Improving Health Outcomes in Patients with ATTR-CM Through the Development of a Shared Decision-Making Tool (Decision Aid)

Overview

Transthyretin Amyloid Cardiomyopathy (ATTR-CM) is a progressive, lifethreatening condition associated with significant morbidity, mortality, and burden on patients, caregivers, and healthcare systems. The multisystemic disease course is often characterized by diagnostic delays, complex treatment pathways, and evolving therapeutic options, which can create uncertainty and decisional conflict for patients and clinicians alike. Shared decision-making (SDM) has emerged as a critical component of patientcentered care1, particularly in complex and preference-sensitive conditions such as ATTR-CM. Well-designed decision aids can support meaningful dialogue between patients and healthcare professionals by improving understanding of disease trajectory, treatment options, risks and benefits, and alignment of care with individual patient values and goals.

The Marcy Shulman Memorial Organization Grant Program, established by the Colburn-Keenan Foundation, supports organizations improving the lives of individuals with chronic illnesses, particularly within the bleeding disorders community. Proposals should enhance health and quality of life, with examples including educational programs and facility upgrades. Virginia Hannah Brown Scholarship

The purpose of the Fund shall be to provide scholarships to graduating seniors from Tazewell High School located in Tazewell, Virginia, or from Richlands High School in Richlands, Virginia, who are furthering their education at any accredited two or four year college, university and/or technical school. The scholarship is primarily intended to provide financial assistance to students who have potential, but who may not otherwise qualify for other academic or athletic scholarships because of GPA or lack of involvement in extracurricular activities.

Scholarships will be sent directly to the school and will be made co-payable to the school and the student.

Scholarship amount is $3,000 per year, paid as $1,500 each semester upon meeting renewal criteria.

Each scholarship is renewable for up to four (4) years if the student remains enrolled in an accredited college, university or technical school; completes fifteen (15) credit hours each semester; and maintains a 3.0 GPA.

We administer a number of scholarships, which range from the traditional academic and athletic awards to corporate and special circumstances offerings. These scholarships are open to all residents of Marshall County, as long as other requirements are met.

John Ruff Scholarship Fund

The purpose of the fund is to provide one or more annual scholarships to students (children or grandchildren) whose families are currently policyholders with the Farmers Mutual Insurance Co. of St. Joseph and Marshall Counties. 

The scholarships will be used to fulfill educational needs to post secondary education to include four year institutions, two year institutions, and vocational trade and technical career areas.

The John Ruff Scholarship Fund is not renewable annually to previous recipients.

Obermier Scholarship Fund in Memory of John N. Obermier, Edith M. Obermier and Mary Belle Obermier

Purpose

Mary Belle Obermier established this scholarship with a gift from her estate in 2012. Her intent was to help academically deserving students who need financial assistance to be able to attend college and to give preference to qualified students who are committed to a career in the United States Navy. The preference for naval students is in recognition of the US Navy careers of her deceased husband, John, and his deceased first wife, Edith, who were both veterans of World War II.

Criteria:

• Applicants must demonstrate financial need (economically disadvantaged).
• Applicants must have a current academic GPA of 3.0 or higher.
• Applicants must be enrolled/accepted to an institution of higher learning prior to applying.
• Applicants must submit one letter of recommendation from a teacher, employer, or clergy.

Preference in the selection process will be given in the following order to qualified students who are:

The AE Foundation Community Grants aim to support US and Canadian non-profit organizations focused on enhancing the mental health and well-being of young people. Eligible organizations must be certified 501(c)3 non-profits or registered charities, and grant funding should be utilized within 365 days of receipt. Marshall High School Scholarship

The Alison Shuemake Scholarship is for a Marshall High School senior and graduates pursing an academic or technical degree or certification.

Applications are open year-round. After we receive all application requirements, we internally review applications and make decisions within two weeks of receipt. An email will be sent to the applicant to complete follow up requirements.

Scholarship payments are sent directly to the school.

The Christina Sue Lilja Resource Grants support agricultural literacy by providing funds for educators to purchase resources from the Ag Foundation store. In 2025, 100 educators will receive $100 in store credits, selected randomly if applications exceed this number. This initiative, backed by the CSL Foundation, honors Christy Lilja's legacy of advocating for agricultural education. The grants aim to enhance classroom resources and improve students' understanding of agriculture, ensuring access to accurate information.

The Lane E. Warren Scholarship Fund, managed by the Northern Piedmont Community Foundation, offers a $1,000 award to a Madison County resident who is a high school graduate or graduating this year. This scholarship honors Lane E. Warren, a promising student whose life was tragically cut short. • One scholarship will be awarded on a one-time basis and disbursed in August.

Amount:

• No less than $300 depending on available funds.

Proposals are invited for activities related to schizophrenia, bipolar disorder, narcolepsy, and idiopathic hypersomnia. Funding is available for various CME formats, focusing on practical training and understanding treatment complexities. Eligible applicants include U.S.-based organizations that are either non-profit or accredited CME providers. The Andee Jorgensen Scholarship, managed by the Renton Regional Community Foundation, supports students from King County demonstrating financial need and resilience against personal hardships. With an award of $1,000, this scholarship honors civic leader Andee Jorgensen and is available for high school, undergraduate, and graduate students. Preference is given to those engaged in community service. The foundation aims to foster education and empower local youth to pursue their academic ambitions while overcoming challenges.
