Health Zones Program in Philadelphia, PA

A variety of factors influence a person’s health, including access to health care services, healthy food, and transportation. To tackle these challenges and improve health outcomes, the CVS Health Foundation (“the Foundation”) is collaborating with local organizations with a deep understanding of the community through our Health Zones initiative. We are committed to enhancing the coordination of health care and social services while dismantling barriers to health and wellness. Our vision is to empower and support local care systems within each Health Zone, making health care and social support networks more accessible and easier to navigate for everyone.

As part of the Health Zones initiative, we look for communities where many people are facing financial strain and have limited access to essentials like reliable health care, nutritious food, stable housing and local support services. When these basic needs are out of reach, it becomes much harder for individuals and families to stay healthy and manage chronic conditions - and these are the places where our support can make the greatest difference. This RFP will provide a three-year grant of up to $2.25 million to a cohort of nonprofit organizations that can support our Theory of Change to implement strategies that improve health outcomes for residents living in Philadelphia.

The Japan International Award for Young Agricultural Researchers encourages young talent in agriculture, forestry, and fisheries in developing regions. Since its inception in 2007, this award recognizes up to three researchers under 40 who demonstrate exceptional research and innovation potential. Awardees are invited to Japan for a ceremony and receive $5,000 each. Housing Place-Based Services Request for Proposals

Through this RFP, MOHCD seeks nonprofit partners to provide place-based services that help maintain housing stability and reduce displacement among residents of federally subsidized housing properties. These funds will support effective, accessible, and culturally responsive Housing Place-Based Services that advance individual and collective opportunity, support community resilience, and strengthen housing stability.

Services funded through this RFP shall be delivered on-site to extremely low-income residents of federally subsidized affordable housing developments and/or cooperatives.

The AgEnhancement Grant, awarded by Farm Credit East and CoBank, provides funding between $500 to $10,000 annually to support projects that enhance agriculture, commercial fishing, and forest products in the Northeast. Since 1996, over $3.5 million has been distributed to various organizations. The program seeks initiatives that promote diversity and provide opportunities for all-sized farms, ensuring long-term benefits to the community and industry.

The Episcopal Presbyterian Health Trust Grant, managed by the St. Louis Community Foundation, focuses on enhancing health care access and addressing chronic diseases. Grants are awarded biannually, prioritizing maternal and infant health in the second cycle. Organizations are encouraged to submit one Letter of Intent (LOI) annually, ensuring their proposals align with the funding priorities to foster sustainable health solutions in the community.

The Washington Coast Restoration and Resiliency Initiative provides critical funding and technical assistance to coastal communities in Washington State, addressing urgent ecological restoration needs while also creating jobs. Established by the Legislature in 2015, this program aims to enhance the health of the land and water, supporting both local economies and the environment. Projects funded must prioritize ecological protection and restoration, ensuring sustainable benefits for future generations.

The Vermont Housing and Conservation Board offers Organizational Development Grants to support nonprofit organizations in developing affordable housing and conservation projects. Eligible applicants, including municipalities and certain state agencies, can receive up to $33,750, while new organizations may apply for up to $12,000. The program encourages collaboration and continuous improvement to achieve sustainable reductions in these critical public health issues.

The United Way of St. Lucie & Okeechobee offers Community Impact Grants aimed at enhancing health, education, and financial stability in the community. Nonprofits serving these counties are encouraged to apply, provided they meet specific eligibility criteria, including being a 501(c)(3) organization and having operational experience of at least two years. This initiative focuses on addressing critical needs such as food security and housing assistance, fostering a sustainable and impactful community.

The Robert B. and Ruth K. Lee Foundation Grant aims to enhance the quality of life in Greenville County, South Carolina. Established in 2000, the foundation supports diverse areas including arts, education, health, and human services. With no geographic limitations, it primarily funds programs benefiting the local community. Grants typically range from $2,000 to $5,000, emphasizing initiatives for the elderly, children’s programs in the arts, and literary projects. Organizations must be 501(c)(3) exempt to be eligible.

El Paso County Community Development Block Grant

Since 2009, El Paso County Community Development Block Grant funds have been awarded to local jurisdictions and non-profit organizations. Agencies with experience administering federal funding will likely have adequate internal controls and policies to successfully implement a CDBG funded project. Eligible projects are those that serve low-to-moderate income residents (LMI), create jobs for low-to-moderate income clients (LMJ), or serve a low-to-moderate income area (LMA) within El Paso County.

The Greater Milwaukee Foundation's GMF: Cycle 2 - General Operating Support grant focuses on enhancing early childhood education, mental health services, and workforce development. It aims to promote equitable access to quality education and mental health resources, addressing systemic disparities in the Milwaukee area. Organizations seeking funding for innovative projects are encouraged to apply for grants up to $50,000 per year for three years, emphasizing racial equity and community impact.

The RIF Community Priority Grants, initiated by the Rhode Island Foundation, aim to enhance community well-being through targeted support to 501(c)(3) nonprofit organizations in Rhode Island. These grants focus on addressing six identified community priorities, promoting diversity, equity, inclusion, and access.
