Through this initiative, NAFC will provide $20,000 grants to member free and charitable clinics to implement or expand food insecurity screening and patient-centered interventions that improve access to healthy food and nutrition education as measured by the US Household Food Security Module or Hunger Vital Signs and patient education focused on food as medicine for eligible patients in order to increase the percentage of patients with improved food security scores within one year of project implementation.
