To support the successful design, implementation, and learning objectives of the Hospital and Housing Supports Services Integration Pilot, DMAHS is issuing a separate RFP for the Hospital and Housing Integration Technical Assistance Grant Program.
The Technical Assistance Grant Program will fund a qualified organization to provide technical assistance to pilot participants and to support the State in assessing the quality of these pilots. The selected technical assistance provider will serve two core functions:
a. Pilot implementation support, including assisting with the launch and ongoing operations of the pilot, supporting hospital–provider partnerships, developing training material on best practices, facilitating shared learning across pilot sites, and communicating with DCA/DMAHS on pilot progress. Further enumeration of the support that the TA organization will provide to awarded pilot participants will be enumerated in Section V; and
b. Pilot mixed-methods assessment, including conducting analysis of pilot implementation — such as interviews with participating hospitals, Housing Supports Program providers, and other relevant stakeholders and descriptive data analysis — and producing a written synthesis of implementation experiences, effective practices, challenges encountered, and key lessons learned to inform future program design and potential scale.

Indian Equity Fund Small Business Grant
The Indian Country Economic Development Program, created by the 59th Montana State Legislature, is a State-funded program administered by the Montana Department of Commerce. Various programs are funded under the ICED Program, including: (i) the Tribal Business Development Grant; (ii) the Tribal Tourism Grant; (iii) the Indian Equity Fund Small Business Grant; and (iv) the Native American Business Advisors. These programs allow the Office of Indian Country Economic Development at Commerce to continue to articulate measurable successes of the program to tribal communities, the executive branch, and elected leadership.
The Indian Equity Fund Small Business Grant assists startup or expanding Native American businesses in Montana. This money can be used for a variety of activities, such as the purchase of new equipment or the development of a new product line. A total of $320,000 is available to fund Native American businesses in Montana. The funds are available to enrolled members of the eight federally recognized tribes in Montana. All applications must include documentation of tribal enrollment in the form of a Certified Indian Blood Form or Tribal ID. An application will be considered incomplete without this information.

Economic Development Organization Capacity Building Grant
The Opportunities in Rural Economies Economic Development Organization Capacity Building Grant supports EDOs in strengthening their ability to provide outreach to their communities for economic development.
Eligible uses of grant funds may include, but are not limited to, technology investments, including software and hardware upgrades, workflow optimization, staff training, contracted services, marketing to increase program or organization visibility, community engagement efforts and partnership development with local lenders, workforce entities or education providers and other capacity-building uses that align with programmatic strategy. ORE EDO CBGP investments aim to ensure Enrolled EDOs have the resources to act as effective intermediaries between Eligible Businesses and the ORE LPP. Funding is not intended to support routine operations but to help Enrolled EDOs strategically increase their impact within Rural Montana.

Montana Historic Preservation Grant Program
The Montana Historic Preservation Grant (MHPG) Program is a state-funded program created in 2019 via Senate Bill 338, Sections 1 through 17. The MHPG Program is designed to support public or private entities with the preservation of historic sites, historical societies, or history museums through grant funding. ADRC-Supported Programs: Request for Pilot Projects for Early Stage Investigators
CART Discovery Grant
Projects are intended for new/early-stage faculty only (i.e, instructor, assistant professor, or equivalent, with institutional privileges to serve as PI on federal grants).
Areas of research can range from basic, translational and clinical investigations. Studies of pre- clinical phases of disease and approaches useful in differentiating disease processes from normal aging, as well as multi-etiology dementia, early interventions, imaging and biomarkers, are encouraged. Applicants may consult with ADRC scientists for strategic and statistical advice.
Please write a cover letter and include affirmation that this is a NEW pilot project and not supported by federal or non-federal sponsored grants. The applicant may request a project period of up to one year with a budget for DIRECT COSTS of up to $50,000. CART will send payment for these Pilot Grants at the beginning of the project. INDIRECT COST RECOVERY IS NOT ALLOWED.

Build MI Community Grant Initiative
Program Overview & Goals
The Build MI Community Grant is an initiative designed to promote private investment in Michigan communities. Administered by the Michigan Economic Development Corporation (MEDC) on behalf of the Michigan Strategic Fund (MSF), this tool provides access to real estate development gap financing and technical assistance for small scale, incremental redevelopment projects.
The Build MI Community Initiative has been established to re-activate underutilized or vacant space into vibrant areas by promoting capital investment into redevelopment projects being taken on by developers and property owners with limited real estate development experience and familiarizing them with the development process in order to position them to potentially undertake more complex projects in the future.
FY27 Nurse Educator Fellowship Program
The purpose of the Nurse Educator Fellowship Program is to ensure the retention of well-qualified nursing faculty at institutions of higher learning that award degrees in nursing. Awards will be used to supplement the salaries of the nursing faculty selected for the Fellowship. Participation in this program is open to Illinois institutions of higher learning with a nursing program approved by the Illinois Department of Financial and Professional Regulation and accredited by the Commission on Collegiate Nursing Education (CCNE) or the Association Commission for Education in Nursing (ACEN). Nominations must be certified by the institution's Chief Nursing Administrator.

Project Details
A statewide program expanding youth tele-behavioral health services to increase access to youth-serving clinicians with a focus on rural areas, school partners and students ages 12-18. Rural Maine communities often have worse behavioral health outcomes than the national average. Similar models of this program have been successfully implemented in select school districts in response to access barriers faced by remote regions of Maine. While these models have included a Community Health Workers (CHW) embedded in school districts to help with facilitation, proposals are not limited to this model. Additionally, this program is not restricted to facilitation in the school setting only, as youth may also prefer to access appointments at home or in another setting.
Who We're Serving
School-aged students and youth across Maine, with a focus on ages 12-18 in rural zip codes. While Maine is experiencing a behavioral health workforce shortage, rural populations face additional barriers, such as long travel times.
Objective
Expand youth tele-behavioral health services to the school setting to increase access to youth-serving clinicians with a focus on rural school districts and grades 6-12 or ages 12-18. Provide assistance for student referrals both during and outside of school hours.
FRCR Plan Developer Grant
The FRCR Plan Developer Grant provides funding to eligible organizations to develop Farmland Restoration and Climate Resiliency (FRCR) Plans for agricultural producers. These long‑term plans (5–10 years) identify strategies for restoring farmland, implementing climate‑smart practices, and reducing vulnerability to extreme weather events.

Equipment Grant Programs
The Arkansas Department of Agriculture is accepting applications for two equipment grant programs: Breakfast Equipment Expansion Grant and the USDA Equipment Assistance Grant. The purpose of these grants is to help Arkansas schools expand access to nutritious meals, increase the use of Arkansas-grown products, and boost participation in school nutrition programs.
Breakfast Equipment Expansion Grant
The Breakfast Equipment Grant Expansion provides funding to help Arkansas schools enhance their breakfast programs and increase the use of Arkansas‑grown, raised, and processed foods. Through a competitive application process, schools can receive equipment that supports innovative breakfast service models such as grab‑and‑go options, breakfast in the classroom, mobile carts, refrigerated kiosks, and meal preparation tools that feature local products. This initiative strengthens student access to nutritious morning meals, improves meal quality and appeal, and expands purchasing opportunities for Arkansas farmers. All awarded funds must be used by December 30, 2026, and selected projects will demonstrate measurable impacts on breakfast participation and Arkansas product utilization.

This telehealth model connects residents and care teams with specialty providers to address workforce shortages and limited access to geriatric expertise in rural communities, helping improve access to care and reduce avoidable emergency department visits and hospital transfers.

This project is intended to support:
The project ultimately benefits older adults living in rural Maine who may face barriers to specialty care due to workforce shortages, geographic distance, transportation challenges, and limited access to geriatric specialists. Emphasis is placed on individuals with chronic conditions, behavioral health needs, medication-management challenges, and other complex care needs that benefit from timely specialty consultation.
Objective
Enable participating nursing facilities to adopt standardized telehealth workflows that expand timely access to geriatric specialty and geriatric psychiatry services, strengthen care coordination and continuity of care, improve management of chronic conditions, behavioral health needs, and medication-related issues, reduce avoidable emergency department visits and hospital transfers, support aging in place through access to specialty expertise, and enable residents to receive appropriate specialty care within their local facility whenever possible.
Preference may be given to facilities serving rural, underserved, uninsured, and/or MaineCare populations.

MCD Global Health (MCD) was selected as a subawardee to support the implementation of Maine’s technology and innovation initiatives in close collaboration with Maine DHHS and statewide partners. This includes supporting the implementation of a variety of digital health solutions and modalities, including telehealth, remote-patient monitoring, provider-to-provider consultations, and digital therapeutics deployed across partner sites, with a focus on reducing distance as a barrier to specialty care, behavioral health, and chronic disease management.

The Maine Rural Health Transformation Program (RHTP) expands access to chronic disease management through an Adult Remote Patient Monitoring (RPM) initiative serving individuals with conditions such as hypertension and heart failure. This telehealth model uses connected monitoring devices, remote clinical oversight, and care coordination to support earlier intervention and improve health outcomes for rural patients who may face barriers to ongoing care and monitoring.

The project ultimately benefits adults with hypertension, heart failure, and other chronic health conditions who may face challenges of accessing ongoing monitoring and support. Emphasis is placed on individuals at increased risk for avoidable emergency department visits, hospitalizations, and complications related to chronic disease.
Objective
Enable participating clinical organizations to adopt standardized Remote Patient Monitoring workflows that support proactive management of hypertension, heart failure, other high-risk chronic conditions, and pregnancy-related hypertension. Participation will improve chronic disease management and maternal health outcomes, strengthen patient engagement and self-management, support early identification of clinical deterioration and pregnancy-related complications, improve continuity of care, strengthen care coordination, support early intervention, and reduce avoidable emergency department visits, hospitalizations, and adverse maternal and chronic disease outcomes.
