The CDC Foundation helps the Centers for Disease Control and Prevention (CDC) and the public health system save and improve lives by unleashing the power of partnerships to protect the health, safety and security of all communities, everywhere. The CDC Foundation is authorized by Congress to mobilize philanthropic partners, corporations, organizations and individuals to support the critical health protection mission of CDC and the public health system.

The CDC Foundation manages hundreds of programs each year aimed at protecting communities across the country from a variety of health threats—from chronic disease conditions including cardiovascular disease and cancer, to infectious diseases like malaria and HIV to emergency responses, such as Ebola and Zika.

Contractor for Implementation and Evaluation of Healthy Native Babies Project Materials

This RFP request proposals from Tribes, Tribal Serving Organizations (TSOs) and Urban Indian Organizations (UIOs) [T/TSOs/UIOs] to implement and evaluate the updated Healthy Native Babies Project (HNBP) materials within their service areas. This initiative aims to strengthen culturally relevant infant safe sleep education, increase community awareness and support Native families in reducing sleep related infant deaths. Selected T/TSOs/UIOs will adapt, disseminate and assess the use of HNBP materials in ways that reflect the cultural, linguistic and community-specific needs of the Native populations they serve. 

This project is a rapid turnaround project to support T/TSOs/UIOs in:

• Implementing HNBP materials in clinical, community, and family setting
• Training staff and community partners
• Evaluating reach, effectiveness, and community feedback
• Identifying barriers, facilitators, lessons learned, and best practices for future replication

About Quest for Healthy Communities Champions

Through the Quest for Healthy Communities Champion program, the Foundation will recognize and give monetary awards to up to 15 individuals or organizations across the US that have committed to improving health in their own communities. In the case of recognition to individuals, the monetary award will be given to the tax-exempt charity of their choosing.

The program reflects the importance of recognizing individuals and organizations that are committed to the purpose of Quest, “working together to create a healthier world, one life at a time” and the Foundation’s belief that increased awareness of individuals and organizations doing this work can lead to healthier communities.

Successful applicants will demonstrate their commitment to improving health in their communities through actions such as:

• Impact on community systems via policy, programming, and education in areas such as workforce development, health, environment, housing, and safety
• Ongoing community engagement in the development of innovative solutions to drive improved health outcomes
• Commitment to increasing individual health autonomy and empowerment through the advancement of diagnostic screening and testing

Global Health, K-12 STEM Education and Economic Mobility Grant

Global Health

Lilly Foundation, through its charitable grants, aims to support permissible charitable efforts to improve access to quality healthcare for resource-limited communities in low- and lower-middleincome countries (L/LMICs) and in the United States. Strengthening access to care is essential for enhancing community well-being and supporting economic stability in resource-limited settings.

The Global Health focus area aims to close gaps in quality healthcare by supporting charitable solutions that improve access, bring care closer to where people live, and address the high burden of non-communicable diseases (NCDs) in resource-limited settings. To the extent permissible, the Foundation prioritizes charitable efforts related to cardiometabolic health in resource-limited settings.

K-12 STEM Education

The K-12 STEM Education focus area aims to close the gaps for students from low-income communities by supporting programs that foster new pathways and pipelines to STEM education through focused K-12 efforts, concentrating on the critical middle school years. The Foundation’s K-12 STEM Education grants are focused on initiatives that benefit Marion County, Indiana.

Economic Mobility

The Economic Mobility focus area supports charitable and educational programs that strengthen workforce readiness, postsecondary and skills-based educational pathways, and housing stability to promote long-term prosperity. This support is focused on Marion County, Indiana, with the goal of improving economic opportunity for low-income communities and advancing community well-being.

Individuals worldwide who have made substantial contributions to health education and health promotion can be nominated for a US $50,000 Elizabeth Fries Health Education Award, including those impacting communities in New Mexico. Nominations, submitted by others, must show how the nominee’s work has benefited public health; self-nominations are not allowed.

Through grant-making, AIH partners with mission-aligned organizations by funding initiatives that align with our strategic objectives.

We are Revolutionaries -passionate in our singular pursuit of discovering, developing and delivering innovative targeted therapies for patients living with RAS-addicted cancer. At Revolution Medicines, we believe in challenging the status quo to find new therapeutic approaches to some of the toughest cancers. The New York Health Foundation is offering sponsorship for nonprofits and community organizations to participate in conferences that support healthy communities, consumer empowerment, and veterans' health. This initiative aims to enhance the visibility of innovative work being done in New York, particularly for those historically marginalized. By facilitating attendance at local, state, and national events, the Foundation seeks to empower organizations to showcase their impactful programs and connect with key stakeholders.

The Prevent Cancer Foundation is launching a $100,000 community grant program for 2024-2026, focusing on patient navigation for cancer prevention and early detection. Community-based organizations can apply for funding over two years to enhance their efforts. Priority will be given to applicants demonstrating best practices, established community partnerships, and initiatives that address health disparities. Note that the application cycle opens in 2026, emphasizing the importance of proactive health measures.

The Northern New Mexico Health Grant, funded by Con Alma Health Foundation, aims to enhance healthcare access for underserved communities in New Mexico. Stealth Biotherapeutics is dedicated to supporting innovative programs that enhance disease awareness and therapeutic management. US Department of Health & Human Services: Food and Drug Administration supports innovative, community-based strategies that strengthen education and outreach on products like hydroquinone; awards US $250,000.

Local campus section leaders at medical schools in New Mexico can seek funding for AMA student recruitment, engagement, community service, and policy participation activities. Focused on medical education, public health, and community health education, the American Medical Association Foundation offers up to US $1,500 per school year to support local AMA initiatives.

New Mexico nonprofits, schools, and government agencies hosting community health events can access funding through Gundersen Lutheran for health fairs, workshops, awareness campaigns, and other well-being initiatives. The Thriving Community Impact Fund also considers local membership and community engagement requests that advance physical, mental, and social health, economic stability, and stronger neighborhood environments.

The Costco Wholesale Charitable Contributions program prioritizes support for children, education, and health services within local communities. Nonprofit organizations seeking assistance must be 501(c)(3) certified and focus on monetary requests. Applications are reviewed within 4-6 weeks, factoring in community needs and collaboration. J.B. Hunt Transport Services offers financial support for various community projects through its Company Giving program. Eligible applicants include 501(c)(3) organizations, schools, and civic groups, while organizations that primarily benefit their members or have high management overhead may be excluded. The Michael & Susan Dell Foundation provides grants to organizations focused on improving the lives of children in urban poverty in the U.S., India, and South Africa.
