CMPRP Reinvestment Program

About CMP Funds

A CMP is a monetary penalty the Centers for Medicare & Medicaid Services (CMS) may impose against nursing homes for either the number of days or for each instance a nursing home is not in substantial compliance with one or more Medicare and Medicaid participation requirements for long-term care facilities. 

A portion of CMPs collected from nursing homes are returned to the states in which CMPs are imposed. State CMP funds may be reinvested to support activities that benefit nursing home residents and that protect or improve their quality of care or quality of life. Alaska maintains information about projects funded by CMP funds on the CMPRP website.

Funding Uses

CMP funds may be used for (but not limited to) the following:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs.

ABOUT THE TRUST

The Alaska Mental Health Trust Authority is a state corporation that administers the Alaska Mental Health Trust, a perpetual trust, to improve the lives of beneficiaries. The Trust operates much like a private foundation, using its resources to provide leadership in advocacy, planning, implementing, and funding of a Comprehensive Integrated Mental Health Program.

Beneficiaries of the Trust include the following broad groups of Alaskans with:

• mental illness
• developmental disabilities
• chronic alcoholism and other substance related disorders
• Alzheimer’s disease and related dementia
• traumatic brain injuries

The Trust also works in prevention and early intervention services for individuals at risk of becoming beneficiaries. The Trust considers prevention of these conditions, where possible, to be part of its mandate.

Behavioral Health

The Trust awards mini grants with an annual limit of one application of up to $2,500 per beneficiary depending upon need. Mini grants are only awarded to an agency on behalf of an individual.

This grant program provides Trust beneficiaries with a broad range of equipment, supplies and services to improve their quality of life, increase independent functioning and help them attain and maintain healthy and productive lifestyles. These may include, but are not limited to, therapeutic devices, access to special health care, and other supplies or services that might improve an individual’s ability to function in the community and become self-sufficient.

Deadline

Mini grants are awarded monthly. Applications must be received by the close of business on the 15th of each month. If the deadline falls on a weekend or holiday, the deadline rolls to the next business day.

What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

What is an eligible area?

Rural areas including cities, villages, townships and towns including Federally Recognized Tribal Lands with no more than 20,000 residents according to the latest U.S. Census Data are eligible for this program.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

Applicants must have held 501(c)(3) status for one year before submitting. If your organization has held 501(c)(3) status for over a year, and your believe that your organization has a new approach to an existing social problem or is addressing a previously unaddressed social issue, you are welcome to contact us and request that we consider your organization for a funding opportunity.

Mission

The mission of the Hearst Foundations is to identify and fund outstanding nonprofits to ensure that people of all backgrounds in the United States can build healthy, productive and satisfying lives. Through its grantmaking, the Hearst Foundations support well-established nonprofit organizations that address significant issues within their major areas of focus—culture, education, health and social service—and that primarily serve large demographic and/or geographic constituencies. In each area of funding, the Foundations seek to identify those organizations achieving truly differentiated results relative to other organizations making similar efforts for similar populations. The Foundations also look for evidence of sustainability beyond their support.

Culture Grant

The Hearst Foundations fund cultural institutions that offer meaningful programs in the arts and sciences, prioritizing those that enable engagement by young people and create a lasting and measurable impact. The Foundations also fund select programs nurturing and developing artistic talent. Supported organizations include arts schools, ballets, museums, operas, performing arts centers, symphonies and theaters.

Funding Priorities in Culture

In the recent past, 25% of total funding has been allocated to Culture. Organizations with budgets over $10 million have received 60% of the funding in Culture.

The Hearst Foundations are only able to fund approximately 25% of all grant requests, of which about 80% is directed to prior grantees and about 20% is targeted toward new grantees.

Michael & Susan Dell Foundation Grants

The Michael & Susan Dell Foundation only accepts unsolicited proposals for specific areas within the education, family economic stability and childhood health sectors in select countries where we work, namely the United States, India and South Africa.

As a guideline, the foundation does not fund more than 25% of a project’s budget or more than 10% of an organization’s total annual operating expenses.

The Michael & Susan Dell Foundation has always recognized the power of providing grants to partner organizations that we knew were already working hard to improve the lives of urban children living in poverty. By aligning with organizations that are already making a difference, we continue to make an immediate impact on the lives of thousands of children.

Foundation priorities:

We fund social enterprises that directly serve or impact children or youth from urban low-income communities in the areas of education, health, and family economic stability (including livelihoods and financial inclusion). These social enterprises may be structured as for-profit or nonprofit entities.

We collaborate with a range of organizations focused on creating opportunities for children and families living in urban poverty, with a deep emphasis on measuring impact. Our funding advances projects already making an impact in education, health, and family economic stability. Through these enduring and long-standing partnerships, we create lasting change together.

The Sidney Stern Memorial Trust is devoted solely to the funding of charitable, scientific, medical and educational organizations.

Social Services/ Human Welfare

We fund emergency and critical human services that support people and families in need. This includes food, emergency/transitional housing, job/life's skills training, counseling and other resources and opportunities that build economic self-reliance.

We support innovative programs that improve literacy, learning and academic success for all ages. Training, tutoring, mentoring and enrichment programs are examples of our outreach in this area.

Medical Research, Treatment & Care

We provide funding for promising medical research to aid in the understanding, treatment and prevention of diseases. The Foundation also supports hospice care as well as HIV/AIDs research and education.

Community Engagement

We nurture the spirt and well-being of Northwest communities by supporting cultural, artistic and recreational activities that engage all ages and populations. Foundation grants help sustain arts organizations and programs that express and grow the creative imagination. We also support community sports/recreational programs, centers and activities that promote health, well-being and teamwork.

Geographic Reach

With both family and business roots in the Pacific Northwest, The Foster Foundation takes a regional approach to giving. We target our funding to assist nonprofits engaged in our priority funding concerns within Washington, Oregon, Idaho, Montana and Alaska.

In addressing the founders' original intent, the Foundation will expand our philanthropy into smaller, more diverse communities within this five state area over the coming years. We will continue to support existing grantees. But, we desire to learn about and fund other pioneering initiatives and nonprofit programs that address the underserved and disadvantaged segments of this population―especially children, women and seniors.

Background

Established in 2002 and initiated in 2003, the Alaska Art Fund (AAF), formerly known as the Art Acquisition Fund, stands as a testament to the partnership between Rasmuson Foundation and Museums Alaska.

The Alaska Art Fund fosters a vibrant synergy between Alaskan artists and museums by encouraging museums to collect the works of living Alaskan artists.

In 2022, the program underwent an expansion. In additional to traditional art acquisitions, the program now supports the commissioning of outdoor art, including sculptures and murals. This evolution aims to diversify artistic expressions within museum collections and provide art that is accessible to the entire community.

About Museums Alaska

Museums Alaska supports museums and cultural centers in Alaska and enhances public understanding of their value. Museums Alaska maintains a central office to receive and disburse information about museums, cultural centers and their activities, and to collect and share professional opportunities. Museums Alaska organizes an annual conference to focus on the needs of Alaska museum professionals, volunteers, and their institutions. Museums Alaska currently administers two grant programs funded by the Rasmuson Foundation: the Art Acquisition program and the Collections Management program.

Art Acquisition Fund

The Art Acquisition Fund supports grants for Alaska museums to purchase work by practicing Alaska artists. Initiated in 2003, the project is intended to (1) support living, practicing Alaska artists through museum purchases, (2) enhance the permanent art collections of Alaska museums and (3) encourage Alaska museums to develop formal collections policies. The Foundation also provides a regular venue for contemporary Alaska art in the Foundation office in Anchorage.

Collections Management Fund

This fund was established in 2013 to develop collections management practices through professional expertise, training, and conservation materials and supplies. The program seeks applications from any department for projects that have an emphasis on collections—whether that be the behind-the-scenes work of preserving, cataloguing, and storing the collections, or public facing work of exhibitions, education, and programming.

Community Care Fund

Community care keeps us alive and plants seeds for queer and trans futures.  

Community care is the antidote for isolation, exploitation, and oppression. We create the joy, rest, pleasure, and healing we need to sustain and transform ourselves. For queer and trans people, there are infinite possibilities for how we protect and care for each other—and this fund supports our collective efforts to practice that care.  

For us, community care is any effort to respond to community threats, harms, needs, hopes, and healing. Whether it’s mutual aid, safety planning, opportunities for healing, or any number of practices—we want to help support whatever “community care” means to you.  

Guidelines

For this grant, the definition of ‘community care’ is broad; ‘community care’ is any collective efforts to protect and care for queer and trans communities.

This funding will be unrestricted (unless you want it to be restricted) .

We define “community care” as any effort to respond to community threats, harms, needs, hopes, and healing. We welcome your own definitions and practices of community care. Examples of community care include, but are not limited to:

Of community, by community, for community For decades, community groups and nonprofit organizations throughout the Northwest have worked to advance equity and justice for LGBTQ+ communities in every corner of our region. Our Community Grants Program helps fuel this work through providing consistent, long-term funding.   Since this program’s inception in 1987, we have awarded over $12 million dollars. Our most recent award cycle in 2021 was the largest in Pride Foundation’s history—$2,751,000 to 105 incredible organizations over three years. In 2021, we shifted to a proactive grantmaking model for this program, and began awarding funds without an application process. These programmatic changes were intentional shifts to center racial justice in our work, improve the experience of grantee partners, and align our grantmaking with our organizational priorities to move resources to LGBTQ+ communities most impacted by injustice. In 2024, we will have both a proactive grantmaking model and open application to ensure that all organizations have the space and opportunity to receive funding.

The Dudley T. Dougherty Foundation Vision

The Dudley T. Dougherty Foundation, "A Foundation for All", was established in 2002. It was begun in order to give a clear voice for those who wish to be a part of the many, worthy, forces for change in our world.

The Lawrence Foundation is a private family foundation focused on making grants to support environmental, human services and other causes.

The Lawrence Foundation was established in mid-2000. We make both program and operating grants and do not have any geographical restrictions on our grants. Nonprofit organizations that qualify for public charity status under section 501(c)(3) of the Internal Revenue Code or other similar organizations are eligible for grants from The Lawrence Foundation.  

Grant Amount and Types

Grants typically range between $5,000 - $10,000. In some limited cases we may make larger grants, but that is typically after we have gotten to know your organization over a period of time. We also generally don’t make multi-year grants, although we may fund the same organization on a year by year basis over a period of years.

General operating or program/project grant requests within our areas of interests are accepted. In general, regardless of whether a grant request is for general operating or program/project expenses, all of our grants will be issued as unrestricted grants.

Max and Victoria Dreyfus Foundation Grant

The Foundation will consider requests to support museums, cultural and performing arts programs; schools and hospitals; educational, skills-training and other programs for youth, seniors, and persons with disabilities; environmental and wildlife protection activities; and other community-based organizations and programs.

Alaska Kidney Foundation is committed to granting funds to organizations that are addressing the causes and treatments of kidney disease in Alaska, and supporting Alaskan residents with, or at risk of kidney disease.  Grants may be awarded to individuals, and for special projects, programs, or general operating support for established organizations.

Goals

This grant is designed to support organizations and projects that further the following goals:               

• Alaskans with kidney disease will have emergency relief for basic human needs.
• Alaskans with kidney disease will have timely kidney transplants and donations for all organs will be increased.
• Alaskans at risk of kidney disease will have early screening.
• Alaskans will be educated on the importance of preventing kidney disease.
• Alaskans with kidney disease will be knowledgeable and empowered health care consumers.

About School-Based Healthcare Solutions Network (SBHSN).

Utilizing a unique framework of funding systems offered by the Department of Health and Human Services, managed care organizations, health insurers, and private donors, SBHSN promotes a system of care model (Coaching Model℠) offering a mix of evidenced-based intervention, prevention, and care coordination services to children in grades K-12. The Coaching Model aims to expand quality mental healthcare access on public school campuses and improve children's social, emotional, behavioral, family, and wellness outcomes.

School-Based Mental Health Implementation Grant

In response to the growing number of students who need mental health counseling, the School-Based Healthcare Solutions Network (SBHSN) is accepting applications from Local Education Agencies (LEA), Public and Private Universities, State and local Colleges, Charter School Management Companies, Public Schools, Charter Schools, and Non-Profit Organizations (501c3) to implement and expand mental health program services on local school campuses. Grantees will receive direct funding and reimbursement to support the following activities: 

• Expanding access to School-Based Social and Emotional Learning (SEL).
• Coordinating mental healthcare services with school administration and staff. 
• Delivering mental healthcare services and coordinating academic-support activities to students with a history of attendance, behavior, and poor academic performance.

FUNDING

5-Years, renewable based on meeting performance goals 5-year award ceiling is $5,500,000.

Youth Cultural Activities Grant 

Children and youth are less than 25% of Alaska’s population, but they are 100% of our future. Investing in their mental health today helps our communities thrive for generations. Promoting access to cultural activities helps strengthen two of the strongest protective factors associated with youth mental wellbeing:

• a connection to one’s culture and community and
• the presence of a caring adult outside of one’s immediate family.

Promoting protective factors can also be fun! From organizing fun events to help youth celebrate cultural holidays to hosting Alaska Native youth dance groups, organizations across Alaska are finding exciting ways to promote mental wellbeing, strengthen youths’ cultural connectedness, and build healthy relationships.

Building the protective factors that strengthen youth mental health is especially important today. Alaska’s high school students report rising levels of sadness and hopelessness, and Alaska’s teen suicide rate is increasing. To strengthen mental wellbeing and increase protective factors for youth, the Alaska Children’s Trust will provide up to $1,000 to support work that engages youth in cultural activities. Project ideas include (but are not limited to) activities that:

• Engage youth in subsistence activities;
• Teach youth traditional arts and crafts;
• Promote language learning;
• Engage youth in cultural celebrations.

Applicant can apply for up to $1,000 in funding.

Brian Douglas and Wythe Dornan moved to Juneau in late 1989 to pursue their dreams. They married in 1990, and in the few short years they lived in Juneau they became involved in numerous activities in their community. Wythe worked in a pre-school day care center and was very active in NAEYC-SEA. Brian organized an ultimate frisbee club and a home brewers club in Juneau. Brian and Wythe served as foster parents for several native Alaskan children, and they also volunteered at the Glory Hole. Both supported activities to protect the environment.

Upon the death of Brian and Wythe in late 1993, at the ages of 27, the Douglas-Dornan Foundation was established in their honor. The goals of the Foundation are to support organizations dedicated to enhancing the health, education and welfare of individuals living in Southeast Alaska. Grants are awarded to applicants who demonstrate sufficient need for services, equipment, activities, and/or training that support children, youth, elders and/or other special needs groups.

Decolonizing Wealth Project is excited to launch a new culturally responsive care fund focused on supporting and expanding mental health care for youth in the United States, prioritizing access for LGBTQ+ youth and youth of color. The fund will redistribute a minimum of $15 million in grants over three years starting this year.

Throughout history, young people have been drivers of social change. They have led efforts from Selma to Standing Rock, inviting us all to imagine a better future. Youth today face a world of compounding challenges with the changing technology and social media, climate crises, income inequality, and deadly extremist violence in the United States and beyond. However, the burden of these challenges is not carried equally. Young people of color and queer youth are often forced to navigate a world that refuses to honor their brilliance and embrace their identities. As a result, BIPOC and LGBTQ+ youth are more likely to experience depression, anxiety, or even suicide.

Suicide is the leading cause of death for American Indian, Alaska Native, and Native Hawaiians/Pacific Islanders ages 10-24, and the second leading cause of death for young Black or African Americans (CDC, 2023; NAMI, 2023; ACAMH, 2023) and nearly half of all LBGTQ youth considered suicide, and 60% of those who sought mental health support were not able to access it (The Trevor Project, 2022).

One of the most significant barriers to supporting BIPOC and LGBTQ+ youth mental health is the lack of access to culturally responsive care. Youth of historically marginalized identities are in dire need of access to mental health providers who actively implement culturally responsive practices and care in affirming spaces that honor their identities. Culture also carries unique protective factors, and centering these strengths will ensure healing for both young people and society. 

The Opportunity

A Community-Aligned Fund that Centers Marginalized Youth

In collaboration with an external advisory committee composed of youth leaders and experts and practitioners in the field of adolescent mental health, DWP is launching the Youth Mental Health Fund (YMHF) – a culturally affirming initiative focused on expanding access to community-based mental health care for BIPOC and LGBTQ+ youth between the ages of 12-24. In addition to supporting community-based programs and partnerships, YMHF will also fund work to shift narratives around care and programs that offer culturally grounded healing experiences that center the unique needs and strengths of marginalized youth living at the intersections.

The YMHF fund will invest at least $15 million in grants over the next 3 years, for a minimum of $5 million annually. Alongside grantmaking, DWP will invest in capacity-building opportunities for grantee partners and elevate critical issues undergirding the Fund through communications and field-building efforts.

Fund Priorities: Culturally Responsive Care

Decolonizing Wealth Project seeks to nurture healing, belonging, and empowerment for youth. We will prioritize initiatives that are community-led or engage young people directly in decision-making processes and efforts. Examples of meaningful youth leadership may be in staffing, youth steering committees, youth boards, and processes that generate regular feedback from youth. An external, intergenerational Advisory Committee will review every eligible application.

Applications will be accepted across the following three priority areas:

• Community-Based Programs -
  • Increasing access to culturally responsive mental health care by providing services in places where youth live, learn, and play (e.g. in schools, churches, community centers, particularly in rural areas, etc). (Note: in the case of partnerships between community-based organizations and clinics/health care system units, grants will be awarded to the community-based nonprofit).
  • Youth-led organizations that provide programs to improve mental wellbeing outcomes, such as peer support, skill-building, and youth advocacy. These programs should create spaces where youth can connect, share experiences, and learn coping strategies, while advocating for changes in policies and practices that impact their mental well-being.
• Cultural-Based Healing -
  • Innovative culturally relevant delivery models that promote connection to belonging, culture, and improve mental health care. This may include programs that use creative expression such as: language, movement, art, music, dance, and theater as therapeutic tools to explore identity, process trauma, and build resilience.
  • Storytelling projects that amplify communal and personal stories of mental health journeys to foster solidarity and representation.
  • Heritage-based wellness programs that integrate cultural practices, rituals, and spirituality into mental health approaches for young people.
• Awareness and Advocacy Campaigns -
  • Community-based education programs tailored to equip youth, families, providers, and educators with tools to recognize and address mental health challenges.
  • Campaigns that challenge cultural and systemic stigmas around mental health care and identity, and work to expand access to services and support, as well as post-traumatic growth and joy.
  • Support for organizations working towards systemic change in mental health services and care delivery to dismantle inequities in mental health care systems impacting youth; Priority given to programs that train youth as advocates for systemic change in mental health policies and systems.
