Rural Health Transformation Program: Building Rural Hospital Capacity

The RHT Program is designed to provide funding for investments that will transform and sustain the way care is delivered in rural communities, specifically to improve access and outcomes, especially for individuals who are uninsured and underinsured. 

Scope of Work

The NJDOH’s Rural Health Transformation Program 2026: Building Rural Hospital Capacity focuses on increasing hospitals’ capacity to provide healthcare services to New Jersey’s rural population. The goal is to build long-term capabilities that will serve the target population for years, even after the program’s funding has ended. To achieve this goal, the program supports targeted initiatives that could include improvements in rural healthcare, workforce development and increased accessibility of healthcare services.

By increasing accessibility, supporting improvements and workforce development, the funding will help strengthen the overall health of rural New Jersey communities, in line with the DOH’s vision of making New Jersey the healthiest state in the union. 

Applicants are not required to serve exclusively uninsured or underinsured populations across their entire organization, and programs or positions supported by this grant may interact with or serve insured individuals. However, grant funds may not be used to pay for or support any services or activities that are reimbursable by Medicaid, Medicare, or private insurance. 

The Susquehanna Riverlands Conservation Landscape Mini-Grant Program offers competitive grants ranging from $2,000 to $20,000 for projects enhancing habitat connectivity, sustainable river access, and community collaboration. This initiative is part of a collaborative effort to protect natural resources and improve quality of life along the Susquehanna River in Pennsylvania. Breast Cancer Screening & Prevention Initiative

The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. By enhancing early detection and preventive care, the initiative seeks to reduce barriers to care and improve health outcomes for women who might otherwise go without critical cancer screening services.

The United Way KGC Impact 100 Grant seeks proposals from local nonprofit organizations that address critical health and human service needs in the Kansas City region. Selected organizations will receive a one-year unrestricted grant, enhancing their visibility and connections within the United Way network. This initiative emphasizes diverse approaches in Health, Education, and Financial Stability, ensuring a comprehensive impact across various community sectors.
