The purpose of Activity 5.3c- Rural Pharmacy Medication Management Therapy Program is to improve health outcomes in rural communities through Medication Therapy Management (MTM), tobacco cessation services, and chronic disease prevention and management that are not covered by medical insurance. Program Goals: The Enhancing Rural Cancer Healthcare program aims to implement sustainable policy, systems, and environmental change approaches across the rural areas of 8 Regional Chronic Disease Coalitions to reduce the burden of cancer and other chronic and reproductive diseases. There will also be a focus on supporting/providing cancer survivorship care to residents of rural areas. Residents of the rural areas served by this project will receive culturally specific cancer education, outreach, referrals to appropriate screening services, and survivorship care services. These project efforts may reduce cancer incidence and mortality in rural New Jersey.

• Objective 1.1: Reduce cancer mortality and morbidity in New Jersey rural communities.
• Objective 1.2: Establish PSE change strategies to improve the health and well-being of New Jersey rural communities with relation to cancer primary prevention, early detection, and survivorship.

Activity 5.2a: NJ Rural Southern Physical Activity and Nutrition Program (NJ R-SPAN)

This program will support evidence-based physical activity, nutrition, and obesity prevention initiatives for rural communities by leveraging partnerships and resources from various sectors (e.g., agriculture, transportation, education, healthcare, parks/recreation, business, housing, and defense/military) to address health disparities related to poor nutrition, physical inactivity, and/or obesity in Atlantic, Cumberland, Salem, Cape May counties. This project aims to address the lack of access to care, nutritious food, and safe spaces for physical activity. Objectives include the following:

• Increased access to evidence-based arthritis management & physical activity;
• Increased access to nutritious foods;
• Increased access to preventive and clinical care;
• Increased access to health screenings; and
• Increased proportion of employers offering worksite wellness initiatives.

The Georgia-Pacific Foundation Grant supports organizations in the Metro Atlanta area, focusing on education, entrepreneurship, community enrichment, and environmental sustainability. Established in 1958, the foundation prioritizes investments that align with its mission and values, promoting long-term well-being within communities where Georgia-Pacific operates. Eligible applicants include 501(c)(3) nonprofits, public schools, and local government entities located within 30 miles of GP manufacturing sites.

The Housing for Everyone Grant Competition, part of the 2026 Initiative, aims to support eviction prevention in low-to moderate-income communities. This 20th annual initiative recognizes the destabilizing impact of eviction and displacement on families, which leads to job loss, school disruptions, and health issues. By funding innovative organizations, the competition seeks to provide stability for renters, enabling them to stay in their homes and communities.

Scholarships

We partner with donors to administer scholarships for students across the state. Most NCCF scholarships open for applications on Dec. 16, 2025, and close on March 3, 2026.

Undergraduates, graduates, medical and professional school students and adult re-entry students who are attending community colleges, four-year universities, career/technical schools, teaching credential programs and graduate, medical or professional schools in the United States are eligible for scholarships offered at NCCF.

Each scholarship has specific eligibility requirements defined by the donor when the scholarship was established. Be sure to read the requirements of each scholarship carefully as directed through the universal application and address those elements as needed in your application.

Grayce Wills MacDonald and Ivan A. MacDonald Endowment

Shall support students who attended high school in Cherokee, Graham, Clay, Swain, Macon, Jackson, Haywood, Buncombe, and Henderson counties, and who are pursuing a college or post-graduate level degree in the field of nutrition with emphasis on the study of natural foods and the vegetarian lifestyle. Scholarships may be based on both need and merit.

The Fred and Betty Worsh Scholarship Fund, managed by the North Carolina Community Foundation, offers financial support to residents of New Hanover and Brunswick Counties who are U.S. citizens attending accredited four-year colleges or universities. Scholarships are available to undergraduates, graduates, and adult re-entry students, and may be renewed annually based on academic performance and financial need. Applications open on December 16, 2025, and close on March 3, 2026.

Phyllis G. Meekins Scholarship

The Phyllis G. Meekins Scholarship is granted annually through The LPGA Foundation and the Phyllis G. Meekins Scholarship Fund. Established in 2006, the objective of the Phyllis G. Meekins Scholarship is to provide a need-based scholarship to a female high school senior from a recognized minority background who will be pursuing a full-time course of study and playing collegiate golf at an accredited college or university in the United States.

Qualifications for all scholarships include strong academic programs, community service and recommendations.

Established in 1999 by Marilynn with the proceeds of a golf tournament she organized, the objective of the Marilynn Smith Scholarship is to provide a scholarship to a female high school senior, who is a legal resident or citizen of the United States or Canada, has played golf in high school or in her community and intends to play golf at an accredited college or university in the United States.

The JADE Learning Electrical Technologies Scholarship supports students enrolled in North Carolina technical or occupational schools focused on electrical systems technologies. Eligible candidates include undergraduates, graduates, and apprentices who demonstrate financial need and strong academic performance. The scholarship application requires careful attention to specific donor-defined eligibility criteria.
