• Education and support for the safe and confident use of digital financial tools.
• Fraud and scam awareness, prevention and recovery initiatives, including protection from identity theft.
• Interventions to help households recover from major financial shocks (e.g., healthcare costs, extended family care, job transitions or natural disasters).

Funding Opportunity

The Foundation plans to award 70 $500,000 (USD) grants (to be disbursed over two years) totaling $35 million to community organizations working in select communities where Citi has a presence.

The purpose of Activity 5.3c- Rural Pharmacy Medication Management Therapy Program is to improve health outcomes in rural communities through Medication Therapy Management (MTM), tobacco cessation services, and chronic disease prevention and management that are not covered by medical insurance. This program aims to address medication adherence/polypharmacy and tobacco use for people with or at risk for chronic conditions, delay disease progression, and support overall well-being, with a particular focus on uninsured and underinsured populations. Programs should address chronic conditions such as diabetes, hypertension, heart disease, and hyperlipidemia, in addition to addressing related risk factors such as tobacco product use. The Rural Health Screening and Education program will increase access to chronic disease screening and education for rural residents and expand awareness of chronic disease risk factors and their health impacts. The Rural Health Screening and Education Program aims to improve chronic disease outcomes among rural residents by providing walk-in and scheduled screenings, transportation, insurance enrollment, SDOH referrals, follow-up care coordination, and enrollment in evidence-based education programs to manage chronic conditions.

Goal: Enhance and expand early detection, prevention, and management of chronic diseases through coordinated screening and education initiatives encouraging proactive engagement in preventive care, healthier behaviors, and improvement in long-term health outcomes among residents

Activity 5.3b: Enhancing Rural Cancer-Focused Healthcare (ERCH)

Program Goals: The Enhancing Rural Cancer Healthcare program aims to implement sustainable policy, systems, and environmental change approaches across the rural areas of 8 Regional Chronic Disease Coalitions to reduce the burden of cancer and other chronic and reproductive diseases. There will also be a focus on supporting/providing cancer survivorship care to residents of rural areas. Residents of the rural areas served by this project will receive culturally specific cancer education, outreach, referrals to appropriate screening services, and survivorship care services. These project efforts may reduce cancer incidence and mortality in rural New Jersey.

• Objective 1.1: Reduce cancer mortality and morbidity in New Jersey rural communities.
• Objective 1.2: Establish PSE change strategies to improve the health and well-being of New Jersey rural communities with relation to cancer primary prevention, early detection, and survivorship.
• Objective 1.3: Increased education, outreach, and screening within rural communities related to various cancer, chronic, and reproductive diseases.
