Request for Proposals for Grantees to Develop and/or Provide Services for Older Minnesotans to Live in Community – Live Well at Home Grants

Objective of RFP

The Minnesota Department of Human Services, through its Aging and Adult Services Division (STATE), is seeking Proposals from qualified Responders to develop and/or provide services for older Minnesotans to live and age in the community of their choice. The STATE aims to improve their community’s capacity to develop, strengthen, integrate, and maintain culturally competent home and community-based services (HCBS) for individuals aged 65 and older who are at-risk of long-term nursing facility use and/or spending down into Medical Assistance eligibility. Proposals may include services that support solo agers, family, friends, and neighbors’ caregiving.

The Live Well at Home℠ program aims “to help older adults live well at home by focusing strategic investments to prepare Minnesotans for 2030.” The grants in this RFP are intended to stimulate innovation by providing one-time, start-up funds to test new approaches in housing and HCBS development, and to develop and support core HCBS providers.

FWP Original Off-Highway Vehicle Grant Program

Program Specifics

The Off-Highway Vehicle Grant Program provides project funding with the goal of enhancing and maintaining OHV recreational opportunities for the benefit of OHV enthusiasts in Montana. It provides grant funding for safety, ethics and education, resource maintenance, protection and improvement projects.

Montana State Parks administers the OHV grant program with funds appropriated by the State Legislature, and is assisted in grant allocation by the Off-Highway Vehicle Advisory Committee (OHVAC). The OHVAC is comprised of citizens throughout Montana who have a vested interest in managing OHV opportunities as a valuable resource. 

The OHV Grant Program awards approximately $150,000 in grant funds annually. Applications for trail steward positions are limited to a $20,000 grant request. All other projects do not have a maximum award amount and will be subject to the review panel and OHVAC funding decisions.

• Have a cumulative unweighted GPA of 2.5 or higher on a 4.0 scale
• Have a demonstrated financial need
• This opportunity is open to all applicants, with a focus on those furthest from opportunity and having a demonstrated financial need.

• Have parents who did not attend college
• Show a strong commitment to their community as demonstrated by their involvement in extra-curricular activities and/or community or faith-based service
• Not major in political, social, ethnic studies, and advocacy work

• Plan to major in finance (San Diego only) or computer science or information technology
• Have a cumulative unweighted GPA of 2.5 or higher on a 4.0 scale
• Open to all applicants, with a focus on advancing gender equity in careers in technology
• If selected as a recipient of the Evotek 2026 Technology Scholarship, recipients will be invited to meet monthly with a mentor and, as needed, attend foundation/press events and speak to their experiences as technology majors.

• Have parents who have not received a college degree
• Have a demonstrated financial need

Theo and Evelyn Yakel Scholarship applicants should:

• Be graduating high school seniors who are children, grandchildren and great-grandchildren of members of the Retired Employees of San Diego County, Inc. (RESDC)
  • Letter from the Retired Employees of San Diego County, Inc. (RESDC) on official letterhead verifying that you are a child, grandchild or great-grandchild of a RESDC member is required.
• Plan to enroll full-time at a community college or four-year university in the United States in fall
• Have a cumulative unweighted GPA of 3.0 or higher on a 4.0 scale
• Be United States citizens

Mission

The Biodiversity Fund supports efforts to maintain and strengthen biodiversity in the Duluth-Superior region through preservation and restoration of habitat, help for particular species and ecosystems, planning for changing conditions, research and education. The purpose is to consider now the value to future generations of the species and ecosystem diversity that will remain when/if human population stabilizes.

Biodiversity Fund

The Biodiversity Fund supports projects that preserve and restore habitats, assist vulnerable species and ecosystems, plan for environmental change, and promote research and education in the Duluth-Superior region.

The fund aims to protect the region's biodiversity through conservation, preservation, and restoration of natural resources for the benefit of future generations.

Biodiversity Fund- Large & Multi-Year Grants

The Fund may also support larger initiatives of up to $50,000 per year for up to three years, for projects that require sustained investment to achieve meaningful, long-term impact. ‍

Multi-year requests should demonstrate:

• A clear long-term vision with defined milestones that allow progress to be assessed prior to subsequent years of funding each year
• How the work will scale, adapt, or deepen impact over time
• Strong partnerships, stewardship plans, or systems-level outcomes
• A plan for sustainability beyond the grant period

What We Mean by Biodiversity

‍Biodiversity is the variety of life on Earth, encompassing the diversity of genes, species, and ecosystems and the complex relationships that sustain them. Biodiversity underpins ecosystem stability, climate resilience, and human well-being by providing essential services such as clean air and water, natural food systems, nature-derived medicines, and climate adaptation and regulation.

This grant recognizes that healthy natural ecosystems and sustainable native plant and animal communities are deeply interconnected — environmental degradation often exacerbates social inequities and instability of communities of habitats and ecosystems. Community-led solutions strengthen ecological outcomes.

Biodiversity Fund Priorities

Funded projects should demonstrate strength in several of the following areas. Not every project must address all principles, but competitive proposals will show clear alignment across multiple dimensions.

• Upstream & Preventative Focus
  • Projects address root causes rather than symptoms.
  • Prioritize prevention, restoration, and long-term solutions
  • Reduce risk and vulnerability for people, species, and ecosystems
  • Anticipate environmental and social change rather than reacting after harm occurs

  • Projects are grounded in authentic partnership.
  • Build cross-sector collaboration (e.g., nonprofits, Tribal Nations, schools, governments, researchers, community groups)

• Be graduating high school seniors from a public high school in North San Diego County
• Plan to attend full-time a public community college or university in California majoring in education OR
• Be students who have entered or have been accepted into their fifth year of full-time schooling at a public university in California with the goal of achieving a teaching credential in the state of California
• Have a minimum unweighted GPA of 3.0 on a scale of 4.0
• Have shown a commitment to their community through meaningful community service

The SD2 Scholarship

The SD2 Scholarship applicants should:

• Be graduating high school seniors or current community college students
• Plan to enroll or transfer to an accredited four-year university in the US in fall
• Have a cumulative unweighted GPA of 3.0 or higher on a 4.0 scale (with rare exceptions for extenuating circumstances)
• Plan to major in science, technology, engineering or mathematics
• Demonstrate an interest in the advancement of historically underrepresented populations in the sciences and related fields
• Students from underserved and underrepresented backgrounds are highly encouraged to apply
• Have a demonstrated financial need
• Open to those applicants above with a focus on reducing historical educational disparities for those furthest from opportunity

Expanded Wisconsin Fast Forward Grants

The objective of the Wisconsin Fast Forward (WFF) expanded grant program is to award funds that reimburse the costs of workforce development activities, as specified in the Grant Program Announcement (GPA).

Teacher Training and Recruitment Grants

Community Scholarship Program

Since 1997, the Community Scholarship Program has awarded $59.5 million to more than 15,400 students, making our program the largest non-university scholarship provider in San Diego County. By offering financial assistance, the program helps students dedicate more time to their studies, stay enrolled and complete their degrees.

The Veronica Zerrer and Michelle Dungan Scholarship Fund

The Veronica Zerrer and Michelle Dungan Scholarship Fund applicants should:

• Have resided in San Diego County for at least one year
• Plan to enroll full-time at a four-year university, graduate school, community college, or career/technical school in the United States
• Have an unweighted GPA of 2.5 on a 4.0 scale
• Have a demonstrated financial need
• Open to all San Diego County applicants, with an effort to reduce educational disparities for those who have faced barriers related to sexual orientation, gender identity or gender expression.

Competitive Community Grant- Health & Human Services- Spring Cycle

All people are healthy and safe.

We will support organizations that work to improve the following outcomes in our communities:

• Access to affordable housing
• Food security
• Safety
• Access to health care
• Mental health services
• Healthy neighborhood living conditions
• Senior services

Through direct service, policy advocacy, and/or collaboration efforts, priority strategies will address:

• Affordable housing and shelter for homeless, rent-burdened, and at-risk populations, including eviction prevention
• Access to affordable and high-quality food, including food banks, pantries, and community centers that distribute food and promote healthy eating and work to address “food deserts”
• Efforts to address safety for vulnerable populations, including domestic violence and child abuse prevention, bullying prevention in schools, safety issues specific to immigrant populations, low wage workers and communities of color
• Access to health care, including programs providing un- and under-insured populations with culturally appropriate and equitable access to quality health and wellness programs services. Priority will be given to efforts applying social determinants of health lens to the work
• Mental health services, including school-based services, treatment, prevention, and rehabilitation programs targeting marginalized communities, and groups working on cultural and generational trauma
• Building healthy communities, including built environment and infrastructure, environmental quality (toxins and air/water quality), affordable and reliable transportation, and parks and recreation
• Providing services targeted at senior well-being, including memory care, depression, audiology, dementia, exercise, home care, hospice, and medication guidance.
• Systems change, including public policy, civic engagement, community organizing or public information to improve health and wellbeing of local residents

Grant Sizes

We offer $5,000, $10,000, $15,000, or $20,000 general operating support depending on your annual budget:

• Organizations with an annual budget up to $50,000 may apply for a $5,000 grant;
• Organizations with an annual budget between $50,001 - $150,000 may apply for a $10,000 grant;
• Organizations with an annual budget between $150,001 - $500,000 may apply for a $15,000 grant; and
• Organizations with an annual budget of $500,001 or more may apply for a $20,000 grant.

Tom and Terry Lewis Scholarship

Tom and Terry Lewis Scholarship applicants should:

• Be graduating high school seniors
• Plan to enroll full-time at an accredited four-year university in San Diego County in fall
• Have a cumulative unweighted GPA of 3.0 or higher on a 4.0 scale
• Plan to major in science, technology, engineering or mathematics
• Be a United States citizen
• Have a demonstrated financial need
• Special consideration may be given to students who have participated in extracurricular activities or community service, or have work experience.

The Twomey Family Scholarship, managed by The San Diego Foundation, supports current or former foster youth pursuing higher education. It aims to assist students attending community colleges, universities, or graduate schools in the U.S. Applicants must have a minimum GPA of 3.0 and demonstrate financial need and community commitment. Since 1997, the Community Scholarship Program has awarded over $59.5 million to help students achieve their academic goals.

Community members are aware of, engaged, and involved in issues that affect them.

We will support organizations that work to improve the following:

• Workforce development/job creation
• Support of hourly workers
• Open government
• Civic engagement
• Social and economic justice

Through direct service, policy advocacy and/or collaboration efforts, priority strategies will address:

• Improving access to employment opportunities and economic advancement by low-income and other disadvantaged community members
• Providing career counseling, job training, and other programs to low wage earners to advance their skills
• Encouraging residents to participate in leadership, policy, and funding decisions through workshops, public forums, and leadership training
• Support public policy, civic engagement, community organizing or public information to improve and strengthen local economy

Grant Sizes

We offer $5,000, $10,000, $15,000, or $20,000 general operating support depending on your annual budget:

• Organizations with an annual budget up to $50,000 may apply for a $5,000 grant;
• Organizations with an annual budget between $50,001 - $150,000 may apply for a $10,000 grant;
• Organizations with an annual budget between $150,001 - $500,000 may apply for a $15,000 grant; and
• Organizations with an annual budget of $500,001 or more may apply for a $20,000 grant.

Rhoades to Justice Scholarship Fund

The Rhoades to Justice Scholarship supports determined students from Kansas City public schools who are passionate about justice and committed to pursuing college despite obstacles. Founded by Gary and Christina Rhoades, with generous support from friends and family, the scholarship honors their belief in justice, opportunity, and service. It is inspired in part by the people Gary has met in his own journey from public schools and Kansas City roots to a law career dedicated to advancing civil rights and social justice.
