What does this program do?

This program provides affordable funding to develop essential community facilities in rural areas. An essential community facility is defined as a facility that provides an essential service to the local community for the orderly development of the community in a primarily rural area, and does not include private, commercial or business undertakings.

How may funds be used?

Funds can be used to purchase, construct, and / or improve essential community facilities, purchase equipment and pay related project expenses.

Examples of essential community facilities include:

• Health care facilities such as hospitals, medical clinics, dental clinics, nursing homes or assisted living facilities.
• Public facilities such as town halls, courthouses, airport hangars or street improvements.
• Community support services such as child care centers, community centers, fairgrounds or transitional housing.
• Public safety services such as fire departments, police stations, prisons, police vehicles, fire trucks, public works vehicles or equipment.
• Educational services such as museums, libraries or private schools.
• Utility services such as telemedicine or distance learning equipment.
• Local food systems such as community gardens, food pantries, community kitchens, food banks, food hubs or greenhouses.

What are the funding priorities?

• Priority point system based on population, median household income
• Small communities with a population of 5,500 or less
• Low-income communities having a median household income below 80% of the state nonmetropolitan median household income.

Grant Approval

Applicant must be eligible for grant assistance, which is provided on a graduated scale with smaller communities with the lowest median household income being eligible for projects with a higher proportion of grant funds. Grant assistance is limited to the following percentages of eligible project costs:

Maximum of 75 percent when the proposed project is:

• Located in a rural community having a population of 5,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 60 percent of the State nonmetropolitan median household income.

Maximum of 55 percent when the proposed project is:

• Located in a rural community having a population of 12,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 70 percent of the State nonmetropolitan median household income.

Maximum of 35 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 80 percent of the State nonmetropolitan median household income.

Maximum of 15 percent when the proposed project is:

• Located in a rural community having a population of 20,000 or fewer; and
• The median household income of the proposed service area is below the higher of the poverty line or 90 percent of the State nonmetropolitan median household income. The proposed project must meet both percentage criteria. Grants are further limited.

The Connecticut Department of Public Health, Facility Licensing and Investigations Section is responsible for the oversight and administration of the Long-Term Care Program and seeks innovative projects that support, protect and benefit nursing facility residents.

Connecticut receives a portion of collected Civil Money Penalty (CMP) funds to be used to support projects that improve the overall quality of life and/or care of nursing facility residents.

About CMP Funds

A CMP is a monetary penalty the Centers for Medicare & Medicaid Services (CMS) may impose against nursing homes for either the number of days or for each instance a nursing home is not in substantial compliance with one or more Medicare and Medicaid participation requirements for long-term care facilities. A portion of CMPs collected from nursing homes are returned to the states in which CMPs are imposed. State CMP funds may be reinvested to support activities that benefit nursing home residents and that protect or improve their quality of care or quality of life.

Nursing facility providers, stakeholders and other organizations are encouraged to submit CMP fund grant applications for the development and implementation of quality improvement initiatives that directly or indirectly benefit nursing facility residents. 

Project proposals should demonstrate current and sound evidence-based practices that promote quality of care and quality of life for nursing facility residents. CMP fund grants are only available for quality improvement initiatives that are outside the scope of normal facility operations. 

Funding Uses

The following examples of allowable uses of CMP funds are not all-inclusive. Please see the Allowable and Non-Allowable Uses of CMP Funds download for more information. CMP funds may be used for:

• Assistance to support and protect residents of a facility that closes or is decertified
• Time-limited expenses incurred in the process of relocating residents to home and community-based settings or another facility when a facility is closed or downsized pursuant to an agreement with the state Medicaid agency
• Projects that support resident and family councils and other consumer involvement in assuring quality care in facilities
• Facility improvement initiatives, such as joint training of facility staff and surveyors, or technical assistance for facilities implementing quality assurance and performance improvement programs.

Charitable Contributions

Costco Wholesale’s primary charitable efforts specifically focus on programs supporting children, education, and health and human services in the communities where we do business. Throughout the year we receive a large number of requests from nonprofit organizations striving to make a positive impact, and we are thankful to be able to provide support to a variety of organizations and causes. While we would like to respond favorably to all requests, understandably, the needs are far greater than our allocated resources and we are unable to accommodate them all.

Grant Applications

If the request is under consideration, you may be contacted by staff for any additional information needed. Applications are reviewed within 4-6 weeks, and decisions are made based on several factors, including: type of program; identified community need not otherwise available; indication that evidenced based data will establish measurable results of intended outcomes; community collaboration; broad base of financial support; project budget and operating expenses.

Mission

The DanPaul Foundation will use its resources to help train teachers and parents in early childhood development, protect children from abuse and neglect, stimulate children's personal social responsibilities, and offer them opportunities for enrichment and growth.

The Foundation will also encourage children to be concerned and informed about the environment and the underprivileged, particularly with regard to clean air and water, and adequate housing and nutrition for all.

• Poverty + Neglect: We seek to help those in poverty as well as educate youth about their responsibility to consider the underprivileged and take care of those most in need of life's basic essentials like adequate housing and proper nutrition.
• Refugee Enrichment: We wish to help refugee youth by supporting programs that provide them enrichment and help them transition to life in a new country.

The DanPaul Foundation provides grants to 501(c)3 tax-exempt non-profit organizations as defined by the IRS. The Foundation is interested in providing funding to programs that directly serve the health, education, development, and welfare of the world's youth.

Grants range from a few hundred dollars up to $15,000 per calendar year.

What stage of growth does DRK Foundation typically fund?

Early stage: Organizations who are early stage, which we define as post-pilot and pre-scale. This typically means: 

• Your program, product or service is already being used in the market or in the field, 
• You have early indication that your model is having its intended impact on the beneficiary populations,
• Your organization is relatively young (ideally between two and five years old, although we will consider both younger and older organizations).

Venture funding: In the case of for profits, we typically support Seed to Series A organizations, and never lead rounds; we also generally but not exclusively refrain from participating in financings exceeding a $15M USD post-money valuation.

Identifying and incubating innovative early-stage solutions. We identify, incubate, and develop innovative early-stage solutions with the highest potential for impact. Global Impact Cash Grants go to nonprofit organizations (NPOs) and nongovernmental organizations (NGOs) that address a significant social problem. We're looking for programs that fit within our investment sectors, focus on the underserved, and use technology to improve the reach and efficiency of services.

Mission

Supported organizations include arts schools, ballets, museums, operas, performing arts centers, symphonies and theaters.

Funding Priorities in Culture

In the recent past, 25% of total funding has been allocated to Culture. Organizations with budgets over $10 million have received 60% of the funding in Culture.

The Hearst Foundations are only able to fund approximately 25% of all grant requests, of which about 80% is directed to prior grantees and about 20% is targeted toward new grantees.

The mission of the Foist Family Perseverance Scholarship Fund is to provide financial assistance to adult learners or graduating high school seniors of a high school in Berkshire County, MA, who are pursuing a career in health care/sciences (clinical or research), including nutrition/dietician, nursing, occupational therapy, physical therapy, and neuropsychology. 

About the Foundation

Jesse W. Couch lived a life of zeal, honor, and dedication to the betterment of his community. The Couch family now humbly stewards the foundation he created to carry on his legacy of service for future generations. We believe that impact is best accomplished through partnerships with local organizations that know the people and communities they serve. How and Why Giving Back to Communities is Important to O’Reilly

From O’Reilly Auto Parts’ inception, O’Reilly co-founder C.H. “Chub” O’Reilly instilled the culture of maintaining high standards for every endeavor, including the area of building civic pride. Providing funding and visibility to organizations specializing in providing relief from natural disasters and state of emergency, including providing temporary shelter, food, water, and hygiene or sanitation products to those affected and aiding in cleanup efforts.

Application Requirements

We will consider three principal types of grants:

Project Support Grants support specific projects or programs aligned with our mission. These requests may include some funds earmarked for the overhead costs associated with running a project.

General Operating Support Grants provide limited general operating support for the core operations or organizations whose missions and activities are aligned with our mission. These grants will often help the grantee build organizational, programmatic, and fundraising capacity. Operating support is not intended to help organizations in fiscal crisis. Applicants must have a current strategic or business plan that clearly outlines the organization’s goals and presents a plan for achieving results. Operating support grants must not exceed 15% of an organization’s total agency budget.

Capital Support Grants provide limited support for capital campaigns to fund the acquisition and construction of facilities, existing property renovation, or the purchase of major equipment. The program has a comprehensive approach to funding capital initiatives, which also includes funding for increased program capacity. A feasibility study may be required for capital initiatives to be considered.

The Lawrence Foundation is a private family foundation focused on making grants to support environmental, human services and other causes.

The Lawrence Foundation was established in mid-2000. We make both program and operating grants and do not have any geographical restrictions on our grants. Nonprofit organizations that qualify for public charity status under section 501(c)(3) of the Internal Revenue Code or other similar organizations are eligible for grants from The Lawrence Foundation.  

Grant Amount and Types

Grants typically range between $5,000 - $10,000. In some limited cases we may make larger grants, but that is typically after we have gotten to know your organization over a period of time. We also generally don’t make multi-year grants, although we may fund the same organization on a year by year basis over a period of years.

General operating or program/project grant requests within our areas of interests are accepted. In general, regardless of whether a grant request is for general operating or program/project expenses, all of our grants will be issued as unrestricted grants.

Max and Victoria Dreyfus Foundation Grant

The Foundation will consider requests to support museums, cultural and performing arts programs; schools and hospitals; educational, skills-training and other programs for youth, seniors, and persons with disabilities; environmental and wildlife protection activities; and other community-based organizations and programs.

The Berkshire Taconic Community Foundation

Berkshire Taconic Community Foundation was established in 1987 in Salisbury, Conn., with an anonymous donation of $100,000. Originally an affiliate of the New York Community Trust, the foundation became an independent public charity in 1990. Today, BTCF manages more than 550 funds that put over $14 million into action each year. Berkshire Taconic Community Foundation serves Berkshire, Columbia, northeast Dutchess and northwest Litchfield counties. This unique area where New York, Connecticut and Massachusetts meet contains 2,200 square miles of natural beauty and small-town charm that is home to nearly 230,000 people.

Tabor Foundation Fund

Who can apply: Residents of northwest Litchfield County, Conn., northeast Dutchess County, N.Y., or southeast Columbia County, N.Y., who plan to undertake education or training in a health care or medical field (nursing, EMT, physical therapy, counseling, etc.) and to seek employment in their community afterwards. Non-residents working in the area may also apply.

The committee will review requests for scholarships according to the following criteria:

• Impact of funding on the applicant’s ability to attain a health care qualification
• Ability of the applicant to move forward in his/her career through attainment of a health care qualification
• Commitment of the applicant to a healthcare career in the communities that serve Northwest CT or Northeast Dutchess County

Community Partnership Award

The Mutual of America Foundation Community Partnership Award recognizes outstanding nonprofit organizations in the United States that have shown exemplary leadership by facilitating partnerships with public, private or social sector leaders who are working together as equal partners, not as donors and recipients, to build a cohesive community that serves as a model for collaborating with others for the greater good.

Each year, the Mutual of America Foundation sponsors a national competition in which hundreds of organizations demonstrate the value of their partnership to the communities they serve, their ability to be replicated by others and their capacity to stimulate new approaches to addressing significant social issues.

Six organizations are selected by an independent committee to receive the Community Partnership Award.

• The Thomas J. Moran Award is given to the national award-winning program and includes $100,000 and a documentary video about the program.
• The Frances R. Hesselbein Award is given to a partnership that is addressing social challenges in more than one community, or which demonstrates the potential to be replicated in other communities. This recipient receives $75,000.
• Four other organizations are named Honorable Mention recipients for their programs, and each receives $50,000.

Since its inception in 1996, the Community Partnership Award has recognized 262 partnerships from cities and towns across America. Like so many of our clients working in the nonprofit community, Mutual of America is dedicated to having a direct, positive impact on society.

Purpose of Grants

Made possible through the Southbury Community Trust Fund, grants provide support for a variety of programs that benefit Southbury residents. Priority is given to proposals that demonstrate collaboration and strong partnerships.

Possible Projects

• Social and Community Services: Support Southbury residents by providing access to basic needs, transportation, and affordable housing.
• Wellness and Health Education: Promote a healthy, vibrant community by encouraging access to prevention programs, fitness, and access to care for all ages.
• Enhancement of Youth Development: Foster positive identity, strong values, social competencies, commitment to learning, community service, empowerment, leadership, and resilience for Southbury youth.
• Aging and Livable Communities: Develop and promote programs that enable older adults to age in place successfully. May include: connecting older adults with benefits and services; reaching people who are underserved; home maintenance, chore, and home safety programs; and building age-friendly communities.
• Arts and Culture: Promote access to performing arts and fine arts programs for community enrichment.
• Environmental Awareness: Encourage the preservation, protection, and conservation of land, water, and wildlife.
• Cultural Heritage: Support Southbury’s tangible cultural heritage and town history.

About Our Women & Girls Fund Grants

Generous gifts and bequests from caring donors have created four permanent endowment funds serving the Northeast, Norwich, Southeast and Windham areas. Together, the four funds are advancing equal opportunities for women and girls throughout Eastern Connecticut, using a lens of equity and social justice in our grants, advocacy and action.

Our grantmaking priorities reflect the challenges identified in The Status of Women and Girls in Eastern Connecticut:

• Empower women and girls to make positive decisions – Examples include mentoring, leadership, career exposure, character development.
• Promote healthy relationships and prevent domestic violence – Examples include teen dating violence prevention education, domestic violence shelters and transitional housing, victim advocacy.
• Advance family economic security – Examples include training for jobs and career advancement, programs to overcome barriers to financial security, and other supports for low-income women.
• Support access to women’s health, mental health and reproductive health services – Examples include counseling, health education, health care access.
• Support public policy, advocacy and leadership to promote equity – Examples include statewide policy groups, training programs for grass-roots organizations. Please call the program staff to discuss your ideas.
• Help New London women, ages 62 and older, with limited incomes to remain in their homes (Southeast only) – Examples include meal delivery, nursing services.
