Connecting Families: Grantees to Provide Support Services to Families of Children with Disabilities

Objective of RFP

The Minnesota Department of Human Services, through its Disability Services Division (STATE), is seeking Proposals from qualified Responders for the Connecting Families Project: to provide support to Minnesota families of children with disabilities through individual and group support services, resource connections and education on Charting the LifeCourse (CtLC). The term of any resulting contract is anticipated to be for up to one (1) year.

To be considered under this RFP, Responders must be a current Minnesota family support organization serving families of children with disabilities residing in Minnesota and must demonstrate a clear understanding of the needs of these families. The leadership of this project will include a parent of a child with a disability, ensuring that family perspectives are embedded in program planning, decisionmaking, and service delivery.

To meet these needs, the STATE is offering grants for the following two goals:

• Family Connection and Support: Reduce isolation by providing individual and group support and linking families to essential resources.
• Empowering Families through Charting the LifeCourse (CtLC): Introduce and guide families and professionals in using the CtLC framework and tools to envision a good life, identify supports, and engage in meaningful planning.

The scholarships we award vary each year based on the unique criteria of each scholarship. In 2025, we awarded over $3 million in scholarships to over 500 students, including $1.3 million to 110 students in western NC through our Disaster Relief and Resilience Scholarship .

Depending on the scholarship, it may pay for tuition, room and board, books, fees and/or other related educational expenses.

Hermann K. Brauer Memorial Scholarship Fund

Students must be preparing for a professional career in medicine, dentistry, nursing, engineering or other science, teaching or law. The scholarship will not be used for career preparation in veterinary medicine, funeral direction, mortuary science, or in the ministry. The student must demonstrate clear financial need. Edward A. Bouchet Award

This award promotes the participation of underrepresented minorities in physics by identifying and recognizing a distinguished minority physicist who has made significant contributions to physics research and the advancement of underrepresented minority scientists. The program will help publicize the lecturer's work and career development to the physics community, especially among minority physics students. The Lectureship consists of a stipend of $5,000 plus support for travel to an APS meeting where the recipient will receive the award and give a presentation. In addition, the lecturer should arrange to give up to three additional talks at academic institutions where the impact of the visit on minority students would be significant. The purpose of these visits would be to deliver technical lectures or lectures about his/her specialty, to visit classrooms when appropriate, to assist the college/university in pre-college outreach efforts where appropriate, and to talk informally with both faculty and students about research and teaching careers in physics. Financial support will also be provided for travel to these lectures. It will be presented annually.

The Springfield Women’s Club Scholarship Fund, administered by the Community Foundation of Western Massachusetts, aims to support women over 19 from Hampden County pursuing two- or four-year degrees. Eligible students can apply through a single online application that matches them with various scholarships, providing up to $3,000 in financial assistance. This initiative encourages women to further their education while addressing the unique needs of the community.

The Springfield Teachers Club Scholarship Fund, managed by the Community Foundation of Western Massachusetts, supports graduates of Springfield public high schools residing in Franklin, Hampden, and Hampshire Counties. The fund offers scholarships up to $3,000 to eligible students enrolling in accredited post-secondary schools. Eligibility includes students planning to enroll in accredited post-secondary institutions in the U.S., primarily from Franklin, Hampshire, and Hampden Counties.

The Lucius H. Tarbell and Dorothy J. Tarbell Scholarship Fund, administered by the Community Foundation of Western Massachusetts, provides financial support for students attending Western New England University. The Alice McCaffrey Scholarship Fund, established in 2013 by Saints John and Paul parishioners, honors Father Joseph McCaffrey on his 25th priestly anniversary. Named after his mother, Alice, the fund supports seminarians pursuing pastoral experiences beyond seminary training. Over the past four years, it has awarded $69,000 to nine seminarians, enriching their training by engaging in mission work across Brazil and Mexico. Applicants will be notified of their awards in April to plan their summers accordingly.

Scholarships

The CBCF scholarship is open each year. Scholarships awarded will be for the following academic year. Scholarships are available only to students who graduated from a high school in one of the counties in the CBCF Service Area: Aransas, Bee, Jim Wells, Kleberg, Nueces, Refugio and San Patricio. Thanks to our generous donors, the Coastal Bend Community Foundation (CBCF) offers scholarships to students from over 100 funds. 

Flinn - Dorothy and John Flinn Memorial Scholarship

This scholarship is exclusively available to a Sinton or Taft ISD graduating senior, with a minimum 2.0 GPA, and the student must complete an essay on the applicant’s community service.

Booker T. Washington Scholarship

For African-American graduating high school seniors from Saline County High School.

Background

The Booker T. Washington scholarship fund was started with the proceeds from the sale of the former Booker T. Washington Negro Center, located at 345 N. Front. It was designed for African American children and was founded as a private enterprise by Miss Janet Porter in 1939. After her death in 1943, the center was operated by the Porter family with aid from the Community Chest. In 1978 the City completed construction of the Carver Center, and the Porter family felt the Booker T. Washington Center was no longer needed. They closed the center, donating all the equipment to the Carver Center, and the property was given to the YWCA. The Y sold the property and used the proceeds to establish the Booker T. Washington Scholarship Fund.

Scholarship recipients are eligible to apply for renewal by meeting the following:

• Recipient must be enrolled as a full-time student and provide a copy of their transcript.
• Recipients must be in good standing and maintain a minimum 3.0 cumulative GPA.
• Recipients may receive this scholarship for a total of 4 years.
