Rural Health Transformation Program Funding Opportunity For Public Health Dental Hygiene Teams in Local Health Departments 

About the Program

This project is 100% funded by the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS. The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics.

The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP) — a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth.

Rooted in Health: Rural Dental Access

A key component of Kentucky’s Rooted in Health: Rural Dental Access initiative involves expansion of the PHDH teams that have been serving multiple counties in Kentucky since 2013. Through this grant opportunity, the Kentucky Department for Public Health (DPH) is soliciting applications from qualified organizations to establish new or expand current PHDH teams to expand access to rural dental care. The newly created programs must demonstrate commitment to providing services including:

• Preventive dental hygiene services for children and adults
• Dental hygiene-focused oral health services for residents of long-term care settings as defined in Kentucky’s Dental Practice Act KRS 216B
• Teledentistry-enabled preventive care, education, triage, and care coordination

Program Objectives

Funded PHDH programs must demonstrate their willingness and ability to:

• Increase access to school-based preventive dental hygiene services for all students, regardless of ability to pay.
• Deliver dental hygiene services in long term care facilities through teledentistry consultation with university dentists.
• Utilize teledentistry to expand access to care, improve quality and continuity of care, and enable more rural residents to stay in region for dental care and specialty consultations.
• Integrate Community Health Workers (CHWs) and/or social determinants of health (SDOH) screenings to support patient navigation and continuity of care.
• Achieve measurable improvements in oral health outcomes and service utilization.
• Operate in a cost-effective, sustainable, and compliant manner. Compliance is defined as adherence to the provision of hygiene services to the aboveidentified populations within KRS 313.

Department for Public Health (DPH) - Rural Health Transformation Program Funding Opportunity to Establish or Expand Community Paramedicine Programming

Funding is designed to support early program development and scaling. Community paramedicine programs are not expected to be fully sustainable during initial funding periods. Awards may vary depending on the proposed budget provided by the applicant and additional funding will be provided in subsequent years.

Program Objectives

Funded programs must demonstrate their willingness and ability to:

• Design and deliver community paramedicine services that address low-acuity, preventative, and high-utilization patient needs in Kentucky’s rural communities.
• Reduce unnecessary emergency department utilization and repeat visits after discharge by supporting community members where they are.
• Strengthen partnerships with rural service sites such as Local Health Departments, Federally Qualified Health Centers, rural clinics, and behavioral health partners, including Community Mental Health Centers (CMHCs).
• Connect community members to the right care before and after emergencies by repositioning EMS as a front-line connector to care.
• Operate in a cost-effective, sustainable, and compliant manner. Compliance is defined as adherence to KBEMS standards for EMS agencies. 

The Ramseyer Ministry Education Grant, offered by the Kentucky Annual Conference and the Board of Ordained Ministry, aims to support clergy by providing funds for educational expenses. This scholarship seeks to uphold high professional and academic standards among clergy serving the conference. The Norfolk Southern In-Kind Donation Grant supports organizations seeking physical donations of materials that align with their focus areas, including safety, sustainability, workforce development, and thriving communities. This grant is available year-round, with no application deadline. Recipients must be located within Norfolk Southern territory and are encouraged to apply for resources that will enhance their community projects and initiatives, fostering growth and resilience.

The Kentucky Opioid Abatement Grants: Prevention program aims to address the severe impact of the opioid epidemic on communities by distributing settlement funds for prevention-focused efforts. Established by the Kentucky Opioid Abatement Advisory Commission, this initiative prioritizes youth prevention, overdose reduction, and collaborative projects. Non-profit and government entities can apply for funding, which has a ceiling of $1 million. Priority will be given to projects that demonstrate effective use of funds and address specific regional service gaps.

The Norfolk Southern Trades on Track SkillPointe Scholarship offers $5,000 scholarships for students pursuing trades education related to the rail industry. Eligible candidates must be enrolled in a post-secondary institution and focused on railway-related trades. With over 1,000 scholarships funded and a mission to close the skills gap, this initiative empowers individuals towards high-demand careers without requiring a four-year degree, ultimately strengthening the workforce and economy.

The Tourism Development Incentive Grant Program in Kentucky aims to boost tourism by providing financial incentives for the development and rehabilitation of tourism projects. Eligible projects include attractions, theme restaurants, and lodging facilities, with investments starting at $1 million. The program allows up to 25% recovery of approved development costs over ten years, enhancing the state's tourism infrastructure and stimulating economic growth through increased visitor engagement.

South Arts, Inc. offers Professional Development Grants for Arts Organizations to enhance the skills and knowledge of Southern arts entities. These grants aim to support operational stability and growth, focusing on organizations with budgets of $500,000 or less. Eligible applicants include a range of arts organizations within South Arts' nine-state service area. The program emphasizes first-time opportunities and requires a dollar-for-dollar cash match, ensuring a commitment to professional development in the arts.

With support from Toyota Motor North America, the National Environmental Education Foundation (NEEF) is pleased to announce $250,000 in grant funding to make public lands more accessible and enjoyable for people of all abilities. The 2026 grant program will focus on communities near Toyota Motor North America facilities and is restricted to 16 communities, as detailed below. 

Overview

Through the 2026 Driving Mobility and Accessibility on Public Lands grant, NEEF aims to:

• Increase the capacity of local organizations and government agencies to address mobility and accessibility considerations on public lands; and
• Improve the level of access, comfort, and enjoyment experienced by public lands visitors of all abilities, together with their families and friends.

The grant will provide up to $20,000 per grantee to increase physical or digital access, provide adaptive recreation equipment or programming, or perform accessibility assessments on public lands in communities near 16 Toyota Motor North America facilities.

Kentucky public entities, private nonprofits, and federally recognized tribes needing water or waste infrastructure in rural areas can access funding through USDA: Rural Development for acquisition, construction, or improvement projects. Youth sports facilities and recreation programs in eligible rural communities may benefit when they require clean drinking water, sewer, solid waste, or stormwater systems and related costs.

The Tree Campus Higher Education Program in Kentucky, led by the National Arbor Day Foundation, encourages colleges to cultivate their community forests. This initiative promotes environmental awareness, improves campus aesthetics, and supports student well-being through green spaces. By meeting five essential standards, institutions can earn annual recognition, fostering pride and commitment to sustainability. Participation benefits both the campus community and the planet, emphasizing the vital role trees play in our health and environment.

The Southern Partners Fund's Discretionary Fund Grant aims to support grassroots community organizing for social, economic, and environmental justice in the Southeastern United States. It provides financial resources and technical assistance to rural organizations and leaders focused on transformative change. Eligible applicants include self-governed rural groups engaging in advocacy and community development. The fund prioritizes education reform, immigration rights, and voter engagement, allowing up to two requests per year, with a maximum award of $3,500.

The Hubert E. Sapp Regular Grant Cycle, by Southern Partners Fund, aims to empower rural communities in the Southeastern U.S. through grassroots leadership and organizing. The fund provides support to nonprofit organizations focused on social, economic, and environmental justice. Eligible applicants are community-based organizations with budgets under $350,000. The initiative honors Hubert E. Sapp's legacy, promoting equitable relationships that drive real transformation and community decision-making.

The Appalachian Landscapes Project - Capital Grants Application Fall 2025 by the Open Space Institute aims to enhance conservation across the Appalachian Mountain region. With a focus on biodiversity, climate resilience, and community benefits, this initiative supports the acquisition of land and conservation easements. As a vital climate refuge, the Appalachians play a crucial role in safeguarding habitats and providing public recreation opportunities for the 26 million residents living in this area.

The Southern Documentary Fund (SDF) offers a Production Grant aimed at supporting Southern documentary filmmakers. With funding made possible by the MacArthur Foundation, SDF prioritizes nonfiction projects in production that showcase the rich tapestry of Southern stories. Up to seven grants of $15,000 each will be awarded to help elevate underrepresented voices and narratives. Applicants must have ties to Southern states and meet specific eligibility criteria to apply.
