Community Health Worker Hub

The New Jersey Department of Health (NJDOH), Division of Family Health Services (FHS), Reproductive and Perinatal Health Services, announces a competitive request for proposals (RFP) to continue the New Jersey Community Health Worker Hub (CHW Hub).

The CHW Hub will be an organization that has demonstrated experience and deep expertise working with community-based organizations; expertise in health and social sectors; experience working directly with managed care organizations (MCOs) in NJ on supporting community health programs and working with NJ health care benefit systems; and strong understanding of the allied health landscape. The CHW Hub will be responsible for ensuring NJ has a strong CHW workforce by working with organizations across the state in continuing the ongoing certification efforts for CHWs and sustainability for the CHW workforce. The CHW Hub will also actively support and advocate for the integration of CHWs into standard healthcare practice. The CHW Hub will summarize feedback acquired from interaction with the community (e.g., community members, CHWs, CBOs) and share with the NJDOH for their awareness and action. 

The CHW Hub will have authority granted by the State to award subcontracts to appropriate organizations

The Community Health Worker (CHW) Hub NOFA, funded by the New Jersey Department of Health, aims to enhance the CHW workforce across the state. The initiative focuses on certification efforts and integration of CHWs into standard healthcare practices. Eligible applicants include municipal governments, higher education institutions, hospitals, and non-profits. The program supports public health improvements and emphasizes the importance of timely reporting and progress toward grant objectives to ensure continued funding.

The Korean American Community Foundation (KACF) offers Community Grants to support organizations in New York and New Jersey addressing economic security for low-income Korean and Asian Americans. Grants of up to $100,000 are available for projects focused on education, workforce development, and community advocacy. Organizations must be registered 501(c)(3) nonprofits with a minimum operational history of six months. KACF prioritizes comprehensive support and measurable outcomes in its funding decisions.

The SNAP STEPS program, initiated by the New Jersey Department of Labor and Workforce Development, aims to empower SNAP recipients by providing essential training, career services, and employment opportunities. Through strategic partnerships, the program focuses on developing skills and creating pathways to self-sufficiency. Participants receive case management and supportive services to facilitate their transition into unsubsidized employment, ultimately enhancing workforce participation and community engagement across New Jersey.

The UPSKILL: NJ Incumbent Worker Training Grant Program aims to assist New Jersey employers by reimbursing up to 50% of training costs for current frontline employees, helping them acquire essential skills for future roles. Open to all businesses, the program particularly targets nine strategic sectors outlined in the Governor's Economic Development Plan, including Advanced Manufacturing, Clean Energy, and Technology. This initiative supports workforce development and enhances New Jersey's economic competitiveness.

The NJ Women and Minority Veterans in Construction Trades Grant Program aims to eliminate economic barriers for women and minority veterans by providing essential training and support for employment in construction trades. This initiative connects military members with quality career opportunities, ensuring they receive the skills necessary for success in registered apprenticeship programs and other industry occupations, ultimately enhancing New Jersey's workforce and economy.

The NJ Pre-Apprenticeship in Career Education Grant supports individuals preparing for Registered Apprenticeship programs in New Jersey. By partnering with at least one Registered Apprenticeship sponsor, successful applicants will help participants gain industry-specific skills and classroom instruction. This initiative aims to enhance career pathways, drive economic development, and promote educational attainment, ultimately leading to better job opportunities and higher wages for those disconnected from the workforce.

The Community Progress Makers Fund, an initiative by the Citi Foundation, aims to empower local organizations by providing $50 million in unrestricted funding. This program, established in 2015, will support 50 selected community organizations over three years with grants of $1 million each, alongside technical assistance and opportunities for collaboration. The fund prioritizes organizations with strong community ties and a proven impact in areas such as affordable housing, economic development, financial health, and workforce readiness.

The Youth Transitions To Work (YTTW) Grant Program, led by the New Jersey Department of Labor and Workforce Development, aims to support high school juniors, seniors, and out-of-school youth in transitioning to high-skill, high-wage employment. By facilitating connections to labor demand occupations, the program promotes long-term career growth and encourages youth to pursue further education and lifelong learning opportunities. This initiative strengthens the workforce and fosters economic development in New Jersey.

The Programmatic Support - Education Pillar Award by Honda aims to enhance workforce readiness in the manufacturing sector. It supports nonprofit organizations and schools focused on hands-on programs in design, software/data analytics, and AI/machine learning. Eligible applicants must be US-based and have 501(c)(3) status. The initiative prioritizes projects that train individuals for manufacturing roles, develop technical skills, and introduce automation to prepare for evolving industry demands, ultimately fostering economic prosperity in local communities.

The NJ Growing Apprenticeship in Nontraditional Sectors (GAINS) Grant aims to foster the growth of Registered Apprenticeship programs approved by the USDOL. This initiative is designed to enhance economic development by promoting skills and educational attainment, ultimately leading to better-paying careers. With a funding amount of $5,000,000, the program encourages participation across a variety of sectors, welcoming all apprenticeable occupations to apply and contribute to New Jersey's workforce development.

The Women and Minorities in Construction Trades Program Grant aims to support women and minority veterans in overcoming economic barriers to access training in construction trades. This initiative offers funding for eligible organizations to provide comprehensive training, placement services, and support to prepare these veterans for successful careers in the construction industry. With a funding amount of $1,500,000, the program is focused on enhancing workforce diversity and inclusion within the sector.

Rural Health Transformation Program_Advancing Technology, Prevention, and Workforce Capacity

The Rural Health Transformation Program 2026: Advancing Technology, Prevention, and Workforce Capacity focuses on expanding healthcare access, strengthening preventive services, integrating technology-driven solutions, and supporting training and capacity building to improve health outcomes in rural New Jersey. 

The NJ Citizenship and Integration Program aims to enhance citizenship preparation services for lawful permanent residents. It focuses on improving English proficiency, U.S. history knowledge, and civic understanding while providing assistance for naturalization applications. With a funding limit of $80,000, the program supports initiatives that promote cultural integration, leadership development, and access to essential legal services, fostering inclusive communities throughout New Jersey.

The NJ Hispanic Entrepreneurship Assistance Grant Program supports the growth of Hispanic businesses by providing culturally relevant education and mentorship. Managed by the Center for Hispanic Policy, Research and Development (CHPRD), this initiative aims to empower new entrepreneurs and enhance community development. Eligible applicants can seek funding for multiple programs, focusing on enhancing public-private partnerships and training opportunities. Applicants can seek funding from multiple categories including health and wellness initiatives, aiming to address domestic violence and mental health challenges effectively.

The Pathway to Recovery grant, funded by the New Jersey Department of Labor and Workforce Development, aims to combat the opioid epidemic by providing workforce development and training services to individuals affected by opioid use. With an estimated funding of $5 million for FY25, the program seeks to enhance employability through skills training, career services, and supportive resources. This initiative is essential for fostering recovery and promoting economic stability within communities impacted by the opioid crisis.

The MGM Resorts Foundation Community Grant Fund supports nonprofit agencies providing essential services in areas such as affordable housing, education, and health. Grants are awarded for a one-year period and focus on regions where MGM Resorts employees live and work. Eligible applicants must comply with diversity policies and serve individuals or families. The foundation does not fund governmental agencies or projects exclusively for medical research. This initiative aims to enhance community well-being through targeted support.
