Texas Health Community Impact Grants (Request for Proposals)

Texas Health Community Impact requests proposals to fund local organizations collaborating on innovative solutions that address barriers to community health and wellbeing. Up to $5 million is available across nine counties for the 2027-2028 grant cycle.

These community health improvement grants are made possible by Texas Health Resources—a faith‑based, nonprofit health system that cares for more patients in North Texas than any other provider.

Key Requirements

Applications must fulfill four key requirements: 1. Propose an innovative approach. 2. Demonstrate a collaborative effort between two or more organizations. 3. Align with strategic priorities. 4. Promote sustainability. Read Grant Key Requirements.

Project Implementation

Applicants must demonstrate the ability to begin implementing activities within four months of the execution of the MOU. Exceptions may be considered on a case-by-case basis.

Request for Proposals by Region

The Texas Health Community Impact Grant requests proposals by regions: 1) Collin Region; 2) Dallas and Kaufman Region; 3) Denton, Wise and Parker Region; 4) Southern Region; and 5) Tarrant Region.

Therefore, the Collin Leadership Council will seek innovative proposals to reduce barriers related to NMDOH in Healthcare Access, Literacy & Navigation, as well as Connectedness, through enhanced access to nutrition, chronic disease management, primary care, and social connectedness in 4 high need zip codes: East McKinney (75069), Farmersville (75442), Princeton (75407), and Blue Ridge (75424).

The Wyoming Nonpoint Source Program, part of the WDEQ, aims to enhance water quality through voluntary pollution reduction efforts. It provides $40,000-$80,000 annually via Section 205(J) grants to local agencies for planning and assessment projects, prioritizing efforts in watersheds affected by impaired waterbodies. This initiative encourages collaboration among landowners and various government entities to effectively tackle nonpoint source pollution, which is challenging to identify and manage.

The FACF: Ice Mountain Environmental Stewardship Fund, established in 2002 by Great Spring Waters of America, Inc., aims to sustain the Muskegon River Watershed through conservation and restoration projects. Grants of up to $50,000 are available for initiatives that enhance water quality, reverse degradation, and foster collaboration among organizations. Eligible applicants include 501(c)(3) nonprofits, government units, and educational institutions focused on benefiting the watershed and its communities.

Eligible applicants are encouraged to participate in training sessions, which are free of charge, to better navigate the grant application process.

The Louisiana Forest Legacy Program aims to protect privately-owned forestlands through land acquisition and conservation easements, ensuring the sustainability of vital natural resources. This voluntary initiative, supported by the USDA Forest Service and the Louisiana Department of Agriculture and Forestry, helps maintain timber production, wildlife habitats, and community benefits while preventing forest fragmentation due to development. By engaging willing landowners, the program promotes responsible forestry practices and safeguards the ecological integrity of Louisiana's forests.

The Louisiana Forest Productivity Program offers financial assistance to landowners in Louisiana to establish and enhance timber crops. By supporting forestry practices, this program aims to improve productivity and offset costs associated with tree planting and management. Eligible participants must own at least 10 contiguous acres of suitable land and commit to maintaining the forestry usage for ten years. This initiative plays a vital role in promoting sustainable forestry and enhancing the state's agricultural landscape.

HOME-American Rescue Plan (HOME-ARP)

Congress appropriated $5 billion in American Rescue Plan funds to be administered through the HOME Investment Partnerships Program (HOME) to address the need for homelessness assistance and supportive services. The State of Nebraska was allocated $14 million for the Balance of State. The HOME-American Rescue Plan or “HOME-ARP” was created to perform four activities that must primarily benefit qualifying populations. The funding is available through September 2030.

The Forest Collaboratives Grant Program, established by the Oregon Legislature in 2013, aims to enhance restoration efforts on federal forests in Oregon. By providing funding through a competitive process, it supports collaborative groups in developing and implementing restoration projects that focus on vegetation management on USFS- and BLM-managed lands. The Organization Collaboration Grant Program, offered by the Oregon Watershed Enhancement Board (OWEB), aims to foster strategic collaborations that enhance local organizations' sustainability and ecological outcomes. Eligible applicants include groups that support these efforts, especially those with a watershed council, soil and water conservation district, or land trust. Grants can go up to $75,000, enabling organizations to evaluate structures or pursue mergers to improve service delivery and community engagement.

The Melba Bayers Meyer Charitable Trust, established in 1995, supports various charitable causes in Pensacola, Florida, and Alabama. The trust prioritizes organizations that assist children, indigent Native Americans, and neglected animals. Eligible applicants must be 501(c)(3) organizations focusing on religious, charitable, scientific, or educational purposes. The trust aims to make a positive impact in the community by addressing the needs of vulnerable populations.

Anacostia River Watershed Trash Trap Maintenance RFA

DOEE seeks eligible entities to propose ways to monitor and maintain trash traps, provide environmental workforce development, and educate District residents about pollution that impairs the District’s waterbodies. A successful applicant will propose ways to design and implement a trash trap maintenance program for eight (8) trash traps in the Anacostia Watershed for one (1) year: to collect the trash; to collect data on the amount and types of trash being captured; to recruit, train, and employ District residents to assist; and to coordinate opportunities for volunteers to assist with maintenance and learn about the need for trash reduction in District waters and watersheds The amount available for the 12-month project is $160,000.

The Adams County Fund for the Environment Grant, established by the Adams County Community Foundation, aims to foster environmental stewardship within the community. Grants of up to $5,000 are available to 501(c)(3) nonprofits for projects that educate the public on environmental issues and support local initiatives such as tree planting and recycling. This fund underscores the importance of community involvement in addressing ecological challenges.

The DE Urban and Community Forestry Grant Program aims to enhance Delaware's urban tree canopy through quality tree planting and management projects. It encourages sustainable forestry practices, improves stormwater management, and promotes diversity and inclusion in urban forestry efforts. By funding tree planting and management initiatives, this program seeks to improve air quality and foster community involvement in stewardship of urban forest resources.

Engagement Grants aim to facilitate projects that foster communication and engagement with landowners and the community regarding restoration, acquisition, or resource assessment initiatives. These projects must demonstrate a clear connection to specific geographic areas and address ecosystem goals, ensuring measurable outcomes. Education projects are excluded, as eligibility focuses on activities directly linked to habitat protection and restoration, striving to enhance awareness and skills in watershed management.
