Denise Fleck Memorial Scholarship

The Denise Fleck Memorial Scholarship was established by her family to honor a courageous woman dedicated to the prevention and intervention of tobacco use by teens. Denise was instrumental in the implementation of the “Teens Against Tobacco Use” program in Kentucky and worked extensively with many health-related organizations. This scholarship fund is designed to provide scholarship assistance in the form of grants to help defray college or university expenses of students who are actively involved in community service activities for the prevention and intervention of tobacco use by their peers.

Context and Background

The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP), a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth1 .

Our Plan

Kentucky’s RHTP launches five interrelated initiatives designed to build rural health infrastructure and provide sustainable, long-term improvements. This plan directly supports Kentucky’s rural counties while advancing statewide impact through innovation, technology-enabled care and strengthened workforce recruitment pipelines for both clinical and non-clinical staff. This strategy will help build a resilient, integrated and technology-forward health system across the Commonwealth.

• Rural Community Hubs for Chronic Care Innovation:
  • Establishes local “hub-and-spoke” collaboratives focused on obesity and diabetes prevention and management.
  • These hubs will integrate nutrition, physical activity programs, and digital self-management tools.
• PoWERing Maternal and Infant Health:
  • Expands timely prenatal and postpartum care by deploying telehealth-enabled maternal care teams who will serve maternity-care deserts and high-risk regions, to help mothers and infants receive seamless, high-quality support.
• Rapid Response to Recovery (EmPATH Model):
  • Deploys technology-enabled crisis stabilization and mobile behavioral health response teams to connect individuals with community-based treatment and recovery supports.
• Rooted in Health:
  • Rural Dental Access: Increases access to preventive oral health services through expanded dental hygiene training programs, externships, and investment in Public Health Dental Hygiene (PHDH) teams in Local Health Districts (LHDs).
• Crisis to Care:
  • Integrated Emergency Medical Services (EMS) Response and Coordination: Enhances pre-hospital capacity and trauma coordination through treat-in-place protocols, improved data connectivity and workforce training for rural EMS providers. 

The Kentucky Oral History Commission (KOHC) provides grants to support oral history projects that preserve and document Kentucky's rich history. These grants include Project Grants for creating research projects, Transcription and Indexing Grants for existing interviews, and Presentation Grants for showcasing oral histories through various media. Funding amounts range from $2,500 to $7,500 depending on the grant type. These initiatives aim to foster participation and ensure the preservation of significant narratives across the state.

The Burt Monroe, Jr., Avian Research Fund, established in honor of Dr. Burt L. Monroe, Jr., provides financial support for basic scientific research on birds in Kentucky. Grants of up to $5,000 are available for KOS members, with a two-year approval period. Matching funds from other sources are encouraged. The fund aims to promote avian research within the Commonwealth, while ensuring that administrative costs are not covered, and projects receiving support from other funds in the same year are disqualified.

The Anne L. Stamm Avian Education Fund Grant supports projects that educate young people about Kentucky's birds, honoring Anne L. Stamm's legacy. Open to applicants in Kentucky aged 21 and over, it prioritizes KOS members and teachers. Grants up to $5000 are available to foster interest in bird watching and conservation among children, ensuring the continuation of efforts to protect avifauna in the region.

The Kentucky African American Heritage Commission Grant supports 501(c)3 organizations aiming to preserve and promote African American heritage in Kentucky. Established in 1994, the Commission recognizes significant contributions of African Americans to the state's history. Grants can fund various initiatives, including research, educational programs, and historic preservation projects, fostering awareness and appreciation of Kentucky's rich cultural legacy.
