The Indiana Department of Health (IDOH) accepts applications for CMP funds on the following schedule: March 1 - April 30, July 1 - August 31, and November 1 - December 31, every year. 

About CMP Funds

A CMP is a monetary penalty the Centers for Medicare & Medicaid Services (CMS) may impose against nursing homes for either the number of days or for each instance a nursing home is not in substantial compliance with one or more Medicare and Medicaid participation requirements for long-term care facilities. A portion of CMPs collected from nursing homes are returned to the states in which CMPs are imposed. State CMP funds may be reinvested to support activities that benefit nursing home residents and that protect or improve their quality of care or quality of life.

Funding Uses

The following examples of allowable uses of CMP funds are not all-inclusive. Please see the Allowable and Non-Allowable Uses of CMP Funds download for more information. CMP funds may be used for:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs
• Training to improve quality of care
• Activities to improve quality of life, foster social interaction, and movement and minimize loneliness
• In-person visitation aids

The Community Health Partnerships (CHeP) Trailblazer Award invites proposals for community-engaged research projects aiming to enhance health equity and address social determinants of health. With funding up to $25,000 over two years, the award supports collaborative efforts between university and community partners. Key focus areas include substance use, obesity prevention, and health disparities in rural areas. Projects should demonstrate sustainability and community engagement, ensuring all participants are committed to improving health outcomes in Indiana.

The Community Foundation of Noble County offers a wide range of scholarships to support local students in achieving their academic and career aspirations. 

Noble County Retired Teachers’ Association Scholarship Fund in Memory of Deceased Teachers

For a resident of the West Noble School District who will have a high school diploma or equivalent by the end of June 2026, is or will be enrolled in a certified 1-,2-,3- or 4-year Indiana post-high school program (the award is given in alphabetical rotational distribution among the three school corporations of Noble County.) Pays $500 one-time.  

The Jay Lampton Memorial Scholarship, managed by the Community Foundation Alliance, supports graduating seniors from various Indiana high schools pursuing post-secondary education in auto mechanics. Applicants must demonstrate financial need and have a minimum 2.0 GPA. Selection criteria include academic performance, a letter of recommendation, and a required essay. This scholarship honors Jay Lampton's legacy through education, empowering students to advance their careers in technology.

The Indiana Office of Community and Rural Affairs works with local, state and national partners to provide resources and technical assistance to aid communities in shaping and achieving their vision for community and economic development.

CDBG Construction Grants

Funding for all Community Development Block Grant (CDBG) programs is provided by the U.S. Department of Housing and Urban Development and administered by the Office of Community and Rural Affairs. The State of Indiana distributes CDBG funds to rural communities to support a range of projects, including infrastructure enhancement, downtown revitalization, improvements to public facilities, and economic development initiatives.

The state CDBG program offers competitive grants to units of local government to support rural areas. Within the CDBG program, there are five types of Construction Grants designed to enhance community resilience and livability:

Two rounds of Construction Grants are offered each year, with specific funding guidelines detailed in each round’s application Instructions document. 

Facilities Program

Community facilities enhance the lives of residents in numerous ways. In addition to community facilities, historic preservation projects are eligible for the Facilities Program grant. The goals of the Facilities Program are to support rehabilitation and restoration of historic buildings and public facilities. A local match of 10% of the total project cost will be required for consideration of funding.

The CDBG Construction Grants - Stormwater Improvements Program supports rural communities in Indiana by providing funding for stormwater management improvements. This program aims to mitigate flooding and enhance community resilience through necessary infrastructure upgrades. With a focus on sustainable practices, the grants encourage the use of green infrastructure to protect waterways. A 10% match of total project costs is required. Two application cycles are held annually, streamlining access to essential resources for community development.

The PreservINg Main Street Grant, initiated by the Indiana Office of Community and Rural Affairs, aims to enhance local economies through the preservation of historic properties in Indiana. This program supports eligible communities with funding up to $2 million for downtown revitalization projects, alongside opportunities for capacity building in partnership with Indiana Humanities and Indiana Landmarks. It offers a sustainable approach to historic preservation while fostering community engagement and economic development.
