement by requiring matching funds for grants, fostering a sense of collaboration and awareness of local programs. Organizations must be 501(c)(3) qualified and cannot be private foundations, ensuring that aid reaches those who truly need it.

The L.P. Jenkins Memorial Fellowship Program supports graduate and undergraduate students conducting vital field research on orangutans and rainforest conservation in Indonesia. Established in memory of Mrs. Lorraine Phyllis Jenkins, a passionate advocate for orangutan research, the fellowship prioritizes proposals focusing on previously unstudied orangutan populations. Eligible applicants must provide evidence of a valid research permit to access funds for their studies, ensuring a commitment to responsible and impactful research.

The LFF: Nature-Based Climate Action Grant aims to support initiatives that leverage natural systems to combat climate change. The Asian Cultural Council's Individual Fellowship provides artists, scholars, and arts professionals from Asia and the United States with opportunities for self-directed cultural exchange and research in unfamiliar regions. With a funding limit of $35,000, the fellowship encourages innovative proposals that enhance creative perspectives and relationships among peers, while also ensuring strong eligibility criteria for applicants.

The We Energies Foundation Grants support community development, health services, arts, education, and environmental initiatives. They prioritize projects from qualified 501(c)(3) organizations located within their service areas, aiming to foster relationships that yield lasting community benefits. Funding is restricted to one grant per organization per year, with a preference for supporting specific projects rather than general operating expenses. Overall, they encourage innovative approaches to enhance community well-being and cultural enrichment.

The Affordable Housing and Sustainable Communities (AHSC) Program plays a vital role in California's climate strategy by funding projects that promote affordable housing and sustainable transportation. By reducing reliance on personal vehicles, it aims to decrease greenhouse gas emissions.
