Responsible Fatherhood Initiative: Strong Fathers, Strong Families

The purpose of this RFGA is to solicit Applications for DCY, through the Ohio Commission on Fatherhood, to implement the Responsible Fatherhood Initiative to strengthen father engagement across Ohio. The initiative will provide fathers with resources, support, and encouragement to enhance their parenting role, including a statewide media campaign and locally delivered services.

This RFGA is being issued pursuant to Ohio House Bill 96 and Ohio Revised Code (R.C.) 5180.706. DCY will award a total of $1.8 million annually to support services across 12 counties. Funding is set at $150,000 per county, per state fiscal year. Each Application must clearly identify the specific county to be served. Future phases will expand the Responsible Fatherhood Initiative to include all counties across Ohio.

Applicants may submit up to four applications in a single submission through the GMS system. When applying for multiple counties, each county must be submitted as a separate, clearly labeled document, and all selected counties must fall within the same designated region as defined in Exhibit 1. Each application will be reviewed and scored independently.

Applicants must demonstrate that they have, or will secure, an appropriate service location within the proposed county—whether an existing site it operates or a partner facility—to ensure equal access for fathers. Applications must demonstrate capacity to deliver county-based programming that (1) engages fathers, parents, and mentors, (2) reaches high-need populations, and (3) builds partnerships that strengthen local support systems.

The overarching objective of this Grant is to improve outcomes for children and families by equipping fathers with the tools, resources, and support needed to build stable, positive relationships with their children.

The PLTW Gateway program, in collaboration with Burns & McDonnell, aims to enhance STEM education by expanding access to Project Lead The Way initiatives in eligible communities. By providing grants for program expansion, the initiative encourages schools to implement new PLTW units and train teachers, empowering students with essential skills for future careers. This partnership seeks to inspire a new generation of learners through hands-on, real-world challenges, thereby fostering greater participation in STEM fields across various regions.

The Hardin County Community Foundation offers grants to 501(c)(3) organizations that benefit Hardin County residents. The foundation prioritizes innovative projects that address community needs in areas like education, health, and arts. It seeks to enhance the quality of life, encourage volunteerism, and ensure efficient distribution of social services. However, it does not fund operating expenses, political activities, or projects already in progress. The foundation aims to create long-term community resources and support self-sustaining initiatives.

The Makulinski Family Foundation Grant supports nonprofit and charity organizations, emphasizing sustainable community development. With a commitment to lasting change, MFF actively matches investments to amplify their impact, ranging from 10:1 to 1:2 based on beneficiary needs. The foundation welcomes proposals from various regions, including Ohio, Michigan, and South Carolina, and encourages applications that align with their mission. Interested parties can access the grant request form to submit their proposals.

The PLTW Gateway program, in partnership with Burns and McDonnell, aims to enhance STEM education by supporting schools in eligible counties to implement PLTW Gateway units. With a total grant of $20,000, schools are encouraged to offer at least one unit per academic year, fostering real-world skills and engagement in STEM fields. This initiative seeks to increase access and participation in high-quality STEM learning experiences, ultimately preparing students for future careers.

DOH 59808- Rural Health Transformation Program- Healthier Ohio Initiatives - Preventing and Addressing Chronic Disease- Track One

Project Background:

The Rural Health Transformation Program (RHTP) was authorized by the One Big Beautiful Bill Act (Section 71403 of Public Law 119-21) and empowers state to strengthen rural communities by improving healthcare access, quality and outcomes by transforming the current system. Five key strategic goals of the program, as defined by the Centers for Medicare and Medicaid (CMS) are:

• Make Rural America Healthy Again
• Sustainable access
• Workforce development
• Innovative Care
• Technology Innovation

Ohio’s plan aligns with federal priorities to improve the health of rural residents by building an innovative movement to bridge medicine and preventive behaviors into a holistic and collective impact approach to improve chronic disease outcomes. This Request for Proposals (RFP) is designed to strengthen preventive and clinical care for improved blood glucose and hypertension indicators across Ohio’s 73 non-urban counties (See Appendix B. Applicants could be but not limited to the following:

• State-level associations that support implementation in multiple clinics, health centers, and/or hospitals.
• Regional collaboratives or health system with multiple locations.
• Individual hospitals, health centers and clinics

Project Objective:

Based upon data analysis from Ohio’s State Health Assessment, including a supplemental deep dive into Appalachian health, 2025 Summary Assessment of Older Ohioans, the Ohio Chronic Disease Atlas 2025, and other publicly available data, Ohio seeks to reduce the prevalence and mortality rates associated with chronic disease. The collective aim of the strategies is:

to reduce hemoglobin A1C for patients diagnosed with prediabetes and diabetes by 10% and

to manage blood pressure at clinically appropriate levels for patients diagnosed with hypertension.

The Great Lakes Protection Fund Grant aims to support projects that deliver significant ecological improvements to the Great Lakes region. The Fund seeks transformative initiatives that involve collaboration among diverse stakeholders and rely on scientific rigor. Eligible applicants include non-profits, universities, and businesses, and the projects must demonstrate a positive environmental impact on the Great Lakes ecosystem. Importantly, the Fund does not support advocacy or compliance-related activities.

The AES Ohio Foundation, a leading corporate foundation in the Dayton region, offers Community Impact Grants aimed at improving quality of life and fostering sustainability. With a focus on poverty reduction, workforce development, and promoting diversity, equity, and inclusion, the foundation supports initiatives that address basic community needs. Organizations seeking funding must align with these priorities while demonstrating measurable outcomes and a commitment to equity. The application deadline for 2024 is October 15, 2024.

The F26AS00083 Aquatic Invasive Species Grants are part of the Great Lakes Restoration Initiative for Fiscal Year 2026. These grants, administered by the U.S. Fish and Wildlife Service, aim to support state-level management plans to combat aquatic invasive species in the Great Lakes region. Eligible applicants include Great Lakes states and their designees, with funding awarded through a competitive selection process.

The F26AS00084 Aquatic Invasive Species Grants to Great Lakes Tribes, funded by the U.S. Fish and Wildlife Service, aim to support the development and implementation of Tribal Aquatic Invasive Species Management Plans. Eligible Great Lakes tribes can apply for competitive grants to address invasive species issues within the Great Lakes Basin, including outreach and prevention efforts. This initiative focuses on early detection and control, significantly contributing to the health of the Great Lakes ecosystem.

Eligible applicants must be 501(c)(3) organizations located within a 30-mile radius of SpartanNash facilities. The Foundation ensures that every dollar raised directly benefits partner organizations, with a commitment to community impact. Grants are not available for ineligible entities, including those outside the radius or not aligned with the Foundation's mission. Since its inception in 2015, the Foundation has positively influenced local communities.

The PLTW Launch program, in collaboration with Burns and McDonnell, aims to enhance STEM education by expanding access to innovative educational modules in eligible schools. This initiative supports public and charter schools in targeted counties, focusing on increasing student engagement in STEM fields. Through grant funding, participating schools can introduce new PLTW Launch modules and provide essential training for teachers, fostering a richer learning environment to prepare students for future careers in science, technology, engineering, and mathematics.

The Mud Hens-Walleye Transportation Fund supports schools and nonprofit organizations in northwest Ohio and southeast Michigan by providing transportation grants for attending Toledo Mud Hens baseball and Toledo Walleye hockey games. With awards up to $1,000, the fund aims to eliminate financial barriers for educational field trips to these events. Eligible applicants must demonstrate financial need and adhere to specific guidelines regarding the use of funds for transportation services.

The Specialized Transportation Program (5310) aims to enhance mobility for seniors and individuals with disabilities through federally funded projects. This program supports various initiatives, including vehicle funding, preventive maintenance, and contracted transportation services. With an 80% federal cost coverage, local funding must cover the remaining 20%. The Ohio Department of Transportation oversees this program, ensuring accessible transportation options in urban and rural areas, thereby addressing the unique needs of transit-dependent populations.

The Great Lakes Fisheries Trust offers grants for ecological and biological research aimed at enhancing ecosystem health and sustainable fish populations. These grants support competitive fisheries research, focusing on resilience, sustainability, and stakeholder value. The next funding cycle will open with a request for proposals in late 2026 for the 2027 program year, promoting innovative research and capacity-building efforts within the fisheries sector.

Ohio Department of Transportation

With a mission to provide a transportation system that is safe, accessible, well maintained, and positioned for the future, the Ohio Department of Transportation (ODOT) supports the state highway system and promotes transportation initiatives statewide. As a more than $3 billion per year enterprise, ODOT invests the bulk of its resources in system preservation through maintenance, construction, and snow and ice operations.

ODOT's regional district offices and Central Office divisions work together to fulfill the department's mission of providing safe and easy movement of people and goods from place to place.

Specialized Transportation Program (5310)

Program Goal and Description

The Specialized Transportation Program, as authorized by the Federal Transit Administration, 49 USC Section 5310, is intended to enhance mobility for seniors and persons with disabilities by providing Federal Transit Administration funds for programs in small urbanized and rural areas that serve the special needs of transit-dependent populations beyond traditional public transportation services and Americans with Disabilities Act (ADA) complementary paratransit services. The program provides 80% of the cost of capital projects; the remaining 20% must be provided by local funding, which may be derived from other federal (non-U.S. DOT) transportation sources or the Federal Lands Highway Program.

Operating Assistance

The objective of the 5310 operating assistance project is to provide financial support to subrecipients in the day-to-day transportation operations for seniors and persons with disabilities. Operating assistance funding is explicitly intended to be spent on operations. ODOT will reimburse 50% of the operating cost and the subrecipient is required to provide the 50% local match. All of the local match must be provided from sources other than U.S. Department of Transportation funds. To be eligible for Section 5310 operating funds, an applicant must provide open-door services. FTA defines open door as regular, continuing shared ride surface transportation services that is open to the public or open to a segment of the public defined by age or disability.

Spectrum Digital Education Grants support nonprofit organizations in educating communities about broadband benefits and usage. These grants aim to bridge the digital divide, particularly for seniors and those with disabilities or ethnically diverse backgrounds. Applicants must demonstrate community partnership, address specific needs, and provide measurable impact metrics. With a focus on financial feasibility and meaningful outcomes, this initiative ensures that underrepresented urban and rural communities gain access to essential digital resources, fostering long-term improvement in their lives.

Healthy Aging Fund

The Community Foundation is funding programs that help older adults (60+) stay healthy, connected and independent.

This initiative follows the Ohio Department of Aging's State Plan, shaped with input from the Area Office on Aging of Northwestern Ohio. Priorities of the plan include cognitive health, chronic disease reduction, mental health, social inclusion, and nutrition access, plus engagement opportunities, physical activity, and preventive screenings. The plan is built on research and input from content-area experts.

The Foundation’s Focus

The Healthy Aging Fund is seeking proposals aligned with the State Plan and the Community Foundation's place-based priorities: programs that promote physical and mental health, reduce social isolation and strengthen community connections for older adults.

The Community Foundation seeks to identify collaborative approaches that work well enough to scale. This is a chance for northwest Ohio providers to test new approaches and collaborations: the kind with real potential to increase impact for participants and move the needle on broader community goals.

Funding Priorities

• Multiple partners with complementary expertise, prioritizing diversity of resources across agencies
• Programs delivered through or including senior centers, community centers or long-term care facilities
• Diverse programming that reaches more participants
• Accommodations for specific needs — memory care, vision loss, etc.
• An educational component

The Community Foundation is seeking cross-agency partnerships that expand access to diverse programming and have real potential to scale. Available for projects up to two years. See more in the Grant Guidelines. 

The Township Safety Sign Grant Program, funded by ODOT, allocates $2 million annually to enhance safety signage on Ohio's township roads. For FY 2025, $845,040.42 has been designated for 38 projects across 30 counties, supporting local townships. The Small City Program in Ohio allocates federal funds to small cities with populations between 5,000 and 24,999, not included in Metropolitan Planning Organizations. With an annual budget of $10.5 million, this program supports road, safety, and signal projects. The funding covers up to 80% of eligible construction costs, and a scoring system prioritizes projects based on various factors, ensuring effective use of resources and improved transportation infrastructure for eligible communities.

The AES Ohio Foundation's Power Grants aim to create lasting community impact through innovative programs across its 24-county service area. With $822,000 allocated for 2024, these grants support initiatives addressing basic needs like food and housing, enhancing education, and fostering diversity and inclusion. The Foundation commits to at least two grants annually, prioritizing projects that demonstrate measurable outcomes. This approach aligns with their goal to uplift local communities and empower individuals to improve their lives.

The PLTW Engineering Program Expansion by Burns and McDonnell aims to enhance STEM education in eligible communities. This initiative supports schools in expanding their existing PLTW Engineering programs, providing students with hands-on experiences and essential skills. The program focuses on empowering students through new courses and teacher training, ensuring access for underrepresented populations. By fostering excitement around STEM, Burns and McDonnell is committed to shaping the future workforce and enhancing educational opportunities in critical areas.

The Michigan Botanical Foundation Grant supports research and educational initiatives that enhance the understanding and preservation of Michigan's flora. Grants are available for projects involving rare species, environmental awareness, and educational programs. The foundation welcomes applications from individuals and organizations that align with its goals. Grant amounts typically range from $100 to $1,500, with preference given to projects based in Michigan or the Great Lakes region. Funding decisions are made on a case-by-case basis.

The Great Lakes Foundation Grant supports projects aimed at preserving wilderness areas and promoting marine safety in the Great Lakes region. As a 501(c)(3) charitable organization, the foundation has awarded over $318,212 since its inception in 1993. Grants have funded various initiatives, including breakwater construction and academic research, to enhance appreciation and safety of the marine environment for both members and the general public.
