DOH 59808- Rural Health Transformation Program- Healthier Ohio Initiatives - Preventing and Addressing Chronic Disease- Track One

Project Background:

The Rural Health Transformation Program (RHTP) was authorized by the One Big Beautiful Bill Act (Section 71403 of Public Law 119-21) and empowers state to strengthen rural communities by improving healthcare access, quality and outcomes by transforming the current system. Five key strategic goals of the program, as defined by the Centers for Medicare and Medicaid (CMS) are:

• Make Rural America Healthy Again
• Sustainable access
• Workforce development
• Innovative Care
• Technology Innovation

Ohio’s plan aligns with federal priorities to improve the health of rural residents by building an innovative movement to bridge medicine and preventive behaviors into a holistic and collective impact approach to improve chronic disease outcomes. This Request for Proposals (RFP) is designed to strengthen preventive and clinical care for improved blood glucose and hypertension indicators across Ohio’s 73 non-urban counties (See Appendix B. Applicants could be but not limited to the following:

• State-level associations that support implementation in multiple clinics, health centers, and/or hospitals.
• Regional collaboratives or health system with multiple locations.
• Individual hospitals, health centers and clinics

Project Objective:

Based upon data analysis from Ohio’s State Health Assessment, including a supplemental deep dive into Appalachian health, 2025 Summary Assessment of Older Ohioans, the Ohio Chronic Disease Atlas 2025, and other publicly available data, Ohio seeks to reduce the prevalence and mortality rates associated with chronic disease. The collective aim of the strategies is:

to reduce hemoglobin A1C for patients diagnosed with prediabetes and diabetes by 10% and

to manage blood pressure at clinically appropriate levels for patients diagnosed with hypertension.

Gallia County transportation providers serving seniors and people with disabilities can access funding through the Ohio Department of Transportation to contract with local transit operators for open-door, shared-ride services. Section 5310 Contracted Transportation Service Funding reimburses 80% of eligible capital costs, requiring a 20% local match from non-U.S. DOT sources.

Nonprofit and public transportation providers offering open-door shared-ride services for seniors and people with disabilities in Gallia County, Ohio can seek operating support through the Specialized Transportation Program (5310). Adaptive Sports in Schools Mobility Grants

Request for Proposals

More than 7 million students in the U.S. receive special education services for disabilities in public schools. A 2010 study by the U.S. General Accountability Office found that students with disabilities participated in athletics at significantly lower rates than their peers. Currently, 2,490 k-12 public schools in the U.S. are members of the American Association of Adaptive Sports Programs schools and have adaptive sports programs for students.

The Bridgestone Americas Trust Fund seeks public schools pursuing funding to start adaptive sports programs or increase access to adaptive sports and equipment for children and youth. Bridgestone is offering eight $25,000 “mobility” grants to organizations that can establish new athletic programming for students with disabilities. Organizations must articulate a clear plan to provide access to new athletic programs for students with disabilities in one or more public schools for students grades K-12.

The Scottie W. Cox Memorial Scholarship of NCCCLA supports employees of learning resources centers in North Carolina pursuing advanced degrees in library and information studies. Eligible applicants must have at least a bachelor's degree and be enrolled in accredited programs. This scholarship, administered by the North Carolina Community Foundation, opens for applications on December 16, 2025, and closes on March 3, 2026. The Reid Sasnett Memorial Scholarship Endowment supports graduating seniors from Pungo Christian Academy and Beaufort County high schools pursuing higher education. The scholarship may be renewed at the discretion of the Scholarship Committee, subject to academic criteria and available funds.

The Sammy and Shirley Church Scholarship supports students from Ashe County, NC, including those graduating from the NC School of Science and Math. Eligible candidates must demonstrate academic excellence, physical vigor, and leadership in various activities. Financial need is considered but not the only criterion. Applications open on Dec. 16, 2025, and close on March 3, 2026. Ensure to meet specific eligibility requirements set by the donor for successful application.

The Ruby A. Trivette Ruritan Scholarship, administered by the North Carolina Community Foundation, supports graduating seniors from Ashe and Watauga counties in North Carolina. Eligible applicants must reside in Todd or be active members or children of active members of the Todd Ruritan Club. This scholarship opens for applications on December 16, 2025, and closes on March 3, 2026, providing crucial financial assistance for students pursuing higher education.

The Robert E. Barnhill, Sr. Scholarship Fund provides financial support for North Carolina high school seniors who plan to attend a four-year college or university as full-time students. Eligible applicants are evaluated on leadership potential, community involvement, academic achievement, and financial need. The scholarship may be renewed for up to three additional years, contingent upon maintaining a 2.0 GPA and continuous enrollment. Applications open on December 16, 2025, and close on March 3, 2026.

The RAK Education Foundation Scholarship provides financial assistance to residents of Camden, Chowan, Gates, Pasquotank, or Perquimans Counties who have earned a high school diploma or GED. This scholarship supports students pursuing degrees or certificates in Career and Technical Education fields such as trade education, computer science, business, health science, and more. Applications open on December 16, 2025, and close on March 3, 2026, encouraging eligible students to apply.

The Innovation Grants to Nurture Initial Translational Efforts (IGNITE) program, supported by the National Institutes of Health, aims to fund pharmacodynamic and pharmacokinetic studies. These studies will assess the biological activity of therapeutic agents, including small molecules and biologics, for diseases prioritized by participating institutes. The initiative emphasizes the development of non-opioid analgesics with minimal addiction risks.
