What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

What is an eligible area?

Rural areas including cities, villages, townships and towns including Federally Recognized Tribal Lands with no more than 20,000 residents according to the latest U.S. Census Data are eligible for this program.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

Charitable Contributions

Costco Wholesale’s primary charitable efforts specifically focus on programs supporting children, education, and health and human services in the communities where we do business. Throughout the year we receive a large number of requests from nonprofit organizations striving to make a positive impact, and we are thankful to be able to provide support to a variety of organizations and causes. While we would like to respond favorably to all requests, understandably, the needs are far greater than our allocated resources and we are unable to accommodate them all.

Grant Applications

If the request is under consideration, you may be contacted by staff for any additional information needed. Applications are reviewed within 4-6 weeks, and decisions are made based on several factors, including: type of program; identified community need not otherwise available; indication that evidenced based data will establish measurable results of intended outcomes; community collaboration; broad base of financial support; project budget and operating expenses.

Mission

The DanPaul Foundation will use its resources to help train teachers and parents in early childhood development, protect children from abuse and neglect, stimulate children's personal social responsibilities, and offer them opportunities for enrichment and growth.

The Foundation will also encourage children to be concerned and informed about the environment and the underprivileged, particularly with regard to clean air and water, and adequate housing and nutrition for all.

Beliefs

The DanPaul Foundation believes that children should have ample opportunities for enrichment in their lives, and thus strives to provide many different ways to enrich and expand children's minds through direct programs and monetary support to organizations doing similar work.

We have provided or currently provide grants related to the following program areas:

• Workshops, Conferences, + Seminars: We strive to offer educational workshops, conferences, and seminars for parents and teachers on topics related to early childhood development.
• Student Scholarships: We aim to help students attending post-secondary education institutions by providing need-based and academic scholarships.
• Scientific Endeavors: We desire to advance scientific endeavors which seek to improve the quality of life for everyone in the world.
• Clean Air + Water: We hope to pass on knowledge and practical life skills to youth regarding their personal responsibility to the environment, teaching them about issues surrounding clean air and water.
• Child Advocacy: We believe in protecting children from abuse and neglect and particularly love to support programs that provide education and assistance to children as well as organizations advocating or caring for vulnerable children.
• Homelessness: We want to encourage young people to take a personal interest in seeing that adequate housing and proper nutrition, especially for the underprivileged and homeless, are available.
• Poverty + Neglect: We seek to help those in poverty as well as educate youth about their responsibility to consider the underprivileged and take care of those most in need of life's basic essentials like adequate housing and proper nutrition.
• Refugee Enrichment: We wish to help refugee youth by supporting programs that provide them enrichment and help them transition to life in a new country.

The DanPaul Foundation provides grants to 501(c)3 tax-exempt non-profit organizations as defined by the IRS. The Foundation is interested in providing funding to programs that directly serve the health, education, development, and welfare of the world's youth.

Grants range from a few hundred dollars up to $15,000 per calendar year.

Background

DRK Foundation funds early-stage social impact organizations solving the world’s biggest social and environmental problems using bold, scalable approaches.

What stage of growth does DRK Foundation typically fund?

Early stage: Organizations who are early stage, which we define as post-pilot and pre-scale. This typically means: 

• Your program, product or service is already being used in the market or in the field, 
• You have early indication that your model is having its intended impact on the beneficiary populations,
• Your organization is relatively young (ideally between two and five years old, although we will consider both younger and older organizations).

Venture funding: In the case of for profits, we typically support Seed to Series A organizations, and never lead rounds; we also generally but not exclusively refrain from participating in financings exceeding a $15M USD post-money valuation.  

Global Impact Cash Grants

Identifying and incubating innovative early-stage solutions. We identify, incubate, and develop innovative early-stage solutions with the highest potential for impact. Global Impact Cash Grants go to nonprofit organizations (NPOs) and nongovernmental organizations (NGOs) that address a significant social problem. We're looking for programs that fit within our investment sectors, focus on the underserved, and use technology to improve the reach and efficiency of services.

Mission

The mission of the Hearst Foundations is to identify and fund outstanding nonprofits to ensure that people of all backgrounds in the United States can build healthy, productive and satisfying lives. Through its grantmaking, the Hearst Foundations support well-established nonprofit organizations that address significant issues within their major areas of focus—culture, education, health and social service—and that primarily serve large demographic and/or geographic constituencies. In each area of funding, the Foundations seek to identify those organizations achieving truly differentiated results relative to other organizations making similar efforts for similar populations. Culture Grant

The Hearst Foundations fund cultural institutions that offer meaningful programs in the arts and sciences, prioritizing those that enable engagement by young people and create a lasting and measurable impact. The Foundations also fund select programs nurturing and developing artistic talent. Supported organizations include arts schools, ballets, museums, operas, performing arts centers, symphonies and theaters.

Funding Priorities in Culture

In the recent past, 25% of total funding has been allocated to Culture. Organizations with budgets over $10 million have received 60% of the funding in Culture.

The Hearst Foundations are only able to fund approximately 25% of all grant requests, of which about 80% is directed to prior grantees and about 20% is targeted toward new grantees.

Wisconsin accepts applications for the use of Civil Money Penalty (CMP) funds. 

About CMP Funds

Civil money penalties (CMPs) are fines imposed on nursing facilities that do not meet federal health and safety standards. The State of Alaska receives a portion of the funds collected to be reinvested in support of projects that improve the overall quality of life and/or care of nursing facility residents.

Nursing facility providers, stakeholders and other organizations are encouraged to submit CMP fund grant applications for the development and implementation of quality improvement initiatives that directly or indirectly benefit nursing facility residents. 

Project proposals should demonstrate current and sound evidence-based practices that promote quality of care and quality of life for nursing facility residents. CMP fund grants are only available for quality improvement initiatives that are outside the scope of normal facility operations. 

Funding Uses

The following examples of allowable uses of CMP funds are not all-inclusive. Please see the Allowable and Non-Allowable Uses of CMP Funds download for more information. CMP funds may be used for:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs

Division of Care and Treatment Services

The Division of Care and Treatment Services (DCTS) manages and supports seven care and treatment facilities for people living with psychiatric disorders and intellectual disabilities, community mental health and substance use services, community mental health and substance use treatment services for people in the criminal justice system, and the protection of client rights for people receiving services for intellectual disabilities, mental health, and substance use.

Recovery Housing for Transition-Aged Adults

The Division of Care and Treatment Services is seeking applications for funding to support recovery housing for adults 18-24 who have an opioid and/or stimulant use disorder.

Recovery residences are home-like, residential environments that promote healthy recovery from a substance use disorder and support people recovering from a substance use disorder through the use of peer recovery support.

Recovery housing provides the support needed for young people to transition from adolescence into adulthood with the assistance of services such as peer support, care coordination, and assistance navigating the challenges at this stage of life. Studies suggest that individuals in this age group respond to brief treatment and service models and innovative approaches (Source: Journal of Substance Use and Addiction Treatment). Recovery housing models place an emphasis on choice and self-determination; provide access to voluntary, individualized services and flexible supports; make peer support available while valuing the role of the peer; provide opportunities for social connection; and have a recovery orientation, including support for trauma recovery (Source: SAMHSA). The fourth edition of the American Society of Addiction Medicine’s (ASAM) Continuum of Care, published in October of 2023, added an additional level of care, recovery residence (Source: ASAM Criteria). The addition of this level of care recognizes the importance of a supportive living environment to support clinical services.

This funding will increase support services provided for adults 18-24 with opioid and/or stimulant disorders by:

• Educating, developing, and promoting a modeled approach for recovery housing to be an element of the integrated continuum of care model within the Wisconsin substance use services system.
• Identifying service gaps and areas of improvement to better serve the young adult population.
• Establishing a recovery housing model framework to best serve young adults in Wisconsin.

Preference will be given to:

• Applicants using peers and other supports with experience working with adults 18-24.
• Programs with established relationships to Tribal nation, county, and/or community treatment programs.

The Impact Fund awards recoverable grants to legal services nonprofits, private attorneys, and small law firms who seek to confront economic, environmental, racial, and social injustice. Since our founding in 1992, the Impact Fund has made more than 800 recoverable grants totaling more than $10 million for impact litigation. We award grants four times per year, most within the range of US$10,000 to US$50,000.

Social Justice

The Impact Fund provides grants and legal support to assist in human and civil rights cases. We have helped to change dozens of laws and win cases to improve the rights of thousands. The cases we are funding allege that:

• In Texas and North Carolina, incarcerated people with mental health disabilities are forced to remain in jail despite being found not guilty and unable to proceed with a criminal trial.
• In Orange County, California there are currently 13 gang injunctions under effect, which disproportionately affect young men of color.
• In Chicago, Illinois, the city’s homeless shelter program is inaccessible to people with disabilities.
• In Springfield, Oregon, the city and its police department used excessive force against Black Lives Matter protesters.
• In West Virginia, the state fails to protect children in foster care from abuse and neglect. 
• In Montana, voter suppression laws disadvantage young adults and give priority to gun owners.
• In Gary, Indiana, a gun manufacturer negligently marketed and distributed its guns, leading to an epidemic of gun violence in the city. 
• In Vancouver, British Columbia, the police perpetuate systemic discrimination against Indigenous people through bureaucratic measures.

Environmental Justice

The Impact Fund provides grants to support local litigation for environmental justice. These grants are for cases aiming to help people or communities who are affected by environmental harm or who lack access to basic environmental needs, such as clean water, clean air, adequate waste treatment, and green spaces. The cases we are funding allege that:

• In Centreville, Illinois, the city’s failure to maintain its sewer system has caused raw sewage to flood peoples’ homes, endangering the property and health of a predominantly Black community.
• In Fresno County, California, the California Department of Transportation approved a highway expansion project that would increase air pollution and traffic in one of the state’s most environmentally burdened communities.
• In downtown Milwaukee, Wisconsin, the proposed expansion of a highway would divide the region's Black, Asian, and Latine neighborhoods and cause pollution and ill health.
• In North Dakota, the five-month closure of a highway in response to the Dakota Access Pipeline protests disproportionately affected the livelihoods and health of the Standing Rock Sioux Tribe members.
• In Ontario, Canada, mercury contamination of the English-Wabigoon river system causes catastrophic environmental and health impacts for the Grassy Narrows First Nation.
• In Sacramento, California, the county government and Sacramento Area Sewer District violated the Clean Water Act by discharging raw sewage into nearby rivers.
• In the Eastern Coachella Valley in California, 1,900 residents of the Oasis Mobile Home Park suffer from arsenic-laced drinking water, wastewater contamination, and overcharging for utilities.

Economic Justice

The Impact Fund provides financial and other forms of support to cases fighting for economic justice. From workers' rights to consumer protection for vulnerable populations, impact litigation is a powerful tool to hold corporationss and the government accountable. The cases we are funding allege that:

• In Brooklyn, New York, a prominent mortgage lender engaged in predatory practices, leaving homeowners of color at risk of losing their homes. 
• In Washington, live-in caregivers are unconstitutionally excluded from the state’s wage-and-hour protections.
• In Ravalli County, Montana, the county has created a “modern-day debtors’ prison” by incarcerating people unable to afford pre-trial fees. 
• In San Diego, California, vehicle ordinances target unhoused vehicle owners even when no adequate housing alternative exists.
• In New York, a federal immigration detention facility is violating minimum wage and forced labor laws by forcing detainees to work for just a dollar a day. 
• In Minneapolis, Minnesota, the city and county destroy the property of unhoused individuals and conduct forced evictions from public spaces. 
• In Miami, Florida, insurance companies discriminate against a nonprofit community development corporation renting to tenants with Section 8 rental subsidies.

About The Foundation

“Consistent with the Potawatomi heritage, the mission of the Forest County Potawatomi Foundation is to fight poverty, promote economic opportunity, strengthen communities and provide an example of responsible citizenship by assisting charitable organizations and support the pursuit of education by offering scholarships".

The Potawatomi were once a large nation with much land, but after forced removal and the loss of millions of acres, scattered pockets of Potawatomi across regions of the United States and Canada resulted. Although diminished in number, our ancestors who remained in Wisconsin would eventually become known as the Forest County Potawatomi Community, Bodwe’wadmi, Keepers of the Fire. For many generations, we relied on the environment for sustenance. We recognized the importance of using what one needs and giving away what one can. Our tribe took only what the earth and land was able to replenish. Unfortunately, our people struggled with oppressive poverty that made for a very difficult life. But throughout our history, we have maintained our traditional ways, guided by tribal values that include reverence for the wisdom of our elders, embracing responsibilities for family and tribe, respect for the environment, and a willingness to share.

With the advent of Indian gaming, our tribe was able to establish and build two casinos — the first in Forest County, followed by one in Milwaukee. The revenue generated by gaming has had an impact on our way of life beyond our imagination. Our people now have good jobs, good homes, exceptional health care, and are closer to the realized vision of self-sufficiency. The resulting impact of these two businesses on the surrounding communities has been positive as well, through our family-supporting jobs and shared revenue payments that fund services for citizens. Through our casinos and tribal ventures, our tribe is the largest employer in Forest County, and among the top employers in Milwaukee.

When building the Milwaukee business, we realized early on that simply being located in the community wasn’t enough. We longed for a way to express our belief in giving back. We were committed to investing in, and providing, promising futures to individuals and families in the community. And so it was in 1999, that the Forest County Potawatomi Foundation was born with these guiding principles: to help fight poverty, promote economic opportunity, strengthen communities and provide an example of responsible citizenship by assisting charitable organizations. To date, the Foundation has proudly contributed over $30 million to charitable causes in both Forest County and in the Greater Milwaukee area.

But the story does not end here. Through the work of our Foundation, our tribe’s vision of building a better tomorrow for future generations continues … the fire still burns.

The Foundation is a result of the rich history and beliefs of the Potawatomi tribe. The cornerstone of those beliefs is the Circle of Life. Paralleling the seasons of nature, the Circle of Life is a demonstration of the Potawatomi reverence for all living things and nurtures the belief that they are a self-reliant and self-determined nation of people. Difficult times have sometimes made that self-reliance seem impossible to achieve. The Potawatomi people have witnessed both young and old struggle against the oppression of poverty. They have learned much in their past struggles, and the Circle of Life guides them to use the fruits of these lessons to help others overcome their obstacles.

Therefore, a major effort of the Foundation and its funds is to assist charitable organizations that help those people who are faced with economic challenges. As a result, the Foundation targets its resources to those areas with a high percentage of individuals with low and very low incomes.

Mercyhealth Community Scholarship

Seven $2,000 scholarships are available to graduating seniors or individuals already enrolled in college programs throughout Mercyhealth’s service area in Wisconsin and Illinois. Five scholarships will be given to students pursuing general health care careers and two specifically for nursing.

About the Foundation

Jesse W. Couch lived a life of zeal, honor, and dedication to the betterment of his community. The Couch family now humbly stewards the foundation he created to carry on his legacy of service for future generations. We believe that impact is best accomplished through partnerships with local organizations that know the people and communities they serve. Mercyhealth Autism Support Fund

When a child is newly diagnosed with autism, the cost of resources can be overwhelming. Mercyhealth Development Foundation's Autism Support Fund can help.

The Autism Support Fund was created to help families cover the costs of additional beneficial resources and therapies that have very limited outside funding, such as:

• Social skills classes
• Sensory equipment
• Safety equipment
• Respite care
• Therapeutic materials

You may submit an application for one item per quarter, for a maximum of $500 per year in Rock County and $1,000 per year in Walworth County.

Individual gifts, when joined with others given through the Community Foundation for the Fox Valley Region, ensure the impact lasts forever so that our community will always flourish.

We are a tax-exempt, publicly supported nonprofit organization managing charitable funds — mostly permanent endowments — created by many families, businesses and organizations.

These funds generate grants for the long-term benefit of the people of the Fox Valley region of Wisconsin.

Isabelle Rowe Nursing Scholarship

After more than sixty years of married life together, Julian did everything he could to care for his wife, Isabelle, at home when she was stricken with Alzheimer’s disease. Although he had his own health challenges and it was a struggle, even with the help of family and neighbors, he was certain that no one could care for her as lovingly and conscientiously as he could. Finally, he couldn’t anymore.

Julian was heartbroken when Isabelle needed to go into a care facility in 2005. He rarely missed a day of sitting by her side in the cheerful room, furnished with two of their favorite chairs from home. Like a friendly and loyal watchdog, he was attentive to how she and the other residents were treated by the staff, and he was relieved to see the kindness and respect she was given by all. He was especially impressed by the gentle, patient care given by the certified nursing assistants, who have the most contact with these vulnerable but often difficult patients. As he got to know the staff at the care facility, his admiration for the nursing assistants grew. He learned that some of them were in school studying to advance their credentials toward a nursing degree. He also learned that some of them were single mothers, struggling to make ends meet on a CNA’s salary, and he decided to do something that could help. In 2007, Julian collaborated with Waupaca Area Community Foundation to establish the Isabelle Rowe Nursing Scholarship Fund.

The scholarship has continued to grow beyond the initial gift with sound investment and contributions from family and friends. Since the scholarship was established, both Julian and Isabelle have passed, but their legacy continues as the Rowe family honors their memory through this scholarship.

Award Amount:

Number of awards may vary, for tuition and books, payable for the fall semester of college:

• $2,000 non-renewable scholarship award(s) for students planning to attend a 4-year college/university, or
• $1,000 non-renewable scholarship award(s) for students planning to attend a 2-year college or technical school.

Gupta Family Foundation is a private, nonprofit foundation headquartered in Herndon, Virginia, USA. Our mission is to support organizations that provide focused intervention in the lives of people who have been disadvantaged in some way to help them become self-reliant. We take a very broad view of “disadvantage” to include anything that holds a person back from realizing their potential, such as poverty, physical or mental disability, social alienation, etc. The foundation also supports relief agencies that serve people affected by emergencies such as natural disasters.

The foundation evaluates and awards annual and multi-year grants ranging from $5,000 to over $250,000 (USD). Our focus is on funding smaller organizations all around the world that are led by individuals with a deep personal commitment to their missions. 

Our selection criteria include:

• Mission alignment
• The organization is run by the founder or, if not, by a successor who embodies the original inspiration, passion and commitment of the founder.
• At least 90% of grant monies reaches the intended beneficiaries.
• The organization is non-sectarian, i.e.,
  • It does not, directly or indirectly, support or condone the proselytization of any religion,
  • It is not supported by or affiliated to a religious organization.

The Dickinson Area Community Foundation is a permanent, charitable endowment created by the community, for the community. The Foundation is made up of 101 charitable funds, each named and designated by donors to benefit the area. 

Mission Statement

Grants are available from the following funds to 501(c)(3) non-profit organizations and agencies or organizations affiliated with non-taxable organizations (i.e., schools, churches, cities) located in or servicing people in the Dickinson County and surrounding Michigan and Wisconsin areas.

Veterans Assistance Fund

Funds available for grants to non-profit organizations that support veterans of the United States Armed Forces.

Civic Improvement Fund

Funds available for collaborative project(s) to benefit or serve the Iron Mountain, Kingsford, and Breitung Township communities. The Grant Committee includes a representative from each community.

Community Health Fund

Funds available for grants for youth and senior health-related programs (i.e., programs designed to reduce behavioral risk factors such as substance abuse, alcohol/binge drinking, smoking, and depression; and to encourage healthy activities). 

Environmental Fund

Funds available for grants for projects that improve or create methods of enhancing natural resources in the region.

Fine Arts Fund

Funds available for grants for projects in the area of theatrical, musical, and performing arts.

Health Care Fund

Funds available for grants for programs involved in prevention, primary physical health care, educational, and other health related services.

Homeless Prevention Fund

Funds available for grants for programs that provide assistance to people in need to prevent loss of homes

Hunger Fund

Funds available for grants to organizations that provide food to people in need.

Iron Mountain/Kingsford Kellogg Youth Fund

Funds available for granting to applicants from the Iron Mountain/Kingsford area by the Iron Mountain/Kingsford Youth Advisory Committee (YACs) for youth-related programs.

The Dudley T. Dougherty Foundation Vision

The Dudley T. Dougherty Foundation, "A Foundation for All", was established in 2002. It was begun in order to give a clear voice for those who wish to be a part of the many, worthy, forces for change in our world.

We are a foundation whose purpose is to look ahead towards the future, giving the past its due by remembering where we came from, and how much we can all accomplish together. We aim to make the critical difference on our planet by recognizing and having respect for our ever changing world. We respect all Life, the Environment, and all People, no matter who they are.

Division of Care and Treatment Services

The Division of Care and Treatment Services (DCTS) manages and supports seven care and treatment facilities for people living with psychiatric disorders and intellectual disabilities, community mental health and substance use services, community mental health and substance use treatment services for people in the criminal justice system, and the protection of client rights for people receiving services for intellectual disabilities, mental health, and substance use.

Tele-Behavioral Health

The Division of Care and Treatment Services is seeking applications from behavioral health providers for funding to:

• Expand opportunities for people to access tele-behavioral health services.
• Reduce the technology and location barriers that exist for many people to access tele-behavioral health services.
• Reduce disparities in access to behavioral health care for underserved populations.

Tele-behavioral health can maintain continuity of care and preserve the patient-provider relationship when in-person visits are not possible. It can also fill gaps in access to behavioral health care caused by shortages of providers.

Wisconsin Medicaid updated its policies during the COVID-19 pandemic to allow Medicaid members to receive telehealth services from an originating site (see ForwardHealth Topic #22739). An originating site is defined as where the patient or member is located, including a home, pharmacy, homeless shelter, assisted living provider, and/or school (previously, only hospitals, nursing homes, and physician offices were designated as originating sites for care)

Some communities still face barriers in accessing tele-behavioral health services. Access to technology and broadband can present barriers for certain populations, including older adults, the economically disadvantaged, and people who live rural areas.

DHS is addressing these barriers by funding behavioral health care providers who provide or plan to provide services in areas of Wisconsin that lack behavioral health care. Agencies awarded grants under this funding opportunity must partner with non-traditional sites to establish tele-behavioral access hubs. Non-traditional sites include schools, pharmacies, long-term care facilities, public libraries, community centers, homeless shelters, food pantries, and other publicly accessible locations in underserved areas. Grant funds can be used to purchase technology for the provider and non-traditional site, install and maintain internet service, support for using the technology, and remodel spaces.
