What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

What is an eligible area?

Rural areas including cities, villages, townships and towns including Federally Recognized Tribal Lands with no more than 20,000 residents according to the latest U.S. Census Data are eligible for this program.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

Charitable Contributions

Costco Wholesale’s primary charitable efforts specifically focus on programs supporting children, education, and health and human services in the communities where we do business. Throughout the year we receive a large number of requests from nonprofit organizations striving to make a positive impact, and we are thankful to be able to provide support to a variety of organizations and causes. While we would like to respond favorably to all requests, understandably, the needs are far greater than our allocated resources and we are unable to accommodate them all.

Grant Applications

If the request is under consideration, you may be contacted by staff for any additional information needed. Applications are reviewed within 4-6 weeks, and decisions are made based on several factors, including: type of program; identified community need not otherwise available; indication that evidenced based data will establish measurable results of intended outcomes; community collaboration; broad base of financial support; project budget and operating expenses.

These deficiencies—if left unhindered—will continue to grow and deny further opportunities to better conditions for families today and future generations.

The Foundation for the Mid South was established to bring together the public and private sectors and focus their resources on increasing social and economic opportunity. Our approach is straightforward and long term: enable communities to develop solutions to better conditions and improve lives.

Our Priorities

The Foundation for the Mid South focuses on strengthening four priorities that, we believe, are the bedrock for prosperous communities and a strong region.

Our Education Opportunities work supports efforts that help students succeed academically—by strengthening traditional education systems and programs and community institutions serving students.

In Healthy Opportunities, we work to promote physical and mental well-being in order to comprehensively improve the health status in the region.

In order to break the cycle of generational poverty, our Economic Opportunities work is helping people become financially secure so they can move up the economic ladder.

Education

A strong foundation of education is essential not only to individuals but to the economic success of our region. Many companies struggle to find graduates with necessary skill sets. It is important that we achieve greater graduation rates, increased college enrollment numbers, and stronger workforce preparedness levels. The national high school graduation rate is 78.2%, but Mid South states have lower rates. Males of color are particularly at risk; nationally, only 52% of black males graduate from high school in four years.

Foundation for the Mid South offers enrichment opportunities and builds skills necessary for success in high school, college and the 21st century workplace. We aim to ensure that all youth, regardless of income, geography or race, receive equitable access to an effective learning environment.

We partner with individuals and organizations committed to improving high school graduation rates, college preparedness and life outcomes of students. Current areas of focus include: tutoring, coaching, career development and learning and study strategies.

Economic Opportunities

Mid Southerners make poor financial decisions because they do not have adequate financial information and training. This limits residents’ ability to acquire wealth and creates a generational cycle of poverty.

The Foundation for the Mid South will use financial competency and asset building programs to improve the financial stability of approximately 300,000 households in the region.

We provide programs and information to enable residents to manage their finances effectively and grow their wealth:

Healthy Opportunities

Improve individual and community health outcomes in chronic disease, mental health, and access to care.

Mid South residents have more health problems and lower life expectancy than those in other parts of the nation. Unhealthy behaviors and lack of health services contribute to these problems. Approximately 30% of adults and 20% of youth in the Mid South are obese, leading to high healthcare costs, lost wages, or even premature death. Approximately 1 million adults suffer from poor mental health, stressors which decrease learning or the ability to work. If not addressed, they can develop into serious mental disorders.

Foundation for the Mid South seeks to improve health for individuals and communities. Health is not a standalone issue; it is an interrelated part of improving our region, enabling people to learn effectively and improve their economic situations.

We aim to improve individual and community health outcomes through:

Mission

The mission of the Hearst Foundations is to identify and fund outstanding nonprofits to ensure that people of all backgrounds in the United States can build healthy, productive and satisfying lives. Through its grantmaking, the Hearst Foundations support well-established nonprofit organizations that address significant issues within their major areas of focus—culture, education, health and social service—and that primarily serve large demographic and/or geographic constituencies. In each area of funding, the Foundations seek to identify those organizations achieving truly differentiated results relative to other organizations making similar efforts for similar populations. The Foundations also look for evidence of sustainability beyond their support.

Whether providing a scholarship to a deserving student, supporting a rural health clinic or bringing artists into schools so children can see firsthand the beauty of the arts, the Foundations’ focus is consistent: to help those in need, those underserved and those underrepresented in society. Since the Foundations were formed in the 1940s, the scale and capabilities of the grant making have changed, but the mission has not.

Culture Grant

The Hearst Foundations fund cultural institutions that offer meaningful programs in the arts and sciences, prioritizing those that enable engagement by young people and create a lasting and measurable impact. The Foundations also fund select programs nurturing and developing artistic talent. Supported organizations include arts schools, ballets, museums, operas, performing arts centers, symphonies and theaters.

Funding Priorities in Culture

In the recent past, 25% of total funding has been allocated to Culture. Organizations with budgets over $10 million have received 60% of the funding in Culture.

The Hearst Foundations are only able to fund approximately 25% of all grant requests, of which about 80% is directed to prior grantees and about 20% is targeted toward new grantees.

Types of Support

Program, capital and, on a limited basis, general and endowment support

The Civil Monetary Penalty (CMP) Reinvestment Projects program aims to improve quality of life and care by equipping nursing home staff, administrators, and stakeholders with technical tools and assistance to enhance resident care. Applications for these CMP funds are accepted through the Louisiana Department of Health, Office of Aging and Adult Services.

About CMP Funds

CMP is a monetary penalty the CMS or the State may impose against nursing facilities when a facility is not in compliance with one or more Medicare and Medicaid participation requirements for long-term care facilities. CMP funds collected from nursing homes are deposited into the Nursing Home Residents’ Trust Fund established by Louisiana Legislature in accordance with the OBRA of 1987.

CMP Reinvestment Projects utilize these funds to support projects that benefit nursing home residents and protect or improve their quality of care or quality of life. The CMP Reinvestment Projects were created as an effort to drive improvements in quality of care for nursing home residents. The program aims to improve quality of life and care by equipping nursing home staff, administrators, and stakeholders with technical tools and assistance to enhance resident care.

Funding Uses

CMP funds may be used for (but not limited to) the following:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs.

As part of its continued commitment to the nation’s most vulnerable populations, the Centers for Medicare & Medicaid Services (CMS) has launched the Civil Money Penalty Reinvestment Program (CMPRP) to improve quality of life by equipping nursing home staff, administrators and stakeholders with technical tools and assistance to enhance resident care. CMPRP is one of several initiatives CMS has underway to strengthen safety and health outcomes for nursing home residents

Michael & Susan Dell Foundation Grants

The Michael & Susan Dell Foundation only accepts unsolicited proposals for specific areas within the education, family economic stability and childhood health sectors in select countries where we work, namely the United States, India and South Africa.

As a guideline, the foundation does not fund more than 25% of a project’s budget or more than 10% of an organization’s total annual operating expenses.

The Michael & Susan Dell Foundation has always recognized the power of providing grants to partner organizations that we knew were already working hard to improve the lives of urban children living in poverty. By aligning with organizations that are already making a difference, we continue to make an immediate impact on the lives of thousands of children.

Foundation priorities:

We fund social enterprises that directly serve or impact children or youth from urban low-income communities in the areas of education, health, and family economic stability (including livelihoods and financial inclusion). These social enterprises may be structured as for-profit or nonprofit entities.

Partnerships

We collaborate with a range of organizations focused on creating opportunities for children and families living in urban poverty, with a deep emphasis on measuring impact. Our funding advances projects already making an impact in education, health, and family economic stability. Through these enduring and long-standing partnerships, we create lasting change together.

Philanthropy is our commitment to communities in which we operate and broader society. We focus our resources on a limited number of key projects that can deliver valuable benefits from our contributions and those of our partners. We give priority to innovative, high-quality projects that meet the following criteria:

Valero Energy Foundation 

Valero, its employees and the Valero Energy Foundation are committed to improving the quality of life in communities where we work and live, fulfilling a mission that was set from the beginning. We set the standard for corporate citizenship and promotes prosperity in our communities. 

The Valero Energy Foundation -- a public charity and Valero's philanthropic arm -- funds nonprofit agencies that serve predominantly disadvantaged children and families. In 2019, the Foundation contributed appoximately $40 million in assistance in the communities where Valero has major operations.

The Founder

The Cowles Charitable Trust was first established in 1948 by Gardner “Mike” Cowles, Jr. (1903-1985). Born into the Cowles publishing family of Des Moines, Iowa, Mike was the youngest of Gardner Cowles and Florence Call Cowles’ six children. A newspaper editor and publisher by trade, he was committed to his family’s traditions of responsible, public-spirited, and innovative journalism as well as philanthropy. 

The Cowles Charitable Trust supports the arts, education, the advancement of ethical journalism, medical and climate research.

Ameriprise Financial Grantmaking

At Ameriprise Financial, giving back is deeply rooted in our culture. We’ve initiated positive change in the communities where we live and work for more than 120 years. We believe our community involvement enables us to actively live our values. Through grant making, volunteerism and employee and financial advisor gift matching programs, we support a diverse group of over 6,000 nonprofits across the country.

Meeting Basic Needs

At Ameriprise Financial, we help clients achieve financial security and peace of mind. That’s satisfying, meaningful work. We also help the people in our neighborhoods who struggle to meet basic needs such as where their next meal comes from, where they’ll sleep tonight or how they’ll find a higher wage job. We’re here to help them through the three platforms of our Meeting Basic Needs focus area.

Consideration is given to applications addressing the following: 

• Hunger
• Food banks, food shelves and food pantries, daily meal programs or meal services for the homebound
• Hunger-relief programs targeted to meet the special needs of children, ethnic populations or veterans
• Food programs run by nonprofits where hunger is not their sole focus, for example a youth meal program at the YWCA or a backpack program run by a Boys & Girls Club
• Shelter
• Emergency shelter, including youth homelessness
• Transitional housing, permanent supportive housing and efforts to end chronic homelessness
• Housing-first models (programs quickly providing housing and then addressing needed services)
• Achieving and maintaining home ownership, repair and maintenance efforts helping keep seniors, veterans and other populations in their homes 
• Adult Self-Sufficiency: Programs serving adults age 21 and older that help address the following areas:
• Basic hard and soft skills that help adults achieve economic and family stability

We believe communities should be strong, healthy and resilient. We want livable places for all, where neighbors look out for one another, cultural events are well-attended and people pull together in times of crisis and joy. Volunteer Driven Causes: Ameriprise employees and financial advisors are outstanding volunteers who serve in teams and also as individuals bringing personal skill-sets to nonprofits. Volunteering is part of the culture at Ameriprise and we are proud to support communities through contributions of both service and financial resources. 

Funding for Volunteer-Driven Causes is determined by current Ameriprise volunteerism. In general, funding is in proportion to the size of the Ameriprise volunteer team supporting a nonprofit. A team may include employees, financial advisors and/or staff or a combination of any Ameriprise volunteers. 

GPOA Foundation Grant

The GPOA Foundation is a grant-making, independent foundation which supports programs that serve children in Orleans Parish, Louisiana.

The GPOA Board has adopted the following priorities for the coming three years, in our effort to level the playing field for the most vulnerable children and youth. In brief, the outcomes we want to see are:

• Children who are ready for kindergarten
• Children who can read on-level by third grade
• At-risk Youth who are equipped for work and independence

Funding Priorities

The GPOA Foundation DOES consider funding

• Nonprofit organizations that serve children and youth in Orleans Parish
• Operational costs of programs (including staff and materials)
• Multi-year requests for three years of declining support will be considered for organizations which have clear impact objectives and an evaluation plan. However, grants will be approved a single year at a time.
• Collaborative grant request, involving up to three organizations working on a common outcome, are encouraged.

That is why we are expanding healthy choices for communities and creating more equitable health outcomes by working to eliminate the social and structural barriers to health.

Our approach

The Humana Foundation partners with local communities and trusted community organizations in Louisville, KY, Texas, Florida and Louisiana to create a network of support for people confronting life’s challenges, regardless of their age, race, ethnicity, or gender identity.

We connect people to various programs and services committed to suicide prevention, substance abuse treatment, foster grandparent programs, and more.

We collaborate with community ecosystems to advance nutritional literacy and food security so people can have consistent and reliable access to healthy food. From awareness initiatives for seniors to community food systems and food mapping pilot programs, we empower people on their health journey.

We ground our work in the needs and desires of those we serve, performing data analytics to advance collective learning and creating real, measurable impact that improves lives.

Disaster Philanthropy

The Humana Foundation allocates funding each year to provide emergency relief to communities in crisis with the goal of supporting equitable, long-term health and recovery.

Our goals

We partner with Humana Inc. to provide relief to Humana associates, facilities and Humana plan members affected by disasters. We respond to calls from national, regional or local leaders for emergency assistance. We support preparedness efforts that ensure a more equitable response to disasters.

Our approach

Disaster funds provided by the Humana Foundation, along with matching donations made by Humana Inc. associates, are primarily directed to immediate relief and long-term community health and recovery.

Additional funding may also be made available for disaster preparedness in communities that are planning and training for a more equitable response to disasters, which accounts for unique needs across age, race, ethnicity and gender identity.

Awareness. Education. Prevention.  These are three key tenets of Bayer Fund’s investments in health and wellness.  With a focus on cancer and cardiovascular disease, we provide resources to organizations dedicated to educating and/or providing services to patients, caregivers and at-risk populations.

Health & Wellness Grants

Programs that Bayer Fund will support include those that focus on patients and their families needing assistance with issues to managing cardiovascular disease and cancer. This includes education, disease awareness, and supportive services for these diseases. In areas related to cardiovascular disease, priority (though not exclusivity) will be given to charitable organizations that focus on programs and support for the African American community due to the high incidence rate of disease in this population.

Giving in this category will focus on support services such as education, disease awareness, and general access to care (e.g., housing for families traveling for cancer or heart care).

All funding requests and budgets must be for program activities and expenses that start after funding decisions are made. All programs must be completed within one year of the start date, except in limited situations where longer term programs have been agreed upon. Grant award amounts vary, depending on the size of the community, the type of programming, and the reach of the organization.

About School-Based Healthcare Solutions Network (SBHSN).

Utilizing a unique framework of funding systems offered by the Department of Health and Human Services, managed care organizations, health insurers, and private donors, SBHSN promotes a system of care model (Coaching Model℠) offering a mix of evidenced-based intervention, prevention, and care coordination services to children in grades K-12. The Coaching Model aims to expand quality mental healthcare access on public school campuses and improve children's social, emotional, behavioral, family, and wellness outcomes.

School-Based Mental Health Implementation Grant

In response to the growing number of students who need mental health counseling, the School-Based Healthcare Solutions Network (SBHSN) is accepting applications from Local Education Agencies (LEA), Public and Private Universities, State and local Colleges, Charter School Management Companies, Public Schools, Charter Schools, and Non-Profit Organizations (501c3) to implement and expand mental health program services on local school campuses. Grantees will receive direct funding and reimbursement to support the following activities: 

• Expanding access to School-Based Social and Emotional Learning (SEL).
• Coordinating mental healthcare services with school administration and staff. 
• Delivering mental healthcare services and coordinating academic-support activities to students with a history of attendance, behavior, and poor academic performance.

FUNDING

5-Years, renewable based on meeting performance goals 5-year award ceiling is $5,500,000.

RESOLVE NOLA Community Impact Grant

Understanding evident gaps in accessible mental health care and the need for the coordination of services for young individuals navigating the profound impact of community violence and collective trauma in New Orleans, United Way of Southeast Louisiana (UWSELA), in collaboration with the partners of the Resilient, Equitable Systems for Overcoming Loss and Violence Everywhere (RESOLVE) Grant, is pleased to announce the launch of the RESOLVE NOLA Community Impact Grant. This initiative is designed to address the critical shortage of comprehensive mental health resources and addiction services tailored to the unique needs of youth and their families in New Orleans. 

Under the guidance of UWSELA and the Resiliency in Communities after Stress and Trauma (ReCAST) Advisory Board Members, this grant program aims to allocate funds strategically, fostering expanded and fair access to trauma-informed community violence support for youth of color in New Orleans communities plagued by chronic poverty. The overarching goal is to establish a seamlessly coordinated system of trauma-informed, community-based services specifically tailored to mitigate the profound impacts of collective trauma and community violence on youth of color in communities facing chronic poverty. 

This grant initiative is a vital step toward building resilience and fostering positive change within the youth communities of New Orleans. 

Focus Areas

The project must focus on activities that address behavioral health disparities and social determinants of health in the areas below:

• Substance Abuse and Addiction
• Excessive Alcohol Use
• Physical and Emotional Trauma

Funding

Applicants can request grants for up to $25,000.

The Gladys Brooks Foundation was created under the will of Gladys Brooks Thayer of New York. 

Its purpose is to provide for the intellectual, moral and physical welfare of the people of this country by establishing and  supporting non-profit libraries, educational institutions, hospitals and clinics.

Scope of Grants Considered

The Foundation will consider major grant applications for innovative projects in the fields of libraries, education, hospitals and clinics.

Grants for Libraries 

Grant applications will be considered generally for resource Endowments (print, film, electronic database, speakers/workshops) capital construction and innovative equipment. Projects fostering broader public access to global information sources utilizing collaborative efforts, pioneering technologies and equipment are encouraged.

Grants for Educational Institutions

Grant applications from universities, colleges and secondary schools will be considered generally for:

• educational endowments to fund scholarships based solely on educational achievements, leadership and academic ability of the student; 
• endowments to support fellowships and teaching chairs for educators who confine their activities primarily to classroom instruction in the liberal arts, mathematics and the sciences during the academic year;
• erection or endowment of buildings, wings or additions thereto of buildings, and equipment for educational purposes;
• capital equipment for educational purposes.

Grants for Hospitals & Clinics

Grant proposals from hospitals and clinics where the proposal addresses a new health need, an improvement in the quality of health care or reduced health costs with better patient outcomes will be considered generally for:

• endowments for programs;
• erection or endowment of buildings, wings of or additions to buildings;
• capital equipment.

 AmeriHealth Caritas Foundation Grant

Mission and Focus Areas

The AmeriHealth Caritas Foundation is dedicated to preventing and mitigating adverse childhood experiences (ACEs), which will help drive positive outcomes for children's health and help set a stronger foundation for a lifetime of wellness. In addition to preventing and mitigating ACEs, it promotes protective and compensatory childhood experiences (sometimes referred to as “PACEs”), which can counteract the effects of ACEs.

Grants are made based on three pillars:

• Empowered families - Work to ensure that families have the resources they need to thrive through:

The Foundation expects to make one-year project-specific grants totaling up to $1.945 million, with awards ranging from $100,000 to $250,000.

Beaird Family Foundation Grant

The majority of our grants support qualified nonprofit organizations in the Shreveport-Bossier City area of North Louisiana. 

Our Mission

Carolyn and Charles established a Foundation that continues their work under the direction of their family’s second, third and fourth generations and an Executive Director. The Foundation awards grants to qualified nonprofit organizations, mostly in the Shreveport and Bossier City region of Louisiana. It has supported a wide range of interests, including but not limited to education, housing, health care, social services, the arts and community development. From its small beginnings in 1960, the Carolyn W. and Charles T. Beaird Family Foundation has grown, enabling its Board to award over $1,400,000 to nonprofit organizations in 2020.

The Carolyn W. and Charles T. Beaird Family Foundation is committed to improving the Shreveport and Bossier City region of Louisiana through assisting organizations to add opportunity, freedom of action and choice, self-betterment and a climate for change to the lives of the people they serve.

The Foundation’s priority is to fund organizations in, or projects which take place in, the Shreveport and Bossier City region, but it may also fund requests from other locations, especially those with the potential to be replicated in its geographic scope if proven effective.

The Foundation is particularly interested in supporting those programs which are small, local, innovative and perhaps even unpopular. The Foundation leaves to others the funding of grants to individuals and the support of large national causes such as the American Cancer Society and the American Red Cross.

Collaborative Grants

In 2014, United Way of Southeast Louisiana embarked on a journey to renew our approach to creating lasting change. The journey began with a carefully orchestrated listening process, including community conversations across our seven-parish service area. Participants discussed their aspirations and the challenges they face. Some of the barriers that emerged included low wages, limited education, inadequate health care, and unaffordable housing.

Through research and engagement, it became clear that poverty is the fundamental issue in the region.

We introduced our Blueprint for Prosperity – our plan to eradicate poverty informed by community voices and validated by external research – to present a compelling path toward a stronger, prosperous, and more equitable Southeast Louisiana for us all. As a part of the Blueprint, we also introduced our programmatic grants – designed to support distinct programs or projects – in alignment with our new prosperity outcomes framework (see below).

Today, our grantmaking is rooted in addressing the complex interplay of symptoms and drivers of poverty in the region and supports the vision of equitable communities where all individuals are healthy, educated, and economically stable.

In 2022, we hosted another series of community conversations to listen to residents and refine the most pressing issues in our communities and pathways to maximize local impact. Once again, we were able to identify the emerging barriers to prosperity for households across each of our seven parishes.

As a result, we’re launching our second cycle of collaborative grant process – rooted in equity, community voice, and data – to foster systems-level change across the region. 

Criteria

UWSELA seeks grant requests across five priority areas aligned with our Blueprint’s prosperity outcomes that we believe will produce long-term change to sustain our mission of to end poverty. In particular, we will consider proposals that focus on one or more of our four outcomes and five priority areas and align with our guiding principles.

Prosperity Outcomes

Data, community conversations, and partner convenings conducted in 2014 and 2015 resulted in four interrelated outcomes that describe the region United Way and residents want to see.

• Stability Today: Families have the skills, resources, & opportunities to access basic needs.
• Prosperity Tomorrow: All families have the social, emotional, and financial assets to create a better future.
• Personal Wellness: People of all ages enjoy a high quality of life and well-being.
• Vibrant Communities: All communities are safe, equitable, and thriving.

Priority Areas

Additional community conversations held drilled down into the four prosperity outcomes, resulting in five priority areas for each parish. Once again, we will give priority to those programs that address these areas for each parish.

• Youth Development: Investing in organizations with a primary focus on serving youth, including both academic and recreational programming, to support the learning, well-being, and prosperity of our next generation.
• Mental Health: Investing in organizations that provide direct counseling and substance abuse treatment to expand the capacity of local providers supporting those facing mental health challenges and to create a more supportive and healthy community.
• Financial Empowerment: Supporting workforce development, community-level economic development, and emergency assistance programs to provide all residents pathways to attaining the skills, opportunities, & supports that undergird economic stability for communities and families.
• Access to Resources: Investing in personnel and community-facing platforms to ensure residents are aware of and able to access existing local supports & resources.
• Coordinating & Convening: Supporting efforts to drive coordination between community-serving organizations with the goal of enhancing accountability, information-sharing, and referrals across the ecosystem, resulting in holistic services and solutions for residents.

The following six guiding principles represent the core values of our work and, by extension, the work we want to see from and with our partners:

• Lived Experience: We amplify the voices of those we are serving and allow their needs and aspirations to guide our work.

• Shared Responsibility: We believe success requires the unique contributions of the entire community, including individuals, families, schools, nonprofits, the faith-based community, funders, governments, and the private sector.
• Systems Change: We embrace our work as holistic and dynamic, impacting people, place, practice, and policy.

Funding

Eligible collaboratives must support a group of organizations and stakeholders taking a unified approach to systems change. The grants may be used for: 

• Planning: Up to $25,000 in funding to help collaboratives come together to create a steering committee, common agenda and working groups.
• Infrastructure-Backbone: Up to $50,000 in funding to support backbone organizations or functions.
• Infrastructure-Shared Measurement: Up to $50,000 in funding to support shared measurement systems.
• Implementation: Up to $100,000 in funding to support systems-change strategies developed by working groups.
• Continuous Improvement: Up to $50,000 in funding to conduct evaluations. 

UWSELA intends to be a long-term partner in supporting collaboratives. As such, at the end of a funding cycle, grantees will have the opportunity to reapply for a funding extension based on a successful evaluation of the work. Funding for the second and third year of the grant is contingent on available dollars and the funded collaborative's performance in the previous year.
