Context and Background

The Commonwealth of Kentucky has been awarded funding through the Centers for Medicare & Medicaid Services (CMS) Rural Health Transformation Fund (RHTF). This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP), a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth1 .

Our Plan

Kentucky’s RHTP launches five interrelated initiatives designed to build rural health infrastructure and provide sustainable, long-term improvements. This plan directly supports Kentucky’s rural counties while advancing statewide impact through innovation, technology-enabled care and strengthened workforce recruitment pipelines for both clinical and non-clinical staff. This strategy will help build a resilient, integrated and technology-forward health system across the Commonwealth.

• Rural Community Hubs for Chronic Care Innovation:
  • Establishes local “hub-and-spoke” collaboratives focused on obesity and diabetes prevention and management.
  • These hubs will integrate nutrition, physical activity programs, and digital self-management tools.
• PoWERing Maternal and Infant Health:
  • Expands timely prenatal and postpartum care by deploying telehealth-enabled maternal care teams who will serve maternity-care deserts and high-risk regions, to help mothers and infants receive seamless, high-quality support.
• Rapid Response to Recovery (EmPATH Model):
  • Deploys technology-enabled crisis stabilization and mobile behavioral health response teams to connect individuals with community-based treatment and recovery supports.
• Rooted in Health:
  • Rural Dental Access: Increases access to preventive oral health services through expanded dental hygiene training programs, externships, and investment in Public Health Dental Hygiene (PHDH) teams in Local Health Districts (LHDs).
• Crisis to Care:
  • Integrated Emergency Medical Services (EMS) Response and Coordination: Enhances pre-hospital capacity and trauma coordination through treat-in-place protocols, improved data connectivity and workforce training for rural EMS providers. 

Wyoming Division of Victim Services

Speaking as one voice for victims of crime. The mission of the Wyoming Division of Victim Services (DVS) is to cultivate social change and access to victim services through the provision of resources, education, and program support and development.

Victim of Crime Act (VOCA) Funds

The purpose of VOCA is to provide direct services to victims of crime. "Services to victims of crime" are those activities directly benefiting individual crime victims and ensuring the availability of comprehensive services to all victims of crime in Wyoming. Each year a portion of VOCA funding is set-aside for special projects. Services under this program are used to:

respond to the emotional and physical needs of crime victims;

assist primary and secondary victims of crime to stabilize their lives after victimization;

assist victims in understanding and participating in the criminal justice system; and

provide victims of crime with a measure of safety and security.

VOCA funds are awarded on a competitive, non-formula basis, to eligible victim service programs. VOCA funds are used to support the provision of services to all victims of crime.

The ME Agricultural Conservation Easement Program (ACEP) supports the preservation of agricultural land by limiting nonagricultural uses and enhancing conservation values. It offers two components: Agricultural Land Easements (ALE) and Wetland Reserve Easements (WRE). ALE helps protect croplands and grasslands on working farms, while WRE focuses on restoring degraded wetlands. Additionally, the program includes the Wetland Reserve Enhancement Partnership (WREP) to improve wildlife habitat through collaborative efforts.

The Chuckanut Health Foundation offers Community Grants to support critical health programs and infrastructure, with awards ranging from $2,600 to $26,000 annually. Established in 1983, the foundation focuses on fostering a healthy community by addressing health needs across six areas, including early childhood and health care access. Emphasizing community-led initiatives, these grants aim to empower local organizations to enhance the well-being of residents. Over $18 million has been awarded to over 280 non-profits since inception.

The NE Agricultural Conservation Easement Program (ACEP) aims to protect agricultural viability by limiting nonagricultural land uses and enhancing conservation values. It includes Agricultural Land Easements (ALE) to safeguard croplands and Wetland Reserve Easements (WRE) for wetland restoration. Through partnerships, USDA's ACEP facilitates high-priority wetland protection and wildlife habitat improvement. Applications are accepted year-round, with automatic consideration for future funding cycles after ranking dates.

The NV Agricultural Conservation Easement Program (ACEP) aims to enhance agricultural viability and conservation values by limiting non-agricultural uses of eligible land. It includes Agricultural Land Easements (ALE) to protect croplands and Wetland Reserve Easements (WRE) to restore wetlands. This program supports landowners in preserving their land's agricultural integrity while promoting wildlife habitat through partnerships. Applications are accepted year-round, with future funding cycles available for late applicants.

The SC Agricultural Conservation Easement Program (ACEP) supports landowners in protecting agricultural viability and conservation values by limiting non-agricultural land uses. Creative & Performing Arts

The Pincus Family Foundation’s Creative and Performing Arts portfolio supports programs where young people use art as a tool to address the structural inequities they face. Five strategic priorities:

• Arts for Social Change Focus – Integrating artistic practice with community organizing, policy advocacy, and/or systemic change efforts
• Hyperlocal Community Integration – Deep community embedment with extensive local partnerships
• Teaching Artist Investment – Commitment to the professional development and well-being of teaching artists
• Cultural Responsiveness – Programming that incorporates community cultural wealth with community input
• Environmental Justice Integration – Authentic integration of environmental themes from community-identified priorities

Target Constituency: Organizations serving preK-12 youth from marginalized communities with arts for social change programming that connects them to the power of art to question and transform systems.

Education

High school readiness for many low-income students, largely is a result of poor preparation in middle school. PFF is funding programs/projects that support middle school engagement and focus on achievement filling a funding gap. 

Target Constituency: We support non-profit organizations serving middle school students who face gaps in comprehensive high school readiness. These organizations implement innovative programming that creates challenging yet nurturing environments—preparing students for achievement while inspiring them to become engaged, confident citizens ready to contribute to their communities.

Health & Wellness

Goal: Advance children’s health and well-being by supporting nonprofits that strengthen social conditions, empower youth, and build healthier environments in marginalized communities.

Target Constituency: Nonprofits whose work strengthens social determinants of health for children in marginalized communities.

Play & Recreation

Goal: Advance children’s social-emotional development and physical well-being by supporting nonprofits that create inclusive play-based programming, competitive sports opportunities, and outdoor recreational experiences in under-resourced communities.

Target Constituency: Non-profits whose work promotes access to quality recreational programming for children ages 6-18 in under-resourced communities.

For all grants, the organization’s mission and scope must align with the Foundation’s mission. For the latter, these grants can be either unrestricted or restricted to a particular program or project. The type of grant and what it is used for will dictate the goals and objectives to be met.

Additionally, the Wetland Reserve Enhancement Partnership (WREP) supports collaborative efforts for wetland protection and enhancement, ensuring sustainable agriculture and conservation of natural resources.

The UT Agricultural Conservation Easement Program (ACEP) aims to protect agricultural viability and conservation values by limiting nonagricultural land use. It includes two components: Agricultural Land Easements (ALE) which help protect croplands, and Wetland Reserve Easements (WRE) that restore degraded wetlands. USDA collaborates with partners through the Wetland Reserve Enhancement Partnership (WREP) to enhance wetland protection and improve wildlife habitat. Applications are accepted year-round, with automatic considerations for future funding cycles.

The VT Agricultural Conservation Easement Program (ACEP) aims to protect the agricultural viability of eligible lands by limiting nonagricultural uses and enhancing conservation values. It includes Agricultural Land Easements (ALE) to help landowners safeguard croplands and grasslands, and Wetland Reserve Easements (WRE) to restore degraded wetlands. Additionally, the program offers the Wetland Reserve Enhancement Partnership (WREP) to leverage resources for high-priority wetland protection and improvement of wildlife habitat.

The WA Agricultural Conservation Easement Program (ACEP) focuses on preserving agricultural land through Agricultural Land Easements (ALE) and Wetland Reserve Easements (WRE). This program helps landowners protect their land's agricultural viability and conservation values by limiting non-agricultural uses. To participate, landowners must collaborate with eligible partners and adhere to specific compliance requirements. The USDA's NRCS plays a key role in funding and supporting these conservation efforts, ensuring sustainable agricultural practices in Washington.

The Gilmore Inclusive Arts Grant supports projects that engage individuals with disabilities in Northeast Indiana's arts communities. Annually, applications open in May and close in September, with funding up to $25,000. Organizations must be 501(c)(3) compliant, and priority is given to impactful projects. Collaboration is encouraged, ensuring diverse participation in the arts. This grant aims to foster inclusivity and enhance the cultural landscape by empowering individuals with disabilities.

The Leonard Rice Memorial Scholarship, administered by the ACEC Colorado Foundation, supports full-time students pursuing engineering degrees in Colorado. With a generous $7,500 award, it aims to assist those specializing in engineering geology, water engineering, or natural resource engineering. Eligible applicants must be U.S. citizens enrolled in ABET-accredited programs. This scholarship not only promotes academic excellence but also enhances awareness of consulting engineering as a viable career path, contributing to the future of engineering in the state.

Kaiser Permanente's Washington Region offers sponsorships to nonprofit organizations, schools, and government agencies that align with its mission to improve community health. With a focus on sustainability, equity, and relationship-building, the program evaluates proposals based on their potential to create lasting value and positive exposure. Ineligible requests include support for religious activities, political campaigns, or individual medical funding, ensuring that resources are directed towards community-driven initiatives that enhance public health.

The Oklahoma 21st Century Community Learning Centers (21st CCLC) Program aims to enhance educational opportunities for students in low-performing schools. By providing academic enrichment, after-school activities, and family engagement, this program supports students in meeting Oklahoma's academic standards. Eligible applicants include local educational agencies, non-profits, and community organizations, ensuring diverse participation. The initiative focuses on a collaborative approach to improve educational outcomes and foster community involvement in children's education.

The WV Agricultural Conservation Easement Program (ACEP) aims to safeguard agricultural viability by limiting non-agricultural land use. It offers Agricultural Land Easements (ALE) and Wetland Reserve Easements (WRE) to protect and enhance the conservation values of working farms and wetlands. Through financial assistance, NRCS supports eligible partners in maintaining land for agriculture and restoring degraded wetlands. This program is crucial for preserving West Virginia's agricultural landscape and promoting sustainable land management practices.

The Benwood Foundation Grants focus on fostering shared prosperity in Chattanooga by partnering with diverse stakeholders. They prioritize inclusive growth, particularly for Black and Latinx communities, and support initiatives in urban vitality, talent, economic opportunity, leadership engagement, and cultural belonging. The foundation is committed to creating equitable pathways to success and addressing systemic barriers. Proposals are invited from strategic partners aligned with these goals, emphasizing collaboration for greater community impact.

The Champlin Foundation, a private funder established in 1932, has allocated over $705 million to support Rhode Island's nonprofit organizations through capital grants. Focusing on diverse sectors such as Arts, Education, and Healthcare, the Foundation aims to enhance the stability and growth of these organizations by funding essential equipment and facility needs. This targeted support is vital for long-term sustainability, addressing various community needs while prioritizing equity and inclusion in its grantmaking processes.

The WV Agricultural Conservation Easement Program (ACEP) focuses on conserving agricultural viability through Wetland Reserve Easements (WRE) and Agricultural Land Easements (ALE). By limiting non-agricultural uses, the program enhances wetlands and grazing lands, providing vital habitat restoration. NRCS supports landowners with technical and financial assistance to restore and protect wetlands. Applications are accepted year-round, ensuring ongoing opportunities for eligible participants to engage in conservation efforts and preserve ecological integrity in West Virginia.
