The Chichester DuPont Foundation provides grants focused on environmental, educational, healthcare, and social service initiatives, emphasizing support for underprivileged children in Delaware. Organizations are encouraged to develop relationships with trustees to enhance funding success. Grants typically cover one year, with potential for multi-year funding in special interest areas. Nonprofit organizations exempt under Section 501(c)(3) are eligible, while individuals and for-profit entities are not. Creating Health Access for Next Generation Equity (CHANGE) Grant Program

Launched in 2023, the AstraZeneca Foundation’s signature Creating Health Access for Next Generation Equity (CHANGE) program works to improve access to quality healthcare.

Beginning April 1, 2026, the AstraZeneca Foundation will accept grant requests of $200,000 - $220,000 for review and consideration for its next cohort of CHANGE grant awardees. Organizations receiving funding in 2026 may be considered for renewed funding on an annual basis for two additional years, contingent upon meeting objectives, funding availability and approval by the Foundation’s Board of Trustees.

The intent of the grants is to provide general operating support for organizations working in innovative, collaborative ways at the community level to help improve access to quality healthcare in their community during the grant year and beyond. Our aim is for funded organizations to maximize the benefit to community members and community-based organizations and to work toward continuing their services beyond potential AstraZeneca Foundation funding.

Individuals without health insurance often face significant barriers to obtaining prescription medications and receiving the counseling necessary to manage chronic conditions effectively. Through this initiative, NAFC will provide $30,000 grants to member free and charitable clinics and pharmacies to establish or expand Medication Therapy Management programs aligned with recommendations from the American Pharmacists Association. Funding will support activities such as staff certification in MTM, development of standardized protocols, and patient education to help individuals better understand their medications, manage side effects, and support overall treatment plans. By strengthening pharmacist-led care and improving communication between providers and patients, this initiative seeks to enhance medication adherence, improve chronic disease management, and advance better health outcomes for vulnerable patient populations.

The Dr. Susan Holstine Behavioral Health Professions Scholarship, established in 2022 by the Chuckanut Health Foundation, honors Dr. Holstine's commitment to accessible behavioral health services. This scholarship aims to support local students pursuing careers in behavioral health, alleviating financial barriers and empowering the next generation. By investing in education, we strengthen our community and encourage future leaders in mental health care.

Food as Medicine Initiative

The Food as Medicine Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to address food insecurity as a critical driver of health outcomes for uninsured and medically underserved patients. Research continues to demonstrate the strong connection between food insecurity and poor health, including increased risk of chronic conditions such as diabetes, hypertension, obesity, and certain cancers. Through this initiative, NAFC will provide $20,000 grants to member free and charitable clinics to implement or expand food insecurity screening and patient-centered interventions that improve access to healthy food and nutrition education as measured by the US Household Food Security Module or Hunger Vital Signs and patient education focused on food as medicine for eligible patients in order to increase the percentage of patients with improved food security scores within one year of project implementation.

Breast Cancer Screening & Prevention Initiative

The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. By enhancing early detection and preventive care, the initiative seeks to reduce barriers to care and improve health outcomes for women who might otherwise go without critical cancer screening services.

Bristol Myers Squibb Foundation Inc supports independent medical education to enhance healthcare professional competence and improve patient health outcomes. Their mission is to be a global leader in innovative medical education, focusing on evidence-based activities that address critical gaps in knowledge. By fostering excellence in healthcare, they aim to ensure measurable improvements in performance and patient outcomes across various disease areas.

The Will Rogers Motion Picture Pioneers Assistance Fund offers crucial financial support and counseling for individuals in the theatrical exhibition and distribution sectors facing health-related challenges. This fund assists with medical expenses, housing emergencies, and essential services like vocational rehabilitation and equipment aid. Eligible applicants must have a minimum of three years in exhibition or five years in distribution. The fund aims to provide a safety net for those impacted by illness, injury, or significant life changes.

Wegmans Family Foundation Inc focuses on cultivating healthy communities through support for hunger relief, health initiatives, education, and youth programs. The NAFC Capacity Fund supports investments that strengthen clinic and pharmacy infrastructure and organizational effectiveness. This funding is designed to help NAFC members build the systems, tools, and capabilities needed to operate more efficiently, improve quality, and expand their ability to serve patients over time.
