The Wisconsin Department of Health Services (DHS) seeks to strengthen the Wisconsin rural health workforce by developing and expanding high-quality and impactful community health worker (CHW) programs in clinical and community settings. There are three main components to this funding opportunity:

• Hiring and supporting CHWs to carry out services in rural communities
• Training CHWs on core competencies and standards
• Building organizational capacity to sustain quality CHW service provision

Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.

Call for Applications:

Community-Led Seed Grants

AHW’s Call for Applications: Community-Led Seed Grants supports community-driven efforts to design new strategies, test innovative ideas, and foster greater collaboration to build a foundation for future endeavors to address Wisconsin’s leading health challenges and produce deeper impact on health and health equity in Wisconsin’s communities. Eligible community-based organizations may request up to $50,000 for projects completed within a 12-month period, beginning Jan. 1, 2027.

To foster a collaborative approach to award selection, applicants will participate in a peer review process to assess proposals submitted to this funding opportunity by their fellow applicants. For the submitted application to be considered for funding, completion of all assigned reviews is required.

Following funding determinations, successful community-led applicant teams will be matched with an eligible MCW faculty member from a pre-formed pool developed through the Call for MCW Academic Partner for Community-Led Grants to serve as an additional resource to the funded community-led projects. 

Interdisciplinary research teams led by eligible UW School of Medicine and Public Health faculty in Wisconsin can apply for University of Wisconsin Madison’s Collaborative Health Sciences Program, with awards up to US $600,000 over three years. Funding supports innovative projects improving health, health care, or health equity, including community health education and community-engaged research benefiting Wisconsinites.

Community organizations and institutions improving health information literacy and outreach in Wisconsin can access funding through the National Library of Medicine’s Express Health Information Outreach Awards; awards US $5,000 for community health education and health information technology projects. Grants support partnerships that expand access to reliable, culturally appropriate health information, especially for underrepresented populations.

Nonprofits and community organizations in Wisconsin running physician-led, evidence-based health programs can seek funding for community health education, access, and equity initiatives. Grants from Wisconsin Medical Society Foundation help support projects addressing health literacy, mental and behavioral health access, disease prevention, and other innovative, measurable efforts to improve health outcomes and reduce disparities statewide.

Network member organizations can apply for US $25,000 to improve access for underrepresented populations, train health professionals, and integrate NLM resources into community health programs.

The Measurement and Evaluation Graduate Student Award, offered by SHAPE America, aims to support graduate students specializing in psychomotor measurement and evaluation. This scholarship assists with professional expenses, ensuring candidates can focus on their studies. Eligible applicants must be current SHAPE America members and pursuing relevant master's or doctoral degrees. The award prioritizes those who have completed their first year of study, promoting excellence in health and physical education across the U.S.

Grants from Society of Health and Physical Educators help Wisconsin community health education and K–12 physical education professionals gain national recognition for excellence in measurement and evaluation. Current SHAPE America members in the Measurement and Evaluation SIG who have significantly advanced the SIG’s mission may be nominated for this honor award.

Experienced SHAPE America members in Wisconsin and nationwide who advance measurement and evaluation in health and physical education may be eligible for this lifetime achievement recognition. Grants from Society of Health and Physical Educators honor substantial scholarly impact in community health education, K–12 education, physical fitness, dance, and related fields, supporting leaders whose work benefits community health programs.

Health and physical education professionals in Wisconsin seeking support for projects that advance opportunities for girls and women in sports can apply for this award, which offers US $1,000 for research, programs, or leadership development. With a mission to advance health and physical education, Society of Health and Physical Educators provides community health educators with annual funding honoring the NAGWS legacy.

Nonprofit and community organizations working on community health education, social justice, workforce development, or environmental stewardship in Wisconsin may be eligible for grants that advance these priorities and demonstrate measurable local impact. AES Hawaii Foundation considers programs serving communities near its renewable energy projects as well as initiatives with broader or statewide reach.

Focused on community and public health education, the Elizabeth Fries Health Education Award invites nominations (no self-nominations) recognizing substantial contributions, and is administered by the CDC Foundation in partnership with the James F. and Sarah T. Fries Foundation.
