Best Community Health Grants in Indiana

The ACT on Health Equity Community Solutions Challenge Grant, backed by the Astrazeneca Foundation, aims to enhance health equity by funding community-based nonprofit programs. The Express Health Information Outreach Awards aim to enhance public health by increasing access to biomedical information for U.S. health professionals and the general public. These awards support projects that improve health information literacy among consumers and caregivers, ensuring they can effectively find and utilize health information. Focused on partnerships and outreach to underrepresented communities, the initiative seeks to promote healthy behaviors, prevent illness, and improve health outcomes through informed decision-making.

CommonSpirit Health offers the Community Health Improvement Grant to enhance health equity and service systems for underserved populations. Through collaborative efforts with 501(c)(3) organizations, this initiative provides financial support for essential health projects. Grant amounts range from $19,500 for small grants to $150,000 for standard grants, varying by hospital. Participating hospitals include those under Catholic Health Initiatives and Dignity Health, aimed at addressing significant community health needs.

The Health Information Outreach Award, funded by the National Library of Medicine, aims to enhance access to essential health information for underrepresented populations and healthcare professionals. This initiative focuses on improving health literacy and equity through diverse projects, including training, technology access, and community outreach. Eligible organizations from select Midwestern states can apply, prioritizing innovative solutions that address health disparities and promote informed health decisions among vulnerable communities.

Indiana Department of Health

The Indiana State Department of Health became the Indiana Department of Health (IDOH) in July 2021 after the Governor signed legislation updating the agency's statute. That same year the Department was also awarded accreditation by the Public Health Accreditation Board, joining the ranks of 36 other states and territories.

MCH Strategic Inititatives Funding

The Indiana Department of Health Maternal and Child Health (MCH) Division is accepting applications to fund organizations to implement MCH programs focused on reducing infant mortality and improving outcomes for maternal, child, and adolescent health.

Purpose of MCH Strategic Initiatives Funding

This funding opportunity’s purpose is to support Indiana’s goal of improving Maternal and Child Health outcomes by funding competitive grants for community-driven projects that address their unique needs and the MCH Strategic Initiatives Priorities. This opportunity is a two-year, performancebased grant. At the end of the first two project years, a performance award opportunity (up to the original awarded amount) will be available for those recipients that demonstrate improvement in their selected outcomes.

Our organization started in 1997, when ChoiceCare sold its health maintenance organization to Humana for $221 million. The proceeds were invested and The Health Foundation of Greater Cincinnati was created, serving a 20-county region in Southwestern Ohio, Northern Kentucky and Southeastern Indiana. The foundation was charged with changing the way health care was delivered—especially for under-resourced communities.

Interact For Health: Event Sponsorship

Interact for Health provides sponsorships as a way to support organizations within Greater Cincinnati with funding beyond our grantmaking work. Sponsorships are a competitive process, and funding is limited. We prioritize sponsoring events that serve our priority populations, align with our strategic plan, and benefit (and are located in) our 20-county service area.

In general, sponsorship opportunities are meant to benefit organizations that are not current grantees of Interact for Health. 

The nccPA Health Foundation's Oral Health - Community Outreach Grant offers up to $2,500 for PA-led initiatives that enhance oral health awareness and treatment. Open to certified PAs, PA students, and organizations, this grant aims to empower applicants to bridge healthcare gaps in oral health. Applications are accepted year-round, promoting continuous engagement in advancing community health. This initiative highlights the vital role of PAs in enhancing access to oral healthcare services.

The SHAPE America Hall of Fame Award recognizes individuals who have made significant contributions to health and physical education, ensuring these fields remain central to educational programs. This prestigious award honors dedicated professionals, including educators and leaders, who have demonstrated exemplary commitment and leadership in promoting physical activity, health education, and related disciplines. Through their efforts, they enhance the quality of education and the well-being of students across the nation.

The Curriculum and Instruction Honor Award, presented by SHAPE America, recognizes outstanding contributions to health and physical education curriculum research. This prestigious award highlights the importance of effective instructional practices that prepare students for active and healthy lives. Award recipients are selected based on their scholarship in health education and physical education, demonstrating commitment to excellence since the organization's founding in 1885. SHAPE America advocates for quality education and promotes the well-being of preK-12 students across the United States.

The E.B. Henderson Award, presented by SHAPE America, honors minority members who have demonstrated exceptional commitment to scholarship, mentorship, and advocacy for ethnic minorities within health and physical education. Established in 1885, SHAPE America serves over 200,000 professionals, promoting high-quality education standards. The award recognizes individuals who have significantly contributed to increasing the participation of underserved populations in the field. Limited to two awards annually, it highlights the importance of diversity and inclusion in health education.

The Helen M. Heitmann Young Scholar Award, presented by SHAPE America, honors young professionals who have made significant contributions to research in curriculum and instruction within health and physical education. Eligible candidates must be current SHAPE America members and have 3 to 6 years of experience post-doctorate. This award emphasizes active involvement and scholarship in the field, encouraging innovative research that enhances educational practices for preK-12 students.

The Luther Halsey Gulick Award, presented by SHAPE America, recognizes individuals with over ten years of membership who have made outstanding contributions to health and physical education. This prestigious honor is awarded annually to a candidate selected for their exemplary leadership and service in the field. Nominations are carefully evaluated based on the candidate's impact on the profession and their ability to inspire future generations to lead healthy, active lives.

The Measurement and Evaluation Graduate Student Award, offered by SHAPE America, aims to support graduate students specializing in psychomotor measurement and evaluation. This scholarship assists with professional expenses, ensuring candidates can focus on their studies. Eligible applicants must be current SHAPE America members and pursuing relevant master's or doctoral degrees. The award prioritizes those who have completed their first year of study, promoting excellence in health and physical education across the U.S.

The National Girls and Women in Sports (NAGWS) Legacy Award, established by SHAPE America, honors exemplary contributions to increasing opportunities for women in sports. This annual award recognizes current SHAPE America members who submit impactful proposals related to women's sports, with a monetary prize of $1000. The award will be presented at the National Convention & Exposition, with a special deadline of November 1st. Nominations can include various initiatives aimed at empowering women in sports.

The AES Hawaiʻi Foundation Grant supports communities near AES Hawaiʻi’s renewable energy projects, focusing on social justice, education, environmental stewardship, and community health. Preference is given to nonprofit organizations in these areas, but statewide entities may also apply. Successful projects should align with the Foundation’s impact pillars, demonstrate measurable community benefits, and address unmet needs, ensuring long-term positive effects on the local community.

The Barbara A. Cooley Master’s Scholarship, offered by SHAPE America, is a $1,000 annual award for master's level students enrolled in accredited health education programs in the U.S. Recipients can use the funds as they wish and receive a complimentary one-year student membership with SHAPE America. The scholarship aims to support future educators and enhance the quality of health education, promoting research and professional practice in the field.

The Bill Kane Undergraduate Scholarship, awarded by SHAPE America, provides $1,000 annually to undergraduate students majoring in school-based health education at accredited institutions in the U.S. or territories. Eligible candidates must be full-time students with a GPA of at least 3.25. Alongside the scholarship, recipients receive a complimentary one-year membership with SHAPE America, promoting professional development in health and physical education.

Better Health Together (BHT) is committed to enhancing community wellness through sponsorships. By collaborating with over 100 organizations in eastern Washington, BHT addresses critical health issues and supports events that elevate community voices. Organizations seeking funding should submit applications at least 6–8 weeks prior to their event to ensure timely review. BHT values partnerships that align with its mission and aims to invest in innovative solutions for community health challenges.

The See, Test & Treat Grant offers funding for a pathologist-led cancer screening program aimed at underserved communities. With a focus on health education, the program provides vital cervical and breast cancer screenings, ensuring same-day results and follow-up care. Eligible institutions can apply for funding up to $20,000 for single-year projects or up to $50,000 over three years. This initiative addresses health disparities, empowers communities, and enhances the visibility of pathology in healthcare.

Awards

Robert F. Gagel, M.D. Community Leadership Award

The Robert F. Gagel, M.D. Community Leadership Award recognizes a healthcare professional who has made outstanding contributions to improving osteoporosis care and increasing community awareness about bone health. This award will be presented annually to a healthcare professional residing and working in the United States or its territories.

The Executive Committee of the Bone Health and Osteoporosis Foundation’s (BHOF’s) Board of Trustees will review nominations and select the recipient.

The award winner will be notified and announced in May, in recognition of Osteoporosis Awareness and Prevention Month. The recipient will receive a plaque and a financial award (currently $1,000) to support community service efforts initiatives related to osteoporosis and bone health.

The AACR June L. Biedler Prize for Cancer Journalism

Prize Summary

The AACR June L. Biedler Prize for Cancer Journalism was established by the AACR in 2015 to raise awareness of the critical role that the media play in educating the public about cancer and cancer research.

Funded by a major bequest to the AACR from cancer research pioneer Dr. June L. Biedler, a scientist, mentor, and former member of the AACR Board of Directors, this prize showcases outstanding examples of cancer journalism and recognizes individual professional journalists who have produced accurate, informative, and compelling stories that enhance the public’s understanding of cancer, cancer research, cancer advocacy, or cancer policy.

The prize provides an unrestricted cash award of $5,000 and a commemorative award.

The McCullough-Hyde Foundation is excited to announce the launch of the Community Granting Program, as part of the expanded philanthropic mission of the Foundation. It is anticipated that more than $200,000 in funding will be available annually through this grant-making program.

The Community Granting Program is a philanthropic program that makes investments that address significant health needs of local communities. The Foundation will partner with and provide funding to local organizations that are leading the way to build healthier communities.

The McCullough-Hyde Foundation invites proposals from organizations for programs and projects that focus on education, prevention programs, awareness efforts, wellness opportunities and direct care in the following areas:

Programs and projects that prevent, manage, and reduce the burden of chronic conditions including, but not limited to, heart disease, hypertension, stroke, and diabetes. Eligible initiatives may focus on education, early detection, care coordination, wellness promotion, and access to treatment and support services.

Programs and projects that promote and sustain healthy lifestyle choices, including, but not limited to, physical activity, nutrition, tobacco cessation, and preventive care. Eligible initiatives should be community-centered and designed to empower individuals and families to take control of their health through activities, education, engagement, and improved access to wellness opportunities that support active living, particularly among youth and families.

Programs and projects that improve access to and quality of mental health and substance use services, reduce stigma, and promote emotional well-being across all age groups. Eligible initiatives may include, but are not limited to, prevention, early intervention, treatment, recovery support, education, and outreach related to conditions including, but not limited to, anxiety, depression, and addiction.

Programs and projects that address the underlying social and economic conditions influencing health outcomes, including, but not limited to, access to healthcare, food, housing, transportation, education and employment. Eligible initiatives should aim to reduce barriers to health and wellness by improving the environments where people live, work, and play.

We are Revolutionaries -passionate in our singular pursuit of discovering, developing and delivering innovative targeted therapies for patients living with RAS-addicted cancer. About the Awards

The late Dr. James F. Fries, formerly professor emeritus of medicine at Stanford University, came up with the idea for the James and Sarah Fries Prize for Improving Health in a 1987 ascent of Nepal’s Makalu—one of the world’s highest peaks—when his party became trapped in a severe snowstorm and was ultimately unable to scale the mountain.

Following that experience, Dr. Fries returned to sea level and set about establishing a foundation to support a Nobel-like prize for health that now annually awards a $100,000 prize to an an outstanding leader judged by an expert panel to have done the greatest good for the greatest number of people in the field of health.

In 1992 the Elizabeth Fries Health Education Award was established in honor of Elizabeth Fries, daughter of James and Sarah Fries. The $50,000 prize is awarded annually to an individual judged to have made substantial contributions to their area of health education and health promotion. Sarah Fries remained active in the Fries Awards for Health program until her death in May 2017, and Jim Fries remained active in the awards until his death in November 2021.

The CDC Foundation is honored to partner with the James F. and Sarah T. Fries Foundation, which established and funds the award. As of 2016, the CDC Foundation manages and administers the Fries Awards for Health program, which include the James and Sarah Fries Prize for Improving Health and the Elizabeth Fries Health Education Award.
