South Carolina Community Development Block Grant
The South Carolina Community Development Block Grant (“State CDBG Program”) is designed to aid units of general local government in improving economic opportunities and meeting community revitalization needs, particularly for persons of low- and moderate-income. The State CDBG Program has been funded through the State since 1982 by the U.S. Department of Housing and Urban Development (HUD) under the Housing and Community Development Act of 1974, as amended (Title I).
CDBG Program Goals and Outcomes
The SC Department of Commerce seeks to improve the well-being of all South Carolinians in a manner that supports and enhances a high quality of life. The State CDBG Program supports the agency’s efforts to strengthen communities through revitalization and improvement of neighborhoods, public infrastructure, and the local economy.
The three overarching goals of the HUD CDBG program are to provide decent housing, economic opportunities and a suitable living environment. Within the context of these goals, each project funded by the State must also meet one of three outcomes identified by HUD: affordability, accessibility, or sustainability.
Local Match/Leveraging
CDBG projects are expected to leverage other public and private investments and serve as a catalyst for future development.
There must be a 10% match of the total CDBG request, unless otherwise approved, which can come from a variety of committed sources including other grants, loans, waiver of fees, public or private investments, and documented volunteer or in-kind contributions. Matching funds must be immediately available for the project at the time of application.
Community Development Block Grant Categories
There are three broad grant program categories: Community Development, Business Development, and Regional Planning. The Community Development Program is further broken down into several subcategories to address infrastructure, community facilities, and local priorities.
Community Development Programs
Description
This program is designed to produce outcomes that improve citizens’ quality of life and create a competitive environment for jobs and investment by addressing priority community development needs. Activities should contribute to healthy, safe and sustainable neighborhoods and communities. These grants are designed to improve the quality of life for distressed and LMI communities. Community Development projects will compete within the following subcategories that have the same general submission requirements, except that the Ready to Go Program will not be funded in a competitive funding round.

Local Priorities Program
Outcome and Priorities
This program is designed to address unique local and regional needs and priorities for addressing those needs through community development, including projects that reflect priorities established in conjunction with the S.C. Department of Commerce Business Incentives & Community Development Division. Projects may reflect community development needs that are not typically funded through the other State CDBG programs and generally should not be funded by other HUD partner programs, such as HOME and NHTF (affordable housing), ESG (homelessness) or HOPWA (special needs). These funds will be used for alternative grant activities and partnerships that meet the community development needs of eligible municipalities. Local Priority projects could include historic preservation, innovation, energy conservation, parks, and trails/greenways. New or expanded public service activities are also eligible. Projects should leverage other funding or include local funding; impact identified needs and demonstrate strong community support while meeting a National Objective and all other requirements.

NAFC Patient Care Fund
The NAFC Patient Care Fund supports direct patient services that expand immediate access to care. This funding is designed to help NAFC members meet urgent patient needs by supporting clinical supplies, expanded services, contracted providers, transportation assistance, medication access, and other direct care resources.

Breast Cancer Screening & Prevention Initiative
The Breast Cancer Screening & Prevention Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to lifesaving breast cancer screening and prevention services for uninsured and medically underserved. Through this initiative, the NAFC will provide $30,000 grants to members to establish or strengthen breast cancer screening programs in alignment with evidence-based guidelines from the U.S. Preventive Services Task Force and the American Cancer Society. Funding will support activities such as improving patient identification and referral systems, increasing access to mammography services through partnerships or mobile units, strengthening staffing and technology capacity, and providing patient education and case management to ensure follow-through with recommended screenings. By enhancing early detection and preventive care, the initiative seeks to reduce barriers to care and improve health outcomes for women who might otherwise go without critical cancer screening services.

Food as Medicine Initiative
The Food as Medicine Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to address food insecurity as a critical driver of health outcomes for uninsured and medically underserved patients. Research continues to demonstrate the strong connection between food insecurity and poor health, including increased risk of chronic conditions such as diabetes, hypertension, obesity, and certain cancers. Through this initiative, NAFC will provide $20,000 grants to member free and charitable clinics to implement or expand food insecurity screening and patient-centered interventions that improve access to healthy food and nutrition education as measured by the US Household Food Security Module or Hunger Vital Signs and patient education focused on food as medicine for eligible patients in order to increase the percentage of patients with improved food security scores within one year of project implementation.

Medication Therapy Management (MTM) Initiative
The Medication Therapy Management (MTM) Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to improve medication access, adherence, and patient understanding among uninsured and medically underserved populations. Individuals without health insurance often face significant barriers to obtaining prescription medications and receiving the counseling necessary to manage chronic conditions effectively. Through this initiative, NAFC will provide $30,000 grants to member free and charitable clinics and pharmacies to establish or expand Medication Therapy Management programs aligned with recommendations from the American Pharmacists Association. Funding will support activities such as staff certification in MTM, development of standardized protocols, and patient education to help individuals better understand their medications, manage side effects, and support overall treatment plans. By strengthening pharmacist-led care and improving communication between providers and patients, this initiative seeks to enhance medication adherence, improve chronic disease management, and advance better health outcomes for vulnerable patient populations.

Healthcare Access Initiative
The Healthcare Access Initiative, supported by the McKesson Foundation and administered by the NAFC, aims to expand access to essential healthcare services for uninsured and medically underserved individuals. Free and charitable clinics and pharmacies play a critical role in ensuring that patients who fall outside traditional healthcare systems can receive timely, high-quality care. However, many clinics face barriers related to staffing, care coordination, technology, and patient outreach that limit their ability to meet growing community needs. Through this initiative, NAFC will provide $20,000 grants to member clinics and pharmacies to strengthen programs that increase access to primary care, specialty services, and preventive care. Funding will support innovative and practical approaches that reduce barriers to care, improve patient engagement, and expand the capacity of clinics to serve vulnerable populations.

Mary C. Dillon Scholarship

Award:
Established by the Dillon Family, this scholarship is awarded to a student whose primary residence is in Torrington and who pursues, or intends to pursue, a degree in nursing at an accredited university or college. Preference is given to applicants with a record of volunteering with the Red Cross or other health-related organizations. This scholarship is not financially need-based. (No FAFSA required)

SVNA Educational Enrichment Program Scholarship

Consistent with the SVNA’s mission to provide the best home health, hospice and preventative care, this scholarship supports health care professionals pursuing continued education in a health care field who reside in Canaan, Cornwall, Goshen, Kent, Norfolk, North Canaan, Salisbury, Sharon or Warren.

The NAFC Response & Emergency Needs Fund is designed to address urgent and unforeseen challenges that threaten operations or patient access to care within NAFC member organizations. These organizations often operate with limited reserves, making them especially vulnerable to unexpected repairs, equipment failures, workforce disruptions, or community emergencies. This fund provides rapid, flexible support to help stabilize operations and ensure continuity of care for uninsured and underserved patients.

The Alex Sevcsik Opportunity Scholarship is open to students who are graduating seniors from the Academy of Arts, Careers, and Technology, or students who have received services from the Eddy House. Preference will be for students who are interested in pursuing Business or Automotive Technologies. Qualifying students will be planning to attend any 2- or 4- year college or university, community college, or trade school in or out of state.

Valerie Rose Pinnette Memorial Scholarship Fund
Award Average:
Established by Ed and Rosalie Pinnette in memory of their daughter, Valerie. This scholarship is awarded to a graduating high school student who resides in and attends school in NCCF's 20-town service area. Preference is given to students pursuing a degree in environmental sciences.
Kieth and Serena Cockrell Scholarship
This program was established in honor of long-time HBCU and UNCF supporters Kieth and Serena Cockrell, who are passionate about education and paving the way for future leaders. Eligible applicants must be permanent residents of North Carolina and a high school senior graduating from any public, private, or charter high school in Mecklenburg County, Union County, or Cabarrus County during this current Spring 2026 term.
Renewable Up to $30,000 Over a Four (4) Year Period. Up to $7,500 Per School Year.
Dr. John and Mary Martin Scholarship Fund
This fund was established to honor Dr. Martin and Mrs. Martin’s service as educators. The fund shall provide scholarship assistance to support the college-going ambitions of graduating seniors of Grandview High School who aspire to teaching careers in the field of pre-K through grade 12 education. Preference shall be given to students who are underrepresented in their area of focus within the education field; for example, males of color in pre-school or elementary school.
