Program Goal and Description

The Specialized Transportation Program, as authorized by the Federal Transit Administration, 49 USC Section 5310, is intended to enhance mobility for seniors and persons with disabilities by providing Federal Transit Administration funds for programs in small urbanized and rural areas that serve the special needs of transit-dependent populations beyond traditional public transportation services and Americans with Disabilities Act (ADA) complementary paratransit services. The program provides 80% of the cost of capital projects; the remaining 20% must be provided by local funding, which may be derived from other federal (non-U.S. DOT) transportation sources or the Federal Lands Highway Program.

Vehicle Funding

Available Vehicles: Applicants can refer to the Vehicle Catalog & Selection Guide for more information on the vehicles offered through the Section 5310 program. The vehicle cost worksheet will list the available options and pricing for each vehicle.

Vehicle Use: Vehicles are to be used for program-related needs for which a Section 5310 grant is made. Subrecipients are encouraged to the extent feasible to also provide service to the general public, on an incidental basis if such service does not interfere with transportation services for seniors and people with disabilities. The recipient shall use the vehicle in the project or program for which it was acquired as long as needed, even if the project does not continue to receive federal funding.

Southeast Ohio Health and Wellness Foundation Fund community grants

DOH 59808- Rural Health Transformation Program- Healthier Ohio Initiatives - Preventing and Addressing Chronic Disease- Track One

Project Background:

The Rural Health Transformation Program (RHTP) was authorized by the One Big Beautiful Bill Act (Section 71403 of Public Law 119-21) and empowers state to strengthen rural communities by improving healthcare access, quality and outcomes by transforming the current system. Five key strategic goals of the program, as defined by the Centers for Medicare and Medicaid (CMS) are:

• Make Rural America Healthy Again
• Sustainable access
• Workforce development
• Innovative Care
• Technology Innovation

Ohio’s plan aligns with federal priorities to improve the health of rural residents by building an innovative movement to bridge medicine and preventive behaviors into a holistic and collective impact approach to improve chronic disease outcomes. This Request for Proposals (RFP) is designed to strengthen preventive and clinical care for improved blood glucose and hypertension indicators across Ohio’s 73 non-urban counties (See Appendix B. Applicants could be but not limited to the following:

• State-level associations that support implementation in multiple clinics, health centers, and/or hospitals.
• Regional collaboratives or health system with multiple locations.
• Individual hospitals, health centers and clinics

Project Objective:

Based upon data analysis from Ohio’s State Health Assessment, including a supplemental deep dive into Appalachian health, 2025 Summary Assessment of Older Ohioans, the Ohio Chronic Disease Atlas 2025, and other publicly available data, Ohio seeks to reduce the prevalence and mortality rates associated with chronic disease. The collective aim of the strategies is:

to reduce hemoglobin A1C for patients diagnosed with prediabetes and diabetes by 10% and

to manage blood pressure at clinically appropriate levels for patients diagnosed with hypertension.

The CALT Emerging Traditional Artists Program, funded by Mid Atlantic Arts Foundation, supports early-career Appalachian traditional artists with $5,000 grants and personalized professional development. Artists engage in learning opportunities, including workshops and mentorship, to enhance their skills. Eligible applicants must be traditional artists from designated Appalachian counties in Ohio, Virginia, or West Virginia, at least 18 years old, and in good standing with Mid Atlantic Arts. This initiative fosters cultural preservation and artistic growth in the region.

By providing grants for program expansion, the initiative encourages schools to implement new PLTW units and train teachers, empowering students with essential skills for future careers. This partnership seeks to inspire a new generation of learners through hands-on, real-world challenges, thereby fostering greater participation in STEM fields across various regions.

Burns and McDonnell, a 100% employee-owned firm, partners with Project Lead The Way (PLTW) to enhance access to STEM education. By expanding PLTW Computer Science programs in local schools, they aim to engage students in real-world challenges and equip them with essential skills. Eligible schools must be located in specific counties and serve a majority of students from low-income backgrounds. This initiative supports the growth of STEM education, fostering a brighter future for communities through enhanced learning opportunities.

The PLTW Computer Science program, supported by Burns and McDonnell, aims to enhance STEM education in various communities by funding schools to implement PLTW courses. This partnership promotes student engagement with real-world challenges, preparing them for future careers in technology. With a total grant of $20,000, schools must offer multiple PLTW courses over two years, ensuring access to quality education for underserved populations.

The PLTW Engineering program, in partnership with Burns and McDonnell, aims to enhance STEM education by providing grants to public and charter schools in specific counties. This initiative empowers students to engage with real-world challenges through hands-on learning experiences. Eligible schools must offer PLTW Engineering courses, ensuring access to quality education for students from diverse backgrounds. The program also promotes the management of forests for timber harvesting while maintaining ecological benefits, ensuring sustainable land use practices for future generations.

The CALT Black Appalachian Storytellers Fellowship, supported by the Mid Atlantic Arts Foundation, celebrates and preserves the rich traditions of Black Appalachian storytelling. This fellowship honors those who embody the history and culture of African Americans in the region. Each year, fellows are recognized at the National Black Storytelling Festival and Conference, receiving awards crafted by Dr. Dena Jennings. 501(c)(3) pet welfare organizations in Muskingum County, Ohio can apply for Nestlé Purina’s Animal Welfare Grants to support pet well-being.
