Context and Background

The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP), a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth1 .

Our Plan

Kentucky’s RHTP launches five interrelated initiatives designed to build rural health infrastructure and provide sustainable, long-term improvements. This plan directly supports Kentucky’s rural counties while advancing statewide impact through innovation, technology-enabled care and strengthened workforce recruitment pipelines for both clinical and non-clinical staff. This strategy will help build a resilient, integrated and technology-forward health system across the Commonwealth.

• Rural Community Hubs for Chronic Care Innovation:
  • Establishes local “hub-and-spoke” collaboratives focused on obesity and diabetes prevention and management.
  • These hubs will integrate nutrition, physical activity programs, and digital self-management tools.
• PoWERing Maternal and Infant Health:
  • Expands timely prenatal and postpartum care by deploying telehealth-enabled maternal care teams who will serve maternity-care deserts and high-risk regions, to help mothers and infants receive seamless, high-quality support.
• Rapid Response to Recovery (EmPATH Model):
  • Deploys technology-enabled crisis stabilization and mobile behavioral health response teams to connect individuals with community-based treatment and recovery supports.
• Rooted in Health:
  • Rural Dental Access: Increases access to preventive oral health services through expanded dental hygiene training programs, externships, and investment in Public Health Dental Hygiene (PHDH) teams in Local Health Districts (LHDs).
• Crisis to Care:
  • Integrated Emergency Medical Services (EMS) Response and Coordination: Enhances pre-hospital capacity and trauma coordination through treat-in-place protocols, improved data connectivity and workforce training for rural EMS providers. 

The Performing Artists Directory Grants, offered by the Kentucky Arts Council, aim to support professional performing artists in dance, music, theater, and storytelling. Eligible applicants, who must be active employees of public K-12 schools and members of specified educator associations, can request grants of $250 for music or arts needs. This initiative aims to enhance creative education across the states, excluding AK, HI, MA, MI, NY, and WI. Applications received by July 15 will be notified by November 30.

Focused on arts education and public participation, this program invites Kentucky public libraries to fund teaching-artist residencies that are open and marketed to the community. Libraries can integrate disciplines like dance, storytelling, painting, or writing into educational or health-related programming, using funds primarily for teaching artist fees and up to 30 instructional in-person hours.

The WHAS Crusade for Children Grant supports organizations, schools, and hospitals that assist children with special needs up to age 18 in Kentucky and southern Indiana. Grants are aimed at direct services, specific programs, and equipment that benefit these children. Applicants must be in good IRS standing, and grants cannot fund administrative costs or be awarded to individuals. Since 1954, the Crusade has distributed over $217 million to further this mission.

Nonprofit organizations serving children’s health, education, or social services in Kentucky can apply for Kosair Charities grants to enhance the well-being of children. Funding supports healthcare, research, education, social services, and child advocacy initiatives, and may benefit health-focused programs improving outcomes for children in the state.

The KAS Special Research Grant Program supports faculty in Kentucky higher education institutions, focusing on undergraduate education. Awards are available for one year, with potential for renewal through a competitive application process. Eligible projects span various fields of science, including biology, chemistry, and environmental science. The Kentucky Academy of Science aims to enhance scientific discovery and understanding, making a significant impact on education and research in Kentucky.

The Summer Undergraduate Research Grants, offered by the Kentucky Academy of Science, support undergraduate students at Kentucky colleges and universities in their faculty-supervised research during the summer. These grants cover essential budget items such as supplies, travel, and stipends, fostering scientific discovery and understanding in Kentucky. Eligible students must work under a faculty member who is a Kentucky Academy of Science member, ensuring a collaborative and enriching research experience.

The Kentucky Academy of Science offers the KAS Undergraduate Research Supply Grants to support undergraduate students in Kentucky conducting research under faculty supervision. These grants provide essential supplies and expendables that are not typically available from their institutions. This initiative aims to promote scientific discovery and understanding while empowering students to engage in meaningful research activities.

The 2027 Continuing Education Grant is made possible by funding from the Grants to States program administered by the Institute of Museum and Library Services (IMLS).  This grant aligns with Kentucky Department for Libraries and Archives' (KDLA) five-year strategic plan to enhance Institutional Capacity to “enhance the knowledge and skills of public library and archival staff through professional development and training opportunities. This grant will only cover conferences taking place between October 1, 2026, and September 30, 2027.  The goal is to provide essential funding to Kentucky's public libraries, enabling them to expand and enhance their staff’s learning opportunities.

Awards shall be disbursed as follows-

• $5000 large libraries (40+ full-time staff)
• $4000 medium libraries (15 – 39 full-time staff)
• $3000 small libraries (6 – 14 full-time staff)
• $2000 very small libraries (1 – 5 full-time staff)

Local governments certified under the Certified Local Government (CLG) Program, pursuant to Title 36 Section 61.5 of the Code of Federal Regulations, are eligible to compete for annual CLG grant funds. The Funding Priorities are: Preservation Education, Community Outreach & Education, Updating Documentation, Preservation Building Trade Schools & Workshops, Surveys, Historic Context & Thematic Studies, and National Register Nominations. 

CSX Charitable Investments provide essential in-kind donations to nonprofit organizations focused on supporting military members and community first responders. The program also includes occasional donations of railroad materials and equipment based on availability. CSX prioritizes support for organizations operating within its service areas, promoting community betterment and development.

The MFA Oil Foundation Grant offers cash support to nonprofit organizations focused on enhancing communities where MFA Oil operates. With a mission to bolster rural areas, the foundation prioritizes initiatives that develop youth leadership, agricultural education, and community problem-solving. Eligible applicants include tax-exempt nonprofits and accredited educational institutions. However, grants are not available for operating expenses, political lobbying, or individual support. This initiative seeks to empower organizations that create sustainable change in their communities.

The JFNY Grant for Arts & Cultural Exchange supports U.S. non-profit organizations aiming to enhance understanding of Japanese arts and culture. Eligible projects include performances, workshops, and exhibitions, with priority given to those with additional funding and in areas with limited access to cultural events. Applications must be submitted at least one month prior for online projects and three months for others. The grant covers expenses like travel, publicity, and artist fees, and is reimbursed upon project completion.

The Centers of Biomedical Research Excellence: Development Phase (COBRE-D) program aims to enhance biomedical research capacity in IDeA states, which historically receive low NIH funding. By providing critical support for early-career investigators and essential infrastructure, COBRE-D facilitates a long-term vision for institutions. This initiative not only supplements start-up packages for new hires but also establishes research facilities and improves administrative services, paving the way for less-resourced institutions to transition into the COBRE E/S program.

Community Foundation of Henderson County's Scholarship Program began in 1984. Thanks to the generosity of passionate donors, over 2,000 traditional and non-traditional students have been provided more than $10 million in scholarships to pursue higher education.

Pisgah Chapter of Trout Unlimited William L. Arbuckle Memorial Scholarship

Recipients of this award must be accepted to an accredited degree granting college or university within the Southern Appalachian Region. Recipients must be pursuing an undergraduate and/or graduate degree in the field of fisheries, aquaculture, freshwater biology, or other related fields. 

The Linda Neville Trust Grant provides funding to charitable organizations that support services for the blind and visually impaired in Kentucky. Eligible applicants include 501(c)(3) organizations, schools, and libraries located within 15 miles of a Dollar General store. The Dollar General School Library Relief Grant Program, managed by the Dollar General Literacy Foundation, provides crucial support to K-12 public school libraries that face significant damage due to disasters such as fires or federally recognized acts of terrorism. This grant aims to restore essential library resources, ensuring that affected communities can continue to access vital educational materials and support. Libraries must be within 15 miles of a Dollar General store and meet specific eligibility criteria.

The Dollar General Literacy Foundation offers Family Literacy Grants to nonprofit organizations that support family literacy initiatives. Eligible applicants must provide services such as adult education, children's education, and parent-child engagement activities. Organizations must be located within 15 miles of a Dollar General store and have a valid IRS tax ID. This funding aims to enhance literacy skills for families, empowering individuals to achieve their educational goals.

The Katharine M. Grosscup Scholarships in Horticulture, established by the Garden Club of Allegheny County in 1981, honors the legacy of passionate gardener Katharine MacConnell Grosscup. These scholarships, aimed at supporting students in horticulture and related fields, provide awards of up to $3,500 for education and research. Eligible candidates include U.S. citizens or permanent residents enrolled in specific states with a minimum GPA of 3.5. This initiative promotes a love for gardening and conservation.

The KYUMC Good Samaritan Ministries Grant aims to empower charitable and educational initiatives that address spiritual and physical needs in Kentucky. With grant awards up to $50,000, it supports organizations offering primary health care to low-income and uninsured individuals. Eligible applicants include ministries connected to the Kentucky Annual Conference, provided they align with its mission. This grant fosters sustainable ministries while prioritizing non-discrimination and equal opportunity.

Female Kentucky college students pursuing environmentally related majors at eligible in-state public universities can receive scholarships to help cover tuition and related expenses. Offered by the Community Foundation of Louisville, awards of US $5,000 support full-time sophomores, juniors, or seniors with at least a 3.0 GPA and demonstrated financial need.
