Broad Pragmatic Studies PCORI Funding

The Patient-Centered Outcomes Research Institute (PCORI) intends to issue a Broad Pragmatic Studies (BPS) PCORI Funding Announcement (PFA), seeking to fund patient-centered comparative clinical effectiveness research (CER) projects. 

Research Initiative Highlights

CER compares at least two available health treatments, services or care delivery strategies. PCORI seeks proposals that compare clinical interventions (e.g., pharmacological and nonpharmacological treatments, screening and diagnostic approaches) and healthcare delivery system interventions (e.g., workforce, technologies, healthcare service delivery designs) to evaluate potential benefits and harms of different treatment and care delivery options, thus generating evidence that empowers patients, caregivers and the broader healthcare community with information to make informed choices that reflect their needs and preferences.

Special Areas of Emphasis

For Cycle 2 2026, PCORI is particularly interested in submissions that address the following Special Areas of Emphasis (SAE) within the BPS funding opportunity. The purpose of identifying these SAEs is to encourage submissions to these areas, rather than limiting submissions to these topics. Applicants addressing one of the below SAEs should identify the area that is best associated with their research approach:  

• Addressing Obesity
• Diabetes Prevention, Care and Treatment
• Management of Urogynecological and Pelvic Pain
• Pain Management for Individuals Living With Sickle Cell Disease

In PCORI-funded research, patients and other research partners are expected to meaningfully contribute their lived experience or professional expertise to ensure studies are patient-centered, relevant and useful for healthcare decision making. Applicants must address PCORI’s Foundational Expectations for Partnerships in Research — an evidence-based framework supported by tools and resources to guide effective engagement strategies. Applicants are also strongly encouraged to consider the inclusion of a broad range of patient-centered burdens and economic outcomes. 

Broad Pragmatic Studies PFA: Category 1, Category 2 and Category 3 Study Specifications

PCORI will commit up to $120 million in direct costs through this funding opportunity. This funding opportunity contains three categories for funding:

• Category 1: Category 1 will support research projects with direct costs up to and including $5 million.

• Category 2: Category 2 will support research projects with direct costs greater than $5 million and up to a maximum of $12 million. Applications for funding under Category 2 will be required to provide additional information in the Letter of Intent (LOI) and application to justify the size and scope of the proposed research projects and to explain how additional funds are necessary with respect to the scale, scope and complexity of personnel and material resources needed to address the research question.

• Category 3: Category 3 will support research projects with direct costs up to $12 million that utilize PCORnet® to conduct observational studies and/or pragmatic trials that are national in scale. Category 3 PCORnet Studies® must include two or more Clinical Research Networks and share study progress, performance metrics and best practices with the network to promote continuous learning and improvement. The Health Access Heroes Awards, initiated by Sun Life Financial, aims to enhance health access and outcomes for underserved communities. This initiative highlights the crucial connection between oral health and diabetes, focusing on prevention and management. It recognizes impactful nonprofits and unsung heroes who contribute significantly to community health. Selected awardees will receive a $10,000 donation to support their charitable efforts, promoting diabetes awareness, management, and overall health equity.

The Nutrition Obesity Research Centers (NORCs) program, funded by the National Institutes of Health, invites applications for core centers that enhance nutrition and obesity research. It supports research core services, pilot and feasibility programs, and enrichment activities, aiming to improve research efficiency and productivity. This funding opportunity encourages multidisciplinary approaches to tackle obesity and nutrition issues, ultimately contributing to a healthier population.

This funding opportunity invites applications for a Research Coordinating Center (RCC) to lead a consortium aimed at optimizing anti-obesity medication (AOM) strategies for children and adolescents. This funding opportunity aims to support urgent research evaluating policies that impact health behaviors related to obesity. It is designed for studies that can only be performed with expedited review due to the time-sensitive nature of the research. Applications must demonstrate a compelling need for immediate funding to explore new policies or programs affecting dietary habits and physical activity, with the expectation that successful proposals will receive funding within four months.

The National Cancer Institute (NCI) invites applications for studies on mechanisms by which bariatric surgery influences cancer risk. This funding opportunity encourages exploration beyond short-term outcomes like weight loss and diabetes. The goal is to support innovative proof of concept studies, emphasizing feasibility without prior development in the literature. Unpublished data is not allowed, but references from preprints with a DOI are acceptable. Deadlines for applications span from February 2025 to November 2027.

The Community Partnership Fund by Adventist HealthCare offers program grants over $5,000 to 501(c)(3) nonprofit organizations addressing health disparities in Montgomery and Prince George’s Counties. Applicants must demonstrate alignment with key health priorities, including access to care, promoting healthy behaviors, and educational equity. This initiative aims to enhance community wellness and equity through measurable impacts in underserved populations. First-time applicants are required to consult with the Community Partnership Fund Team before applying.

The NHLBI Career Transition Award (K22) supports postdoctoral fellows and research trainees in the NHLBI Division of Intramural Research, facilitating their transition to independent research roles at extramural institutions. Key deadlines for applications include October 12, 2024, and February 12, 2025, among others, providing ample opportunity for qualified investigators to advance their careers.

The NHLBI Career Transition Award (K22) program supports postdoctoral fellows in transitioning their research to extramural institutions. The program includes a mentored intramural phase (up to two years) followed by an extramural phase (three years), totaling five years of support. Applications have multiple deadlines, including October 12, 2024, and February 12, 2025, among others. Successful transition depends on the awardee's progress during the mentored phase, evaluated by NHLBI.

The Patient-Centered Outcomes Research Institute (PCORI) intends to release an Implementation of Findings From PCORI's Research Investments PCORI Funding Announcement (PFA) on Aug. 4, 2026, seeking to fund implementation projects that promote the uptake of peer-reviewed findings from specific PCORI-funded patient-centered comparative clinical effectiveness research (CER) in the context of the body of related evidence. This PFA is an open competition opportunity for principal investigators to propose implementation projects that move evidence toward practical use in improving health care and health outcomes. Applicants may propose projects with budgets up to $1.8 million in total direct costs.

PCORI seeks to fund implementation projects that incorporate active, multicomponent strategies that will lead to uptake and integration of evidence generated by PCORI-funded research in practice settings. Implementation should target specific end users with a clear interest in — and ability to benefit from — the evidence. Proposed implementation projects will adapt findings as needed to facilitate uptake in the proposed settings and accomplish scale-up to reach larger numbers and/or scale-out to reach broader populations and settings. Applicants must demonstrate commitment and buy-in — including from leadership as well as frontline staff critical to the success of the project — from proposed implementation sites across multiple health systems, such that they provide a supportive context and infrastructure for undertaking the proposed project. In addition, projects should involve regional and national stakeholder organizations positioned to extend uptake in broader venues.  

Applications must be guided by an established conceptual model or framework and, where possible, by evidence regarding effective strategies for implementing evidence-based practices and interventions. Additionally, applications must include a rigorous evaluation plan that documents the successful execution of the implementation approach and the impact of the project on health care and health outcomes as feasible and appropriate within the project scope.

Sun Life’s Health Access Grant Program aims to enhance access to health services, specifically targeting diabetes and oral health in local communities. With funding options ranging from $25,000 to $100,000, this program supports 501(c)(3) charities focused on diabetes prevention, management, and oral health care. By fostering access to essential health services, Sun Life is committed to building healthier communities and addressing health disparities faced by underprivileged populations.

The program emphasizes innovative initiatives that address healthcare disparities and improve patient outcomes through education, advocacy, and support services. Applications are evaluated based on design, capacity, and potential impact, with a preference for programs over event-based funding.

The Therapeutic Giving program by Johnson & Johnson Innovative Medicine focuses on supporting nonprofit organizations that enhance patient access to treatment in cardiovascular and metabolic health. Applications are encouraged for initiatives that promote education, empower patients, and address healthcare disparities. Eligible organizations must be IRS 501(c)(3) certified, with a preference for programs demonstrating measurable outcomes. The funding period for local organizations extends from January to October 15th, while national organizations can apply in September.

The Hillman Innovations in Care Grant seeks innovative, nursing-driven proposals to address the impact of racism on health equity for marginalized communities. This includes addressing health disparities faced by BIPOC, LGBTQ+, and economically disadvantaged populations. The program encourages bold, evidence-based interventions that enhance healthcare delivery and outcomes. Grants of up to $600,000 will support projects that demonstrate potential for broad replicability and sustainability over a 36-month period.
