Community Health Workers Grants: Strengthening the Rural Health Workforce in Wisconsin

Funding opportunity summary

The Wisconsin Department of Health Services (DHS) seeks to strengthen the Wisconsin rural health workforce by developing and expanding high-quality and impactful community health worker (CHW) programs in clinical and community settings. There are three main components to this funding opportunity:

• Hiring and supporting CHWs to carry out services in rural communities
• Training CHWs on core competencies and standards

Coachella Valley Mountains Conservancy

Our mission is to protect the natural and cultural resources of the Coachella Valley: the scenic, wildlife, cultural, geologic, and recreational resources that make this such a splendid place for people and all the other life forms with which we share this special place.

The Coachella Valley Mountains Conservancy is a state agency established in 1991 to pursue this mission, and is directed by a 20-member governing board. The Conservancy is an exceptional example of local communities, state elected officials, non-profit organizations, and business and environmental interests working together for the good of all. Since its inception, the Conservancy and its partners have conserved over 106,000 acres.

Proposition 4 Grant Program

The objectives of the Safe Drinking Water, Wildfire Prevention, Drought Preparedness, and Clean Air Bond of 2024 (“Proposition 4”) are to target the most urgent climate needs by investing in preventing devastating wildfires, providing clean, safe drinking water, and boosting water supplies, and protecting California’s iconic landscapes. Proposition 4 project investments aim to assist communities that have been historically underserved by allocating a minimum of 10 percent of available funds to severely disadvantaged communities and a minimum of 40 percent of available funds to disadvantaged communities. 

The Conservancy has approximately $11 million available for local assistance grants over the life of its Proposition 4 Grant Program. The minimum local assistance grant amount is $50,000 for implementation projects.

The NEF Research Grant Programme supports individual researchers in developing countries of the Asia-Pacific region. It aims to enhance their research skills in nature conservation through financial assistance for scientific projects. Eligible applicants must be full-time researchers or doctoral students, able to communicate in English, and not currently funded by NEF. Projects should involve fieldwork and aim to submit results within one to two years. The programme encourages sustainable research practices while strengthening local capacities.

The WHALE TAIL Grants Program, managed by the California Coastal Commission, supports initiatives that engage children and the public with California's coast and watersheds through hands-on education and stewardship. The initiative fosters collaboration while prohibiting funding for operating costs, travel, and faith-based advancements.

The Mosquito Control Grant Program in Ohio, administered by the Ohio Environmental Protection Agency, aims to combat mosquito-borne viruses like Zika and West Nile. Texas Health and Human Services

Texas Health and Human Services, also known as the Texas Health and Human Services Commission (HHSC), helps more than 7.5 million Texans every month. The agency supports the needs of mothers, children and families through programs such as Medicaid, CHIP, SNAP, women’s health and behavioral health services.

HHSC provides long-term services and supports for seniors and people with disabilities, and operates state supported living centers for residents with intellectual and developmental disabilities and state psychiatric hospitals. HHSC also regulates health care providers, professions and facilities to protect the health and safety of Texans.

Pediatric Tele-Connectivity Resource Program for Rural Texas

Executive Summary

The Texas Health and Human Services Commission (HHSC), the System Agency, is accepting applications for the State Fiscal Year 2026 Pediatric Tele-Connectivity Resource Program for Rural Texas Grant Program. The purpose of this program is to connect Rural Hospitals (RHs) and Rural Health Clinics (RHCs) with (i) pediatric specialists and pediatric subspecialists who provide Telemedicine Medical Services (TMS) or (ii) an institution of higher education that is a member of the Texas Child Mental Health Care Consortium established under Chapter 113 of the Texas Health and Safety Code. The Indiana Office of Community and Rural Affairs works with Indiana communities to build relevant and economically thriving places where people want to live, work and grow.

Mission: The Indiana Office of Community and Rural Affairs works with local, state and national partners to provide resources and technical assistance to aid communities in shaping and achieving their vision for community and economic development.

CDBG Planning Grant - Environmental Assessment Plan

The Planning Grant Program leverages Community Development Block Grant (CDBG) funding from the U.S. Department of Housing and Urban Development to encourage communities to plan for long-term, holistic improvement. The Office of Community and Rural Affairs (OCRA) seeks applications from local units of government for programs funded through the State CDBG program. Eligible local units of government are counties, cities, or incorporated towns not located within an entitlement community. Unincorporated areas must apply through the county in which they are located. Eligible planning activities consist of all costs associated with data gathering, study, analysis, and preparation of plans and the identification of actions to implement such plans.

The Columbia County Children's Fund Grant, administered by Innovia Foundation, focuses on supporting local organizations that provide essential programs for children. Eligible applicants include public charities, federally recognized tribes, and public entities serving Columbia County. The grant prioritizes educational enrichment, counseling, and medical services for disadvantaged children. Innovia Foundation encourages applications from organizations within its 20-county region, ensuring that funding contributes to meaningful community impact.

The Josephine Goodyear Foundation, established in 1912, provides approximately 50 grants annually totaling around $300,000 to support non-profit agencies assisting indigent women and children, primarily in the Greater Buffalo area. Applications are accepted from tax-exempt organizations with specific deadlines. The foundation's mission has expanded beyond medical facilities to address broader needs, ensuring vital support for vulnerable populations.

Joseph G Markoly Foundation Inc

When Joseph Markoly originally established the foundation more than thirty years ago, its focus was on Alzheimer’s and heart disease support, funded through a number of the larger national disease-related organizations. Over time the focus has shifted to helping small to mid-size organizations in our local communities which support children and seniors in a variety of effective ways. 

Markoly Foundation was established in honor of Joseph G. Markoly, an entrepreneur and philanthropist who wanted to make a positive impact in people’s lives. Joseph’s family was affected directly by both Alzheimer’s and heart disease having lost a family member to each condition. For that reason, the foundation dedicates a majority of their resources to providing grants to organizations that focus on the treatment of each of these diseases. In addition, Markoly Foundation supports a variety of programs dedicated to helping children and seniors by providing services, supplies, programs and research in areas impacting these demographics.

Joseph G. Markoly Foundation Grant Support for Elder Organizations

The Markoly Foundation takes pride in supporting local organizations that provide services for elders in our community. Seniors face a number of challenges that come with aging, especially when they don’t have loved ones who are able to assist them with their needs. We provide grants for a variety of senior-related programs from Caregiver Education and Support to Emergency Meal Delivery and Transportation Services.

We are dedicated to providing funding for local nonprofit programs that improve the quality of life for all, but particularly for the children and elders in our community.

Helpers Community Grants

Helpers Community cares deeply about nonprofits that enrich the lives of individuals with developmental disabilities. Helpers Community is dedicated to supporting nonprofit agencies and individuals that help those who provide residential care, recreation, and job training programs to Adults with developmental disabilities. Helpers Community grants fund 501(c)(3) organizations that serve individuals with developmental disabilities, with priority given to those in the state of California.

Organizations that qualify through their commitment to serve Adults with developmental disabilities can apply for grants for infrastructure and materials for specific programs or projects, including facility improvement projects, but not for general operating support and/or salaries.

The amount of funding requested, keeping in mind our typical grant awards range from $15,000 to $25,000, and if any other potential funders have been approached or have committed to fund a portion of this infrastructure need.

The LP Community Grant, funded by the Louisiana Pacific Foundation, supports local nonprofits and public schools in North America. Ranging from $10,000 to $20,000, grants are determined by community needs and team member requests. Established in 1973, the LP Foundation has contributed over $22 million to enhance education, environmental efforts, and community development. Tree Research and Education Endowment Fund (TREE Fund)

TREE Fund is a 501(c)3 nonprofit whose mission is to support scientific discovery and dissemination of new knowledge and best practices in the fields of arboriculture and urban forestry.

TREE Fund’s research and education programs shape industry best practices, improve workforce safety and contribute to the long-term sustainability of our communities in the following crucial ways:

Lucas Tree Experts Integrated Vegetation Management Endowed Research Fund

Since 1926, John Lucas Tree Expert Co. has worked with investor-owned utilities, electric cooperatives, municipalities, railroads, and pipeline customers throughout the United States and Canada. Innovation has always been a core value, and Lucas remains committed to innovation that makes the industry safer, greener, and more efficient. The Lucas Tree Experts Integrated Vegetation Management Endowed Research Fund is a new endowment fund of Tree Research and Education Endowment (TREE) Fund created to support research in Integrated Vegetation Management in the United States and Canada.

This fund program was created in 2026 to help celebrate Lucas Tree’s 100th anniversary. Lucas Tree’s anniversary marks a century of innovation, safety, and stewardship in utility vegetation management.

Projects are expected to be completed within one to three years. No project may receive more than one award from this program.

Naples Children Foundation

Grant Guidelines:

The Naples Children Foundation (NCF) was established in 2000 to transform the lives of vulnerable children in Collier County by investing in, and partnering with, proven programs that bridge gaps in education, health care and child advocacy to deliver measurable results.

In 2005, we commissioned The Study of Child Wellbeing, an assessment of the needs of children in Collier County that found significant shortfalls in basic services including pediatric oral health, early childhood education, child advocacy, pediatric medical services and out-of-school services. In 2010, a follow up study was conducted identifying additional voids including childhood hunger, which is now a key area of focus. Further studies conducted in 2017 and 2022 confirmed the persistent need for targeted initiatives to bridge these gaps. These studies led to the development of — and reinforce — the following seven NCF Strategic Initiatives, each designed to address specific areas of need:

• Healthcare and Oral Health (est. 2006)
• Early Learning (est. 2008)
• Hunger (est. 2011)
• Mental Health, Vision and Out-of-School (est. 2012)

Every dollar we generate fuels high-impact solutions and, since its inception, NCF has transformed entire communities and positively impacted the lives of 385,000 children through its strategic investments in close to 90 nonprofit organizations. The Barney Family Foundation Grant focuses on improving early education and advancing medical research, particularly in cancer and Alzheimer’s. It supports organizations that demonstrate a clear plan to enhance children's life experiences and educational opportunities for those in grades K-8. Grants typically range from $5,000 to $50,000, prioritizing established entities while welcoming startups. Eligible applicants must have 501(c)(3) status, and the Foundation does not fund individual requests or political activities.

Loene Pooler Guthrie Scholarship Fund

The Loene P. Guthrie Scholarship fund benefits music students in Benton, Linn, and Lincoln counties who plan to engage in the teaching of piano, voice, and orchestral music.

This scholarship fund promotes the teaching of piano, voice and orchestral music by aiding music students in Benton, Linn, and Lincoln Counties who plan to engage in the teaching of piano, voice, and orchestral music. Students who will graduate from Linn, Benton, or Lincoln County high schools or graduates of those high schools currently studying music at an institution of higher learning may apply. The Linn-Benton Chapter of the Oregon Music Teachers Association reviews the applications and selects individuals to perform at an audition open to the public.

The Core State Injury Prevention Program (Core SIPP) aims to enhance public health infrastructure and partnerships to prevent unintentional injuries through data-driven strategies. Focusing on key areas such as traumatic brain injury, falls among older adults, and drowning, it seeks to improve community safety by strengthening data use and fostering collaborations. This initiative supports states in identifying local injury concerns, thus promoting evidence-based prevention approaches and increasing the capacity for sustained public health actions.

Grant Information

• Characterize the occurrence of trifluoroacetic acid (TFA) and other ultrashort-chain organofluorine compounds included in the Sixth Unregulated Contaminant Monitoring Rule (UCMR 6) in representative U.S. drinking water systems.
• Evaluate changes across existing full-scale treatment processes using source and finished water samples to identify treatment trains that demonstrate measurable removal or reduction of these compounds.
• Develop a response and communication protocol to support utilities when communicating the results of the sampling and potential detection of TFA and other UCMR 6 ultrashort-chain organofluorine compounds.
