What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

What are the funding priorities?

• Priority point system based on population, median household income
• Small communities with a population of 5,500 or less
• Low-income communities having a median household income below 80% of the state nonmetropolitan median household income.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

Idaho facilities wishing to apply for reinvestment program funds must use the CMS forms found on the Civil Money Penalty Reinvestment website.

About CMP Funds

A CMP is a monetary penalty the Centers for Medicare & Medicaid Services (CMS) may impose against nursing homes for either the number of days or for each instance a nursing home is not in substantial compliance with one or more Medicare and Medicaid participation requirements for long-term care facilities. A portion of CMPs collected from nursing homes are returned to the states in which CMPs are imposed. State CMP funds may be reinvested to support activities that benefit nursing home residents and that protect or improve their quality of care or quality of life.

Nursing facility providers, stakeholders and other organizations are encouraged to submit CMP fund grant applications for the development and implementation of quality improvement initiatives that directly or indirectly benefit nursing facility residents. 

Project proposals should demonstrate current and sound evidence-based practices that promote quality of care and quality of life for nursing facility residents. CMP fund grants are only available for quality improvement initiatives that are outside the scope of normal facility operations. 

Funding Uses

The following examples of allowable uses of CMP funds are not all-inclusive. Please see the Allowable and Non-Allowable Uses of CMP Funds download for more information. CMP funds may be used for:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs

As a corporate citizen, IPC believes that its responsibility extends to supporting and participating in activities and organizations that help maintain and improve the quality of life within the communities we serve. Serving those who depend on us is at the center of everything we do. We all prosper by committing to the needs, safety, and success of our customers, communities, employees, and shareholders.

IPC’s contributions of financial and non-financial or “in-kind” support are investments in the economic and social viability of the communities we serve. But the benefits to the community and IPC are not always measurable, and subjectivity is a part of every contribution decision. It is the intent of IPC that its contributions strike a reasonable balance between the need to be accountable and responsible to our shareowners and customers and the desire to contribute to the economic and social needs within communities we serve.

 Community Contributions

Community contributions range from support for capital fund drives to sponsorship of nonprofit fundraising events to non-cash donations like awarding retired company vehicles to emergency-response organizations.

IPC has established five general categories of contributions.

These categories are:

• Health and Human Services:
• To assure the availability of adequate health care and human service support for employees and other members of the public and to promote the development of new, workable responses to health and human services needs, this category includes:
  • Support of organizations dedicated to this cause, and other federated campaigns.
  • Hospitals and other health care facilities.
  • Agencies for senior citizens and the elderly.
  • Youth agencies.
  • Local health and human service organizations.
• Agencies receiving primary funding from federated drives (e.g., Unites Way) to which IPC separately contributes are generally not eligible for additional health and human services operating funds. Exceptions may exist in the case of capital fund drives.

• Education:
  • Priority is given to education to ensure the continued availability of highly skilled, educated men and women to enrich local communities and from among whom IPC may draw employees, to encourage continuing research and development in academic areas relevant to IPC, and to promote standards of educational excellence.
• Culture and Arts:
  • Contributions made in this area are intended to encourage high standards of excellence and provide a pool of creative resources in the visual and performing arts for the enrichment of our employees and the community. This category includes support to orchestras, museums, theaters, performing arts programs, cultural arts programs, public radio and television, and non-academic libraries.
• Civic and Community:
  • Support for organizations and/or programs addressing civic and community needs demonstrates our commitment to increasing citizen awareness and participation in the management of community life. This commitment extends to supporting organizations that provide services for adequate housing and neighborhood improvements, local civic and community projects, conservation activities, and the support of groups desiring to develop workable solutions to community issues.
• Environment and Recreation:
  • This category provides contributions to organizations and projects that support environmental stewardship, including clean and renewable energy. It also supports the expansion and preservation of recreation opportunities.
• Other:
  • This category includes support for organizations not included in the four preceding categories, but which have a benevolent or philanthropic purpose relevant to IPC and its communities. 

IPC intends to respond to requests for contributions in a timely manner, and generally not more than 60 days following submission of the completed application.

Idaho Veterans Support Fund

The Veterans Support Fund was established by Idaho Statue, Section 65-209, Idaho Code, to fund programs to support veterans of the armed forces of the United States.

The Division's policy is to fund programs operated both within and outside state and local government, which support Idaho veterans. Veterans Support Funds are not provided to support the basic needs of individual veterans or their families.

Priority will be given to proposals that will serve the greatest number of veterans, and for which there is no other source of funding.

Since its inception, the Foundation, which is still led by Lester T. Sunderland's descendants, has focused on supporting construction projects, awarding grants to nonprofits in the Kansas City region and other markets traditionally served by the Ash Grove Cement Company. 

 The Foundation prefers to make grants for construction and special interest projects rather than for annual operating expenses. 

Grants for planning, design, construction, renovation, repairs and restoration of facilities are considered. Areas of interest include higher education, youth serving agencies, health facilities, community buildings, museums, civic projects and energy efficient affordable housing projects sponsored by qualified tax-exempt organizations. 

Funding Areas

In recent grant cycles, the Board of Trustees has awarded the majority of grants in four broadly defined areas:

Higher Education

In 2022, the Foundation awarded $49 million to more than 35 educational organizations. Grantees included community colleges, private colleges, and public universities.

Human Services

The Foundation awarded $30 million to human service nonprofits in 2022, and many grants in this area went to groups that provide essential services to youth and families. Grantees included a range of youth-focused groups, including Reconciliation Services, the Nebraska Youth Justice Initiative in Omaha, Nebraska and the Sno-Valley Senior Center in Carnation, Washington. 

Arts and Culture

Arts and culture projects received $23 million in 2022, including grants to the Springfield Art Museum in Springfield, Missouri, the Omaha Zoo and the Seattle Aquarium in Seattle, Washington. Health Care and Hospitals.

Health Care and Hospitals

This is a growing area of need in many communities the Foundation serves. In 2022, $65 million was awarded to hospitals and health-care groups to build and improve their facilities.

The Sunwest Bank Charitable Foundation invests in outstanding local organizations and employee programs designed to build a philanthropic corporate culture and strengthen our communities.

Our primary focus is the care of vulnerable children and families and supporting nonprofit organizations that meet food, housing, health, safety, and education needs. The Sunwest Bank Charitable Foundation encourages bank employees to get involved through the Annual Mission Trip, Volunteer Grants, and Matched Donation programs.

Since inception, we have issued grants to organizations that meet the needs of homeless, hungry, abused, unemployed, at-risk, and otherwise disadvantaged children and families in Arizona, California, Idaho, and Utah.

Grant Guidelines

The Sunwest Bank Charitable Foundation builds prosperous communities by fostering philanthropic corporate culture and advancing the impact of effective charitable organizations. Through its Community Grants program, the foundation provides annual cash grants to nonprofit organizations benefiting the communities served by Sunwest Bank. These grants are aimed at addressing the unique needs of our communities. Community Grants are awarded through an open application process to eligible organizations. All organizations interested in applying for a Community Grant must apply through the Sunwest Bank Charitable Foundation’s online grant portal, which is linked via the Sunwest Bank website. Incomplete applications or those that fail to comply with the Grant Guidelines will be disqualified. Submission of an application does not guarantee funding. 

Rural Health Care Access Program (RHCAP) - Dental Care

(RHCAP) exists to assist rural and underserved Idaho communities in improving access to primary medical and dental health care. RHCAP awards are limited to a maximum of $35,000 for a one-year funding cycle. Proposed projects are required to address needs in areas designated as Health Professional Shortage Areas (HPSA) and Medically Underserved Areas (MUA). Applicants may submit grant proposals that improve access to primary care and/or dental health services in any of the three assistance categories (Idaho Code 39-5906), including, community development, telehealth, and other (such as recruitment and retention).

RHCAP seeks to improve access to primary medical and dental care by strengthening healthcare systems, removing barriers that prevent people from obtaining healthcare, and developing partnerships to better serve communities. Barriers may be socioeconomic, cultural, geographic, or reflect the inadequate availability of clinicians. Although access barriers to mental health services may exist, the RHCAP statute limits projects exclusively to primary medical care and dental health services (Idaho Code 39-5902)

To maximize the impact of our financial support, the Foster Foundation cultivates long-term partnerships with organizations whose work aligns with our priority funding issues. By identifying well run nonprofit programs with the vision and capacity to get things done, we continue to make sound investments in the people, communities and future of the Pacific Northwest.

Priority Areas

Building strong communities benefits all of us. Improving community life encompasses not only meeting critical needs such as food, housing, healthcare, education and employment, but also enriching community spirt and well-being through the support of artistic expression, cultural programs and sports/recreational opportunities.

We seek to identify and fund under-resourced opportunities to make a difference in these four areas:

Social Services/ Human Welfare

We fund emergency and critical human services that support people and families in need. This includes food, emergency/transitional housing, job/life's skills training, counseling and other resources and opportunities that build economic self-reliance.

Education

We support innovative programs that improve literacy, learning and academic success for all ages. Training, tutoring, mentoring and enrichment programs are examples of our outreach in this area.

Medical Research, Treatment & Care

We provide funding for promising medical research to aid in the understanding, treatment and prevention of diseases. The Foundation also supports hospice care as well as HIV/AIDs research and education.

Community Engagement

We nurture the spirt and well-being of Northwest communities by supporting cultural, artistic and recreational activities that engage all ages and populations. Foundation grants help sustain arts organizations and programs that express and grow the creative imagination. We also support community sports/recreational programs, centers and activities that promote health, well-being and teamwork.

Geographic Reach

With both family and business roots in the Pacific Northwest, The Foster Foundation takes a regional approach to giving. We target our funding to assist nonprofits engaged in our priority funding concerns within Washington, Oregon, Idaho, Montana and Alaska.

In addressing the founders' original intent, the Foundation will expand our philanthropy into smaller, more diverse communities within this five state area over the coming years. We will continue to support existing grantees. But, we desire to learn about and fund other pioneering initiatives and nonprofit programs that address the underserved and disadvantaged segments of this population―especially children, women and seniors.

Awareness. Education. Prevention.  These are three key tenets of Bayer Fund’s investments in health and wellness.  With a focus on cancer and cardiovascular disease, we provide resources to organizations dedicated to educating and/or providing services to patients, caregivers and at-risk populations.

Health & Wellness Grants

Programs that Bayer Fund will support include those that focus on patients and their families needing assistance with issues to managing cardiovascular disease and cancer. This includes education, disease awareness, and supportive services for these diseases. In areas related to cardiovascular disease, priority (though not exclusivity) will be given to charitable organizations that focus on programs and support for the African American community due to the high incidence rate of disease in this population.

Giving in this category will focus on support services such as education, disease awareness, and general access to care (e.g., housing for families traveling for cancer or heart care).

All funding requests and budgets must be for program activities and expenses that start after funding decisions are made. All programs must be completed within one year of the start date, except in limited situations where longer term programs have been agreed upon. Grant award amounts vary, depending on the size of the community, the type of programming, and the reach of the organization.

Rocky Mountain Power Foundation

The Rocky Mountain Power Foundation is the philanthropic arm of Rocky Mountain Power. Our mission, through our charitable investments, is to support the growth and vitality of our communities.

Apply for a Grant

The foundation manages its grants in four cycles. This helps the foundation carefully review similar requests to ensure maximum benefit.

• Educational and research organizations both public and private, from early childhood through university level (proposals due March 15)
• Environment, civic and community enhancement and other organizations not covered in other categories (due June 15)
• Arts and culture organizations dedicated to performing arts, visual arts, historic preservation, cross-cultural education and related (due September 15)
• Health, safety and wellness organizations including youth organizations, senior citizen centers, dependency and prevention programs and related (due December 15)

The foundation allocates grants that best serve community interests. Grants generally are less than $10,000 with most between $2,000 and $5,000.

About Employer Grant

Employer Grants are available to Idaho employers who are increasing their current workforce and/or retraining existing workers with skills necessary for specific economic opportunities or industrial expansion initiatives.

The maximum amount of funds available to an employer is $500,000 per grant and $750,000 over a 10-year period.

• Promote innovation in talent development.

Best Practices

• Encourage replication of best practices in talent pipeline development.

• Provide timely assistance to businesses while shifting focus to broader talent pipeline development strategies that serve multiple employers.

Economic Growth

• Support growth of the economy by assisting employers with job creation and integration of technology, specifically through the development of skills in their existing and/or new employees.

Return on Investment

• Provide a return on investment to Idahoans as evidenced by increased wages, job creation, capital investment, retention of Idaho’s workforce, credential attainment, and/or customer satisfaction (employer and trainee).

Community Care Fund

Community care keeps us alive and plants seeds for queer and trans futures.  

Community care is the antidote for isolation, exploitation, and oppression. We create the joy, rest, pleasure, and healing we need to sustain and transform ourselves. For queer and trans people, there are infinite possibilities for how we protect and care for each other—and this fund supports our collective efforts to practice that care.  

For us, community care is any effort to respond to community threats, harms, needs, hopes, and healing. Whether it’s mutual aid, safety planning, opportunities for healing, or any number of practices—we want to help support whatever “community care” means to you.  

Guidelines

For this grant, the definition of ‘community care’ is broad; ‘community care’ is any collective efforts to protect and care for queer and trans communities.

This funding will be unrestricted (unless you want it to be restricted) .

We define “community care” as any effort to respond to community threats, harms, needs, hopes, and healing. We welcome your own definitions and practices of community care. Examples of community care include, but are not limited to:    

• Mutual aid and financial relief 
• Community organizing, policy advocacy, political education and mobilization

Of community, by community, for community For decades, community groups and nonprofit organizations throughout the Northwest have worked to advance equity and justice for LGBTQ+ communities in every corner of our region. Our Community Grants Program helps fuel this work through providing consistent, long-term funding.   Since this program’s inception in 1987, we have awarded over $12 million dollars. Our most recent award cycle in 2021 was the largest in Pride Foundation’s history—$2,751,000 to 105 incredible organizations over three years. In 2021, we shifted to a proactive grantmaking model for this program, and began awarding funds without an application process. These programmatic changes were intentional shifts to center racial justice in our work, improve the experience of grantee partners, and align our grantmaking with our organizational priorities to move resources to LGBTQ+ communities most impacted by injustice. In 2024, we will have both a proactive grantmaking model and open application to ensure that all organizations have the space and opportunity to receive funding.

The goal of the NNLM is to advance the progress of medicine and improve public health by providing U.S. health professionals with equal access to biomedical information and improving individuals' access to information to enable them to make informed decisions about their health.

NNLM offers funding for projects that improve access to health information, increase engagement with research and data, expand professional knowledge, and support outreach that promotes awareness and use of NLM resources in local communities.

Engagement & Outreach Award

The purpose of the Engagement & Outreach Award funding to build partnerships and collaborations with local community groups and other key groups. Applications should demonstrate how it will:

• reach underserved populations with health information outreach projects,
• use community-led approaches to increase access to health information by working through libraries and other members to support a highly trained workforce in biomedical, health information, data science resources, etc., and
• support specific Region 4 Core Aims, the NNLM mission, and the NLM 2017-2027 Strategic Plan

NNLM Region 4 Core Aims

Project proposals are intended to support the following core aims of NNLM Region 4 program:

• Construct a rich culture built on a foundation of inclusive programming designed to attract and engage a diverse regional network.
• Use community-driven approaches to increase access to health information.
• Work through libraries and other members to support a highly trained workforce in biomedical, health information, and data science resources.

Potential Project Ideas

Potential projects may include but are not limited to:

• Training unaffiliated, minority, urban or rural health professionals in effective use of electronic health information resources for evidence-based practice with an emphasis on NLM databases and NNLM resources.
• Exhibiting or presenting at local meetings or organizational events to share completed outreach projects and promote NLM products and services
• Evaluating health information and training needs of a target audience and implementing a training plan for that audience
• Programs on locating and evaluating consumer health information found on the Internet

• Developing supplemental educational materials using NLM resources for educators teachers
• Train-the-trainer projects that enhance the skills of library/organization staff and other consumer health information intermediaries to train a target population on locating and evaluating health information.

About IWCF

In the Fall of 2001, a small group of women founded an organization whose mission it was to inspire women with the power of collaborative philanthropy – Idaho Women’s Charitable Foundation. Thirty-four motivated women joined during our inaugural year. Today, the membership is more than 400 strong.

Just as our organization has grown, so has our ability to make a difference. We have invested more than $3.3 million in over 140 local projects with our pooled-fund grants. These grantees range north to McCall, west to the Oregon border and east to Gooding. One of the unique aspects of IWCF membership is the opportunity to designate $500 of your contribution to organizations of your choice. Those designations, to more than 700 other grantees, have increased our overall giving to nearly $6.2 million.

IWCF Pool-Fund Grants

IWCF seeks to fund carefully planned and skillfully executed projects, programs, services, or items that address demonstrated needs in Southwestern Idaho.

In our evaluation process, we prefer requests that:

• Address critical needs in Southwestern Idaho
• Are innovative and creative approaches to community challenges
• Include plans for sustainability beyond IWCF funding
• Allow IWCF to contribute a substantial percentage of funding

Interest Areas

IWCF will fund grants between $20,000 and $30,000. (applicants must select one interest area):

• Cultural Arts: Visual and performing art (music, dance, visual, traditional crafts, film, public art, performance, etc.) and humanities for all ages, abilities, and income levels, especially underserved populations.
• Education: All ages and all forms of learning (mentoring, childcare/preschool, instructional or equipment upgrades, pilot projects, early learning, literacy, and enrichment). 
• Environment: Regional environmental issues (flora, fauna, land, air, water) such as resource and wildlife conservation, mitigation, reclamation, parkland improvements, or environmental education.
• Financial Stability: Poverty, homelessness, housing, self-sufficiency, work/job training, and unique, rarely-offered programs targeting underserved populations including referral services, food/clothing collection/distribution, case management, or mentoring programs.
• Health: General health care, domestic violence, child abuse/parenting, substance abuse, suicide, disabilities, and wellness or recreation programs. 
• Rural Communities: Addresses critical needs in towns and unincorporated areas with a population less than 30,000. Organizations which are based in urban areas, but provide services in rural areas, may apply, but the services covered by the grant request must be delivered exclusively in rural areas.

Intermountain Community Care Foundation

The mission of the Intermountain Community Care Foundation is to support Intermountain’s identified health priorities, improve access to healthcare services, and increase healthy behaviors for low-income, uninsured or medically underrepresented populations in Utah and Southeast Idaho (Cassia County and Minidoka County).

Through identifying and supporting existing programs and services that provide access to these populations, the Foundation is dedicated to helping people live the healthiest lives possible.

What we fund

Our funding aligns with priorities identified during our Community Health Needs Assessment. The CHNA identifies significant community health needs, especially for low-income residents in Utah and Southeast Idaho communities.

Purpose of the Professional Development Award

The purpose of this funding is to strengthen the professional knowledge and experience of individual network members within Region 4 in data science or health information access/delivery, build professional capacity in support of the mission of the NNLM, and enable libraries and other organizations to train staff in skills needed to deliver reliable health information.

Professional development awards are intended to expand beyond the organizations you associate with as a normal part of your profession. For example: MLA and MLA Chapter meetings will not be considered for health science librarians.

Potential Project Ideas

Professional development opportunities such as classes, conferences, or workshops for individuals or groups of individuals associated with NNLM institutional members. Ideas include but are not limited to:

• Introduce students to the health sciences, trusted National Library of Medicine (NLM) resources, and develop their professional skills.
• Prepare individuals for a leadership position in a hospital or academic health sciences library.
• Host a reading club with learning outcomes focusing on health information access issues that can be used to improve library programming that are more impactful to your underrepresented communities.
• Costs associated with arranging for an expert speaker to give a talk, workshop, class, or lecture.
  • NOTICE: Applications that include speaker fees over $1000 MUST sufficiently justify the expense.
• Educational activities that relate to Diversity, Equity, Inclusion, and Accessibility and prepare professionals to better serve underserved, minority, and underrepresented communities.

Award Focus & Impact

Applicants should plan to describe how their professional development activity will improve their ability to provide improved health information access related library services to their communities.

Applicants should consider submitting proposals that support:

• An INDIVIDUAL to attend a workshop, conference, or meeting (example topics - patient safety, diversity, equity, inclusion, and accessibility, health information access, data services, etc.).
• NETWORK MEMBER INSTITUTIONS to host a CE event, workshop, or speaker (example expenses - speaker fees. reading club materials, funding licenses to online training services, hosting a virtual book club discussing diversity, equity, inclusion and accessibility, etc.)
• STUDENTS currently enrolled in a library and information science, iSchool, or health occupations program (this funding is ideal for students interested in exploring health information as a career path or the role that information plays in improving the knowledge of health care providers).
• ONLINE CONFERENCES EXPRESS APPLICATION registration costs (under $500) for individuals interested in attending an online workshop, conference, or meeting that meets the purpose and alignment with NNLM and NLM mission and strategic plan

Successful applications will demonstrate how community-based health organizations, public health organizations, and libraries work together in addressing the health information needs of NNLM priority populations. (see NNLM Overview, section "Underrepresented Populations" for details).

NNLM Region 4 Core Aims

Project proposals are intended to support the following core aims of NNLM Region 4 program:

• Construct a rich culture built on a foundation of inclusive programming designed to attract and engage a diverse regional network through effective membership management.
• Use community-driven approaches to increase access to health information.
• Work through libraries and other members to support a highly trained workforce in biomedical, health information, and data science resources.

About School-Based Healthcare Solutions Network (SBHSN).

Utilizing a unique framework of funding systems offered by the Department of Health and Human Services, managed care organizations, health insurers, and private donors, SBHSN promotes a system of care model (Coaching Model℠) offering a mix of evidenced-based intervention, prevention, and care coordination services to children in grades K-12. The Coaching Model aims to expand quality mental healthcare access on public school campuses and improve children's social, emotional, behavioral, family, and wellness outcomes.

School-Based Mental Health Implementation Grant

In response to the growing number of students who need mental health counseling, the School-Based Healthcare Solutions Network (SBHSN) is accepting applications from Local Education Agencies (LEA), Public and Private Universities, State and local Colleges, Charter School Management Companies, Public Schools, Charter Schools, and Non-Profit Organizations (501c3) to implement and expand mental health program services on local school campuses. Grantees will receive direct funding and reimbursement to support the following activities: 

• Expanding access to School-Based Social and Emotional Learning (SEL).
• Coordinating mental healthcare services with school administration and staff. 
• Delivering mental healthcare services and coordinating academic-support activities to students with a history of attendance, behavior, and poor academic performance.

FUNDING

5-Years, renewable based on meeting performance goals 5-year award ceiling is $5,500,000.
