The Georgia Department of Community Health (DCH) is one of Georgia’s four health agencies serving the state’s growing population of over 10 million people.

DCH serves as the lead agency for Medicaid and PeachCare for Kids®, and also oversees the State Health Benefit Plan, Healthcare Facility Regulation Division and State Office of Rural Health, impacting one in four Georgians.

Through effective planning, purchasing and oversight, DCH provides access to affordable, quality health care to millions of Georgians, including some of the state’s most vulnerable and underserved populations.

GREAT Health Program

Description

Georgia’s Rural Enhancement And Transformation of Health (GREAT Health) program will bring about a transformation of health in rural Georgia. Achieving this vision means rural populations are healthier, live longer, have an improved quality of life, and can both live and work in the communities they love; rural places have healthcare that is high quality, more abundant, more accessible, and more effective; and rural progress creates systems-level change that leverages technology, drives innovation, and improves quality, while maintaining a patient focus.

The GREAT Health program will do this through five initiatives:

• Transforming for a Sustainable Health System
• Strengthening the Continuum of Care
• Connecting to Care to Improve Healthcare Access
• Growing a Highly Skilled Healthcare Workforce
• Leveraging Technology for Healthcare Innovation

Who Can Participate?

Georgia has 126 rural and partial rural counties designated as the program’s areas of focus, based on the Health Resources and Services Administration’s (HRSA) rural grants eligibility analyzer. Healthcare facilities and organizations serving these counties will be eligible to apply for funding through the program’s initiatives. Visit the list of eligible counties.

In addition to the 126 eligible rural and partial rural counties, 93 hospitals serving these rural areas and with Center for Medicare and Medicaid Service (CMS) designations of Critical Access Hospitals, Sole Community Hospitals, or Rural Referral Centers, were invited to participate in Initiative 1 and its associated value-based care model. The Clean Vessel Act Grant Program, initiated by the Georgia Department of Natural Resources, provides essential funding for constructing and maintaining sewage pumpout stations for recreational boaters. With 75% of project costs covered, the program aims to improve environmental protection by preventing sewage discharge into waterways. Grant recipients must ensure facilities are accessible and operational during reasonable hours, promoting public awareness and use of these crucial waste management systems.

The project titled 'Aquatic Fauna Response to Ecosystem Restoration in Southwest Florida' aims to assess the effects of the Comprehensive Everglades Restoration Plan (CERP) on aquatic fauna. It will utilize a Monitoring and Assessment Plan (MAP) to evaluate responses of aquatic communities and inform future restoration efforts. This initiative is crucial for documenting CERP benefits and guiding further ecosystem restoration, ultimately benefiting the public by tracking progress in ecological recovery.

The Truist Foundation Grants aim to empower communities by providing resources for economic mobility and supporting small businesses, particularly those owned by racially and ethnically diverse individuals. The foundation focuses on enhancing job navigation for disadvantaged workers and building sustainable business ecosystems through collaboration and innovation. By addressing barriers to growth and promoting access to capital and education, the Truist Foundation strives to create equitable opportunities for all.

The ACT on Health Equity Community Solutions Challenge Grant, backed by the Astrazeneca Foundation, aims to enhance health equity by funding community-based nonprofit programs. Since its inception in 2021, it has allocated over $2.6 million to initiatives addressing nutrition, healthcare access, and lung health for historically marginalized populations. Eligible US-based 501(c) organizations can apply, with a focus on projects serving underserved communities and improving health outcomes through targeted support.

Amgen is a leading biotechnology company dedicated to advancing medicine through Independent Medical Education (IME). It funds accredited educational programs that empower healthcare providers to enhance patient care. IME funding supports critical areas such as oncology, bone health, cardiovascular education, and more. Amgen's commitment ensures that these educational activities remain unbiased and compliant with applicable guidelines, fostering innovation and improved health outcomes for patients worldwide.

State Farm's Good Neighbor Citizenship Company Grants aim to enhance community safety, development, and education. Focused on long-term impact, they support initiatives in auto and home safety, disaster preparedness, affordable housing, and educational programs. Eligible recipients include nonprofits, educational institutions, and government entities. However, funding is not available for individuals, religious programs, or organizations outside the U.S. By addressing critical community needs, these grants foster safer and more resilient neighborhoods.

The Appalachian Landscapes Project Capital Grant, offered by the Open Space Institute, supports vital conservation initiatives aimed at preserving biodiversity and enhancing landscape connectivity. Established in 1974, the OSI has protected over 2.5 million acres in the Eastern US and Canada, focusing on land that ensures clean drinking water and wildlife habitats. The Community Foundation for the CSRA offers funding to support charitable organizations in Georgia and South Carolina. With a focus on arts, education, health, and services for people in need, the foundation encourages innovative proposals. Grants may reach up to $15,000, supporting impactful community initiatives. Organizations must demonstrate long-term benefits and meet specific eligibility criteria, ensuring funds are used effectively to enhance community welfare and quality of life for residents.

The Robinson Foundation Grant, established in 2016, aims to alleviate hardships faced by individuals and communities, demonstrating God's love through charitable contributions. Organizations eligible for funding must be 501(c)(3) public charities. The foundation prioritizes fiscally responsible projects with the potential for tangible results, while not supporting capital projects.

The Doris Duke Foundation's Environment Program grants aim to foster a resilient environment through effective conservation efforts. By supporting U.S. nonprofit organizations, the foundation seeks to protect biodiversity, address climate change, and advance equity, particularly for marginalized communities. The program emphasizes land conservation, natural climate solutions, and inclusive practices, ensuring that conservation efforts benefit both wildlife and people. This initiative is pivotal for enhancing the health of ecosystems and communities across the United States.

The Neighborhood Builders Program, supported by Bank of America, empowers 501(c)(3) nonprofits through a two-year leadership development initiative. Participants receive funding, training, and networking opportunities to enhance their impact in communities. Since its inception, the program has invested over $346 million, training more than 4,000 leaders. The initiative focuses on advancing social mobility and economic opportunities, ensuring affordable housing, and fostering resilient communities. This program is vital for nonprofits aiming to strengthen their leadership and community engagement.

The Ribbons of Hope Grant, awarded annually by the Ribbons of Hope Foundation since 2012, aims to enhance the lives of women and children through education, health, economic independence, social well-being, and human rights initiatives. Non-profit organizations with a strong operational history and a commitment to impactful programs are encouraged to apply for this $100,000 grant, which supports both ongoing and self-sustaining projects.

The Roy A. Hunt Foundation's Environment Initiative Grants aim to support projects that enhance ecological health and sustainability. With an annual budget of approximately $3.4 million, the foundation prioritizes initiatives that promote responsible environmental practices, clean energy, and waste reduction. Grants between $25,000 and $75,000 are available for projects at various levels, focusing on innovative solutions that foster community partnerships and address root causes of environmental challenges.

The Diminished Tree Species Grant Program focuses on restoring native tree species crucial for biodiversity and forest health. The Oak Hill Fund, a private entity, encourages project-specific applications, prioritizing organizations that meet eligibility criteria under Section 501(c)(3) of the IRS. Hurricane Response Marine Debris Removal Fund

2027 Request for Proposals

National Fish and Wildlife Foundation (NFWF) and the National Oceanic and Atmospheric Administration (NOAA) established the Hurricane Response Marine Debris Removal Fund to support projects to assess, remove, and dispose of marine debris in and around coastal communities impacted by hurricanes and other episodic storm events.

The Hurricane Response Marine Debris Removal Fund will award grants to remove damaging marine debris from coastal areas of communities impacted by hurricanes or other natural disasters. Priority will be placed on debris removal efforts that reduce impacts on communities and industry as well as prevent further harm to habitats and fish and wildlife populations. Funding has been made available by the FY 2025 American Relief Act, P.L. 118-158.

The Hurricane Response Marine Debris Removal Fund will award up to $11 million in grants in 2027. While there is no minimum or maximum expected award amount, funding request amounts should be appropriate relative to the overall scale and impact of the project.

The Georgia Outdoor Stewardship Grant Program, established in 2018, provides funding for parks, trails, and conservation efforts across Georgia. Administered by the Georgia Department of Natural Resources, the program supports projects that protect wildlife and natural resources. Eligible applicants include local governments and conservation-focused organizations. This initiative, backed by strong voter support, aims to enhance outdoor recreation and safeguard critical lands for future generations.

The BRACE Grant Program, administered by the Keep Georgia Beautiful Foundation, aims to combat litter, blight, and illegal dumping while promoting community code enforcement. Applications are accepted from January to September, with a requirement to submit requests at least three months before the program’s start date. GM emphasizes charitable contributions in the communities where it operates, ensuring that organizations align with its values. The grant amount is not limited and is assessed based on the proposed program's merit and needs.

The Growing Healthy Kids Grant, funded by The Sprouts Healthy Communities Foundation, aims to support community-based programs that enhance children's nutrition access and health education. With grant amounts ranging from $5,000 to $10,000, this initiative focuses on hands-on programs that promote healthy habits among children and their families. Eligible organizations must be 501(c)(3) nonprofits located in areas where Sprouts Farmers Market operates, helping to ensure that the funds directly benefit local communities.

The Georgia Environmental Protection Division (EPD) offers the Recycling and Waste Diversion Grant to support projects aimed at reducing solid waste and promoting recycling in Georgia. This grant provides financial assistance for initiatives that enhance recycling infrastructure, improve waste reduction efforts, and encourage innovative recycling processes. Eligible projects include those focused on hard-to-recycle materials and the development of hub and spoke systems. This funding aims to foster sustainable practices and measurable outcomes in waste management.

The R. Howard Dobbs, Jr. Foundation Grant aims to enhance educational opportunities, access to health services, and environmental stewardship in Georgia. Focused on public K-12 education, the foundation prioritizes innovative teaching practices, professional development, and scalable projects. Additionally, it supports equitable conservation efforts and health initiatives, particularly in maternal and adolescent mental health. The foundation welcomes inquiries from public charities under 501(c)(3) status, emphasizing operational support for mission-critical functions.
