The federal Rural Health Transformation Program (RHTP) was created by H.R.1 (Section 71401 of Public Law 119-21) on July 4, 2025. It is a federal initiative to help states support rural communities in improving health care access, quality, and outcomes by transforming the health care delivery ecosystem. The RHTP focuses on promoting innovation, strategic partnerships, infrastructure development, and workforce investment in rural communities. The federal program will grant up to $50 billion to states over five budget periods.

This RFP focuses on the development of community telehealth access points (TAPs). TAPs are dedicated spaces in community locations where members of the public may access telehealth appointments. TAPs may be located at sites such as schools, pharmacies, local public health offices, long-term care facilities, libraries, community-based organizations, and Tribal health clinics or other Tribal offices. Telehealth services provided at TAPs may include, for example, screenings, provider consultations, and remote visits for primary care, behavioral health, and oral health. TAPs increase access to timely, high-quality health care for rural residents who may not have a health care facility nearby or broadband service at home, or who may feel more comfortable seeking care in a trusted community setting. All applicants must be located in Minnesota, and the TAP must be located in a rural community. 

The James M. Cox Foundation Grant offers support to nonprofit organizations focused on conservation, early childhood education, family empowerment, and health. Organizations must be 501(c)(3) tax-exempt and located in areas where Cox Enterprises operates. Grant applications are accepted year-round, with deadlines before Trustee meetings. Preference is given to organizations with a history of local engagement. This foundation embodies the philanthropic spirit of its founder, James M. Cox, by giving back to the community.

Rural Clinical Fellowship for Advanced Practice Providers and Other Clinicians

It is a federal initiative to help states support rural communities in improving health care access, quality, and outcomes by transforming the health care delivery ecosystem. The RHTP focuses on promoting innovation, strategic partnerships, infrastructure development, and workforce investment in rural communities. The federal program will grant up to $50 billion to states over five budget periods.

This RFP supports the development of rural clinical fellowship programs to prepare and train advanced practice providers, behavioral healthcare professionals, and oral healthcare professionals to deliver comprehensive rural clinical care. Proposed programs should be advanced, rural-focused, community-based, supervised post-graduate clinical training and didactics that complement prior academic experiences and address gaps in clinical skills to prepare providers to manage the health needs of rural Minnesotans. 

Fellowships are expected to vary in duration, focus, and format based on the needs of different provider types. All programs must offer a structured learning environment providing mentored clinical education and didactics. Fellowships may be 12-18 months long. For example, a rural clinical Nurse Practitioner fellowship may be structured to meet two days per month for a 12-month fellowship. 

Proposed fellowships must:

• Train clinicians who are currently practicing or intend to practice in rural primary care, including oral health, behavioral health, maternal health, and other areas of primary care practice
• Strengthen clinical competencies
• Cultivate knowledge, skills, and cultural competence related to rural practice, solidifying providers’ rural professional identities 
• Include interprofessional learning and practice
• Provide mentorship
• Increase clinicians’ ability to handle ethical issues that arise when practicing in rural communities
• Strengthen rural workforce wellness and resiliency
• Increase clinicians’ overall readiness to deliver comprehensive care to rural Minnesotans

The Carolyn L. Kuckein Student Research Fellowship, awarded by Alpha Omega Alpha Honor Medical Society, supports first and second-year medical students conducting research over 2 to 12 months. With a total funding of $7,000, students can receive reimbursement for travel expenses up to $1,000 for presenting their findings at national conferences. Eligibility requires enrollment in schools with active AΩA chapters, while those taking a year off for research or holding a PhD are ineligible.

The L.B. Research and Education Foundation Grant Program supports U.S.-based nonprofits focusing on innovative education and health solutions for underserved populations. Eligible organizations can apply by submitting a 400-word concept paper, and selected applicants may receive funding ranging from $10,000 to $75,000. The Foundation prioritizes projects that foster critical thinking and creativity, address health disparities, and encourage collaboration. There are no application deadlines, promoting year-round engagement.

The Esther Tomljanovich Strategic Initiatives grant from the Medica Foundation supports transformative projects aimed at advancing health equity. With a funding capacity of up to $200,000 over two to three years, the initiative focuses on innovative proposals in healthcare workforce development, women's health, aging services, dental programs, and housing stability. Grantees are expected to demonstrate measurable outcomes and foster collaboration within their communities, emphasizing the importance of addressing unique health needs.

The Cleveland-Cliffs Foundation, established in 1962, focuses on enhancing educational opportunities, environmental protection, and community health in the regions it operates. As a leading steel manufacturer, Cleveland-Cliffs is committed to sustainability and responsible corporate citizenship. The foundation supports 501(c)(3) organizations that address key challenges in education, the environment, and community well-being, ensuring impactful investments that help communities thrive across North America.

The Rupert Dunklau Foundation Grant supports organizations with 501(c)(3) status, focusing on Lutheran Higher Education, Health and Human Care, and Faith Strengthening. Since its establishment in 1968, the foundation has aimed to provide financial resources to ministries within the Lutheran tradition, particularly in Nebraska and surrounding states. Eligible projects must align with the foundation's mission to promote the welfare of mankind through educational and health initiatives.

The Great Lakes Federal Grant Navigation Program offers free assistance to local governments in accessing critical federal funding from the American Rescue Plan Act and the Infrastructure Investment and Jobs Act. This program aims to simplify the grant application process, especially for smaller municipalities, by providing one-on-one support and facilitating collaboration among cohort members. By participating, cities can maximize the impact of federal dollars and strengthen their communities in the wake of economic challenges.

The Archie D & Bertha H Walker Foundation supports nonprofits addressing systemic racism and promoting arts. Established in 1953, the foundation aims to enhance community welfare by funding organizations that dismantle barriers to equity and foster artistic expression. Eligible applicants must hold 501(c)(3) status and operate within specified counties in the Minnesota Twin Cities area. The foundation does not accept unsolicited proposals, focusing instead on impactful community engagement.

CommonSpirit Health offers the Community Health Improvement Grant to enhance health equity and service systems for underserved populations. Through collaborative efforts with 501(c)(3) organizations, this initiative provides financial support for essential health projects. Grant amounts range from $19,500 for small grants to $150,000 for standard grants, varying by hospital. Participating hospitals include those under Catholic Health Initiatives and Dignity Health, aimed at addressing significant community health needs.

The Skin Lightening Products Public Awareness and Education Grant aims to enhance public knowledge about the health risks of using mercury-containing skin lightening products that are illegally sold in the U.S. This initiative, funded by the Minnesota Department of Health, emphasizes culturally relevant and community-focused educational activities to effectively reach diverse populations. It seeks to inform individuals about the dangers associated with these products and promote safer alternatives.

The DGLF Youth Literacy Grants, established by the Dollar General Literacy Foundation, aim to enhance literacy among underperforming students in K-12. Bristol Myers Squibb Foundation Inc supports independent medical education to enhance healthcare professional competence and improve patient health outcomes. Their mission is to be a global leader in innovative medical education, focusing on evidence-based activities that address critical gaps in knowledge. By fostering excellence in healthcare, they aim to ensure measurable improvements in performance and patient outcomes across various disease areas.

The Will Rogers Motion Picture Pioneers Assistance Fund offers crucial financial support and counseling for individuals in the theatrical exhibition and distribution sectors facing health-related challenges. This fund assists with medical expenses, housing emergencies, and essential services like vocational rehabilitation and equipment aid. Eligible applicants must have a minimum of three years in exhibition or five years in distribution. The fund aims to provide a safety net for those impacted by illness, injury, or significant life changes.

The Dawn Foods Foundation Corporate Grants program is dedicated to enhancing community well-being through strategic investments in key areas like hunger prevention, education, community vibrancy, and volunteerism. With a strong commitment to improving lives, the foundation supports initiatives that promote health, literacy, and creative arts, while also encouraging team members to engage in volunteer efforts. Though applications for 2025 are closed, opportunities will reopen in 2026.

Wegmans Family Foundation Inc focuses on cultivating healthy communities through support for hunger relief, health initiatives, education, and youth programs. They prioritize collaborations with local nonprofits to enhance community well-being, offering assistance in key areas like economic mobility and youth engagement. Despite their wide-ranging support, they have specific ineligibility criteria, such as not accommodating personal requests or solicitation on their property, aiming to direct resources effectively where they are needed most.

The Dr. Susan Holstine Behavioral Health Professions Scholarship, established in 2022 by the Chuckanut Health Foundation, honors Dr. Holstine's commitment to accessible behavioral health services. This scholarship aims to support local students pursuing careers in behavioral health, alleviating financial barriers and empowering the next generation. Eligible entries must address healthcare affordability and be published in 2025, with a maximum of three submissions per author. This award aims to recognize innovative insights that can inform policymakers and stakeholders in the healthcare sector.

The NIHCM Foundation's Television and Audio Journalism Award honors outstanding television and audio reporting that contributes to enhancing the affordability, accessibility, and quality of healthcare in the U.S. With a prize of $20,000, the award seeks entries that showcase creativity and originality while providing new insights relevant to health plans, policymakers, and consumers. Eligible entries include stories broadcasted in 2025, focusing on critical healthcare issues.

The Excellence in Medicine Awards, presented by the AMA Foundation, honor exemplary physicians and medical advocates committed to volunteerism, community engagement, and leadership. Since 2005, this prestigious program has celebrated over 100 honorees, recognizing their vital contributions to underserved populations. Awardees receive a $2,500 grant honorarium to support a nonprofit health organization of their choice, collectively awarding over $250,000 to enhance community health and promote equitable care across the nation.
